Abstract

Background Colorectal carcinoma has several pathways that play a role in normal colonic mucosa development into carcinoma, one of microsatellite instability (MSI) pathway. This pathway results from a deficiency in one of the MMR proteins that normally repair genome damage, leading to microsatellite instability high (MSI-H) and excessive mutations. MSI-H is closely associated with Lynch syndrome and often experiences resistance to chemotherapy treatment, one of which is 5-fluorouracil (5-FU), so it is necessary to conduct initial screening in colorectal carcinoma to assess MMR status on patient with Lynch syndrome . This study aims to see relationship between MMR status and chemotherapy response in colorectal carcinoma. Methods This study is cross sectional study using 30 archival block and slaid samples of all colorectal carcinoma cases from hemicolectomy and biopsy results that have been histopathologically diagnosed in Anatomic Pathology Section of Faculty of Medicine Unsri/RSMH Palembang January 2017-December 2021. Each sample was immunohistochemically stained using four antibodies namely anti-MLH1, anti-MSH2, anti-MSH6 and anti-PMS2 (ventana). Interpretation was qualitative by assessing cell positivity. Categorized into intact and missing with cut-off point value of >10% positive declared intact. MMR deficiency was considered if there was loss of cell positivity, at least one MMR CPI marker and MMR proficiency if all MMR CPI were intact. Analysis of relationship between MMR status and chemotherapy response was performed using Fisher's exact test. Results Fisher's exact test showed an association between MMR status and chemotherapy response. The results were significantly significant with p-value of 0.045 where MMR deficiency status had a 6 times chance of not responding to chemotherapy compared to MMR proficiency. There was no association between MMR status and age, gender, histopathological subtype and tumor location with p-values of 0.139, 1.000, 0.657 and 0.174 respectively. Conclusion There was a significant association between MMR status and chemotherapy response.

Abstract

Reticulocyte hemoglobin (Ret-He) is a parameter used to assess the amount of hemoglobin in reticulocyte, thus Ret-He describes the mean of hemoglobin content distribution in the reticulocyte, so that the Ret-He examination can be used as a parameter to diagnose iron deficiency anemia early. Screening of IDA which are cheap and easy is still needed. To analyze whether Ret-He can be a parameter in determining iron deficiency anemia in children. This study is a diagnostic test study. The research data was obtained from outpatient and outpatient laboratory data that showed hypochromic microcytic anemia from July to October 2017 in Department of Pediatrics Moh. Hoesin Hospital Palembang. Data analysis was done using SPSS software. The result of research done by comparison between Ret-He value and transferin saturation value (gold standard). The total number of 120 subjects consisted of 52 subjects in IDA group (TfSat <15%) and 68 subjects in non-IDA group (TfSat ≥15%). The IDA incidence of hypochromic microcytic anemia group in Moh.Hoesin Hospital is 43.3%. The best cut off point of Ret-he is 21.95pg. Based on calculation by using Cut off point 21,95pg, the data obtained NDP 97,8%, NDN 90,5%, RKP 57,6, RKN 0,13 and AUC value 99,8%. Ret- He can be used as a method to detect ID is easy and cheap

Abstract

Epiphora, or excessive tearing, is a frequent yet under-recognised adverse effect of docetaxel, a taxane-based chemotherapeutic agent used in breast cancer treatment. Inflammatory and fibrotic changes within the lacrimal drainage system are hypothesised as the primary mechanisms. This study aimed to evaluate the correlation between docetaxel administration duration and epiphora severity, alongside the impact of cumulative dosage and patient age. A retrospective observational study was conducted at Mohammad Hoesin Hospital Palembang, involving 25 breast cancer patients who reported epiphora during docetaxel chemotherapy. Patients with pre-existing ocular conditions, prior lacrimal surgery, or concurrent use of epiphora-inducing medications were excluded. Epiphora severity was assessed using the Munk score, and statistical analyses included Pearson’s correlation and multiple regression. Findings demonstrated a strong positive correlation between epiphora severity and docetaxel duration (r = 0.645, p < 0.05), cumulative dosage (r = 0.618, p < 0.05), and patient age (r = 0.703, p < 0.05). Limitations include the retrospective design, small sample size, and subjective assessment tools. These results emphasise the importance of monitoring for epiphora during docetaxel therapy, with early ophthalmologic intervention recommended. Further prospective studies are needed to clarify pathophysiological mechanisms and optimise management strategies for chemotherapy-induced epiphora.

Abstract

Urtikaria Kronis Spontan (UKS) berdampak signifikan pada kualitas hidup pasien. Autologous Plasma Skin Test (APST) dan C-reactive protein (CRP) merupakan skrining praktis pada UKS. Hasil positif disebab- kan oleh sitokin inflamatorik saat degranulasi sel mast dan dapat berkorelasi dengan keparahan UKS. Studi analitik observasional dengan desain potong-lintang dilakukan selama bulan Juli-September 2021 melibatkan lima puluh pasien UKS berusia 17-51 tahun. Keparahan penyakit dinilai menggunakan Urticaria Activity Score (UAS7). Hasil APST dinilai berdasarkan kriteria Sabroe. Nilai serum CRP diukur menggunakan high sensitiv- ity CRP (hsCRP). Hasil APST positif ditemukan pada hampir semua pasien (95%) dan hanya empat pasien dengan CRP positif (8%). Keparahan berkorelasi bermakna terhadap diameter urtika (P = 0,038) dan nilai CRP (P = 0,041). Analisis odds ratio (OR) menunjukkan bahwa pasien dengan hasil APST positif memiliki risiko 32 kali mengalami UKS sedang-berat. Tidak ditemukan korelasi bermakna antara nilai APST dan serum CRP pada UKS.

Abstract

Background: Tracheostomy is a procedure that aims to maintain the airway so that air can enter the lungs and bypass the upper airway. Tracheostomy causes physiological changes, especially in terms of communication, which leads to reduced voice-related quality of life. Fenestrated tracheal cannula and speaking valve can assist patients in generating voice. Objective: To compare the V-RQOL scores of the fenestrated tracheal cannula and speaking valve users. Methods: Analytic observational study using a cross-sectional design. Data collection was done through interviews with the V-RQOL questionnaire guide. The study was conducted in September-December 2022 at the THTBKL Polyclinic of Dr. Mohammad Hoesin Hospital Palembang. Data were analyzed with IBM SPSS 26. Results: 42 patients were using tracheal cannula, with 21 patients each with a fenestrated and speaking valve tracheal cannula. Patients with fenestrated tracheal cannula had lower quality of life in the physical, social, and total domains than those in the speaking valve group (poor QoL in the physical domain: 66.67% vs. 9.52%; social domain: 85.71% vs. 57.14%; total domain: 76.19% vs. 4.76%; p<0.001). From the multivariate analysis, the tracheal cannula type influenced the patient's quality of life was (p<0.001). The probability of someone with a fenestrated tracheal cannula will experience a poor QoL is 76.2%, while speaking valve is 4.7%. Conclusion: The quality of life of patients with a fenestrated type of tracheal cannula is significantly lower than that of speaking valve tracheal cannula users in both the physical, social, and total domains as measured by the V-RQOL.

Abstract

Background: Snakehead fish (Channa striata, CS) has traditionally been used in Indonesia as a nutrient to promote wound healing. It contains protein highly similar to human protein, which may be involved in wound healing via biological pathways. We aim to determine the effectiveness of the topical 5% CS extract gel in promoting wound healing. Methods: A double-blind, placebo-controlled randomized clinical trial was conducted from June to August 2022 at the Department of Dermatology, Venereology, and Aesthetics of Dr. Mohammad Hoesin Hospital. We allocated 32 post-skin surgery participants with grade I acute superficial wounds on the face, neck, trunk, and extremities into 2 groups with one receiving 5% CS (n=16) and the other receiving placebo (n=16). Both groups applied the treatments twice daily. Assessments of wound healing using modified REEDA (redness, edema, ecchymosis, drainage, approximation) score, wound size, and adverse effects were done at follow-up days 1, 3, and 7. Results: Both groups showed REEDA score improvements on days 1, 3, and 7, with significant difference between both groups identified on day 3 (p=0.038). Wound size improved at each follow-up in all participants, with the 5% CS group being slightly better than the placebo group on day 3 (p>0.05). A mild adverse event was found in only one participant in the 5% CS group (3.1%). Conclusion: Both 5% CS extract gel and placebo had good safety profile and were effective in promoting wound healing. 5% CS extract gel was better at reducing REEDA score.

Abstract

Background: HIV infection often leads to endocrine complications, including hypogonadism which characterized by low serum testosterone, potentially affects quality of life and contributes to metabolic issues, such as muscle weakness, depression, osteoporosis, and cardiovascular disease. Some studies have indicated a connection between testosterone levels and CD4 T lymphocyte counts.   Aim: To evaluate the correlation between total serum testosterone levels and CD4 T lymphocyte counts in naive HIV-infected men. Methods: A cross-sectional analitycal study was conducted at Dr. Mohammad Hoesin General Hospital, Palembang, from April to October 2024. Involving 64 naive HIV-infected men. Total serum testosterone and CD4 T cell counts were measured and analyzed. Results: Participants mean age is 36.52 ± 11.03 years with BMI of 19.52 ± 3.71 kg/m². Most subject (62.5%) are reported men who had sex with other men. Poor sleep quality was found in 81%, mild depressive symptoms in 38.1%, and mild erectile dysfunction in 28.6%. Hypogonadism occurred in 32.8% of subjects (28.1% secondary and 4.7% primary). Mean number of testosterone level was 446.75 ± 260.95 ng/dL, with a median CD4 count of 23.5 cells/µL. A significant positive correlation was observed between testosterone levels and CD4 counts (r = 0.313; p = 0.012) although testosterone levels did not significantly differ across HIV clinical stages (p = 0.464). Conclusion: There is a significant positive correlation between serum testosterone levels and CD4 T lymphocyte counts in naive HIV-infected men. Hormonal disturbances may arise in early infection. Hence, testosterone assessment is recommended during the initial evaluation on HIV patients.

Abstract

Background Matrix metalloproteinase-9 (MMP-9) is a protease that plays an important role in various biological processes of breast cancer. This marker may be useful in determining the prognosis and become a promising therapy, especially in breast carcinoma with an aggressive subtype including HER2 enriched and triple negative. Both subtypes have high MMP-9 expression, but it is necessary to differentiate them because they have different biological and molecular aspects. This study aims to compare the density of MMP-9 between triple negative and HER2 enriched breast carcinoma subtypes and their relationship with clinicopathological characteristics. Methods This is a cross sectional study using 32 samples of paraffin blocks and slides of breast carcinoma patients with triple negative and HER2 Enriched subtypes at the Department of Anatomical Pathology Faculty of Medicine Sriwijaya University/Dr. Mohammad Hoesin Hospital Palembang for the period January 1, 2016 to December 31, 2019. Each sample was stained with MMP-9 antibodies immunohistochemistry (polyclonal, Abcam). Comparative analysis of MMP-9 density between the two subtypes was carried out using the median value based on the Mann-Whitney analysis. MMP-9 density was categorized as high and low based on the ROC curve and analyzed by using chi-square test to assess its association with clinicopathological characteristics. Results Mann-Whitney test showed that the MMP-9 density was higher in the HER2 enriched subtype (median 12.0) than triple negative (median 8.8) with a p value of 0.632. The relationship distribution of high MMP-9 density was found in the category of age group >40 years old (60.0%), positive lymphovascular invasion (86.7%) and histological grade III (80.0%) with p values of 0.798, 0.402 and 0.691. Conclusion MMP-9 density in HER2 enriched subtype was higher than triple negative subtype, but statistically not significant.
