Abstract

Purpose: The Boyolali district has never reached the cervical cancer screening coverage target for women of childbearing age, aged 30-50 years. In 2023, cervical cancer screening coverage reached 1.7% of the target of 63%. So this study aims to evaluate the implementation of the cervical cancer screening program in Boyolali District in 2024. Methods: This study used a descriptive, observational, quantitative design, with analysis using a logic model that included the program implementation components of inputs, activities, and outputs. Sampling was purposive, with 28 respondents selected for their roles in the cervical cancer screening program. Primary data were collected using a questionnaire instrument. All data obtained were then processed and analyzed descriptively using Microsoft Excel. Results: The evaluation of the cervical cancer screening program in Boyolali District shows that the input components, particularly regulations and human resources, remain inadequate. In terms of activities, passive and active services, socialization, advocacy, and monitoring and evaluation are still limited. As for cervical cancer screening in Boyolali District in 2024, coverage was only 2.44% of the target, with 6.53% of tests returning positive results. Conclusion: The implementation of the cervical cancer screening program in Boyolali District in 2024 was not optimal. Improvement efforts are needed in regulations, resources, and the intensity of activities to increase screening coverage.

Abstract

Abstract

Abstract

Background: Roberts syndrome (RBS) is a rare autosomal recessive disorder, with no estimates of prevalence have been published. It is caused by mutation in the “Establishment of cohesion 1 homolog 2” (ESCO2) gene on the short arm of chromosome 8, and characterized by symmetrical limb reduction, craniofacial anomalies, growth retardation, mental retardation, cardiac, and renal abnormalities. Objective: The aim of this case report is to describe the findings of a suspected case of Roberts syndrome born at Dr. Sardjito General Hospital and to examine the diagnostic process in suspected cases of Roberts syndrome in low-income countries such as Indonesia.Case: A 32-year-old woman came to the hospital at 29 weeks' gestation. Antenatal sonography screening showed a singleton pregnancy with multiple congenital anomalies: labiognatopalatoschisis, short humerus with absent antebrachial structures. After counseling about the fetal condition and prognosis, the pregnancy was terminated by Misoprostol induction. Post-natal examination revealed an extremely low birth weight baby with labiopalatoschisis, ear malformation, ventricular septal defect, patent foramen ovale, diaphragmatic eventration, bilateral phocomelia, syndactyly, and a Congenital Talipes Equinovarus (CTEV) of the left foot. A Roberts syndrome was suspected, and a cytogenetic testing was initially planned, however it was not done due to the high cost of the test and its availability. The baby’s condition deteriorated and later died on the second day after birth.Discussion: A suspicion of Roberts syndrome may begin during prenatal ultrasound of a fetus with multiple congenital anomalies. Mortality rates are high in severe clinical manifestations. Follow-up management for survivors involves a multidisciplinary medical approach. Conclusion:  Prenatal findings of limb abnormalities should be evaluated carefully to detect any associated anomalies which may links to a specific syndrome, thus helping a clinician predict prognosis, and further decide the most beneficial patient management.

Abstract

To describe a rare and atypical case of secondary postpartum hemorrhage (PPH) induced by uterine scar dehiscence in conjunction with an unexplained uterine laceration in the cervico-isthmic region. We report the case of a 29-year-old woman, gravida 2 para 2, who had significant vaginal bleeding and signs of hemorrhagic shock 10 days post-repeated cesarean section. The bleeding was not controlled despite initial resuscitation and medical care. Transabdominal ultrasonography revealed signs of pathology at the previous cesarean scar site. Due to persistent hemodynamic instability, an emergency laparotomy was conducted, exposing active hemorrhage from uterine scar dehiscence, as well as a laceration extending caudally into the cervico-isthmic area, without a clearly identifiable cause. Definitive management required total hysterectomy to achieve hemostasis. This case highlights the importance of consideration of structural causes when medical treatment fails, as early surgical intervention can be life-saving.
