Abstract

Indonesia has developed the National Social Insurance System with the establishment of the National Health Insurance program and organized by BPJS Kesehatan. One of payment way of health services for the hospital in the era of JKN is Indonesian Case Base Group (INA-CBG’s), that is the payment of claims from BPJS to hospital on a package of services based on the packet of disease diagnosis and procedure.This study aims to analyze efforts by hospitals to cover the cost deficiency of INA CBG’s claims in Privat Hospital in Medan. This research type is descriptive with qualitative and quantitative. Qualitative is done by in-depth interview, and quantitative is done by calculating real cost and cost of INA-CBG’s claim and by Activity Based Cost method. The counting result with ABC system,The results showed that the efforts made by hospitals to cover INA-CBG’s claim cost deficits are utilizing the help of health professionals from students from private hospital schools / academies, length of service delivery, setting out service packages for general patients, and providing patient pickup facilities. Suggested to to the Hospital to evaluate the financial risks the hospital receives from services, practice student performance, and distribute the cost of INA CBG’s claims well in order to improve the quality of health services in accordance with the standards.

Abstract

Introduction: The majority of workers in the informal sector have not received occupational health services. This study aims to explore health issues and assess the community's need for Occupational Health Services within the group of farmers.Methods: The activities are categorized into three parts: 1) Self-Assessment Survey (ASA). Data collection for the Self-Assessment Survey (SAS) activity is conducted using a questionnaire instrument, with a total of 10 farmer respondents collected to answer the questionnaire consisting of 5 assessments: work environment, personal protective equipment and safety, work performance, farmer’s health, and workplace hazards. 2) Village Community Assembly. Data collection for the Village Community Assembly activity is done through a Focus Group Discussion (FGD) with 35 study participants determined using a purposive sampling technique. 3) Establishment of occupational health effort posts. The implementation of the establishment of occupational health effort posts is done through a joint assembly with the village community and a facilitator team. Data analysis is conducted descriptively based on a Needs assessment. Musculoskeletal complaints, inadequate pesticide use for health requirements, and insufficient use of personal protective equipment were found.Results: The components of a needs analysis are based on considerations such as problem gaps, resource availability, support, and funding. The establishment of occupational health effort posts provides an appropriate platform for farmers to become acquainted with, prevent, early detect, and improve workers' health.Conclusion: Healthcare personnel needs to continuously provide support and health services through enhancement, guidance, and training to the farming community so that they can prevent and improve their health.

Abstract

Introduction: Stunting is a chronic malnutrition condition caused by a numeuros of factors, including inadequate maternal nutritional nutrition during pregnancy, inadequate exclusive breastfeeding, and infectious diseases. There are also some indirect factors .Objective: The objective of this study was to investigate the risk factors for stunting in children aged 6-36 months in the Sambirejo Health Center area. study. Method: This study is an observational analytic study using a cross-sectional approach that gathers data via a questionnaire. Sampling Techniques using the Non- Probability Sampling technique, and the sample size was estimated using the Lemeshow formula. The Prevalence Ratio (PR) was calculated to evaluate the strength of the link between the factors assumed to be linked with stunting. Result and Discussion: Of the 65 samples,  Four children were stunted, while 61 were not. Infants who were not exclusively breastfed, infants with a history of LBW, infants with a history of SBL, and family planning of 2 years both were risk factors for stunting in this study. Conclusions: Some variables in this study had no impact on the incidence of stunting. In this study, the risk factors for stunting were LBW, SBL, and infants without exclusive breastfeeding, and child spacing was a neutral factor for the occurrence of stunting in children aged 6-36 months.

Abstract

Urban fisherman have low to medium economic income. Limited income for fisherman becomes an economic burden. Fisherman consume cigarettes to the point of addiction, and thus do not prioritize family needs. The research objective was to analyze the economic burden of urban fisherman and compare the economic burden between smoker and non-smoker fisherman. The respondents were 50 smoker fisherman and 50 non-smoker fisherman in Medan Belawan. The research method uses the t test to see the difference in the economic burden of smoker and non-smoker fisherman. The results showed 76% of smoking fisherman believed that smoking was harmful to health. There is a significant difference between the economic burden between smoking and nonsmoking . The economic expense for a smoker is 42.8% or IDR846,400 of your monthly income. However, only 8% of fisherman have health complaints due to smoking. Smokers' priority places cigarettes in second place after basic needs. It is recommended that the government not stop providing knowledge about the evils of smoking to fisherman and stop the entry of cigarettes in fishing areas so it can reduce the economic burden on low-income people.

Abstract

MSME stands for micro, small and medium enterprises run by individuals and small business entities. Business development by MSMEs aims to build a national economy based on a just economic democracy. MSMEs play an important role in the Indonesian economy by expanding opportunities and employment, increasing the Gross Domestic Product (GDP), and providing a safety net, especially for people with low incomes to carry out productive economic activities. Therefore, there is a need for support for improving the welfare of MSMEs. Denai Bird's Nest as part of Deli Serdang Regency which is located in a coastal area, most of the villagers use processed marine products for consumption and used as material for creative economic activities. But the processing is still traditional. MSME business development must be carried out so that MSME actors can develop their business, namely through product variant development innovations, branding, and digital marketing. The mentoring process in the context of empowering MSME actors must be carried out in order to generate added value for MSME products. Greater added value can play a role in increasing economic growth and have an impact on increasing the welfare of MSME actors.

Abstract

A disease that has a high prevalence and reduces ` quality of life of sufferers is hypertension. Hypertension is also known as the silent killer, which sufferers often don't realize because there are no specific symptoms. Until now, hypertension is still a big challenge for Indonesia. The number of people with hypertension continues to increase every year and this disease is often found in primary health care. It turns out that in reducing hypertension, you can take advantage of plants that grow in the village which are rarely used by the local community, namely moringa leaf tea and lemongrass which grow along the area in the village near the bird's nest. The content of Moringa leaf tea with protein is two times higher than yogurt, vitamin A is seven times higher than carrots, potassium is three times higher than bananas, calcium is four times higher than milk, vitamin C is higher than oranges while lemongrass has a high citral content. capable of providing anti-inflammatory effects that are used to reduce hypertension

Abstract

Biogas is renewable energy that can be used as substitute of LPG and generator fuel. The utilization of waste into biogas has been implemented in Tadukan Raga Village, Sinembah Tanjung Muda (STM) Hilir District, Deli Serdang Regency since 2018 by building a facility for processing biomass into biogas by utilize rumen waste and cow dung at pilot scale. This facility has been handed over by the Ministry of Environment and Forestry to the community to manage independently. However, there is a problem in the lack of understanding of the community in the management, development and maintenance of this facility. This problem resulted in the abandonment of the biogas plant so that production and operations not running and some equipment requiring repairs. The purpose of this program is to strengthen and empower observer groups and users of biogas production facilities in Tadukan Raga Village. Therefore it is necessary to carry out: an analysis of the sustainability of the biogas plant program and the use of biogas as an alternative to overcoming livestock manure pollution in the program of Desa Binaan USU 2022, repair of biogas reactor and piping and the use of biogas stoves modified from LPG stoves.

Abstract

ABSTRACT In an effort to achieve universal health coverage in Indonesia, the government introduced the National Health Insurance program administered by BPJS Kesehatan as the single payer for healthcare services. This social insurance supports the utilization of healthcare services by the public, both in government-owned and private healthcare facilities. This study aims to identify factors influencing patient preferences in choosing between government-owned or private hospitals. This analytic survey research with a cross-sectional approach was conducted at Grandmed General Hospital (RSU Grandmed) and Drs. H. Amri Tambunan Regional General Hospital (RSUD Drs. H. Amri Tambunan). The study involved 393 patients, comprising 198 patients from RSU Grandmed and 195 patients from RSUD Drs. H. Amri Tambunan. Samples were selected using incidental sampling techniques. Data were collected using a questionnaire based on the valid and reliable Andersen healthcare utilization model. The statistical tests used in this study were the Chi square test and multiple logistic regression. The data showed no significant differences in predisposition and health behavior characteristics among patients choosing RSU Grandmed and RSUD Drs. H. Amri Tambunan (p>0.05). Additionally, differences between patients who choosing RSU Grandmed and RSUD Drs. H. Amri Tambunan were only observed in waiting times (p<0.001) and type of visits (p<0.05), while other aspects showed no significant differences (p>0.05). Logistic regression indicated that hospital choice was influenced by the availability of hospitals near the residence (OR: 3.047, p<0.01), the number of hospitals in the vicinity (OR: 1.554, p<0.05), type of visits (OR;0,618, p<0,05) and waiting time (OR: 2.138. p<0.001). It can be concluded that waiting time and type of visits are the only factors distinguishing patient who choose RSU Grandmed and RSUD Drs. H. Amri Tambunan. Since waiting time is a modifiable aspect by hospitals, it is highly recommended that both hospitals reduce waiting times by increasing service efficiency. Keywords: BPJS Kesehatan, Utilization, Government Hospital, Private Hospital  ABSTRAK Dalam rangka mewujudkan cakupan kesehatan semesta di Indonesia, pemerintah memperkenalkan program Jaminan Kesehatan Nasional yang dijalankan oleh BPJS Kesehatan sebagai pembayar tunggal pelayanan kesehatan. Jaminan sosial ini mendorong pemanfaatan layanan kesehatan oleh masyarakat baik di fasilitas kesehatan milik pemerintah maupun swasta. Penelitian ini bertujuan untuk mengetahui faktor yang berhubungan dengan preferensi pasien antara rumah sakit milik pemerintah atau swasta. Penelitian ini merupakan penelitian survei analitik dengan desain potong lintang yang dilaksanakan di Rumah Sakit Umum (RSU) Grandmed dan Rumah Sakit Umum Daerah (RSUD) Drs. H. Amri Tambunan. Populasi pada penelitian ini adalah seluruh pasien rawat jalan RSU Grandmed dan RSUD Drs. H. Amri Tambunan yang menggunakan BPJS Kesehatan dan sampel sebanyak 393 pasien yang terdiri dari 198 pasien RSU Grandmed dan 195 pasien RSUD Drs. H. Amri Tambunan digunakan sebagai sampel. Sampel dipilih dengan menggunakan teknik pemilihan insidental. Data pada penelitian ini dikumpulkan dengan teknik survei dengan menggunakan kuesioner yang valid dan reliabel yang disusun berdasarkan model pemanfaatan layanan kesehatan Andersen. Uji statistik yang digunakan pada penelitian ini adalah uji kai kuadrat dan regresi logistik binomial. Hasil penelitian ini menemukan bahwa tidak terdapat perbedaan karakteristik jenis kelamin, umur,  status perkawinan, suku, pendidikan, pekerjaan, pendapatan, jenis pembiayaan, moda transportasi, jarak tempuh ke rumah sakit, ketersediaan tenaga kesehatan, ketersediaan pemeriksaan penunjang, ketersediaan dan jumlah rumah sakit di sekitar tempat tinggal, klinik tujuan, proses administrasi, maupun alasan memilih rumah sakit di antara pasien yang memilih RSU Grandmed dan RSUD Drs. H. Amri Tambunan (p>0,05). Perbedaan antara pasien yang memilih RSU Grandmed dan RSUD Drs. H. Amri Tambunan hanya terlihat pada waktu tunggu (p<0,001) dan jenis kunjungan (p<0,05). Regresi logistik menunjukkan bahwa pilihan rumah sakit memiliki hubungan dengan ketersediaan rumah sakit di sekitar tempat tinggal (OR:3,047, p<0,01), jumlah rumah sakit di sekitar tempat tinggal (OR:1,554, p<0,05), jenis kunjungan (OR:0,618, p<0,05) dan waktu tunggu (OR:2,138, p<0,001). Dapat disimpulkan bahwa waktu tunggu dan jenis kunjungan merupakan faktor yang membedakan pasien yang memilih antara RSU Grandmed dan RSUD Drs. H. Amri Tambunan. Dikarenakan waktu tunggu merupakan aspek yang dapat dimodifikasi oleh rumah sakit, sangat disarankan pada kedua rumah sakit untuk menekan waktu tunggu dengan peningkatan efisiensi pelayanan. Kata Kunci: BPJS Kesehatan, Pemanfaatan, Rumah Sakit Pemerintah, Rumah Sakit Swasta

Abstract

ABSTRAKPelayanan kesehatan yang diterima peserta Jaminan Kesehatan Pemerintah dilakukan melalui mekanisme rujukan berjenjang. Sistem ini dikeluarkan untuk memenuhi aspek ketersediaan fasilitas kesehatan dan kepuasan peserta Jaminan Kesehatan Nasional. Sistem rujukan merupakan pelimpahan tanggung jawab terhadap masalah kesehatan secara vertikal  dan horizontal. Tujuan penelitian ini untuk menganalisis apa saja yang menjadi faktor/determinan penurunan utilisasi, Menganalisi ketersedi aan supplyside pelayanan kesehatan.Penelitian ini menggunakan metode kualitatif dengan pendekatan deskriptif eksploratif melalui observasi dan wawancara mendalam dengan informan. Penelitian telah dilakukan sejak 23 Januari hingga 25 Januari di Rumah Sakit Haji Medan.Dari hasil dan pembahasan penelitian ini dapat disimpulkan bahwa rendahnya utilisasi Rumah Sakit Haji Medan disebabkan oleh beberapa faktor utama, antara lain: masalah administrasi dan sistem seperti sistem rujukan, pilihan rumah sakit dan pilihan pasien mengenai citra pelayanan dan peralatan rumah sakit; keterbatasan sarana dan prasarana; sumber daya manusia yang tidak sesuai baik secara kuantitas maupun kompetensi. Disarankan agar rumah sakit dan pegawai mengupayakan dan meningkatkan pelayanan kompetensinya agar sesuai dengan standar dan mekanisme yang telah diterapkan.Kata Kunci    : Sistem rujukan berjenjang, sarana, prasarana
