Abstract

 Depression and HIV-related stigma, among other factors, have been inversely linked independently with adherence to antiretroviral therapy (ART) among the youth. However, the processes through which the various factors influence this relationship is not fully known. Guided by Social Action Theory, we examined the interactive mechanisms through which depression, HIV-related stigma, and self-efficacy influenced ART adherence and whether or not these relationships are moderated by gender. A total of 450 HIV-positive youth (13–24 years) in Malawi receiving ART participated in this cross-sectional study. Moderated mediation analyses were conducted using Hayes’ PROCESS macro 2.11 in SPSS. ART adherence was measured by pill count. Findings showed that self-efficacy mediated the effects of depression and stigma on ART adherence. The analyses also revealed that gender moderated both the direct and indirect influence of depression and stigma (via self-efficacy) on ART adherence. Furthermore, self-efficacy simultaneously mediated and moderated the relationship between stigma and ART adherence. The interactive mechanisms through which various factors influence ART nonadherence must be considered to design effective interventions. To reduce the impact of depression and stigma on ART adherence, medication self-efficacy should be bolstered while taking gender in consideration. Keywords: Adolescent, Antiretroviral adherence, Malawi, Moderated mediation, Social action theory, Youth Abstrak Pengaruh Efikasi Diri Terhadap Hubungan antara Depresi dan Stigma HIV dengan Kepatuhan Terapi ART pada Remaja di Malawi. Depresi dan stigma HIV, di antara faktor-faktor lain, berhubungan terbalik secara independen dengan kepatuhan terapi antiretroviral (ART) pada remaja. Akan tetapi, dalam prosesnya faktor yang memengaruhi hubungan ini belum sepenuhnya diketahui. Berdasarkan Teori Perilaku Sosial, penelitian ini dilakukan bertujuan untuk mengkaji mekanisme interaktif depresi, stigma HIV, dan efikasi diri yang memengaruhi kepatuhan ART, dan untuk mengetahui apakah hubungan ini dimoderasi oleh gender atau tidak. Sebanyak 450 remaja dengan HIV-positif (13–24 tahun) di Malawi yang menerima ART ikut berpartisipasi dalam penelitian potong lintang ini. Analisis mediasi moderated dilakukan dengan menggunakan Hayes 'PROCESS macro 2.11 pada SPSS. Kepatuhan ART diukur menggunakan jumlah pil. Hasil penelitian menunjukkan bahwa efikasi diri memediasi efek depresi dan stigma pada kepatuhan ART. Hasil analisis juga mengungkapkan bahwa jenis kelamin memoderasi pengaruh langsung dan tidak langsung dari depresi dan stigma (melalui efikasi diri) terhadap kepatuhan ART. Lebih lanjut,efikasi diri secara bersamaan mediasi dan moderasi hubungan antara stigma dan kepatuhan ART. Mekanisme interaktif dengan berbagai faktor yang memengaruhi ketidakpatuhan ART harus dipertimbangkan untuk merancang intervensi yang efektif. Untuk mengurangi dampak depresi dan stigma terhadap kepatuhan ART, efikasi diri pengobatan harus didukung saat mempertimbangkan jenis kelamin. Kata kunci: Kepatuhan antiretroviral, Malawi, Mediasi tingkat menengah, Teori Perilaku Sosial, Remaja

Abstract

Penelitian ini mengenai karakteristik subjektif tidur pada klien rawat inap dewasa di Rumah Sakit X Depok. Desain penelitian ini deskriptif sederhana dengan pendekatan cross-sectional. Sampel yang terlibat berjumlah 72 pasien dengan teknik pengambilan sampel consecutive sampling. Responden mengisi kuesioner berisi skala tidur Verran dan Snyder-Halpern. Hasil penelitian didapatkan bahwa pasien mengalami gangguan tidur tingkat sedang (47,22%) hingga berat (25%). Efektivitas tidur pasien juga hanya berada pada tingkat sedang (50%) hingga rendah (19,44%). Pasien juga mensuplementasikan tidurnya dengan tingkatan sedang (38,88%) hingga tinggi (22%). Penelitian ini dapat digunakan sebagai penelitian dasar untuk meneliti faktor-faktor yang memengaruhi pola tidur pasien rawat inap. Abstract Sleeping Subjective Characteristics of Adult Patient at X Hospital Depok. This research explored subjective sleep characteristics of hospitalized adult patients in X Hospital Depok. Simple descriptive design was used in this research. Consecutive sampling ­was applied on this study involving 72 patients. Patients were asked to fill the questionnaire about Verran and Snyder Halpern sleep scale (VSH Sleep Scale). Results showed that patients experienced sleep disturbance with moderate levels (47,22%), moreover (25%) patients experienced severe sleep disturbance. Patients felt that sleep effectiveness in moderate levels (50%), moreover (19,44%) patients felt that sleep effectiveness only in low levels. Sleep supplementation also experienced by patients with moderate levels (38,88%), moreover (22%) patients experienced sleep supplementation in high levels. This research can be used as a pilot study to find out factors that affecting how patients sleep in the hospital environment. Keywords: adult patients, hospitalized, subjective sleep characteristics, VSH Sleep Scale

Abstract

Introduction: Self-control and self-effi cacy are necessary for would-be mothers to enhance their birth labor satisfaction. This research was conducted in Ratu Zalecha public hospital in Martapura and in Banjarbaru public hospital in South Borneo with the aim to identify the relationship between self-control and self effi cacy on the one hand, and birth labor satisfaction on the other hand.Methods: Cross-sectional approach with correlative analysis was applied for this research, using 125 samples chosen from normal childbirth using consecutive sampling techniques. Results: The result of Chi-Square correlation test showed that there were some relationships between self-control as well as self-effi cacy and birth labor satisfaction (ρ value 0.002,α: 0.05) and (ρ value 0.000, α: 0.05). There were four variables resulted from double logistic analytical regression that play signifi cant roles in birth labor satisfaction, i.e., self-control, self-effi cacy, family income, and parity.Conclusion: The most infl uencing variable in birth labor satisfaction was family income. Labor carers should provide enough support so that would-be mothers can view the process of childbirth as a positive and pleasant experience; hence, birth labor satisfaction will surely be tremendous.Keywords: birth labor satisfaction, self-control, self-effi cacy

Abstract

Intoduction: Hospitalized neonates may experience pain caused by invasive procedures. Sucrose and non-nutritive sucking are non-pharmacological analgesics. The aimed of this study was to examine the effectiveness of sucrose and non-nutritive sucking administration on pain and crying duration of neonates during invasive procedures.Method: The study used quasi-experimental design with post-test only control group design approach. This study used nonprobability sampling technic with consecutive sampling. The sample consisted of 45 neonates, divided into three groups, each group of 15 neonates.Result: The results showed that the pain response and the crying duration were insignificantly different between the sucrose group and the NNS, respectively p = 0.635 and p = 0.848.Discussion: Age was identified as a confounding variable that effected pain responses. Provision of sucrose and NNS proven to reduce pain as non-pharmacological pain management for neonates during invasive procedures.

Abstract

Background: The increase in COVID-19 cases in Indonesia has resulted in changes in the hospital workflow, including the staffing process and scheduling, especially in the isolation units. Nurse managers are working hard in the scheduling system to ensure high-quality care is provided with the best human resources. Objective: This study aimed to explore the experiences of nurse managers in managing staff nurses’ work schedules during the COVID-19 pandemic. Methods: A qualitative descriptive design was used in this study. Eleven nurse managers from three COVID-19 referral hospitals were selected using purposive sampling. Data were collected using online semi-structured interviews. Thematic analysis was used for data analysis, and data were presented using a thematic tree. Consolidated criteria for reporting qualitative research (COREQ) checklist was used as a reporting guideline of the study. Results: Four themes were developed: (i) Nurse shortage, (ii) Strategically looking for ways to fulfill the workforce, (iii) Change of shift schedule, and (iv) Expecting guidance from superiors and compliance from staff. Conclusion: The lack of nurse staff is a problem during a pandemic. Thus, managing personnel effectively, mobilizing and rotating, and recruiting volunteers are strategies to fulfill the workforce during the pandemic. Using a sedentary shift pattern and sufficient holidays could prevent nurses from falling ill and increase compliance with scheduling. In addition, a staffing calculation formula is needed, and top nursing managers are suggested to provide guidance or direction to the head nurses to reduce confusion in managing the work schedule during the pandemic.

Abstract

This study aims to identify variables that are related to job satisfaction during the COVID-19 pandemic. The research design used descriptive correlation with a cross-sectional approach. The method used is a purposive sampling technique with a sample of 185 nurses who work in the inpatient and outpatient rooms of Hospital X. Data collection uses a questionnaire in the form of google form. The results showed that there was no significant relationship between the characteristics of gender, education, marital status, work unit and career path, work experience and job satisfaction of nurses. However, the variables of age, head coaching competence and motivation have a significant relationship with nurse job satisfaction. As for the multivariate results, the constant value obtained is -0.5841, with the variables that influence coaching and motivation. In conclusion, the job satisfaction of implementing nurses during the COVID-19 pandemic is still low and the variables related to job satisfaction of nurses during the COVID-19 pandemic are coaching and motivation. Abstract

Background: The non-optimal implementation of patient discharge planning is still an important problem in reforming the care system. This non-optimality will have an impact on increasing the length of stay, high patient recurrence rates, and decreasing levels of patient satisfaction and loyalty.Purpose: to explore in-depth experiences of nurses related to nurses' perceptions, support and obstacles in carrying out patient discharge planning.Methods: This research is a qualitative research with a phenomenological approach. Participants in this study were the implementing nurses and team leaders who worked in inpatient rooms. Recruitment of participants is carried out by providing electronic recruitment leaflets to the head of the room. Data analysis used the thematic analysis of the Collaizi method which consisted of six stages, namely preparation of transcripts, coding, categories, themes, interpretation and validation.Results: The results of this study contained 6 (six) themes, namely: 1) The purpose of the patient discharge action plan; 2) The process of carrying out discharge planning at the hospital; 3) SOP for implementation of discharge planning is carried out from the beginning of the patient's treatment; 4) Factors influencing discharge planning; 5) Nurse's motivation in carrying out discharge planning; 6) Obstacles for nurses related to planning to go home which is often overlooked due to the high mobility of nurses and not having time to write.Conclusion: The most important continuity of nursing care activities, one of which is the implementation of discharge planning in accordance with the SOP by involving all health workers and documented from the time the patient enters the treatment room until the patient is planned to go home.Keywords: Patient Discharge Planning, Nursing Care Management, Military Hospital, Qualitative

Abstract

Background: The risk management program will run well with commitment and good management. This study aims to describe the experience of nurse managers in implementing risk management in hospitals. Research methodology: this study used a descriptive qualitative approach. The data collection technique used Focus Group Interviews with 12 nurse managers consisting of 1 Sub Installation Head, 7 Unit Heads, and 4 Team Leaders. Interviews were carried out three times, and each implementation consisted of four participants. Analysis using thematic content and consolidation for reporting qualitative research (COREQ) was used as a guide to research reporting. The research found three inhibiting factors in implementing risk management: 1) Busyness, 2) Lack of knowledge, and 3) Lack of structured monitoring and evaluation. Conclusion: The active participation of nurse managers in implementing risk management is very influential in the successful implementation of risk management in hospitals. Good time management, increasing the level of knowledge, and supported by a systematic risk management monitoring-evaluation system means that risk management will be carried out well

Abstract

Perawat sebagai tenaga kesehatan yang paling terdepan dalam penanganan covid-19 berisiko tinggi tertular penyakit. Posisi ini membuat banyak perawat yang mengalami masalah kesehatan dan terpapar bahaya di tempat kerja. Tujuan penelitian ini adalah menganalisis keterjaminan kesehatan dan keselamatan diri perawat pasca pandemi melalui kajian policy brief. Metode penelitian ini menggunakan desain literatur review. Pendekatan menggunakan PEO. Literatur dikumpulkan dari beberapa peraturan di Indonesia dan hasil penelitian lainnya yang mengulas tentang keterjaminan kesehatan dan keselamatan diri perawat selama dan pasca pandemi. Hasil temuannya menyebutkan bahwa regulasi terkait kesehatan dan keselamatan kerja sudah tertera tetapi masih dirasa kurang tegas. Banyak pengelola fasilitas pelayanan kesehatan tidak melaksanakan kegiatan yang telah diamanahkan oleh peraturan. Disisi lain fokus penanganan masih kepada fisik belum sampai kewajiban berkala untuk mengatasi masalah psikologis perawat. Hal ini juga diperparah belum ada standar yang dibuat oleh organisasi profesi keperawatan untuk memperkuat pentingnya kepedulian pengelola untuk menjamin kesehatan dan keselamatan diri perawat. Kesimpulan dari kajian ini adalah belum optimalnya keterjaminan kesehatan dan keselamatan diri perawat selama dan pasca pandemi. Rekomendasi menjadi pertimbangan bagi pemangku kebijakan dalam menjamin kesehatan dan keselamatan diri perawat sehingga pelaksanaan asuhan keperawatan di fasilitas pelayanan kesehatan tetap berjalan dengan optimal

Abstract

This study aims to optimize the communication of the SBAR method in nurse handover that encourages a culture of patient safety and effective communication. The method used is a pilot study starting from the process of identification, analysis, prioritization of problems, plan of action, implementation, evaluation, and follow-up plan. The results showed that the application of guidelines and Standard Operating Procedures (SPO) with socialization and handover assistance with SBAR method communication had a good effect, as evidenced by the trial application of the guidelines obtained data there was an increase in the percentage of vital sign mentions of 100%, the results of the latest nursing assessment increased to 40%, the delivery of nursing diagnoses and recommendations for further nursing actions by 60%. The hospital organization is in a stage of movement change where after implementation, the Nursing Services Department and the head of the room try to maintain its sustainability and make the organizational culture in the hospital. In conclusion, the activities that have been carried out during the socialization and mentoring process provide a change in behavior and a good mindset for nurses in implementing and cultivating the implementation of handover with SBAR communication in order to realize quality nursing services.
