Abstract

The World Health Organization (WHO) 2021 stated that tuberculosis is still one of the main world health problems where tuberculosis is the 13th leading cause of death in the world and in 2020 tuberculosis became the infectious disease that caused the most deaths in the world after COVID-19. The National Strategy for Tuberculosis Prevention in Indonesia 2020-2024 explained that the scope of finding tuberculosis patients has not reached the target. Based on the Health Profile of the Salak Health Center in 2021, the achievement of the Pulmonary Tuberculosis program (Incidince Rate) was 21 suspected patients or 10.6% of the target of 198 suspected patients. The purpose of the study was to analyze the low number of passive and active case finding, and analyze community empowerment towards the implementation of the Pulmonary Tuberculosis Program in the Working Area of the Sungai Salak Community Health Center. This type of research is qualitative, descriptive, with a case study design. The research location is in the Working Area of the Sungai Salak community health center, Indragiri Hilir Regency. The research period is January to August 2023. In-depth interview research informants there were 5 people consisting of 3 main informants (tuberculosis Program Holders at Puskesmas, Analysts, Doctors) and 2 Supporting In-Informants (Head of Puskesmas and Tuberculosis Program Holders at the Health Office), while the Focus Group Discussion informants there were 6 people consisting of village heads, RT leaders, RW leaders, community leaders, PMOs, tuberculosis cadres. The form and flow of passive examination of tuberculosis patients are in accordance with the SOP. The obstacle is that sometimes patients who come for treatment to the puskesmas find it difficult to remove their sputum at that time. Active case discovery has not been in accordance with Standard Operating Procedures (Screening and Household Contact of tuberculosis patients) because nothing has been done. It is expected that the role of tuberculosis officers can ensure that tuberculosis cases are reported regularly through the tuberculosis program information system.Keywords : Community Empowerment, Passive Case Discovery, Active Case Discovery, Sungai Salak Community Health Center, and Tuberculosis Program

Abstract

Up Respiratory Infection (URI) is one of the leading causes of death in children under five years of age in the world. The incidence of ARI in Rokan Hulu Regency is in the first place with the number of 4118 cases in the 2019 period. The highest ARI cases were found in Ujung Batu Health Center, Rokan Hulu Regency, compared to Puskesmas Kunto the incidence of ISPA reached 211 cases. The research objective was to determine the factors that influence the incidence of upper respiratory tract infections at the Ujung Batu Health Center, Rokan Hulu Regency. The research method used quantitative type and cross sectional design. The population in this study were all households in the Ujung Batu Health Center, with a sample of 157 people. The sampling technique used simple random sampling. Using primary data, with observation sheet instruments and questionnaires. Data analysis used univariate, bivariate and multivariate analysis. The results showed that there was an effect of the wall effect on the incidence of ARI (pvalue = 0.017), and there was an effect of ventilation with the incidence of ARI (pvalue = 0.000 <0.05). In addition, there is no effect of road facilities, cooking fuel and house floors with the incidence of ARI at the Ujung Batu Health Center, Rokan Hulu Regency. It is hoped that the puskesmas will be able to enter with the local community in disseminating the conditions for a healthy house on a regular basis, improving the condition of the walls and ventilation of the house which are still not in accordance with the standards of a healthy house. Bibliography: 35 References (2009-2015)Keywords: Up Respiratory Infection, factors

Abstract

Workload is an important concern for the nursing profession which will have an impact on the performance of nursing staff in providing nursing services. This study aims to determine the results of measuring workload and performance of nursing staff in the Outpatient Installation of Dumai City Regional Hospital. The research method was quantitative with a cross-sectional study design. The research was conducted at the Dumai City Hospital Outpatient Installation from June-July 2023. The population of this study was 33 people, the sample used was 33 respondents taken using the total sampling technique. Workload measurement is carried out using the work sampling method through observation. Performance is obtained using secondary data. The data analysis used was univariate analysis and bivariate analysis with Pearson correlation, chi-square and linear regression on work sampling variables only. The research results showed that the calculation of heavy workload in orthopedic and neurosurgery clinics was 93.75%. With direct productive activities in wound care (19.23%) and indirect productive activities in completing the SPPK (Proof of Health Services) (13.01%). Nursing staff with a heavy workload was 27.28%, and a light workload was 72.72%. Nursing staff with good performance was 75.8% and very good 24.2%. There is no relationship between workload and the performance of nursing staff (p-value= 1.000, OR=0.750). Research Ethics Pass Letter Number: 145/KEPK/UHTP/VI/2023 dated June 19 2023. Evaluation and restructuring of work systems include reducing unproductive workloads, adjusting working hours, or allocating resources more efficiently and training and competency development of nursing staff.

Abstract

The existence of Aedes aegypti larvae is evidence of the discovery of a population of Aedes aegypti mosquitoes that influence the occurrence of Dengue Hemorrhagic Fever. The larvae-free rate in Labuh Baru Timur Subdistrict was found to be 85%, which shows that it has not reached the national standard above or equal to 95%. The research aimed to determine the factors related to the presence of Aedes aegypti mosquito larvae in Labuh Baru Timur Subdistrict, the working area of ​​the Payung Sekaki Community Health Center, Pekanbaru City in 2023. This research method was quantitative using Observational Analytical with a cross-sectional design. The research was carried out in February - July 2023. The sampling technique was quota sampling of 100 heads of families. The data analysis used is univariate and bivariate. The results of the research showed that there was a relationship between knowledge, attitudes, eradicating mosquito nests, the color of the water reservoir, the location of the water reservoir, and the role of larva monitoring cadres (Jumantik) with the presence of Aedes aegypti mosquito larvae. It is hoped that the Payung Sekaki Community Health Center can provide KIE (Communication, Information, and Education) to the community regarding Dengue Hemorrhagic Fever (DHF), especially how to prevent the presence of Aedes aegypti mosquito larvae and the involvement of Jumantik Cadres is expected to be more active and effective.

Abstract

Abstract : Sidomulyo Public Health Center experienced an increase in Dengue Hemorrhagic Fever (DHF) cases from 2023 to 2024, marked by a low Larvae Free Index (LFI) of only 4.3%, far below the 95% standard. This study aims to determine the relationship between Climate Change Adaptation (APIK) and the presence of Aedes mosquito larvae, which causes DHF, in the Sidomulyo Health Center's working area. This is a quantitative research with a cross-sectional design, conducted in RW.10, Tuah Karya Village, in March 2025. The study population consisted of 850 households, with a sample size of 90 households selected using Simple Random Sampling. Data collection tools included questionnaires and observation sheets, and data were analyzed using univariate and bivariate analysis with the Chi Square test. The results showed that factors related to larvae presence with a P-value ≤ α 0.05 were: humidity (P-Value 0.019; OR 3.889), knowledge (P-Value 0.019; OR 3.714), and the "3M Plus" mosquito prevention efforts (P-Value 0.050; OR 3.047), while the presence of vegetation had no significant correlation. These findings highlight the importance of routine larvae monitoring based on seasonal patterns to accelerate mosquito breeding site eradication. Preventive efforts can include consistent implementation of 3M Plus, healthy and adaptive housing, and active community participation.Keywords: DHF, Mosquito Larvae, Climate Change, Adaptation

Abstract

Dalam rangka menurunkan angka kematian ibu dan angka kematian bayi untuk pencapaian Millennium Development Goals, ditetapkan kebijakan bahwa setiap ibu yang melahirkan, biaya persalinannya ditanggung oleh pemerintah melalui Program Jampersal, namun dalam pelaksanaannya masih ditemukan ketidakpuasan pasien terhadap pelayanan kebidanan. Penelitian ini bertujuan untuk menganalisis pengaruh dimensi mutu pelayanan kebidanan terhadap kepuasan pasien program Jampersal meliputi keandalan bidan, ketanggapan bidan, jaminan bidan, empati dan bukti langsung. Penelitian ini dilakukan di RSUD Rokan Hulu dengan populasi semua pengguna Jampersal yaitu sebanyak 92 orang dan pada penelitian ini seluruh populasi dijadikan sampel. Penelitian ini menggunakan desain potong lintang. Pengumpulan data dilakukan dengan data primer dan data sekunder. Dilakukan analisis data yaitu analisis univariat, analisis bivariat (kai kuadrat) dan analisis multivariat (regresi logistik ganda). Proporsi pasien program Jampersal yang memiliki kepuasan terhadap mutu pelayanan kebidanan sebanyak 67 orang (72,8%) dari 92 sampel. Variabel independen dengan hubungan yang signifikan dengan kepuasan pasien program Jampersal adalah variabel bukti langsung dengan nilai p < 0,05 (0,003). In order to reduce maternal mortality rate and infant mortality rate, in achieving the Millennium Development Goals set policy, that every mother who gave birth, her labor costs borne by the government through assurance of delivery program, but in practice still found dissatisfaction of patients to obstetric care. This study aimed to analyze the influence of the quality dimensions of midwifery care on patient satisfaction assurance of delivery program include reliability midwife, responsiveness midwife, assurance midwife, and direct tangible. This study was conducted in Rokan Hulu District General Hospital with a population assurance of delivery all users and as Dimensi Mutu Pelayanan Kebidanan terhadap Kepuasan Pasien Program Jampersal Quality Dimensions of Midwifery Care on Patient Satisfaction Jampersal (Delivery Assurance) Program Herlina Susmaneli, Ani Triana many as 92 people on the entire study population sampled. This study used a cross sectional design. The data was collected primary data and secondary data. Data analysis is performed univariate, bivariate analysis (chisquare) and multivariate analysis (Multiple Logistic Regression). The proportion of patients who had an assurance of delivery program satisfaction with the quality of obstetric care by 72.8%. Independent variables with significant association with patient satisfaction assurance of delivery is a concrete manifestation of variables with p value < 0.05 (0.003).

Abstract

Pharmaceutical service is a direct and responsible service to patients related to pharmaceutical preparations with the intention of achieving definite results to improve the quality of life of patients. The purpose of this study was to obtain in-depth information on drug management in the Pharmacy Unit of RSUD Tengku Sulung. The research method used descriptive qualitative with purposive sampling with the number of informants 7 people. Data collection through in-depth interviews, direct observation and document review using content analysis. The results, on the input component obtained information that the lack of human resources, facilities and infrastructure are inadequate and there is no depot and SIM RS. Policies have been made in the form of Drug Management Guidelines in the Pharmacy Unit. In the process component, information is obtained that selection is based on the hospital formulary and national formulary, planning using the combination method, procurement using e-catalog and direct purchase, receipt is carried out by checking and adjusting the order letter, order invoice, and the drug received, storage based on dosage form, classification of types of drugs, medical devices, B3 and arranged alphabetically with the FIFO and FEFO systems, distribution using individual prescribing and Floor Stock systems, deletion is carried out by involving third parties, control is carried out by controlling drug inventory and quality, administration based on recording, reporting and financial administration. The output component obtained by the availability of drugs is still not effective and efficient. In conclusion, the pharmaceutical supply management system in the Pharmacy unit of RSUD Tengku Sulung is not in accordance with the established standards. Suggestions, the need for additional human resources, the fulfillment of infrastructure such as depot procurement and SIMRS, monitoring and evaluation, improved communication during drug selection and budget planning in the fulfillment of facilities and infrastructure.
