Abstract

Background: Perineal tears are one of the complications that often occur in normal childbirth and can cause pain, bleeding, and impaired recovery of postpartum women. Factors that influence the incidence of perineal tears include fetal position, elasticity of perineal tissue, and technique of delivery. Optimising Fetal Positioning (OFP) and the use of Birthing Ball are non-pharmacological interventions that aim to help optimal fetal positioning and improve pelvic mobility during labour. Purpose: This study aims to determine the effectiveness of Optimizing Fetal Positioning and Birthing Ball on the incidence of perineal tears in labouring women. Method: The study used a quasi-experimental design with a relational empirical approach. The study sample was 40 normal labouring women who were divided into two groups, 20 intervention group respondents and 20 control group respondents. Data were collected through observation of intervention implementation and examination of perineal conditions after delivery. Data analysis used the Chi-Square test. Results: The proportion of perineal tears in the intervention group was lower than the control group, but the statistical test results showed no statistically significant relationship (p-value > 0.05). Nevertheless, there was a tendency to decrease the incidence of perineal tears in mothers who received OFP and Birthing Ball interventions. Conclusion: The combination of Optimising Fetal Positioning and Birthing Ball is not statistically effective in reducing the incidence of perineal tears, but has the potential to provide clinical benefits. Suggestion: Future research is recommended to use a larger sample size and control for other confounding factors.

Abstract

Background: Breast engorgement occurs due to increased milk production combined with irregular breastfeeding. This can be treated pharmacologically and non-pharmacologically by applying aloe vera compresses. Purpose: To analyse the effectiveness of aloe vera compresses on breast swelling and pain in postpartum mothers. Method: This study uses a quasi-experimental design, specifically a one-group pretest-posttest design. This study involved all patients in Desember 2025 was 104 mothers. The sample size was calculated using the finite population correction formula, resulting in a sample of 50 mothers. The instruments used to obtain research data were the Wong Baker Pain Rating Scale, Six Point Engorgement Scale (SPES), and aloe vera compress SOP. The data were analysed using the Wilcoxon test. Results: The results of the study explained that before the intervention, 32 (64%) respondents showed moderate breast engorgement and 33 (66%) respondents showed mild breast engorgement. Before the intervention, 38 (76%) respondents experienced moderate pain. After receiving the intervention, 37 (74%) respondents experienced mild pain. Conclusion: The results of the study indicate that aloe vera compresses are effective in reducing swelling (p=0.000) and breast pain (p=0.000) in postpartum mothers at Kartini General Hospital in Jepara. Suggestion: Future researchers are also advised to conduct studies by including a control group as a comparison group so that the differences between groups can be clearly distinguished.

Abstract

General Background: Breast milk production during the postpartum period is a critical factor in successful breastfeeding, yet many mothers experience challenges in achieving adequate lactation. Specific Background: Traditional herbal practices such as empon-empon consumption are widely used in Indonesian communities as part of postpartum care, believed to support lactation through bioactive compounds. Knowledge Gap: Previous studies have primarily examined general herbal benefits, with limited contextual evidence on the relationship between empon-empon consumption and breast milk production within independent midwifery practices. Aims: This study aims to analyze changes in breast milk production before and after consuming empon-empon herbal drink among postpartum mothers at PMB Dwi Ningrum, Demak. Results: Using a quasi-experimental one-group pretest–posttest design involving 79 respondents, findings showed an increase in the proportion of mothers with smooth breast milk production from 24.1% before intervention to 60.8% after consumption, with statistically significant differences based on the Wilcoxon test. Novelty: This study provides empirical, context-specific evidence integrating local cultural practices and lactation outcomes within community-based midwifery services. Implications: The findings suggest that empon-empon herbal consumption can be considered as a complementary approach to support lactation, while emphasizing the importance of comprehensive breastfeeding management, including maternal nutrition, psychological condition, and family support. Highlights • Breast milk flow increased substantially after herbal consumption period• Significant statistical difference observed between pretest and posttest conditions• Community-based traditional practices documented within midwifery service context Keywords Postpartum Period; Empon-Empon Herbal Drink; Breast Milk Production; Postpartum Mothers; Pretest Posttest Design

Abstract

Sleep quality is one of the common problems experienced by postpartum mothers due to physical, hormonal, and psychological changes, as well as adaptation to the new role as a mother. Sleep disturbances during the postpartum period can affect maternal health and the quality of infant care. One of the non-pharmacological interventions that can be used to improve sleep quality is mindfulness therapy. This study aimed to determine the effect of mindfulness therapy on the sleep quality of postpartum mothers. The study used a pre-experimental design with a one-group pretest–posttest approach. The research subjects were postpartum mothers who met the inclusion criteria. The intervention in the form of mindfulness therapy was administered in three sessions over one week according to standard operating procedures. Sleep quality was measured using the pittsburgh sleep quality index (psqi) questionnaire. The pre-test measurement was conducted before the mindfulness intervention, while the post-test measurement was carried out after all intervention sessions had been completed. The results showed that the average psqi score before the intervention was 12.3, indicating poor sleep quality among postpartum mothers. After mindfulness therapy was administered, the average psqi score decreased to 6.4, indicating an improvement in the respondents’ sleep quality. Statistical analysis was conducted to compare psqi scores before and after the intervention. The wilcoxon test results showed a significant difference between pre-test and post-test psqi scores with a p-value < 0.05, indicating that mindfulness therapy had a significant effect on improving postpartum mothers’ sleep quality. Based on these findings, it can be concluded that mindfulness therapy is effective as a non-pharmacological intervention in improving the sleep quality of postpartum mothers and may be considered for implementation in postpartum maternal healthcare services.
