Abstract

Pharmaceutical services are direct and responsible services to patients related to pharmacy to achieve real results and improve patients' quality of life. This study aims to analyze the improvement of pharmaceutical services at the Kimia Farma 36 Ijen Pharmacy using the IPA (Importance Performance Analysis) method. The method used is descriptive research with a quantitative approach through questionnaire instruments. The SERVQUAL (Service Quality) model was used to measure the pharmaceutical service questionnaire. Sampling by the Accidental sampling technique. Questionnaires were distributed to patients or families of patients who redeem prescriptions at the Pharmacy in February 2023. The questionnaire refers to the research of Parasuraman, A., Zeithaml, V. A., & Berry adapted to the research context in the pharmacy. The dimensions are empathy, reliability, responsiveness, assurance, and tangibility, which are divided into importance and performance. The IPA method will analyze performance as the X axis and Importance as the Y. The results show that the service aspects require urgent improvement to improve service quality and patient satisfaction. Aspects that need to be improved include product completeness, the physical appearance of the pharmacy building, cleanliness, comfort, and neatness of the pharmacy, as well as the appearance and service of pharmacy officers. Overall, the results of this analysis provide clear guidance on the priorities for improvements that need to be made at Pharmacy Kimia Farma 36 Ijen.

Abstract

Diabetes melitus tipe 2 merupakan penyakit kronis yang memerlukan pengendalian optimal untuk mencegah komplikasi. Keberhasilan terapi dipengaruhi oleh berbagai faktor, termasuk kepatuhan, manajemen diri, dan karakteristik pasien. Penelitian ini bertujuan menganalisis determinan outcome terapi pada pasien DM tipe 2 di Poli Rawat Jalan RS X Kota Blitar. Penelitian menggunakan desain analitik cross-sectional pada 83 responden yang dipilih secara purposive sampling. Tingkat kepatuhan diukur dengan Medication Adherence Report Scale (MARS-5) dan manajemen diri dengan Diabetes Self-Management Questionnaire (DSMQ). Outcome terapi dinilai dari kadar glukosa darah sewaktu (GDS) dan glukosa darah puasa (GDP). Hasil menunjukkan kepatuhan (p=0,001), manajemen diri (p=0,003), dan lama menderita DM (p=0,048) berhubungan signifikan dengan outcome terapi, sedangkan karakteristik seperti jenis kelamin, usia, pendidikan, dan pekerjaan tidak berhubungan secara signifikan. Analisis regresi logistik mengidentifikasi kepatuhan sebagai determinan utama (OR=3,280; 95% CI: 1,085–9,913). Dapat disimpulkan bahwa peningkatan kepatuhan pengobatan dan manajemen diri berperan penting dalam keberhasilan terapi pada pasien DM tipe 2.

Abstract

Penyakit tuberkulosis (TB) adalah infeksi menular yang terutama mempengaruhi organ paru-paru dan disebabkan oleh bakteri Mycrobacterium tuberculosis yang ditularkan melaui perantara ludah atau dahak pada saat batuk. Keberhasilan terapi TB tidak hanya bergantung pada ketepatan pemberian obat, tetapi juga pada kepatuhan pasien selama terapi dan rasionalitas. Tujuan penelitian adalah untuk mengetahui rasionalitas pengobatan pasien TB dengan terapi obat antituberkulosis (OAT) di Rumah Sakit X Kabupaten Kediri. Metode yang digunakan dalam penelitian ini adalah observasional dengan deskriptif retrospektif berdasarkan rekam medis pasien rawat inap TB. Teknik pengambilan sampel secara total sampling sehingga diperoleh sebanyak 81 pasien. Hasil dianalisa secara deskriptif univariat yang disajikan secara distribusi frekuensi serta persentase. Hasil penelitian menunjukkan rasionalitas penggunaan OAT pada pasien TB diperoleh hasil yaitu tepat indikasi 81 pasien (100%), tepat pasien 81 pasien (100%), tepat pemilihan obat 81 pasien (100%), tepat dosis 81 pasien (100%), tepat cara pemberian 81 pasien (100%), dan tepat interval waktu 81 pasien (100%). Hal ini menunjukkan bahwa penggunaan OAT di Rumah Sakit X Kabupaten Kediri telah dilaksanakan secara rasional dan sesuai pedoman pengobatan TB.

Abstract

Diabetes mellitus is an endocrine disease characterized by hyperglycemia, characterized by elevated blood glucose levels. The cause of elevated blood glucose levels is due to ineffective insulin secretion, insulin action, or both. The purpose of this study was to determine the relationship between adherence to oral antihyperglycemic medication and fasting blood sugar levels in patients with type 2 diabetes mellitus. This study employed a non-experimental, descriptive cross-sectional design. This study was conducted at the Balerejo Community Health Center in Madiun in February 2024 with a total sample of 63 patients. The sampling technique used was the consecutive sampling method. The data analysis used in this study was the Spearman Rank test. The results of the most gender characteristics were female 37 people (58.7%), age 46-55 years 35 people (55.6%), high school education 29 people (46.0%), farmer occupation 11 people (17.5%), normal BMI (18.5-22.9 kg / m2) 25 people (39.7%), metformin monotherapy treatment 11 people (17.5%), metformin + glibenclamide combination 24 people (38.1%). Compliance with taking medication was categorized as compliant in 39 people (61.9%), and non-compliant in 24 people (38.1%). Fasting blood sugar (FBS) targets were achieved by as many as 36 people (57.1%), and not achieved by 27 people (42.9%). The results of the analysis showed significant relationship between compliance with oral antihyperglycemic drugs and fasting blood sugar levels in patients with type 2 diabetes mellitus, with a p-value of 0.000 and a correlation coefficient of 0.510. Conclusion: There is a significant relationship exists between compliance with oral antihyperglycemic drugs and fasting blood sugar levels in type 2 diabetes mellitus patients.

Abstract

Diabetes mellitus is a chronic metabolic disease characterized by elevated blood glucose levels, which can cause serious damage to the heart, blood vessels, kidneys, eyes, and nerves. This study aimed to determine the relationship between diabetes self-management education and blood glucose levels in patients with type 2 diabetes mellitus at the Balerejo Public Health Center. This study used an observational analytical study with a cross-sectional design using consecutive sampling. Data were collected prospectively and analyzed using Spearman's Rank Test on 50 patients. The results of the analysis of the diabetes self-management education questionnaire, in the domain of diet with a good category of 32 people (64%), physical exercise 8 people (16%), foot care 34 people (68%), drug therapy 34 people (68%), and glucose level monitoring 17 people (34%). The blood glucose level values ​​​​in the normal range are GDP 21 people (42%), GD2PP 27 people (54%), HbA1c 20 people (40%), and lipid profile values ​​​​of total cholesterol 27 people (54%), LDL 28 people (56%), HDL 29 people (58%), triglycerides 27 people (54%). The results of the Spearman Rank analysis showed that there was a relationship between diabetes self-management education with GDP p-value 0.022; r 0.32, G2PP p-value 0.004; r 0.4, HbA1c p-value 0.036; r 0.3, total cholesterol p-value 0.027; r 0.31, LDL p-value 0.015; r 0.34, HDL p-value 0.008; r 0.37, triglyceride p-value 0.004; r 0.4. Conclusion: There is a significant relationship between diabetes self-management education and blood glucose levels as well as lipid profiles.
