Abstract

ABSTRACT Vitamin D is one of the nutritional components needed by the human body. This vitamin plays a role in maintaining healthy bones, teeth and muscles because it functions in terms of the absorption of calcium and phosphate. The need and function of vitamin D are also very influential on the condition of pregnancy for both mother and fetus because the compound 1,25 dihydroxy vitamin D (1,25(OH)2D) plays a role in the maintenance of cell function in the placenta. Another function of vitamin D is maintaining the mother's immune system properly. Fulfilment of vitamin D should be gradual from early pregnancy, and vitamin D deficiency causes pregnancy disorders and complications such as pre-eclampsia, premature birth, and stunted fetal growth. Examination of vitamin D levels is not included in routine pregnancy checks, so this service activity screens Vitamin D levels in pregnant women as an initial effort to prevent pregnancy disorders that can be triggered by low levels of vitamin D. A total of 24 pregnant women have received explanations and are willing to follow this activity. The service team collaborates with the Tahtul Yaman Health Center and also the PRODIA Laboratory in helping with the implementation of this service. The results of the Vitamin D examination showed that plasma Vitamin D levels were more in the insufficiency and deficiency categories, and only a small proportion had normal Vitamin D plasma levels. Based on these results, vitamin D screening should be included as an antenatal care program, and a thorough follow-up study should be carried out hereafter. Keywords: Antenatal care, Pregnancy, Vitamin D ABSTRAK Vitamin D merupakan salah satu komponen nutrisi yang dibutuhkan oleh tubuh manusia. Vitamin ini berperan dalam menjaga kesehatan tulang, gigi dan juga otot karena memiliki fungsi dalam hal penyerapan kalsium dan fosfat. Kebutuhan dan fungsi dari vitamin D ini juga sangat berpengaruh pada kondisi kehamilan baik untuk ibu maupun janin, karena senyawa 1,25 dihidroksi vitamin D (1,25(OH)2D) berperan untuk pemeliharaan fungsi sel pada plasenta. Fungsi lain dari vitamin D adalah untuk menjaga sistem kekebalan tubuh ibu agar tetap melakukan fungsinya dengan baik. Pemenuhan vitamin D sebaiknya bertahap dari awal kehamilan, defisiensi Vitamin D berdampak pada timbulnya komplikasi kehamilan seperti preeklampsia, persalinan prematur, dan pertumbuhan janin terhambat. Pemeriksaan kadar vitamin D tidak termasuk dalam pemeriksaan rutin kehamilan, sehingga kegiatan pengabdian ini melakukan skrining kadar Vitamin D pada ibu hamil sebagai upaya awal pencegahan timbulnya gangguan kehamilan yang dapat dipicu oleh rendahnya kadar vitamin D. Sebanyak 24 orang ibu hamil telah mendapatkan penjelasan dan bersedia untuk mengikuti kegiatan ini. Tim pengabdian berkerja sama dengan puskesmas Tahtul Yaman dan juga Laboratorium PRODIA dalam membantu terselanggaranya pengabdian ini. Hasil pemeriksaan Vitamin D didapatkan kadar Vitamin D plasma lebih banyak dalam kategori insufisien dan defisien, sebagian kecil yang memiliki kadar plasma Vitamin D normal. Berdasarkan hasil tersebut, sebaiknya skrining Vitamin D dapat dimasukkan sebagai program pemeriksaan rutin kehamilan dan dilakukan kajian lanjutan secara menyeluruh. Abstract

ABSTRACT Background: Beyond controlling blood pressure and blood glucose, a genetic factor may play role in diabetic nephropathy (DN) progression in Type II Diabetes Mellitus (T2DM). Previous studies reported a genetic variant of Angiotensin rs699 (M235T) associated with diabetic nephropathy, with conflicting results across populations worldwide. To the best of our knowledge association of this genetic variant has never been conducted in the Jambi Malay population. Aims The Aims of this study is to the reveal association of the genetic variant of Angiotensin rs699 with diabetic nephropathy in the Jambi Malay population. Methods: This study design was cross-sectional. Totally 70 T2DM patients aged 22-67 years old were participated, as many 41 patients have aged older than 50 years old. Genotyping was performed using tetra ARMS PCR specific for rs699. Result: Patients aged older than 50 who have CTTT genotype have lower risk for suffering DN than CC genotype (p-value 0.029; OR (95% CI) 0.233 (0.06-0.88)), the statistic significant persist in multivariate analysis (p-value 0.030; OR (95% CI) 0.19 (0.04-0.86)). Conclusion: CTTT genotype was protective for DN in patients older than 50 years old. Further study with larger sample size, multi-centre and measuring confounding factors was needed. Keyword: Genetic variant of Angiotensin, rs699, diabetic nephropathy, Type II Diabetes Mellitus, Malay population ABSTRAK Latar belakang: Kendali Tekanan darah dan glukosa darah saja tidak cukup untuk mencegah kejadian nefropati diabetik (DN) pada Diabetes Mellitus Tipe II (T2DM), faktor genetik juga berperan penting. Studi sebelumnya melaporkan varian genetik Angiotensin rs699 (M235T) yang terkait dengan DN, dengan hasil yang berbeda antar populasi. Sejauh studi literatur yang dilakukan, varian genetik ini belum pernah diteliti pada populasi Melayu Jambi. Tujuan dari penelitian ini adalah untuk mengetahui hubungan varian genetik Angiotensin rs699 dengan nefropati diabetik pada populasi Melayu Jambi. Metode: penelitian ini merupakan penelitian potong lintang. Sebanyak 70 pasien T2DM berusia 22-67 tahun berpartisipasi, 41 diantaranya berusia lebih dari 50 tahun. Genotyping dilakukan dengan menggunakan tetra arms PCR spesifik untuk rs699. Hasil: Pasien berusia lebih dari 50 tahun yang memiliki genotipe CTTT memiliki risiko lebih rendah untuk menderita DN daripada genotipe CC (nilai p 0,029; OR (95% CI) 0,233 (0,06-0,88)), analisis multivariat juga menunjukkan hasil serupa (nilai p 0,030; ATAU (95% CI) 0,19 (0,04-0,86)). Kesimpulan: CTTT genotipe adalah pelindung untuk DN pada pasien yang lebih tua dari 50 tahun. Studi lebih lanjut dengan ukuran sampel yang lebih besar, berbagai pusat studi dan mengukur faktor risiko genetik dan non-genetik lain perlu dilakukan. Abstract

ABSTRACT Background: Flat feet is a foot deformity that refers to the loss of the mid-length of the foot. This can cause pain, stiffness, discomfort, and irritation in the ankles and feet. This study attempts to determine the relationship between obesity and the issue of flat feet in the academic community of the Faculty of Medicine and Health Sciences (FKIK) Universitas Jambi (UNJA) Medical Study Program. Method: This observational analytic research used a cross-sectional design with samples of 102 academics from the FKIK UNJA Medical Study Program. The primary data consisted of body weight, height, BMI, and soles of the feet. Bivariate analysis using the Chi-square test. Result: The incidence of flat feet is 20.6%, and 45,7% of flat feet are obese. There is an association between obesity and the incidence of flat foot in the academic community of the FKIK UNJA Medical Study Program. Conclusion: There is a significant relationship between body mass index and the incidence of flat feet in the academic community of the FKIK Jambi University Medical Study Program.

Abstract

Background: Pre-eclampsia (PE) is a hypertensive disorder in pregnancy and a significant cause of maternal and perinatal mortality and morbidity. Failure of spiral artery remodeling due to abnormal apoptosis, triggers disturbances in the mother and the baby’s growth. This study aimed to identify the profile of apoptotic marker genes and histopathological features of the placenta in pregnancies with pre-eclampsia.Methods: This study had used case-control method. Samples were taken from normal pregnancies (n=25) and pregnant women with pre-eclampsia (n=25) using a purposive sampling method from several hospitals in Jambi. qRT-PCR was used to examine apoptotic gene expression from placental tissue and hematoxyline eosin staining to view the placenta’s microscopic appearance. The targeted genes were BCL2-associated X (BAX) and B-cell lymphoma 2 (BCL-2). Histopathological changes of the placenta observed were syncytial node, cytotrophoblast, villous edema, hypervascularization, fibrosis stroma, atherosis, infarction, and thrombosis.Results: Relative BAX genes expression were increased once in placenta pre-eclampsia compared to controls, but not statistically significant (p-value>0.05). There was no difference between the decline of BCL-2 gene expression in pre-eclampsia placenta compared to the control (p-value >0.05). Histopathological changes in the placenta were syncytial node and cytotrophoblast (25 of 25), villous edema (19 of 25), hypervascularization (24 of 25), fibrosis stroma (22 of 25), atherosis (12 of 25), infarction (17 of 25), and thrombosis (24 of 25).Conclusion: The expression of BAX genes in pre-eclampsia tends to increase compared to normal pregnancy, and the expression of BCL-2 decreases.  The histopathological features of pre-eclampsia pregnancy placenta are mostly syncytial nodes, cytotrophoblasts, stromal fibrosis, and thrombosis.

Abstract

Anatomy education has historically been facilitated by cadavers; therefore, students always need a mentor at all times of study. Along with the dense curriculum, students are required to be able to study independently so various media are needed that can facilitate the student learning process.  This study aimed to determine the differences in the level of student knowledge of the anatomical learning materials provided through anatomy atlas, learning videos, and augmented reality (AR) in first-year medical students. This study was quasi-experimental. A total of 100 students became participants in this study that were then divided into 3 groups. Data were collected by giving pre-test and post-test to the student directly. The results of this study showed that the anatomy atlas and augmented reality significantly enhanced students’ knowledge in anatomy learning (p=0,004, p=0,000) but there was no significant difference between the three media towards the increasing student scores(p=0,14).

Abstract

Background: In early 2020, the Coronavirus disease 2019 became an epidemic and called as a pandemic. One of the risk factors related to COVID-19 is blood type. There are some groups who are at a higher risk of exposure to the corona virus and experience more severe degree of severity. This study aims to determine the relationship between ABO blood type and the severity of COVID-19 infection at H Abdul Manap Hospital, Jambi City.Methods: This is an observasional analytic study using cross-sectional approach. Data were collected from medical records of COVID-19 patients from May-December 2020 with a final total of 89 samples, then they would be tested using Chi Square test to get the desired results.Results: The results showed that male patients were higher (53.9%) than female (48.1%). Based on the age range, the highest group was found in 46-59 years (32.6%). Patients with blood type A are more dominant (44.9%). The severity was mostly in the mild-ordinary category (64%). There was no significant association between ABO blood types and the severity of COVID-19 (p=0.94). Conclusion: There was no association between ABO blood types and the severity of COVID-19 statistically.

Abstract

ABSTRACT Diabetes Mellitus (DM) type 2 is a common metabolic disorder characterized by hyperglycemia. The most frequent microvascular complication in diabetes patients is diabetic nephropathy. Dietary management is one of the approaches in DM treatment. Intermittent fasting as a dietary intervention has been shown to improve overall health, weight management, blood pressure, and glomerular filtration rate (GFR). This study aims to investigate the effects of intermittent fasting on urea and creatinine levels in Wistar rats with a diabetes model. This study is a quantitative experimental analysis conducted in vitro in a laboratory setting, using a pretest and post-test group design. There was no difference in urea and creatinine levels before and after the intermittent fasting diet treatment in Wistar white rats. The one-way ANOVA and Kruskal-Wallis tests indicated no significant differences among the groups, as evidenced by the statistical test results with p-values > 0.05 (not significant). Abstract

ABSTRACT Background: Bone markings must be studied and mastered in learning skeletal system anatomy. The short and interesting material in the pocketbook helps students identify preparations during the practicum, increasing their understanding and gaining practical grades. Objective: This research aims to determine the effectiveness of using the Skeletal System pocketbook on grades practical anatomy block of the Integumentary System and Locomotion for medical students in the first semester of the Universitas Jambi in 2023. Method: This research used a quasi-experimental method with a pre-test and post-test control group. The 106 sample students were from the Medical Study Program at Universitas Jambi class of 2023. Result: The mean pre-test and post-test grades for students with pocketbooks were 61.86 and 78.04, while those without pocketbooks were 63.74 and 71.35. In the mean pre-test grade, there was no significant difference between the experimental and control groups' mean grades (p-value = 0.488). Meanwhile, in the mean post-test grades, there was a significant difference between the two groups (p-Value = 0.005), where the experimental group had a higher mean grade. Conclusion: There is a significant difference in using the skeletal system anatomy pocketbook on the grade of anatomy practicum for medical students at Universitas Jambi. Abstract

Background: The PE is leading cause of maternal and neonatal mortality and morbidity worldwide. The etiology of PE remains not yet clearly identified, genetics contributed 20-40% of PE. The variation of the rs4343 gene was significant as a risk factor for PE and the severity of PE with effect sizes varying between studies. To the best of our knowledge, the association of rs4343 in the ACE gene for PE risk has not been reported to the population of Indonesia, especially the Jambi Malays. Aims: This study aims to reveal the association of Genetic Variation of ACE gene rs4343 with preeclampsia. Methods: This recent study was case-control, 78 pregnant women matching based on age and pregnancy number participated in this study. The preeclampsia diagnosis is based on the criteria of the American College of Obstetricians and Gynecologists. The genotyping method was tetra ARMS-PCR. Bivariate statistical analysis was performed. Results: The proportion of the AA genotype and AG genotype was slightly different between the two groups, but all GG genotypes suffered preeclampsia. The recessive model reported the GG/AG genotype has a higher risk of PE than the AA genotype (p-value 0.784; OR 1.16, 95% CI (0.40-3.41)). The allele model reported that the G allele (p-value 0.338; OR 1.59, 95% CI (0.62-4.14)) had a higher risk than the A allele. However, all those differences do not reach statistically significant. Conclusion: Genetic Variation of ACE gene rs4343 Has Lack Association with Pre-eclampsia.

Abstract

Prospective studies indicate that the Atherogenic Index of Plasma (AIP) is a superior marker compared to isolated assessments of triglycerides (TG) and High-Density Lipoprotein (HDL) in predicting Coronary Heart Disease (CHD). Simpang Sungai Duren Community Health Center serves a large population with a high proportion of productive-age and obese individuals. Routine metabolic screening for CHD risk stratification in obese residents has been hindered by limited facilities and healthcare financing systems. This community service initiative aimed to assess AIP for CHD risk stratification in obese individuals. Participants included residents aged 19–64 years with a body mass index (BMI) ≥25 kg/m², no recent lipid-lowering medication use (within 3 months), and an 8–10 hour fasting period. AIP scores (logarithm of TG/HDL ratio) were used to stratify CHD risk. Results were communicated to participants alongside health promotion leaflets. Among 61 participants, AIP-based screening identified 14.8% as high-risk, 26.2% as moderate-risk, and 59% as low-risk for CHD. Modifiable cardiometabolic risk factors in the high-risk group revealed 100% with central obesity and hypertriglyceridemia, and 50% with hypertension and/or low HDL levels. The high-risk cohort exhibited multiple cardiometabolic risks requiring comprehensive management.
