Abstract

Background: Traditional medicine plays a role in alternative treatment systems for diabetes mellitus. Objectives: This study aimed to screen the secondary metabolite of the ethanol extract from robusta coffee (Coffea canephora Pierre) seeds. We aimed to clarify the effects of these extracts on blood glucose, urea, and creatinine levels in Streptozotocin-Induced Diabetic Rats. Material and Methods: Phytochemical screening of extracts was carried out qualitatively according to standard methods. The rats were administered ethanol extracts of C. canephora Pierre seeds at doses of 100, 200, and 300 mg/kg body weight for 28 days. Blood glucose, urea, and creatinine levels were then compared between the control and diabetes groups. Results: The ethanol extract of C. canephora Pierre seeds contains alkaloids, flavonoids, saponins, and tannins. Group doses of 100-300 mg/kg body weight can significantly reduce blood glucose, urea, and creatinine levels. It can be concluded that the ethanol extract C. canephora Pierre seeds is a potential source of antidiabetic. Conclusions: The ethanol extract of C. canephora Pierre seeds shows potential as an antidiabetic agent.

Abstract

Purple leaves are one of the plants that are widely used as medicine. Purple leaves have 3 types of plant varieties, namely purple leaves, green leaves, and white mottled leaves.  Purple leaves have pharmacological activities that can be used as medicine, including antioxidants (improving the deterioration of cells in the body), anti-inflammatory (reducing or suppressing inflammation), antidiabetic (lowering blood glucose levels), analgesics (relieving pain), immunomodulators (increasing the work of the immune system in the body), antihemorrhoids (treating symptoms and complaints of hemorrhoids), and antibacterial (as an antiseptic). The purpose of this study was to determine the antioxidant activity of purple leaf ethanol extract (Graphtophyllum pictum (L.) Griff) in vitro using the DPPH method and to determine its effectiveness as an analgesic and anti-inflammatory against Wistar rats. The results showed that antioxidant activity showed an IC50 value of 72.31 ppm (strong antioxidant). Meanwhile, in the analgetic test, the highest percentage of analgesic effectiveness was found at a dose of 100 mg/kg BW (129.64%) and showed anti-inflammatory activity at a dose of 100 mg/kgBW with a percentage decrease in edem volume of 28.73% at the sixth hour.

Abstract

Hypertension is the primary cause of Chronic Kidney Disease (CKD), as it damages blood vessels in the kidneys. The most commonly used management for hypertension as monotherapy includes Calcium Channel Blockers (CCB) such as Amlodipine and Angiotensin Receptor Blockers (ARB) such as candesartan. Patients with CKD undergoing hemodialysis depend on dialysis machines, which impacts their economic burden. Therefore, a cost analysis review of drugs with the same therapeutic goals is necessary. This was a non-experimental study with a descriptive design. Data were collected through retrospective purposive sampling from January to December 2023 using medical records and payment data for inpatient treatments with hemodialysis that met the inclusion criteria. Direct medical costs included antihypertensive, other medication, service and facility fees, and medical support costs. Of the 100 samples that met the inclusion criteria from January to December 2023, amlodipine was the most commonly used antihypertensive (56%), followed by candesartan (38%). The average direct medical costs were IDR 184.350 for Amlodipine and IDR 561,339 for candesartan. The patient characteristics by gender showed a higher percentage of males (52%) than of females (48%). By age, the majority were in the late adult group (67%), whereas the elderly accounted for (33%). The Amlodipine group achieved more significant cost minimization than the candesartan group. Abstract

Diabetes is characterized by an increasing of blood levels and progressive changes in the structure of pancreatic islet of pancreas. Natural ingredients that contain antioxidants such as some cowpea leaves can be an alternative treatment for diabetes. This study aims to examine the effect of ethanol extract of some cowepea leaves (Vigna unguiculata (L.) Walp) on morphometry (perimeter and diameter) of pancreatic islet of diabetic rat models. The type of research is quasi-experimental with posttest group design. Samples were 30 male wistar rats, age 8 weeks, BW 250-300 grams, that induced by single dose of STZ 40 mg/kg BW. Rats were divided into 5 groups. Group A: normal control, Group B: negative control, Group C: positive control, Group D: DM rats with 100mg/Kg BW extract therapy, Group E: DM rats with 200mg/Kg BW extract therapy, Group F: DM rats with extract therapy 300mg/Kg BW. Blood sugar levels were measured on days 7, 14 and 21. Pancreatic tissue was prepared with 5μm thickness and stained with Hematoxylin Eosin. The sample was observed under 400x magnification microscope. The observation uses Image J software to measure the perimeter and diameter of the pancreatic insula. Data were analyzed by non-parametric Kruskal Wallis and post hoct Mann Whitney statistical tests. Blood glucose level was obtained on 21st day with the highest mean in group B (246.8±156.3) and lowest in group A (67.2 ±7.85), perimeter of pancreatic islet was significantly different (p=0.026), the diameter is significantly different (p =0.046). Ethanol extract of some cowpea (Vigna unguiculata (L.) Walp) leaves with dose of 300mg/kgBB helps to restore the blood and morphometry of pancreatic islet model of diabetic rats.
