Abstract

Background: The current transformation in the hospital care industry demands greater coordination of care across providers and across settings so as to improve quality care, improve outcomes, and reduce spending. Nurses contribute to the transformative changes by being a fully contributing member of the inter-professional team. It requires an enhanced set of knowledge, skills, attitudes, and performance among nurses around wellness and population care with a renewed focus on patient-centered care, care coordination, data analytics, and quality improvement. This study aimed to analyze the associations of ward head direction and incentive with nurse performance at Arun LNG Hospital, Lhokseumawe, Aceh.Subjects and Method: This was a cross-sectional study carried out at Arun LNG Hospital, Lhokseumawe, Aceh. A sample of 74 nurses was selected for this study by exhaustive sampling. The dependent variable was nurse performance. The independent variables were ward head direction and incentive. The data were collected by questionnaire and analyzed by a multiple logistic regression.Results: Nurse performance improved with high motivation (OR= 11.12; 95% CI= 2.66 to 46.49; p= 0.001), explicit ward head direction (OR= 4.37; 95% CI= 1.05 to 18.17; p= 0.043), and existence of incentive (OR= 5.84; 95% CI= 1.30 to 26.17; p= 0.021).Conclusion: Improved nurse performance is associated with explicit ward head direction and existence of incentive.Keywords: nurse performance, ward head direction, incentiveCorrespondence: Irwadi. Abstract

Background: Maternity waiting homes are residential facilities where women who live remotely can wait before giving birth at a hospital or health center. The aim of imple­menting maternity waiting for home strategy is to reduce maternal and perinatal mortality by improving access to skilled birth attendance and emergency obstetric care, particularly for women in rural and remote areas. This study aimed to examine the association between socialization and the use of maternity waiting home in East Aceh.Subjects and Method: This was a cross-sectional study carried out at four community health centers (puskesmas), in East Aceh, Indonesia. A sample of 130 pregnant mothers was selected for this study by stratified random sampling. The dependent variable was the use of a maternity waiting home. The independent variable was socialization and family support. The data were collected by questionnaire and analyzed by multiple logistic regression.Results: Socialization was associated with increased use of maternal waiting home (OR= 14.17; p= 0.002), after controlling from the effect of family support.Conclusion: Socialization is associated with increased use of maternal waiting home, after controlling for the effect of family support.Keywords: maternity waiting home, socialization, family support.Correspondence: Atika Triwulan Dari. Abstract

Background: Hand hygiene is an effective behavior to prevent infection among health workers. This study aimed to determine the associations of motivation and sanction on hand hygiene compliance among health workers at Dr. Pirngadi Hospital.Subjects and Method: A cross-sectional study was conducted at Dr. Pirngadi Hospital, Medan, North Sumatera, Indonesia. A sample of 33 health workers was selected for this study by total sampling. The dependent variable was hand hygiene obedience. The independent variables were sanction and motivation. The data were collected by questionnaire and analyzed by multiple logistic regression.Results: Hand hygiene compliance was associated with the presence of sanction (OR= 3.44; 95% CI= 1.22 to 4.95; p= 0.041) and motivation (OR= 4.77; 95% CI= 2.52 to 6.30; p= 0.029).Conclusion: Hand hygiene compliance is associated with sanction and motivation. Abstract

Obesity in children has become a global health problem throughout the world, either in the developed and developing countries. As for the prevalence of obesity in children is continuously increasing each year so that it becomes serious, complicated, and multicultural health problem. Children who are overweight and obese tend to remain obese when adults  and are likely to develop non-communicable diseases such as diabetes and cardiovascular disease at a younger (WHO, 2012). The objective of the research was to analyze the risk factor for the incidence of obesity in 9-12 year-old children at Harapan 1 Elementary School, Medan, in 2018. The research used analytic method with cross-sectional study with 59 samples. The sampling method was proportional random sampling. The data were gathered by conducting interviews, questionnaires, and direct observation and analyzed by using Chi-Square test and logistic regression test with an SPSS 24. The result of the research showed that 54.2% of the respondents were obese, and 45.8% of the respondents were not with the significant correlation of p=0.05. The risk factors of the incidence of obesity in school-aged children were   eating behavior (p=0.003 < 0.05), physical activity (p=0.008 < 0.05), exposure to television advertisements (p=0.015 < 0.05), and pocket money (p=0.0001 < 0.05). The result of logistic regression test showed that the variable which had the most dominant influence on the incidence of obesity was pocket money at Exp (β)=25.200. The variables of eating behavior, physical activity, exposure to television advertisements, and pocket money had the influence of 80.5% on the incidence of obesity. It is recommended that the school to monitor nutritional status of children through UKS (School Health Unit) in early detection of obesityObesitas pada anak telah menjadi masalah kesehatan global di seluruh dunia, baik di negara maju maupun negara berkembang. Adapun prevalensi obesitas pada anak terus meningkat dari tahun ke tahun, sehingga obesitas pada anak menjadi salah satu tantangan paling serius di bidang kesehatan masyarakat. Masalah obesitas pada anak merupakan masalah yang kompleks dengan penyebab multifaktorial. Anak yang mengalami overweight dan obesitas cenderung tetap mengalami obesitas ketika dewasa dan kemungkinan akan berkembang menjadi penyakit tidak menular, seperti diabetes dan penyakit kardiovaskular pada usia yang lebih muda (WHO, 2012). Tujuan dari penelitian ini adalah untuk menganalisis faktor perilaku berisiko terhadap kejadian obesitas pada anak usia 9 – 12 tahun di SD Harapan 1 Medan tahun 2018. Penelitian ini merupakan penelitian analitik menggunakan desain cross sectional dengan jumlah sampel sebanyak 59 responden. Teknik pengambilan sampel adalah Proportional random sampling. Data dikumpulkan dengan wawancara menggunakan kuesioner dan observasi langsung. Analisis data dilakukan dengan uji Chi-Square dan uji Regresi Logistik Berganda menggunakan Software SPSS 24. Hasil penelitian menunjukkan responden yang mengalami obesitas sebanyak 54,2% dan sebanyak 45,8% responden yang tidak mengalami obesitas. Faktor yang secara bermakna berhubungan (p<0,05) dan menjadi faktor risiko terjadinya obesitas pada anak usia sekolah adalah kebiasaan makan p (0,003 <0,05), aktivitas fisik p (0,008 <0,05), paparan iklan televisi p (0,015 <0,05),  dan uang jajan p (0,0001 <0,05). Hasil uji regresi logistik berganda diperoleh variabel yang paling dominan mempengaruhi kejadian obesitas adalah uang jajan dengan nilai Exp(B)= 25,200. Kebiasaan makan, aktivitas fisik, paparan iklan televisi dan uang jajan mempunyai pengaruh sebesar 80,5% terhadap kejadian obesitas. Selanjutnya, diharapkan pihak sekolah untuk memonitoring pertumbuhan dan perkembangan status gizi anak sekolah melalui Usaha Kesehatan Sekolah (UKS) yang telah ada sehingga dapat mendeteksi adanya obesitas sejak dini

Abstract

Background: The disease pain scale related to the health service in Madani Hospital was more than 1.5% in 2017. Nurse compliance with standard precaution has been proved to reduce the incidence of diseases related to health services. This study aimed to analyze characteristics and normative, formative and restorative supervision which influence the level of nurse compliance with standard precaution.Subjects and Method: This was an analytic observational study with a cross-sectional design. The study was conducted at Madani hospital in Medan, North Sumatera. A sample of 89 nurses in inpatient wards was selected by total sampling. The dependent variable was nurse compliance. The independent variables were nurse characteristics, normative, formative, and restorative supervisions. The data were collected by questionnaire and analyzed by multiple logistic regression.Results: The level of nurse compliance with standar precaution was good (66.3%). Nurse compliance with standar precaution increased with gender (OR= 4.71; 95% CI= 1.14 to 19.85; p= 0.032), normative supervision (OR= 6.00; 95% CI= 19.74 to 1.83; p= 0.001), and restorative supervision (OR= 25.23; 95% CI=2.90 to 219.35; p= 0.001).Conclusion: Gender and the implementation of normative and restorative supervision will increase the level of nurse compliance with standard precaution.Keywords: Nurses compliance, normative, formative, restorative supervisionCorrespondence: Astri Nurhandini. Abstract

Background: Patient safety management means to reduce unnecessary risks and adverse events to the minimum possible level while providing health care. The application of pati­ent safety management had not been maximal in some hospitals. It can cause health problems for patients and health workers. This study aimed to analyze the patient safety manage­ment in the in-patient room at Deli Hospital, Medan, North Sumatera.Subjects and Method: This was a qualitative study conducted in February to July 2018. The three informants were consisted of the head of quality improvement and patient safety, the patient safety team, and the nursing committee at Deli Hospital, Medan, North Sumatera. The data were collected by in-depth interview and observation. The data were analyzed using data reduction, data presentation, and conclusion.Results: The role of hospital organization in implementing management patient safety in the in-patient care had not been maximized. The efforts of increasing the commit-ment of health workers, working environment condi­tion, facility, implementation of standard ope­rating procedure (SOP), and implementing Situation, Background, Assessment, and Re­commendation (SBAR), and implementing sanction, had not been maximize. Communi­ca­tion was not effective. The ability of health workers to perform patient safety had not been maximal as seen from the understanding of a poor safety system. Adverse event report was still of manual type. Number of fall accidents in 2018 was 2 cases.Conclusion: Patient safety management in the in-patient care has not been maximized. Abstract

Abstract Work stress on nurses is a common problem that often occurs in the world of work. Nurses who care for patients with mental disorders tend to experience higher stress than general nurses. This study aimed to determine the relationship between workload and environment and work stress of nurses. This study used a quantitative cross-sectional approach. This study amounted to 105 mental nurses who served in inpatient with accidental sampling. The results of this study found that the workload of 5 respondents (4.8%) with a light workload, 75 respondents (71.4%) moderate, and 25 respondents (23.8%) heavy. The results of the univariate analysis found 34 respondents (32.4%) who reported that their work environment was in the moderate category and 71 respondents (67.6%) were good. The results of the univariate analysis showed that 6 respondents (5.7%) experienced mild stress, 54 respondents (51.4%) had moderate stress, and 45 respondents (42.9%) experienced severe stress. The results of the bivariate analysis with the Gamma correlation test found a positive significant relationship with a very strong relationship strength (p <0.05, r = 0.916) where as many as 51 respondents (68.0%) experienced moderate workload with moderate work stress and were found a negative significant relationship with a strong relationship strength (p <0.05, r = -0.607) where as many as 42 respondents (59.2%) experienced moderate stress in a good work environment. Researchers suggest that the availability of nurses must be adjusted and the workload given must be adjusted to their capacity, while the work environment requires additional lighting to carry out activities optimally, healthy, safe, and comfortable. Abstract

The purpose of this study is to analyze the relationship between responsiveness, assurance, and attention to patient family satisfaction in the Sub-Acute Room of the Aceh Mental Hospital in 2021. The type of research that will be used is a quantitative study. The research location chosen is the Aceh Mental Hospital, as the only public service institution for tertiary psychiatric services in the Aceh region. The population in this study are the families of patients who came to visit patients who were being hospitalized within 2x24 hours or more, and also the families of patients who came to pick up patients who had finished in sub-acute inpatient care after the day of treatment was completed at the hospital. The number of samples obtained is 59 people, so the researcher will take a sample of 60 patients' families within the planned one month research period as respondents to represent the existing population. This study was analyzed using the spearman correlation test. The results of the study show that there is a significant relationship between responsiveness with patient family satisfaction. There is a significant relationship between assurances with patient family satisfaction. There is a significant relationship between attentions with patient family satisfaction.

Abstract

The purpose of this study is to analyze the relationship between culture of initiative and culture of accuracy with performance of implementing nurses at Regional General Hospital dr. Djasamen Saragih, Pematang Siantar City. The type of research used is descriptive research with a quantitative approach. This research was conducted from March to July 2022 at Regional General Hospital dr. Djasamen Saragih, Pematang Siantar City. The population in this study are nurses who are in the Inpatient Agencies of Regional General Hospital dr. Djasamen Saragih, Pematang Siantar City is 116 people. The method of analysis in this study uses pearson correlation test. The results show that there is a significant relationship between culture of initiative and culture of accuracy with performance of implementing nurses at Regional General Hospital dr. Djasamen Saragih, Pematang Siantar City.

Abstract

Kepemimpinan merupakan suatu fenomena sosiologis untuk menggerakkan para pegawai dalam mencapai kinerja yang efektif dan efisien. Dibutuhkan motivasi kerja yang tinggi dalam mewujudkan kinerja pegawai yang maksimal. Peran pemimpin sangat besar untuk menciptakan suasana kerja dengan motivasi kerja yang tinggi. Praktek Manajemen kepemimpinan juga terjadi pada instalasi farmasi rawat jalan RSUD Abdul Manan Simatupang. Dengan tantangan beban kerja yang tinggi seorang pimpinan harus mampu memberikan pengaruh terhadap motivasi kerja bawahannya agar dapat memberikan performa kinerja yang maksimal. Penelitian ini bertujuan untuk mengetahui hubungan kepemimpinan dengan kinerja pegawai, hubungan kepemimpinan dengan motivasi kerja pegawai dan hubungan motivasi kerja dengan kinerja pegawai di instalasi farmasi rawat jalan RSUD Abdul Manan Simatupang. Desain penelitian ini adalah crossectional. Penelitian dilakukan melalui metode kuantitatif dan menggunakan analisis statistik Regresi Linier Berganda untuk mengetahui seberapa besar pengaruh variabel bebas terhadap variabel terikatnya. Penelitian ini menggunakan total sampling pada teknik pengambilan sampel. Berdasarkan hasil penelitian, dapat disimpulkan bahwa kepemimpinan berpengaruh signifikan terhadap kinerja pegawai. Hal ini diindikasikan oleh nilai t-hitung X1 (3.212) > t-tabel (2.00) dan p-value (0,002) <0,05. Dengan kata lain, semakin baik kepemimpinan semakin baik pula kinerja pegawai. Kepemimpinan memberi pengaruh signifikan secara positif terhadap variabel terikat Y (kinerja pegawai). Hal ini diindikasikan oleh nilai t-hitung X2 (6.995) > t-tabel (2.00) dan p-value (0,000) <0,05. Hal ini berarti bahwa semakin baik motivasi kerja, semakin baik pula kinerja pegawai. Peningkatan implementasi manajemen kepemimpinan akan meningkatkan motivasi kerja petugas. Peningkatan motivasi kerja akan memberikan peningkatan kualitas kerja pegawai. Kinerja pegawai dapat meningkat melalui pemberian motivasi yang diberikan oleh pimpinan di instalasi farmasi rawat jalan RSUD Abdul Manan Simatupang. Untuk meningkatkan kinerja karyawan, Pimpinan pada instalasi farmasi rawat jalan RSUD Abdul Manan Simatupang perlu meluangkan waktu untuk memberikan motivasi pada bawahannya.
