Abstract

Struma ovarii is a rare condition, in which thyroid tissue is the predominant element in an ovarian carcinoma. Thyroid tissue may demonstrate the same spectrum of pathological features as in the normal thyroid including benign and malignant changes. The malignant type is very rare, only 5% from all incidents of goiter ovarii. We present a case of papillary thyroid carcinoma arising in a struma ovarii of the right ovary in a 43-year-old female

Abstract

In the most recent publications on Mayer-Rokitansky-Küster-Hauser (MRKH) Myxoma is a benign tumor that generally occurs in the heart, soft tissue, muscle, skin and bone. But in this case, we present myxoma that occurs primarily in the ovary. Ovarian myxsoma is a rare benign tumor.In this case report, it was found that a 12-year-old girl was brought by her parents to the M. Djamil Hospital in the city of Padang, West Sumatra. After performing the gynecological physical examination and ultrasonography, there was a hypoechoic mass with an indeterminate solid part of the right adnexa, which was suspected of being an ovarian malignancy. Then it was decided to carry out a procedure in the form of a conservative surgical staging laparotomy. The results of the Anatomical Pathology examination confirmed the myxoid ovarian appearance and the patient was decided for regular monitoring.

Abstract

Background : Choriocarcinoma is extremely malignant tumor contains of anaplastic trophoblast and prominent hemorrhage, necrosis, and vascular invasion. Choriocarcinoma is a rare, aggressive neoplastic type of trophoblastic disease. This condition grow rapidly and can metastasize to the lung and liver. There are few cases of choriocarcinoma metastases to the spine that have been reported. Case Report : A 28-year-old woman with complaints of haemoptoe and shortness of breath. Previously the patient complained of not feeling the sensation for defecation and micturition. The patient had been diagnosed with choriocarcinoma since 3 years ago, refused to have a hysterectomy and had received chemotherapy 12 times in the first year with choriocarcinoma, but dropped out of chemotherapy in the second year, then came with a worsening condition a year later. The results of the chest x-ray and lumbar MRI showed suspicion of metastases to the lungs and spine. The patient was continued with chemotherapy with paclitaxel and carboplatin and was monitored with periodic beta HCG evaluation. Choriocarcinoma a have different prognoses depending on stage and onset. In these patients, based on FIGO staging, she has reached stage IV and also suspected chemotherapy-resistant Gestational Trophoblastic Neoplasia (GTN), worsened by disobedience to previous treatment. Many patients with GTN require multiple regimens with or without surgery to achieve complete remission.

Abstract

Background : Peritoneal tuberculosis is one of the most challenging forms of extrapulmonary tuberculosis to diagnose. Extrapulmonary TB is very difficult to diagnose because of its non-specific signs and symptoms, thus it sometimes leads to a diagnosis of gynaecological malignancy such as advanced ovarian carcinoma. Case Report : A teenage girl, 12 years old, with a chief complaint of intermittent abdominal pain since the last 1 month. There was no history of an enlarged abdomen, fever and weight loss. Ultrasound examination revealed an irregular adnexal mass suggesting an ovarian malignancy with other possibilities of an infectious process. Diagnostic laparoscopy and peritoneal biopsy were performed on the patient. Intraoperatively, multiple friable nodular-mylar peritoneal masses with abdominopelvic attachments and fibrin fibres and white vesicles were found on the entire surface of the genitalia, peritoneum, and intestines. A mass biopsy was performed with histopathological results showing peritoneal tuberculosis. Currently, the patient is undergoing anti-tuberculosis drugs (OAT) treatment. Discussion : The diagnosis of this disease is difficult to establish. Symptoms are highly variable and non-specific and can lead to the wrong tumour pathology. The clinical characteristics are dominated by changes in general condition, abdominal pain and transit disorders, as well as masses (20 to 25%). The gold standard for definitive PTB diagnosis remains laparoscopy with peritoneal biopsy and subsequent pathological or microbiological confirmation. Yellow/white nodules in the peritoneum observed on the patient's laparoscopic images are the hallmark of wet peritoneal TB. Omental thickening and abdominal cocoon with matted small bowel are other classic laparoscopic findings for peritoneal TB. Abstract

Ovarian cancer is the most lethal disease among gynecologic malignancies; about 1/4 of ovarian cancer patients are in stage IV when diagnosed. Based on the data of Globocan, there were 295,414 newly diagnosed ovarian cancer patients and 184,799 deaths registered in 2018. For ovarian epithelial cancer patients with stage IV disease, the mean overall survival (OS) is around 20 months, and the 5 year-OS is about 20%. About 1 in 5 patients with advanced-stage ovarian cancer have hepatobiliary involvement. The study on HGSOC aims to understand the molecular and genetic features, diagnosis, and prognosis of HGSOC, as well as its response to therapy. The method involves analyzing the genetic traits, chromosomal instability, and dysregulation of signaling pathways associated with HGSOC, as well as the detection of stemness and EMT levels in response to chemotherapy and radiation therapy. The results highlight the unique characteristics of HGSOC, such as nearly universal mutation in and dysfunction of p53, genomic instability, and the role of cancer stem cells and EMT in chemo- and radiation resistance. The implication of the study is the need for a better understanding of the molecular basis of HGSOC for improved diagnosis, prognosis, and treatment strategies.

Abstract

The Infant Mortality Rate (IMR) in Indonesia remains higher than in developed countries, with one of the main contributing factors being Low Birth Weight (LBW) infants. The World Health Organization (WHO) reports that the global prevalence of LBW ranges from 15% to 20% of all births. In Indonesia, the prevalence of LBW based on the 2022 Indonesian Nutritional Status Survey (SSGI) was 6%, while in West Sumatra, the LBW rate was recorded at 4.4%. At Dr. M. Djamil Hospital Padang, data from 2023–2024 showed that the incidence of LBW reached 47.49% in 2023 and increased to 52.92% in 2024. LBW increases the risk of neonatal mortality by up to 20 times and has the potential to cause long-term complications. This study aims to analyze the relationship between maternal factors and low birth weight outcomes at Dr. M. Djamil Hospital, Padang, in 2023–2024. This research used a quantitative study design with a cross-sectional approach. The sample consisted of 58 postpartum mothers who met the inclusion criteria, selected using the Consecutive Sampling technique. The study found that 37.9% of newborns had LBW, 36.2% of mothers were either too young or too old, 37.9% were primiparous, 41.4% had preterm gestational age, and 39.7% experienced hypertensive disorders during pregnancy. Additionally, 43.1% of infants had severe asphyxia based on Apgar scores, 82.8% required NICU care, and 53.4% had comorbid asphyxia-related conditions. Bivariate analysis using the chi-square test showed significant associations between maternal age (p=0.018), parity (p=0.028), gestational age (p=0.024), and pregnancy morbidity (p=0.029) with LBW. There is a significant relationship between maternal age, parity, gestational age, and pregnancy morbidity with the incidence of low birth weight. Effective interventions are needed to reduce the prevalence of LBW and its associated complications. Low Birth Weight (LBW), Infant Mortality Rate (IMR), Maternal Factors, Neonatal Outcomes, Dr. M.
