Abstract

Indonesia's adolescent pregnancy prevalence necessitates comprehensive determinant identification for evidence-based preventive strategy formulation. This research analyzes correlations between reproductive health knowledge, parental role functioning, and peer influence on teenage pregnancy among female adolescents in Ciseeng Health Center, Bogor Regency, West Java Province. Cross-sectional design employed among 65 adolescents aged 15-19 years during October 2024-January 2025 using stratified random sampling. Data collection utilized validated questionnaires based on WHO Adolescent Sexual and Reproductive Health Indicators, Parental Monitoring and Support Scale, and Peer Pressure Inventory with Cronbach Alpha (α=0.84; 0.81; 0.79). Demographic characteristics showed 60% aged 17-19 years, 40% aged 15-16 years, with 55.4% experienced pregnancy validated through medical records. Univariate analysis identified knowledge deficiency (64.6%), parental dysfunction (63.1%), and high peer influence (66.2%). Chi-Square analysis confirmed associations of low knowledge (p=0.001; OR=3.5; 95% CI=1.8-6.7), inadequate parental roles (p=0.002; OR=3.2; 95% CI=1.6-6.1), and dominant peer influence (p=0.001; OR=3.8; 95% CI=1.9-7.2) with adolescent pregnancy. Multivariate logistic regression identified peer influence as strongest predictor (AOR=3.80; 95% CI=1.67-8.65).

Abstract

Contraceptive decision-making is a complex process influenced by an interplay of psychological characteristics and interpersonal dynamics.  which may contribute to dissatisfaction and discontinuation among users. This study examined the effects of conscientiousness as a personality trait and dyadic communication on the quality of contraceptive decision-making among women of reproductive age. A cross-sectional study design was employed.  involving 186 married women aged 20–50 years who were current contraceptive users.  recruited through purposive sampling. Data were collected using the Big Five Personality Scale (α = 0.82).  the Dyadic Communication Scale (α = 0.89).  and the Decision-Making Scale (α = 0.87). Multiple linear regression analysis demonstrated that conscientiousness (β = 0.563.  p < 0.001) and dyadic communication (β = 0.358.  p < 0.001) were significant predictors of decision-making quality.  collectively accounting for 79.2% of the variance (R² = 0.792). In addition.  significant differences in decision-making quality were identified across age groups.  educational levels.  contraceptive methods.  and employment status. These findings underscore the importance of integrating psychological traits and partner communication into contraceptive counseling strategies to improve informed and satisfactory decision-making among women of reproductive age.

Abstract

Preeclampsia constitutes an obstetric complication with 5.3% prevalence in Indonesia, significantly contributing to maternal mortality rates. Cognitive deficiency among pregnant women regarding preeclampsia risk factors represents a fundamental determinant in primary preventive strategies. This research aimed to optimize awareness and comprehensive knowledge of gravidas concerning preeclampsia through implementation of structured health education programs. Research methodology employed participatory action research involving 13 gravida respondents at Posyandu Dahlia XI, Karawang Regency. Comprehensive educational interventions encompassed interactive counseling, applicative demonstrations, and educational media development. Pre-post test evaluation demonstrated significant escalation of optimal knowledge categories from 38.46% to 84.61% of respondents post-intervention. The program successfully generated behavioral transformation, establishment of sustainable peer support mechanisms, and formation of collective community awareness. Implementation of participatory health education proved effective in cognitive enhancement and gravida awareness toward preeclampsia preventive strategies, resulting in sustainable behavioral modification within maternal communities. This intervention model demonstrates significant potential for replication in similar healthcare settings

Abstract

Background: Workplace stress during pregnancy contributes to adverse maternal-fetal outcomes, including preterm birth, low birth weight, and pregnancy-induced hypertension. Limited research has examined this relationship in the Indonesian population. Objective: This study examined associations between workplace stress levels and pregnancy complications among working pregnant women. Methods: Cross-sectional study involving 160 pregnant women in urban Indonesia using the Work Stress Assessment Scale (WSAS), Pregnancy Experience Scale (PES), and comprehensive demographic questionnaires. Multivariable logistic regression analyzed relationships between workplace stress and pregnancy complications, adjusting for demographic and environmental factors. Results: Participants had an average work stress score of 79.52±11.87, pregnancy experience score of 81.88±11.84, and lifestyle and work environment factors of 80.04±11.60. There was a strong positive correlation between all work stress variables (r=0.8180.961, p<0.001). Work stress was significantly associated with pregnancy complications in the initial model (OR=0.077; 95% CI: 0.031-0.190; p<0.001); however, the OR below 1.0 likely reflects healthy worker survivor bias rather than a true protective effect. Work stress lost statistical significance in the final model (OR=0.500; p=0.325) after inclusion of lifestyle and work environment factors, which emerged as the sole independent predictor (OR=0.080; 95% CI: 0.0180.349; p=0.001). Conclusions: While initial models showed significant associations between work stress and pregnancy complications, the final model indicated that concrete work environment factors—such as working hours, rest periods, and physical hazards—were the dominant predictors, mediating the perceived stress-complication relationship. The counterintuitive OR direction and the loss of significance for work stress in the fully adjusted model highlight important methodological limitations of the cross-sectional design. Healthcare providers should incorporate occupational exposure assessment into routine antenatal care, with emphasis on modifiable working conditions.

Abstract

Fear of childbirth remains a significant psychological factor influencing delivery mode preferences among first-time pregnant women. This study aimed to examine the direct effect of tokophobia on non-medically indicated caesarean section (CS) requests among primigravidae and to assess the mediating role of midwifery support in this relationship. A cross-sectional design was employed involving 150 third-trimester primigravid women in Bandar Lampung City, selected through multi-stage cluster sampling across eight sub-districts. Tokophobia was measured using a culturally adapted version of the Wijma Delivery Expectancy Questionnaire Version A, while midwifery support was assessed using a researcher-developed instrument. Preferences for CS were collected through a nominal questionnaire. Data were analyzed using structural equation modeling with bootstrapping. The results showed that tokophobia significantly predicted both CS requests and the level of midwifery support. Furthermore, midwifery support significantly mediated the relationship between tokophobia and CS requests. Interestingly, the mediating effect was positive, indicating that supportive approaches lacking structured cognitive components may unintentionally reinforce CS preference among women experiencing fear of childbirth. These findings highlight tokophobia as a key psychological determinant of non-medically indicated CS demand in urban settings. In practice, integrating structured psychoeducational counseling and cognitive reframing techniques into routine antenatal care is crucial. Additionally, midwifery training programs should be strengthened to include evidence-based psychological intervention skills. Early screening for tokophobia, starting from the first trimester, is strongly recommended to help reduce unnecessary caesarean sections and improve maternal health outcomes.

Abstract

Family support is widely acknowledged as a key determinant of breastfeeding; however, the psychological mechanism linking it to exclusive breastfeeding remains inadequately examined in Indonesia, as no published study has formally tested breastfeeding self-efficacy as a mediator using path analysis frameworks. This study aimed to analyze the mediating role of breastfeeding self-efficacy in the relationship between family support and exclusive breastfeeding among postpartum mothers. A cross-sectional analytical study was conducted with 250 postpartum mothers in Tangerang City and Regency, recruited through purposive sampling across multiple healthcare settings. Data were collected using two validated instruments and analyzed via Partial Least Squares Structural Equation Modeling (PLS-SEM), with indirect effects examined through indirect effect analysis. Family support had no significant direct effect on exclusive breastfeeding (β = 0.097; p = 0.422), yet strongly predicted breastfeeding self-efficacy (β = 0.887; p < 0.001). Breastfeeding self-efficacy significantly predicted exclusive breastfeeding (β = 0.761; p < 0.001), and the indirect effect was confirmed (β = 0.675; p < 0.001), indicating full mediation. The model explained 45.1% of variance in exclusive breastfeeding and 78.6% in breastfeeding self-efficacy, though causal inference remains constrained by the cross-sectional design. These findings demonstrate that family support promotes exclusive breastfeeding only insofar as it simultaneously strengthens maternal psychological confidence. Exclusive breastfeeding promotion programs should integrate self-efficacy-building components as a core intervention element rather than targeting family support in isolation.

Abstract

Maternal diet quality during pregnancy remains critically suboptimal in Indonesia, contributing to the high prevalence of anemia and adverse pregnancy outcomes. This study aimed to analyze the direct and indirect effects of food literacy and eHealth literacy on diet quality through nutritional self-efficacy among pregnant women in urban Indonesia. A cross-sectional analytical study was conducted on 238 pregnant women in the second and third trimesters at Puskesmas Wara Selatan, Palopo City, South Sulawesi. Data were collected using four validated instruments: the Self-Perceived Food Literacy Scale, the eHealth Literacy Scale (comprising the core eHEALS items and a digital information access sub-dimension), the Nutrition Self-Efficacy Scale, and the Minimum Dietary Diversity for Women indicator via 24-hour dietary recall. Partial Least Squares Structural Equation Modeling with 5,000 bootstrap resamples was employed. Results showed that food literacy had the strongest direct effect on diet quality (β=0.689; p<0.001) and significantly predicted nutritional self-efficacy (β=0.453; p<0.001). eHealth literacy significantly influenced nutritional self-efficacy (β=0.484; p<0.001) but did not directly affect diet quality (β=0.162; p=0.254). Nutritional self-efficacy significantly predicted diet quality (β=0.140; p=0.036) and served as a partial mediator in the food literacy–diet quality pathway and a full mediator in the eHealth literacy–diet quality pathway. The model explained 48.5% of diet quality variance and 77.8% of nutritional self-efficacy variance. These findings confirm that nutritional self-efficacy constitutes a critical intervention point for optimizing maternal diet quality in urban primary healthcare settings.

Abstract

Insufficient breast milk production remains a major barrier to achieving exclusive breastfeeding at both global and national levels. In Indonesia, exclusive breastfeeding coverage declined from 88.9% in 2021 to 82.2% in 2023. The use of plant-based herbal galactagogues, particularly Sauropus androgynus (katuk leaves), is widely practiced; however, comparative evidence against active interventions such as lactation education remains limited. This study aimed to compare the effectiveness of katuk leaf decoction and breastfeeding technique education in increasing breast milk production among postpartum mothers. A quantitative study with a quasi-experimental pretest–posttest control group design was conducted involving 32 postpartum mothers selected through purposive sampling from a population of 66, comprising 16 respondents in the intervention group (katuk leaf decoction 150 ml/day for 7 days) and 16 respondents in the control group (breastfeeding technique education). Breast milk production was assessed using a structured observation sheet based on four proxy indicators. Data were analyzed using the Wilcoxon signed-rank test for within-group comparisons and the Mann–Whitney test for between-group comparisons. The results showed that breast milk production scores increased in both groups, with a higher mean increase observed in the intervention group (from 4.44 to 6.56; Δ=2.12) compared with the control group (from 4.19 to 5.94; Δ=1.75). Within-group analysis demonstrated statistically significant improvements in both groups (p=0.001), and between-group comparison revealed a statistically significant difference in score changes (p<0.05). In conclusion, katuk leaf decoction demonstrated a greater increase in breast milk production compared with breastfeeding technique education; however, these findings should be interpreted cautiously due to methodological limitations, and further studies using more rigorous designs are recommended.

Abstract

Anemia banyak dijumpai pada remaja putri dan wanita usia subur. Penyebab utamanya adalah defisiensi zat besi akibat kehilangan darah saat menstruasi, asupan gizi yang tidak memadai, dan gangguan penyerapan zat besi. Pada remaja, anemia berdampak seperti menurunnya daya tahan tubuh, rasa mudah lelah, gangguan pertumbuhan, serta penurunan kemampuan kognitif, psikologis, dan konsentrasi belajar. Kondisi tersebut dapat memengaruhi prestasi akademik dan perkembangan diri. Jika tidak ditangani sejak dini, anemia dapat berlanjut hingga dewasa dan meningkatkan risiko komplikasi saat kehamilan. Tujuan dari pengabdian masyarakat adalah pemeriksaan skrining anemia bagi remaja dan edukasi pencegahan anemiaMetode yang digunakan dalam pengabdian masyarakat adalah dengan melakukan sekrining pemeriksaan anemia menggunakan alat hemoglobin, media edukasi pencegahan anemia dengan poster dan pemberian leflet serta, penyampaian hasil skrining, konseling individu. Pelaksanaan pengabdian masyarakat di Universitas Phatum Thani Thailand, pada tanggal 7 November 2025. Sasaran adalah Remaja diwilayah lingkungan kampus. Teknik pelaksanaan kegiatan yaitu survey permasalahan mitra, penyiapan bahan, proses pelaksanaan kegiatan, evaluasi, tindak lanjut, dan pelaporan kegiatan pengabdaian masyarakat. Hasil  pemeriksaan sekrining dari 30 remaja yang mengalami anemia sejumlah 18 (60%) remaja dengan Hemoglobin dibawah 12 g/Dl dilakukan dengan menggunakan alat digital pemeriksaan hemoglobin dan pemberian edukasi dengan menggunakan brosur dan poster pencegahan anemia.Kegiatan pengabdian masyarakat berhasil mengidentifikasi kasus anemia pada remaja melalui skrining hemoglobin serta pemberian edukasi dapat meningkatkan pengetahuan remaja tentang pencegahan anemia. Anemia pada remaja putri menunjukkan bahwa pengetahuan, status gizi (kurang zat besi), pola makan (buruk), pola haid (menstruasi banyak/lama/tidak teratur), dan sosial ekonomi rendah adalah faktor risiko utama, sementara aktivitas fisik juga berperan karena tubuh butuh lebih banyak energi saat aktif. Remaja putri rentan anemia karena kehilangan darah bulanan saat haid dan kebutuhan gizi tinggi untuk pertumbuhan, yang diperburuk oleh faktor-faktor di atas

Abstract

Electronic medical record (EMR) implementation influences healthcare quality, but psychological mechanisms remain underexplored in midwifery contexts. This study examined EMR's effect on midwifery care quality with job satisfaction as a mediating variable. A cross-sectional study involved 450 midwives from healthcare facilities in DKI Jakarta, West Java, and Central Java from March to June 2025. Participants were selected through a multi-stage cluster sampling design with stratification by facility type. Data were collected using validated self-administered questionnaires measuring EMR implementation (30 items, α=0.952), job satisfaction (25 items, α=0.943), and care quality (35 items, α=0.923). All instruments demonstrated excellent psychometric properties with factor loadings ranging from 0.71 to 0.89 and average variance extracted values of 0.652 to 0.694. Structural equation modeling using SmartPLS 4.0 with bootstrapping (5,000 subsamples) and the Sobel test was used to analyze the direct and indirect relationships among variables. EMR implementation significantly affected care quality (β=0.689, t=12.352, p<0.001, R2=0.508) and job satisfaction (β=0.367, t=5.352, p<0.001, R2=0.135). Job satisfaction positively influenced care quality (β=0.060, t=2.757, p=0.028). Sobel test confirmed partial mediation (Z=2.450>1.96, p<0.05), indicating that job satisfaction mediates the relationship between EMR implementation and care quality. The total effect (c'=0.735) exceeded the direct effect (c=0.713), demonstrating that EMR influences care quality through both technological and psychological pathways. In conclusion, EMR implementation improves midwifery care quality through direct technological pathways and indirect psychological mechanisms, including job satisfaction, supporting holistic implementation strategies that consider human factors alongside technical aspects in maternal healthcare digitalization initiatives.
