Abstract

The 12-lead EKG is a useful first-line diagnostic tool for detecting cardiovascular diseases, including arrhythmias, conduction abnormalities, and myocardial ischemia or infarction. This scoping review aims to discuss the ECG perspective for detecting heart problems. This study was conducted by following the narrative review and development framework. The literature search used ScienceDirect, Scopus, ProQuest, ClinicalKey, and SpringerLink data. The keywords used were "criteria" or "guidelines" and "ECG" or "ECG" or "electrocardiography" or "electrocardiogram" and "detection" and "heart problem" or "heart disease" or "heart problem" or "heart problem." The method uses a narrative review through eight articles spanning 2017–2022. The lighting method uses the PICO analysis framework. The results found four components, namely: 1) automatic diagnosis; 2) 3-D ECG with artificial intelligence (AI) classification; 3) use of the LSTM deep-learning model; and 4) leads using Tpeak-End (Tp-e). In patients with heart problems, the initial examination is an EKG. Thus, the ECG, with the addition of the automatic diagnosis feature, EKG-3D, and the use of a 12-lead ECG with a deep-learning LSTM model, also followed by high sensitivity and specificity, can be used as a tool to perform examinations on patients who require serial ECG examinations. Abstract

Stroke is a non-communicable disease that commonly affects the elderly and can lead to verbal communication disorders, especially when it impacts the brain’s speech center. This study aims to determine the effectiveness of speech therapy using the “Suara AIUEO” pocket book in improving speech ability in patients with non-hemorrhagic stroke. The method used was a descriptive case study involving one patient who experienced motor aphasia due to stroke. The intervention was carried out by providing “AIUEO” speech therapy twice a day for three days, each session lasting 10–15 minutes. Speech ability was measured using the Derby Communication Scale, which includes aspects of expression, comprehension, and interaction. The results showed an improvement in the patient’s communication scores, particularly in expression and comprehension. The patient began to articulate vowels clearly and understand simple instructions. This therapy is not only effective but also simple, affordable, and can be performed by family members at home. With proper education, families can play a crucial role in the rehabilitation process. “AIUEO” therapy is recommended as part of a holistic nursing care approach for stroke patients with speech disorders.

Abstract

Professional identity reflects how nurses perceive and evaluate themselves, shaped by the values, norms, and ethical principles of the nursing profession. This concept encompasses four key aspects: professional values, ethics, knowledge, and leadership. Professional identity contributes to the quality of nursing care and nurses' job satisfaction. This study aims to explore the role of professional identity in these two aspects. A descriptive literature review was conducted to examine the relationship between professional identity, quality of care, and job satisfaction. Following JBI guidelines, the review evaluates nurses’ knowledge, motivation, and practices related to discharge planning. Searches were conducted in PubMed, ScienceDirect, and ProQuest using the keywords "Professional Identity" AND "Nurses," "Quality of Care," AND "Job Satisfaction," focusing on articles from 2019-2024. Results reveal a significant link between professional identity and job satisfaction, with professional identity also influencing burnout potential. Self-directed learning serves as a mediator, while a supportive work environment and positive organizational climate enhance nurse engagement and job satisfaction. A strong professional identity leads to better care quality and job satisfaction, with nurses gaining a clearer understanding of their roles.

Abstract

Objective: Ovarian cysts are commonly detected in pregnancy due to routine prenatal ultrasonography. Most are benign and resolve spontaneously, but some may cause complications, risking both mother and fetus. Case Report: A 23-year-old primigravida at 30-31 weeks of gestation presented for routine antenatal care. She had a history of laparotomy and left salpingo-oophorectomy at 11 weeks 3 days pregnant due to ovarian torsion. A 9.16 × 6.32 cm left ovarian cyst with a depth of 5.13 cm was detected early in pregnancy and managed conservatively. However, due to worsening pain, surgical intervention was performed. At the current visit, she was asymptomatic, and ultrasound confirmed a viable singleton pregnancy with normal fetal parameters. Vaginal delivery was recommended. Conclusion: This case highlights the importance of timely diagnosis and appropriate management of ovarian cysts in pregnancy. A balance between conservative monitoring and surgical intervention is crucial to ensure optimal maternal and fetal outcomes.

Abstract

Background: Myelodysplastic syndrome (MDS) is a hematopoietic disorder characterized by cytopenia, dysplasia, and genetic abnormalities that can transform into acute myeloid leukemia. MDS cases generally occur in children and adults. This case report involves a 75-year-old elderly individual with no history of MDS or family illness. Purpose of the Study: This study serves as a reference for screening the nursing perspective of MDS in the elderly. Methods: This research is a descriptive case report of 1 case. Results: A 75-year-old elderly patient with 1 month of weakness and pallor, blood pressure of 140/90 mmHg with controlled hypertension for 1 year. Laboratory results showed correlation with MDS symptoms: leukopenia (leukocytes 2.85 x 10^3/µl), thrombocytopenia (platelets 1,000/µl), and anemia (Hb 4.3g/dl). Conclusions: Hematological data support the patient experiencing MDS. Additionally, the patient's exposure to pesticides for 40 years also strongly supports the occurrence of MDS as it is a triggering factor for MDS.

Abstract

This case report aims to describe the holistic nursing care provided to three patients following cystectomy surgery due to Endometriosis. The three subjects, Mrs. E (33 years old), Mrs. F (30 years old), and Mrs. G (35 years old), were admitted on the first postoperative day with chief complaints of Acute Pain and Impaired Physical Mobility. Assessment revealed that Mrs. E experienced Acute Pain with a score of 8 on the surgical incision site, required total bed rest, and had elevated Leukocyte levels (15,600/µL). Similar findings in Mrs. F and Mrs. G also indicated acute pain of high intensity and immobility. The primary nursing diagnoses established for all three cases were Acute Pain (D.0077), Impaired Physical Mobility (D.0054), and Risk for Infection (D.0147). Interventions focused on non-pharmacological pain management (deep breathing technique) and gradual early mobilization. Following 3x24 hours of nursing implementation, a significant decrease in pain intensity was observed (Mrs. E's pain decreased from a score of 8 to 3) and an improvement in mobility ability was noted in all three cases. The integrated nursing interventions demonstrated a positive effect on reducing pain and enhancing the functional mobility of the postoperative patients.
ANALYSIS OF SERVICE QUALITY ON OUTPATIENT PATIENT SATISFACTION: LITERATURE REVIEW Prawira, Ricky; Saputra, Dikki

Abstract

Background: Healthcare quality is an essential pillar of modern healthcare systems, directly impacting patient satisfaction. This article comprehensively analyzes the influence of the service quality dimension on outpatient satisfaction in hospitals. Through a systematic literature review of various international studies, it was found that there is a strong positive relationship between patient-perceived service quality and their satisfaction levels. The SERVQUAL model, with its five dimensions (tangibles, consent, responsiveness, assurance, and empathy), has proven to be a dominant framework for improving shared care. However, patient perceptions of these dimensions vary contextually, with a combination of expectations and perceptions being the primary drivers of dissatisfaction. Long waiting times and ineffective communication also significantly impact patient satisfaction. Therefore, improving service quality requires a holistic approach that focuses not only on technical improvements but also on managing patient expectations, enhancing staff communication skills, optimizing administrative processes, and addressing staff well-being. Recommendations include the implementation of digital systems, patient-centered training, and information transparency to create a more satisfying outpatient experience and foster patient loyalty.
