Abstract

Wrong perceptions regarding exclusive breastfeeding can influence exclusive breastfeeding. An example is insufficient breast milk production. This reason is the main reason mothers do not breastfeed exclusively. In this case, the mother feels that her breast milk is not enough, with various complaints such as the breasts getting smaller, the breast milk becoming thinner, the baby crying more often and asking to be breastfed more often (Walyani, 2015). This research aims to determine the relationship between maternal characteristics and social support on the provision of exclusive breast milk to breastfeeding mothers at the Datuk Bandar Community Health Center, Datuk Bandar District, Tanjungbalai City in 2024. This type of research is research with a cross-sectional design. The population and sample were all breastfeeding mothers at the Datuk Bandar Community Health Center, Datuk Bandar District, Tanjungbalai City in 2024 in May-June, as many as 39 people using the total sampling technique using the Chi Square statistical test. There is no significant relationship between age and exclusive breastfeeding. There is a significant relationship between education level and exclusive breastfeeding. There is a significant relationship between work and exclusive breastfeeding. There is no significant relationship between social support and exclusive breastfeeding at the Datuk Bandar Community Health Center, Datuk Bandar District, Tanjungbalai City in 2024

Abstract

Premature Rupture of Membranes (PROM) is a common obstetric complication associated with maternal infection, neonatal morbidity, and adverse pregnancy outcomes if not managed promptly. Comprehensive and evidence-based midwifery care is essential to improve maternal and neonatal safety, particularly in primary healthcare settings. Objective: This case report aimed to describe the implementation of evidence-based midwifery management for a woman with term PROM using Helen Varney's Seven-Step Midwifery Management framework. Methods: A descriptive case report design was employed involving a 26-year-old primigravida (G1P0A0) at 39 weeks of gestation who presented with spontaneous rupture of membranes before the onset of regular uterine contractions. Clinical data were collected through patient interviews, physical and obstetric examinations, labor observations, medical records, and neonatal assessments. Data were analyzed descriptively using a narrative approach based on Helen Varney's Seven-Step Midwifery Management and interpreted according to current evidence-based recommendations. Findings: Following confirmation of PROM, continuous maternal and fetal monitoring was performed until spontaneous labor occurred. The patient delivered a healthy female infant vaginally with a birth weight of 3,000 g, body length of 49 cm, and an Apgar score of 9. The placenta was delivered completely, and the mother remained clinically stable despite experiencing a second-degree perineal laceration that was successfully repaired. No immediate maternal or neonatal complications were observed. Implications: The findings demonstrate that systematic assessment, continuous monitoring, and evidence-based midwifery management can support favorable maternal and neonatal outcomes in women with PROM and may strengthen the quality of maternity care in primary healthcare facilities. Originality: This report highlights the practical implementation of Helen Varney's Seven-Step Midwifery Management in a term PROM case within a primary healthcare setting, providing a structured example of evidence-based midwifery practice.

Abstract

Abstract

BACKGROUND First pregnancy for a prospective mother is a new journey marked by physical and psychological changes resulting in various psychological problems. One of the psychological aspects that influences pregnancy is anxiety. Apart from that, anxiety arises in pregnant women, influenced by the physical changes that occur during pregnancy, namely the stomach getting bigger and the body getting fatter (Utami, 2017). The pregnant women's class program in Indonesia started in 2010 and implementation at the Tarastar Community Health Center, Tarastar District, Humbang Hasundutan Regency began in 2012. One of the expected results from the implementation of the pregnant women's class is an understanding, change in attitudes and behavior of pregnant women so that pregnant women can undergo their pregnancy calmly and comfortably, avoiding physical and mental illness, and childbirth can take place normally, which ultimately contributes to reducing maternal mortality. PURPOSE: To determine the effect on anxiety of primigravida mothers at the Tarastar Community Health Center, Tarastar Subdistrict, Humbang Hasundutan Regency in 2023. The quasi-experimental design METHOD with the normality test of this data uses the Shapiro Wilk Test because it is adapted for small research samples. RESULTS: The conclusion of this study is that there is a difference in the level of anxiety between primigravida mothers who took the class for pregnant women and those who did not take the class for pregnant women. Suggestions are given for research and for research sites and for educational institutions regarding factors related to knowledge and attitudes of post partum mothers regarding Pregnancy Mother classes.
