Abstract

One target of MDG 2015 is to reduce by half the pulation without access to water and basic sanitation. The HealthMinistry of Indonesia has an effort to increase basic sanitation access through National Strategy of CommunityBased Total Sanitation (STBM) started from 2008. According to data from the local primary health care (Puskesmas)Tegalalang II, the sanitation coverage in some area of Taro Village are low. For that reason, in cooperation withHealth Agency of Gianyar, Puskesmas Tegalalang II, the activity of KKN in Taro Village was integrated with STBMprogram focusing on hygiene behavior change in terms of stop open defecation without any subsidy provided.The preparation was started with training of STBM facilitator participated by KKN students. It was then followedby triggering of stop open defecation as well as promoting hand washing with soap in Banjar Tebuana, Taro,Gianyar. The trigering and promoting activities were conducted through participatory rural appraisal (PRA) methodthat utilising Participatory Hygiene and Sanitation Transformation (PHAST). In order to have baseline data insanitation, then verification of sanitation facilities already built in the village was carried out. From the trigeringprocess only 3 families that commited to build toilet. The community always consider that poverty as the mainreason why they could not afford for a toilet. From the verification result, only 21,21% families that have toilet, andonly 38% of those toilet that categorised as hygiene toilet. Moreover, only 55,55% of families that have facilitiesfor washing hand with soap.Based on the trigering and verification result, it is recommended that facilitating thisvillage to rapidly reduce the number of families without access to basic sanitation is very important to continue.The role of sanitarian at Puskesmas cooperated with village government to facilitate their community in pursingbehavior change is also very crucial.

Abstract

ABSTRACT Midwifery as paramedic in a village plays an important role in primary health care in order to pursue Indonesia Sehat 2010. Midwifery should provide good services with adequate tariff for society. As one indicator of service, the use of unit cost method has become an important part in tariff calculation. It was found that knowledge of the local Midwifery about unit cost was quite low. The activity of community services was carried out at IKK-IKP department Faculty of Medicine, on 01st September 2007. This activity aimed to improve knowledge and ability of midwifery of how to use Unit Cost in tariff calculation. The method was practicing on calculate several health care services cost. The result of this activity was midwifery can create adequate tariff base on unit cost.

Abstract

School-age children require sufficient source of energy to support their growth. Some places in which children get their food are school’s canteens and vendors in the vicinity of the schools. Nutritional contents, additive substances (food coloring and preservatives) and food processing influence the quality of food in such areas. In addition, there are also problems related to diarrheal diseases and food poisonings due to consuming low quality food, not only in Denpasar but also in other areas of Bali. We assisted and supervised food sellers on the application food safety principles and the importance of high quality food at school’s canteens and food vendors surrounding the school areas. The results indicated that the sellers showed a good improvement in knowledge and attitude on the importance of food safety, supported by the results of direct observations in the field. The responses of teachers at Primary School No 17 Kesiman, and the sellers are good, indicated by their active participation in assisting the activities. Coordination with the teachers is required to perform continual monitoring on the cleanliness and quality of food sold at school canteens as well as surrounding areas. This will be expected to prevent food poisoning and diseases, which eventually improves nutrition status of the school-age children.

Abstract

The knowledge of farmers on pesticides and its association on health and the environment still remains an issue. Located in the rural area of Bali, the village of Songan, Kintamani, Bangli with its fertile soil accommodates many farm lands. The counseling and assistance included 20 farmers during their working hours to allow direct observation. The counseling and assistance targeted at issues regarding safe uses of pesticides and its association to health and the environment. The knowledge of participants was evaluated by pre- and post-test. Less than a half (40%) were able to identify an association between pesticide use and health, while almost all does not know the chronic effect of pesticides to health and/or the environment. Recommendations target health care facilitators to continue counseling and assistance to the farmers and others in order to maintain a healthier and sustainable future.Keyword: pesticide, health, environment, farmers knowledge

Abstract

Abstract Generally street vendor has little knowledge on hygiene and sanitation of food handling. Usually they trade in a night market or on street sides. There are two markets in Penatih Village that operate night market. Both markets located in a strategic location thus they have many visitors every day. In these two markets there are some vendors that need to be counseled and assisted in order to improve their knowledge on hygiene and sanitation of food handling. The counseling and assistance was conducted every week from 25 September to 10 October 2009, with 10 vendors as target participants. The venues were located in Penatih Market and Agung Market, Penatih Village, East Denpasar. The activities were conducted in the form of counseling and assisting during trading hours towards the participants in regards of hygiene and sanitation of food handling. In order to evaluate and measure the knowledge improvement of the participants, pre test and post test were carried out. The result shows that, most of participants have less knowledge on food additives in terms of usage, types and risk of usage. The participants have good knowledge on usage of clean water for utensil and food washing as well as knowledge on personal hygiene. It is recommended to conduct a continuous and regular counseling to street vendors with the involvement of local primary health care (puskesmas).

Abstract

Introduction: Heavy metals in the environment can accumulate in organisms through the food chain process. Previous studies recorded heavy metal concentrations above threshold limits value in Badung river, which warrants monitoring adverse health outcomes due to consuming fish from this river. This research aimed to estimate the potential risk from heavy metals exposure, namely Pb, Cd, Cu, and Cr, due to fish consumption. Methods: Fish samples were limited to Oreochromis Mossambicus and Oreochromis niloticus, commonly known as tilapia. Twenty samples of composite fish muscles were collected from three fishing sites. Subsequently, the heavy metals present in the samples were quantified using an atomic absorption spectrophotometer (AAS), and health risks were assessed by calculating estimated daily intake (EDI) and target hazard quotient (THQ) for both carcinogenic and non-carcinogenic risks. Results and Discussion: The average concentrations of Pb in Oreochromis Mossambicus (6.35±3.21 mg/kg) and Oreochromis niloticus (6.09±3.07 mg/kg) exceed the threshold limits value for fish products, but other heavy metals remain below. The average EDI from consuming Oreochromis Mossambicus with Pb is 0.0025-0.0026 mg/kg/days; Cu is 0.0037- 0.0062 mg/kg/day; Cr is 0.0001 mg/kg/day. EDI from consuming Oreochromis niloticus with Pb is 0.0015-0.0025 mg/kg/day; and 0.00 mg/kg/day for Cu and Cr. The THQ calculation for carcinogenic and non-carcinogenic health risks showed no health risk from consuming the fish from the Badung River. Conclusion: The study concludes that the consumption of Oreochromis Mossambicus and Oreochromis niloticus from the Badung River was generally safe from potential health risks.

Abstract

Abstrak: Makanan yang mengalami proses fermentasi diyakini dapat memberikan manfaat bagi kesehatan manusia. Proses kedelai yang difermentasi menyebabkan kedelai mudah untuk dicerna dan mampu menghilangkan bau dan rasa langu serta berpotensi mengandung komponen zat gizi yang belum banyak dieksplorasi. Penelitian ini bertujuan untuk menganalisis komposisi profil asam amino ekstrak seredele dan ekstrak tempe. Penelitian ini bersifat eksperimental dengan sampel sebanyak 2 jenis bahan makanan terfermentasi yaitu seredele dan tempe kedelai. Sampel tersebut dibuat dalam bentuk ekstrak dan selanjutnya dianalisis komposisi asam amino dengan metode HPLC. Asam amino yang diujikan adalah sejumlah 15 asam amino           . Hasil peneltiian menun-jukkan bahwa kedua kstrak bahan makanan yaitu ekstrak seredele dan tempe kedelai mempunya komposisi asam amino yang bervariasi dan cukup lengkap dengan 15 asam amino esensial. Seredele yang merupakan makanan khas tradisional Bali memiliki kandungan asam glutamate (glutamic acid) yang lebih tinggi dibandingkan dengan ekstrak tempe, yaitu masing-masing 0.6% dan 0.4%. Kandungan asam amino Aspartic acid dan Tyrosine pada seredele juga ditemukan lebih tinggi daripada tempe kedelai. Adanya kandungan asam amino jenis asam glutamate (Glutamic Acid) yang lebih tinggi dari tempe menunjukkan potensi atau kemampuan untuk mengikat zat besi. Untuk itu disarankan adanya suatu penelitian lanjutan mengenai kemampuan atau potensi mengikat zat besi dari seredele dan tempe kedelai secara in vitro

Abstract

Background: Status of minerals such as zinc, magnesium, and chromium among diabetic patients are considered lower than non-diabetic peoples, both in the serum and intake. Some reviews show that those minerals contribute to carbohydrate metabolism among diabetic patients.Objective: This study aims to explore intake of zinc, magnesium, and chromium among DMT2 in Denpasar.Method: This is a cross-sectional analytic study. The population was all (70) DMT2 patients registered at Chronic Disease Service (Prolanis) at two Primary Health Care (PHC) Denpasar Utara III PHC and Denpasar Timur I PHC. The sample for the study was DMT2 patients and non-diabetic people age 50-70 years reside in Denpasar city. DMT2 patients were randomly selected with simple random technique, while non-diabetic peoples were non-randomly selected. The level of serum chromium was measured by AAS method, whereas microminerals intake were collected by a recall and SQ-FFQ method. Data was analyzed descriptively and differences were tested with two independent t-test at 95% confidence level.Results: The mean of serum chromium DMT2 and non-diabetic were 0.044 mg/L and 0.094 mg/L respectively. The mean±SEM magnesium (DMT2 316.1±22.4 mg/day; non-diabetic 211.0±33.5 mg/day), zinc intake was (DMT2 8.4±0.7 mg/day; non-diabetic 6.2±0.8 mg/day), and chromium (DMT2 10.8±1.3 µg/day; non-diabetic 8.7±0.9µg/day). A significant difference was found between magnesium intake (p=0.01).Conclusion: The study showed that zinc, magnesium and chromium intake is appeared higher among DMT2 patients compare to non-diabetic in Denpasar region.

Abstract

Food intake and body fat parameter among women older than 40 years old in DenpasarBackground: The prevalence of obesity in Indonesia tends to increase, particularly among women older than 40 years old.Objective: This study aimed to identify the associations between nutrient consumption and parameters of body fat among women in Denpasar.Methods: This was an observational analytic study with a cross-sectional approach. The population was women aged older than 40 years old in Denpasar with sampled population were members Dharma Wanita PDAM Kota Denpasar. Eighty out of 200 women were selected through systematic random sampling method. Variables collected in this study included women’s characteristics, nutrient consumption, and body fat parameters: body mass index (BMI), waist circumference (WC), and waist to hip ratio (WHR). Analysis of the relationship between nutrient consumption and body fat parameters using the Spearman Rank correlation.Results: This study found that the majority aged 41-50 years (83.8%), were Balinese  (95.0%), have married  (98.8%), did not use contraception (65.0%), completed higher education (63.8%), worked in informal sector (60.0%), had low income (65.0%), had no history of obesity (80.0%) and no family with degenerative diseases (73.8%). Based on the body fat parameters, most of the women were obese based on BMI (52.5%), WC (70.0%), and WHR (57.5%) categories. There was a significant association between fiber consumption and waist circumference (p=0.021).Conclusions: There was an association between fiber consumption and waist circumference, thus having a balanced and varied diet is recommended to prevent obesity and reduce risk factors for degenerative diseases.

Abstract

Health status and micronutrient intake among vegetarian and non-vegetarian in BaliBackground: Several studies have reported that vegetarian diets have health benefits for those adopting the diets.Objective: This study aimed to compare anemia status and micronutrient intake between vegetarians and non-vegetarians in Bali.Methods: This cross-sectional study was conducted in Bali. A total of 240 samples consisting of 160 vegetarians and 80 non-vegetarians were randomly selected. Hemoglobin (Hb) and fasting blood sugar levels were measured using the cyanmethemoglobin method and enzymatic colorimetric, respectively. Meanwhile, the data on micronutrient intakes were collected by interviews using the semi-quantitative food frequency questionnaire (SQFFQ). The data were analyzed using descriptive statistics, and the Mann-Whitney test was performed to analyze the differences.Results: The study’s results showed that the mean Hb levels in the vegetarian group were significantly lower than non-vegetarians (p=0.002; p<0.05), and the anemia status in vegetarian women were higher (22.5%) than non-vegetarian women (2.5%). The mean fasting blood glucose level in non-vegetarians was significantly higher than in the vegetarian group (p=0.000; p<0.05). There were no differences in zinc (Zn) intake between vegetarians and non-vegetarians, but there were significant differences (p<0.05) in vitamin C, vitamin D, vitamin B12, folic acid, magnesium (Mg), calcium (Ca), and iron (Fe) intakes.Conclusions: There were differences in anemia status and fasting blood glucose levels between vegetarians and non-vegetarians. The fasting blood glucose levels of non-vegetarians were higher than vegetarians, and there were differences in the intake of certain micronutrients between vegetarians and non-vegetarians.
