Abstract

Background: Climate change has the potential to significantly impact public health. Primary health facilities must be prepared to provide effective health services to address the health impacts of climate change. Purpose: To determine the preparedness of community health centers in facing climate change that has an impact on health. Method: The approach used is Preferred Reporting Items for Systematic Review and Meta-analyses (PRISMA). The search format using Patient, Intervention, Comparison, Outcome (PICO) was formulated to facilitate the process of searching literature from databases. The databases used are ScienceDirect, Sage Journal and Scopus from 2020-2024. Researchers use the Mendeley application to detect duplication. Results: The final search results obtained 10 articles, from these articles were obtained preparedness efforts that can be carried out by health centers through routine training, building knowledge, attitudes, and practices of health workers, and building positive perceptions about preparedness. Health centers can respond to the impacts of climate change more effectively with the support of a structured framework. Conclusion: Increasingly extreme climate change has a significant impact on public health, thus requiring better preparedness from health facilities such as community health centers.   Keywords: Climate Change; Health Centers; Preparedness.   Pendahuluan: Perubahan iklim berpotensi m.emberi dampak besar bagi kesehatan masyarakat. Fasilitas kesehatan primer dituntut memiliki kesiapsiagaan untuk memberi pelayanan kesehatan yang efektif dalam merespon dampak kesehatan akibat perubahan iklim. Tujuan: Untuk mengetahui kesiapsiagaan puskesmas dalam menghadapi perubahan iklim yang berdampak pada kesehatan. Metode: Pendekatan yang digunakan adalah Preferred Reporting Items for Systematic Review and Meta-analyses (PRISMA). Format pencarian dengan menggunakan Patient, Intervention, Comparison, Outcome (PICO) dirumuskan untuk memudahkan dalam proses pencarian literatur dari database. Database yang digunakan adalah ScienceDirect, Sage Journal dan Scopus dari tahun 2020-2024.  Peneliti menggunakan aplikasi Mendeley untuk mendeteksi Duplikasi. Hasil: Hasil akhir pencarian didapatkan 10 artikel, dari artikel tersebut didapatkan upaya kesiapsiagaan yang dapat dilakukan oleh puskesmas melalui pelatihan rutin, membangun pengetahuan, sikap, dan praktik tenaga kesehatan, serta membangun persepsi positif tentang kesiapsiagaan. Puskesmas dapat merespons dampak perubahan iklim secara lebih efektif dengan dukungan kerangka kerja yang terstruktur. Simpulan: Perubahan iklim yang semakin ekstrem menimbulkan dampak signifikan terhadap kesehatan masyarakat, sehingga menuntut kesiapsiagaan yang lebih baik dari fasilitas kesehatan seperti puskesmas.   Kata Kunci: Kesiapsiagaan; Perubahan Iklim; Puskesmas.

Abstract

Introduction: Cancer patient care is a complex and ongoing process. The impact of cancer diagnosis and treatment on sexual quality is an important issue in cancer quality of life. Sexual counselling is one of the interventions chosen in handling sexual problems in cancer patients. This review aims to examine the effectiveness of sexual counselling on the sexuality of cancer patients. Methods: Literature searches were conducted through several databases, including PubMed, Scopus, ProQuest, Science Direct, Oxford, ClinicalKey Nursing, EBSCOhost, Springer Link, Taylor & Francis, using predetermined inclusion and exclusion criteria. The selected articles were analyzed based on the intervention method. A total of 15 14,287 articles were extracted and synthesized. Results: 15 intervention articles were obtained showing that sexual counselling effectively affects the sexuality of cancer patients and can be carried out for patients and their partners face-to-face, by telephone, by internet, or via WhatsApp. Conclusion: Sexual counselling programs for cancer patients are effective interventions for sexual problems in cancer patients.

Abstract

Background: Cancer is a major disease that significantly affects global public health. In Indonesia, the number of cancer patients continues to increase, requiring innovative approaches in the cancer care system.Objective: This study aims to evaluate the effectiveness of cancer patient navigator programs in reducing caregiver burden, particularly among cancer patients.Methods: This systematic review followed the PRISMA guidelines and used inclusion criteria that included full-text articles published in English and quantitative studies. Literature searches were conducted in various academic databases using relevant keywords.Results: Out of 3,583 records retrieved, 6 studies met the criteria for further analysis. The results also showed that patient navigator programs can significantly reduce caregiver burden through improved education, emotional support, and access to healthcare services.Conclusion: The implementation of cancer patient navigator programs in Indonesia has the potential to reduce caregiver burden and improve the quality of cancer patient care. Recommendations for further research and health policy are also included.

Abstract

Background: Nurse burnout can reduce performance and quality of service. Nurses who experience burnout syndrome will lose motivation for work due to prolonged responses to emotional, physical, and mental exhaustion. Purpose: To analyze the effectiveness of mindfulness in reducing burnout in implementing nurses. Method: Systematic review using protocols and rules in accordance with the Preferred Reporting Items for Systematic Review (PRISMA), using databases from Scopus, Proquest, Pubmed, SAGE, and Science Direct in 2019-2024, found 220 articles and 8 selected articles. Results: Mindfulness training reduces fatigue levels, produces lower scores for emotional exhaustion and depersonalization, and is higher for personal achievement. Conclusion: Mindfulness in implementing nurses is an effective effort to choose what nurses will think and maintain the attention needed to maintain awareness of current experiences or in providing nursing care, thereby helping implementing nurses reduce stress due to burnout in the long term. Suggestions: This review has the potential to be biased due to the lack of studies and high heterogeneity. Practical mindfulness techniques for nurses still need further research and should be guided by professionals to be effective.   Keywords: Burnout Syndrome; Mindfulness; Implementing Nurses.   Pendahuluan: Burnout perawat dapat menurunkan kinerja dan mutu layanan. Perawat yang mengalami sindrom burnout akan kehilangan motivasi terhadap pekerjaan karena respon yang berkepanjangan terhadap kelelahan emosional, fisik, dan mental. Tujuan: Untuk menganalisis efektifitas mindfullness dalam menurunkan burnout perawat pelaksana. Metode: Sistematik review menggunakan protokol dan kaidah yang sesuai dengan Preferred Reporting Items for Systematic Review (PRISMA). Pencarian literatur berasal dari database, Scopus, Proquest, Pubmed, SAGE, dan Sciencedirect tahun 2019-2024, ditemukan 227 artikel dan sebanyak 8 artikel terpilih. Hasil: Pelatihan mindfulness mengurangi tingkat kelelahan, menghasilkan skor yang lebih rendah untuk kelelahan emosional dan depersonalisasi, serta lebih tinggi untuk pencapaian pribadi. Simpulan: Mindfulness pada perawat pelaksana merupakan upaya yang efektif untuk memilih apa yang akan perawat pikirkan dan mempertahankan atensi yang dibutuhkan untuk menjaga kesadaran terhadap pengalaman saat ini atau dalam melakukan asuhan keperawatan, sehingga membantu perawat pelaksana menurunkan stress akibat burnout dalam jangka waktu yang lama. Saran: Tinjauan ini berpotensi bias karena minimnya studi dan tingginya heterogenitas. Teknik mindfulness yang praktis bagi perawat masih perlu diteliti lebih lanjut dan sebaiknya dipandu profesional agar efektif.   Kata kunci: Burnout Syndrome; Mindfulness; Perawat Pelaksana.

Abstract

Background: The handover process in nursing care is very important because it is directly related to patient safety and continuity of care. A suboptimal handover can increase the risk of medical errors. Purpose: To develop handover guidelines in the inpatient ward of the University of Indonesia Hospital. Method: A case report study on the preparation of handover guidelines was conducted at the University of Indonesia Hospital on September 2 - October 2, 2024. This activity includes assessment, data analysis, action plan (Plan of Action/PoA), implementation, and evaluation. Data collection was carried out through interviews, observations, and questionnaires. The approach used to solve the problem through Kurt Lewin's theory of change. Results: Shows that the implementation of handover is not optimal with variations in methods used, including conferences and handovers at the bedside. The results of the study indicate that the implementation of the handover SOP has not been consistently adhered to by ward nurses and several factors such as time constraints and lack of supervision have a negative effect on the effectiveness of the handover. Conclusion: The implementation of handovers in inpatient rooms has not been optimal due to inconsistencies in the application of SOPs, time constraints, and lack of supervision. The development of handover guidelines with the Kurt Lewin change theory approach is a strategic step to improve the effectiveness and consistency of handovers. Wider dissemination of guidelines and regular monitoring are needed to ensure patient safety and the quality of sustainable nursing care. Suggestion: Handover guidelines need to be updated regularly based on scientific evidence (evidence-based practice) to remain relevant to developments in nursing science. Socialization and implementation activities of handover conferences and handovers at the patient's bedside need to be monitored and evaluated routinely to ensure their effectiveness in improving the quality of patient handovers.   Keywords: Handover; Guidelines; Development; Inpatient; Hospital.   Pendahuluan: Proses serah terima atau handover dalam asuhan keperawatan sangat penting karena secara langsung berhubungan dengan keselamatan pasien dan kontinuitas perawatan. Handover yang tidak optimal dapat meningkatkan risiko kesalahan medis. Tujuan:  Untuk mengembangkan pedoman handover di ruang rawat inap rumah sakit Universitas Indonesia. Metode: Penelitian case report mengenai pengembangan panduan handover, dilakukan di Rumah Sakit Universitas Indonesia pada 2 September – 2 Oktober 2024. Kegiatan ini meliputi pengkajian, analisis data, rencana tindakan plan of action (PoA), implementasi, dan evaluasi. Data dikumpulkan melalui wawancara, observasi, dan pengisian kuesioner. Pendekatan yang digunakan untuk menyelesaikan masalah melalui teori perubahan Kurt Lewin. Hasil: Menunjukkan bahwa pelaksanaan handover belum optimal dengan variasi metode yang digunakan, termasuk conference dan bedside handover. Hasil menunjukan bahwa pelaksanaan SPO handover tidak konsisten dipatuhi perawat ruangan dan beberapa faktor, seperti keterbatasan waktu dan kurangnya supervisi berpengaruh negatif terhadap efektivitas handover. Simpulan: Pelaksanaan handover di ruang rawat inap belum berjalan secara optimal akibat ketidakkonsistenan dalam penerapan SPO, keterbatasan waktu, dan kurangnya supervisi. Pengembangan panduan handover dengan pendekatan teori perubahan Kurt Lewin menjadi langkah strategis untuk meningkatkan efektivitas dan konsistensi handover. Sosialisasi panduan yang lebih luas dan monitoring berkala diperlukan untuk menjamin keselamatan pasien dan kualitas asuhan keperawatan yang berkelanjutan. Saran: Panduan handover perlu diperbarui secara berkala berdasarkan bukti ilmiah (evidence-based practice) agar tetap relevan dengan perkembangan ilmu keperawatan. Kegiatan sosialisasi dan implementasi handover conference serta bedside handover perlu dimonitoring dan dievaluasi secara rutin untuk memastikan efektivitasnya dalam meningkatkan kualitas serah terima pasien.   Kata Kunci: Handover; Pedoman; Pengembangan; Rawat Inap; Rumah Sakit.

Abstract

The Emergency Department is a high-intensity clinical setting that manages critically ill patients who are highly susceptible to sudden clinical deterioration. Early detection through the implementation of the Early Warning System plays a crucial role in preventing unexpected adverse events, with nurses serving as the primary implementers of this system. This review aims to evaluate current evidence on the implementation of the Early Warning System by nurses in the Emergency Department and its impact on the prevention of unexpected adverse events. This study employed a systematic review approach by applying the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines during the article selection process and using the 2023 edition of the Joanna Briggs Institute (JBI) Critical Appraisal Tool to assess the quality of included studies. Search for articles through the ScienceDirect, ProQuest, PubMed, BMJ Journal, and EBSCO databases from 2020 to 2025. and 10 articles were analyzed. The majority of studies reported that the use of the Early Warning System consistently enhances the early detection of patient deterioration, accelerates clinical response, improves triage accuracy, and assists in predicting the need for advanced care such as admission to intensive care units. Implementation by nurses was also associated with improved compliance in vital signs monitoring and a reduction in patient mortality.

Abstract

Outpatient nursing services are an important element for hospitals to produce many patients, particularly inpatients. Outpatient nursing services are currently still not optimal. Nurses are responsible for providing good service to increase patient satisfaction. Aim: This case report aims to describe the implementation of planned changes to optimize the outpatient nursing service planning function. Method: The method used is a case report. Data collection uses interviews, questionnaires, and field observations. The results of data exploration are then analyzed using SWOT (strengths, weaknesses, opportunities, threats) and fishbone diagrams, then formulating a plan of action (PoA), followed by implementation and evaluation. Results: The results of the study showed that the priority problem in nursing services was that the implementation of the planning function was not optimal. Making outpatient nursing service guidelines is a strategy for solving problems in developing outpatient nursing services. Implementation of problem-solving uses a theory of change approach by the stages of Kotter's eight-step plan. Recommendation: Kotter's eight-step plan theory of change approach can be continued by hospital management as a form of initiation of service development.

Abstract

The success of health organizations depends on the quality of human resources including nursing resources. Health organizations can grow and develop if they have staff or nurses with demonstrated high performance. Good performance is directly proportional to the quality of services provided. Performance appraisal is very important to evaluate, assess and measure, to enhance employee capabilities and professionalism. This assessment aims to ensure that organizational goals can be achieved optimally. Furthermore, strategic planning and the role of managers are essential in performance appraisal. With good strategic planning, supported by the optimal role of managers, can enhance staff performance and facilitate the achievement of organizational objectives. Managers who have good abilities in planning, organizing, staffing, actuating and controlling the vision, mission and goals of the organization can be achieved. Objective: This study aims to determine the relationship strategic planning and the role of managers on the performance of nursing resources. Methods : This systematic review was prepared based on Preferred Reporting Items for Systematic Reviews and Meta-Analyzes (PRISMA). Used databases Scopus, ProQuest, Science Direct, Pubmed, Clinicalkey and Google Scholar. Articles used from 2023-2025. Keywords Strategic Planning AND Manager Roles AND Performance Appraisal for Nurses. Joanna Briggs Institute (JBI) and Critical Appraisal Skills Program (CASP) analysis. From 78,200 articles found, 8 articles were analyzed. The results of systematic analysis in the study showed that with appropriate strategic planning and good manager roles can improve nurse performance which has an impact on improving the quality of service and the goals and vision of the organization's mission can be achieved. The results of the study can be concluded that strategic planning and the role of managers have a significant effect on improving performance so that organizational goals can be achieved optimally.

Abstract

Medication errors have been a persistent issue in healthcare, often overshadowing nurses' ability to focus on patient care. Traditionally, nurses have devoted considerable time to calculating medications—dosages, routes, and frequencies—at the expense of other caregiving duties. This literature review explores the adoption of electronic-based systems to reduce medication errors and associated risks in healthcare settings. The research involved examining databases such as Clinical-key-nursing, Science Direct, ProQuest, Embase, and academic articles, using keywords like "medication error" and "electronic-based system." The findings suggest that electronic systems can mitigate harms linked to medication errors by ensuring compliance with the "10 rights of medication administration," which include the right patient, drug, dose, time, route, documentation, assessment, approach, drug interaction, and information. The review analyzed 19 articles, but only ten met the inclusion criteria. The results indicate a positive correlation between the use of electronic systems and a decrease in medication errors, highlighting the potential benefits of technology in enhancing patient safety and care efficiency.

Abstract

Culturally congruent care practices are essential for providing holistic and meaningful care, particularly for multicultural populations and the end-of-life patients. However, national policies have not been fully implemented within hospital internal policies. This study aimed to develop a culturally congruent care practices guideline and a nursing assessment form for the end-of-life patient. A case study employing the Eight-Step Kotter Change Model was used to develop, implement, and evaluate the guideline and form at Hospital X Jakarta. Socialization was conducted with ICU nursing staff and nurse managers. Nineteen out of forty-one study participants were purposively selected to participate in the evaluation. The results demonstrated that the development of the guideline and form successfully improved nurses’ knowledge related to culturally sensitive nursing, with 80% of the evaluation questions showing increased scores. Subsequently, the guideline and form were integrated into hospital policies and electronic medical records to ensure sustainability. A systematic approach using Kotter’s Model effectively facilitated the development of culturally congruent care practices. Leadership support and ongoing competency development are necessary to sustain these changes and enhance the quality of care.
