Abstract

Berdasarkan hasil Riskesdas (2018), prevalensi kasus TB di Indonesia sebanyak 0,42% dari total seluruh provinsi di Indonesia. Jawa Barat masuk pada peringkat ke-3 untuk prevalensi kasus TB Paru tertinggi. Masalah yang dihadapi pasien tuberkulosis dalam pengobatan adalah waktu pengobatan tuberkulosis yang lama, dan banyak pasien tuberkulosis yang sudah bosan minum obat, serta 10,06% pasien tuberkulosis berhenti berobat karena merasa sudah sembuh. Tujuan dari penelitian ini adalah untuk mengetahui Hubungan mekanisme koping dengan kepatuhan minum obat pada penderita TB Paru di Puskesmas Sukra Kabupaten Indramayu. Penelitian ini menggunakan  metode  korelasional, dengan sampel penelitian sebanyak 30 orang yang diambil secara total sampling dari semua semua  pasien  penderita  TB  paru  yang  tercatat  di  Puskesmas  Sukra  Kabupaten  Indramayu, pada tahun 2021. Hasil penelitian menunjukan Responden penderita TB Paru di Puskesmas Sukra Kabupaten Indramayu mempunyai Mekanisme koping yang adaptif sebesar 57% dan 43 % dalam kategori maladaptif. Responden penderita TB Paru di Puskesmas Sukra Kabupaten Indramayu Kepatuhan penderita TB paru mempunyai kepatuhan yang tinggi sebesar 47%, kepatuhan sedang 23% dan kepatuhan rendah 30%.  Hasil analisis Chi-Square dengan hasil p-value sebesar 0,001 lebih kecil dari nilai signifikan 0,05 (p-value <0,05). Kesimpulan dari penelitian menunjukan terdapat Hubungan Mekanisme Koping dengan kepatuhan minum obat pada penderita TB Paru di Puskesmas Sukra Kabupaten Indramayu. Saran diberikan pada keluarga untuk memberikan dukungan yang maksimal pada penderita TB paru terutama yang sedang menjalankan terapi pengobatan, sehingga responden mempunyai koping yanga daptif dan patuh minum obat untuk mendukung kesembuhan

Abstract

Hypertensive emergency is a hypertensive crisis characterized by a sudden increase in blood pressure of 180/110 mmHg without organ damage. Hypertensive disease has increased every year due to low awareness of compliance in taking medication and maintaining a healthy lifestyle in hypertensive patients. The purpose of this case study is to provide nursing care to patients experiencing hypertension urgency. This research method uses a qualitative approach with case studies as the main method, using observation sheets, interviews, and documentation studies. This case study chose one patient as the research subject, namely a patient with hypertensive urgency in the emergency room of Gunung Jati Hospital, Cirebon City. Nursing care management is based on meeting the needs of patients carried out in 1 day. The results of the case study showed that after being given nursing care there was a decrease in blood pressure from 195/110 mmHg to 187/104 mmHg. The conclusion is that the nursing care provided is effective in emergency hypertension patients and meets the needs of patients. The implication of this study is the importance of nursing care in managing hypertension emergencies, which can help in the effective monitoring and management of this disease. In addition, this study also strengthens the understanding of the importance of patient compliance in managing hypertension to prevent complications that can arise.

Abstract

Diabetes mellitus (DM) is a chronic disease that has increased significantly in the last two decades, especially in the elderly population. This study aims to design and implement a comprehensive health education and diabetic wound care program for the elderly in urban communities. Methods. This study involved 30 elderly people who suffer from type 2 diabetes mellitus and live in the Padang Pasir Village area. The research instruments consisted of a knowledge questionnaire on diabetic wound care and a foot care skills observation sheet, which had been tested for validity and reliability in a previous study. Data was collected through questionnaires that were completed before and after the intervention. Results. The average overall knowledge score increased from 57.9  10.5 at the pre-test to 82.6  8.4 at the post-test, with a difference of +24.7 points. The knowledge aspect of foot wound prevention showed the highest increase, from 58.0  9.8 to 85.5  7.5, which reflects that after education, the elderly are more aware of the importance of preventive measures such as the use of appropriate footwear and regular foot examinations. Statistical analysis using paired t-test showed that the increase in knowledge and skills of the elderly after education was significant (p = 0.000). Conclusion. The educational intervention designed in this study was statistically and clinically effective, and can be recommended for wider implementation as part of a strategy to improve the quality of life of elderly people with diabetes.
