Abstract

Introduction: Little do we understand about factors associated with functional disability in instrumental activities of daily living among the elderly living at home. This study aimed to explore the prevalence and determinant factors of functional disability in instrumental activities of daily living (IADL) among the elderly living in a rural area in Bali. Method: Using a cross-sectional design, 1,053 elderly aged 60 years and above living in a rural area in Bali were recruited. A structured interview by trained data collectors was conducted. Questionnaires used were the Lawton IADL Scale, the 5-item of Geriatric Depression Scale, and the adoption of questionnaires for vision, hearing, and communication problems from the Washington Group Short Set of Questions on Disability. A logistic regression model was applied to explore determinant factors. Results: Findings indicated that 26.1% of participants aged 75+ and 52.3% were female. Eight percent (8%) experienced functional disability in IADL, 1.9% lived alone, 11.8% indicated depression, 5.1%, 4.7%, and 1.6% of them had vision loss, hearing impairment, and communication problems, respectively. The strongest determinant factor for functional disability in IADL was depression (OR 7.869; 95% CI 4.657-13.296), followed by age (OR 4.602; 95% CI 2.764-7.663), and hearing impairment (OR 2.903; 95% CI 1.190-7.083). Conclusions: Depression is the strongest determinant factor for functional disability in IADL. Health professionals in rural areas should actively screen for depression to increase the ability of the elderly to fulfill their IADLs. Keywords: aged, depression, epidemiology, Indonesia

Abstract

ABSTRAK Desa Tribuana merupakan salah satu desa yang terletak  di Kecamatan Abang. KabupatenKarangasem, Bali, Indonesia. Sampai saat ini, penyuluhan dan pemeriksaan dini jenis golongandarah pada anak sekolah dasar di Desa Tribuana, Kecamatan Abang Karangasem belum pernahdilakukan. Transfusi darah dari golongan yang tidak kompatibel dapat menyebabkan beberapareaksi transfusi imunologis dan aspek klinis seperti ketidakcocokan pada sistem golongan darahABO dan Rhesus. Luaran dari pelaksanaan kegiatan ini adalah peningkatan pengetahuanmengenai jenis golongan darah sistem ABO dan Rhesus di kalangan masyarakat. Sampelkegiatan ini adalah siswa-siswi kelas 4,5, dan 6 Sekolah Dasar Negeri 2 Tribuana Tahun Ajaran2016/2017. Metode pelaksanaan meliputi persiapan ijin, koordinasi intern pihak STIKES Balidengan mahasiswa serta alat dan bahan yang digunakan. Pemeriksaan meliputi pemeriksaangolongan darah sistem ABO dan Rhesus. Hasil pemeriksaan golongan darah diperoleh dari 63orang yang terdiri dari 35 orang laki-laki dan 28 orang perempuan dengan rincian golongandarah A+, B+, AB+, dan O+ berturut-turut berjumlah 15, 24, 2 dan 22 orang. Kata kunci : Golongan darah, ABO, Rhesus, SDN 2 Tribunana, Abang, Karangasem ABSTRACTTribuana is a village which is located at Abang district, Karangasem regency, Bali, Indonesia.Counseling and early examination of blood type to the group of elementary students atTribuana village, Abang district, Karangasem regency has not been done before. Bloodtransfusion from incompatible types can cause an immunological transfusion reactions andclinical aspects such as the incompatibility of the blood type systems ABO and Rhesus. The aimof this activity was to improve the community knowledge about blood type systems ABO andRhesus. This examination followed by elementary students at grade 4,5 dan 6  of ElementarySchool number 2 Tribuana in academic year 2016/2017. The Actions method consisted ofpermits, internal coordination between Stikes Bali and students also tools and materials used.The examination included blood type systems ABO and rhesus. The test result showed that from63 students, there were 35 male students and 28 were female students. There were 24 studentswho have B blood type, 22 students with O blood type, 15 students with A blood type and only 2students have AB blood type. Abstract

Ambulance services are becoming a trend for early response to emergencies in major cities. The performance of the ambulance service will increase in line with the population and possible problems in the area. EMS services are essential at this time, as any life-threatening condition requires immediate action to prevent disability and death. Ambulances are required to provide assurance to chronically ill patients that the service they provide is adequate and can handle them in case of deterioration. This study aims to describe the frequency of pre-hospital ambulance use in patients with chronic diseases. This research method is an observational study with a retrospective approach to find the frequency of ambulance use by chronic diseases during the period 2019-2021. The results showed the three highest cases of pre-hospital ambulance use in Denpasar City were accident, fire and evacuation services for sick patients. The use of prehospital ambulances by chronic diseases has not reached half of the ambulance services. Stroke, diabetes mellitus and heart disease are the three most common chronic diseases that use pre-hospital ambulance services.

Abstract

Previous studies have shown that slow deep breathing techniques provide benefits in reducing anxiety in general, but there has been no specific review investigating its benefits in inpatient populations. Objective: To provide strong and up-to-date evidence on the effectiveness of slow deep breathing technique as a potential intervention in reducing anxiety in the inpatient population. Methods: This study is a literature review conducted during April-May 2023 using Google Scholar, PubMed, and ScienceDirect. The keywords used were "nafas dalam", "kecemasan", "rawat inap" in Indonesian, and "slow deep breathing," "anxiety," "inpatient care" in English. Results: The literature review identified eight articles that examined the anxiety of patients undergoing surgery and treatment in hospitals in Indonesia. These articles highlighted internal and external factors influencing anxiety. The dominant research approach employed experimental designs with pre-test and post-test measurements involving more than ten respondents. The study found that internal factors influencing anxiety included age, gender, education, health condition, and prior hospitalization experiences, while external factors included access to information, therapeutic communication, environment, and healthcare facilities. Conclusion: The implementation of slow deep breathing techniques is a major factor in reducing anxiety in inpatient populations. Further research is expected to emphasize the use of slow deep breathing techniques as a primary intervention in reducing anxiety levels in patients prior to undergoing surgery.

Abstract

Aims: As people get older, numerous cardiovascular disorders, including hypertension, which many older people have, will become more prevalent. Music therapy, as a non-pharmacological therapy, can be used to reduce blood pressure and anxiety in hypertensive patients. Objective: This literature review aims to determine the effectiveness of types music therapy that can reduce anxiety and blood pressure in the elderly with hypertension. Methods: To conduct a type of literature review, 10 articles that discuss the effectiveness of music therapy in lowering blood pressure and anxiety in hypertensive patients were searched for. The databases used are Pubmed, Google Scholar, and Science Direct, by entering keywords. The inclusion criteria used for data collection were full text, type of research (RCT, quasi-experimental, and pre-experimental), in English and Indonesian, and journal publishing from 2018 to 2023. Result: Ten relevant articles, including 505 respondents, were extracted in this study. Most of the studies (70%) showed significant results, showing that music therapy (traditional, classical, and spiritual) was effective in reducing high blood pressure and anxiety. Favorite music has a stronger effect than foreign music, such as traditional music, which has a deeper emotional impact on the elderly. Conclusion: Music therapy is effective in reducing blood pressure and anxiety in the elderly, and besides that, it is cost-effective and easy to use. Further research on the application of traditional music types is needed to determine the effectiveness of the music.

Abstract

The elderly is likely to experience chronic diseases because there is a decrease in the function of organs in the body. An increase in life expectancy will correspond to an increase in the number of chronic disease sufferers, because every one elderly person has one or more chronic diseases. It is important for the elderly to understand the emergency conditions that may be experienced, because fluctuations in the condition of the elderly can occur in a short time. This Community Service aims to increase the understanding of the elderly regarding emergency conditions in chronic diseases. The method used is through counseling and pre and post tests to determine the understanding of the elderly. After the test was carried out, it was found that there was an increase in the average value of the participants' knowledge of emergency conditions in chronic diseases after being given counseling. With an increase in knowledge, it is hoped that patients will be able to perform first aid and find the right place for further help if chronic disease patients experience emergency conditions at home.

Abstract

Lansia didefinisikan sebagai seseorang yang memiliki usia lebih dari 60 tahun. Adanya peningkatan dalam usia angka harapan hidup mengindikasikan adanya peningkatan jumlah lansia. Seseorang yang telah berada pada usia lansia dari survey memiliki lebih dari satu jenis penyakit kronis. Dua penyakit kronis yang ditemukan sering pada lansia yaitu diabetes melitus dan hipertensi. Lansia yang mengalami permasalahan penyakit kronis dan diperkirakan akan mengalami peningkatan setiap tahunnya. Dua penyakit teratas tersebut disebutkan angka kematian dan angka kesakitan yang tergolong tinggi di dunia, serta juga dikatakan sebagai penyakit dengan peringkat teratas di Indonesia. Kegiatan Pengabdian Kepada Masyarakat memiliki tujuan untuk mendeteksi penyakit kronis pada lansia yaitu diabetes melitus dan hipertensi melalui pemeriksaan Berat Badan, tinggi badan, indeks masa tubuh, tekanan darah dan gula darah sewaktu. Metode dalam pengabdian masyarakat ini dilakukan melalui pemeriksaan kesehatan terbagi menjadi tiga proses kegiatan yaitu persiapan, pelaksanaan dan evaluasi. Hasil analisis menunjukkan responden yang menjadi sasaran kegiatan pengabdian kepada masyarakat memiliki usia yang tergolong lanjut usia. Responden memiliki IMT pada katagori berat badan berlebih. Responden kegiatan mayoritas memiliki tekanan darah lebih dari 140 mmHg dan memiliki kadar gula darah sewaktu kurang dari 200 mg/dL dari pemeriksaan glukometer. Responden pada kegiatan ini belum dapat didiagnosis memiliki hipertensi ataupun diabetes melitus sehingga diperlukan pemeriksaan lanjutan yang lebih akurat.

Abstract

ABSTRAK  Menopause merupakan masa yang penting bagi kehidupan wanita. Seiring memasuki menopause disertai pula dengan kemunculan gejala-gejala menopause atau menopausal symptoms. Masa menopause dan menopausal symptoms yang muncul seringkali menimbulkan ketakutan dan kecemasan pada wanita usia subur yang belum memahami mengenai menopause. Kegiatan Pengabdian kepada Masyarakat ini bertujuan untuk meningkatkan penetahuan wanita usia subur di desa Werdi Bhuwana, Kecamatan Mengwi Badung terkait menopause dan menopausal symptoms. Kegiatan diawali dengan memberikan pre-test bagi peserta kemudian edukasi melalui penyuluhan mengenai menopause dan menopausal symptoms dan diakhiri dengan diskusi serta post-test. Hasil dari kegiatan Pengabdian kepada Masyarakat ini adalah terdapat peningkatan pengetahuan pada wanita usia subur mengenai menopause dan menopausal symptoms. Dari 30 peserta sebanyak 26 (86.67%) peserta memiliki pengetahuan yang baik terkait menopause dan menopausal symptoms dan hanya 4 (13.33%) peserta saja yang pengetahuannya masih kurang setelah diberikan edukasi. Semakin baik pemahaman wanitau usia subur mengenai menopause maka akan semakin baik penerimaan mereka terhadap perubahan yang dialami ketika memasuki masa menopause sehingga akan lebih bijak serta siap menghadapi masa menopause. Tenaga kesehatan memiliki peran penting dalam mengidentifikasi kebutuhan pengetahuan yang diperlukan oleh wanita usia subur ketika memasuki masa menopause dan rutin memberikan edukasi terkait menopause sehingga kecemasan dan ketakutan wanita usia subur ketika memasuki masa menopause dapat teratasi. Kata Kunci: Menopause, Gejala-Gejala Menopause, Edukasi, Pengetahuan  ABSTRACT  Menopause is an important time in a woman's life. Women who are menopause it is accompanied by the onset of menopausal symptoms. Menopause and menopausal symptoms that appear often cause fear and anxiety in women who have not yet understood about menopause. The aim of this study was to increase knowledge of women in the Desa Werdi Bhuwana, Mengwi, Badung related menopause and menopausal symptoms. The activity begins with pre-testing the participants, then educating them about menopause and menopausal symptoms and ends with discussions also post-tests. As a result of this study was there has been an increase in knowledge among women about menopause and menopausal symptoms. Of the 30 participants, 26 (86.67%) had good knowledge of menopause and menopausal symptoms, and only 4 (13.33%) had insufficient knowledge after being educated. The better women understand menopause, the better their acceptance of the changes they experience when they enter the menopausal period, the wiser and more prepared they will be to cope with it. Health professional plays an important role in identifying the knowledge needs of women when entering menopause and regularly providing menopausal-related education so that the anxiety and fears of women e as they enter menopauses can be overcome. Keywords: Menopause, Menopausal Symptoms, Education, Knowledge

Abstract

ABSTRACT Breast cancer and cervical cancer are the most common cause for death in women. The prevalence of breast and cervical cancer are increasing every year. Chemoterapy is usual treatment for cancer which can affect sleep quality. Quality of sleep is important to identify where can impact patient’s quality of life. The purpose of this study was to determine prevalence of poor sleep quality and the domains that contribute to it. This study used quantitative descriptive approach with 80 breast and cervical cancer patients undergoing chemotherapy. Pitssburgh Sleep Quality Index (PSQI) was used to measured the quality of sleep, a score of >5 indicates poor sleep quality. The data was analyze using univariate analysis. Most of the respondent (70%) have poor sleep quality. In the sleep quality domain, the most complaining problem were sleep disturbance (51.2%) and sleep latency (46.2%). Poor sleep quality is a significant problem experienced by patient breast and cervical cancer undergoing chemotherapy. The quality of sleep must be regularly assesed in order to provide appropriate interventions and treatments.  Keywords: Breast Cancer, Cervical Cancer, Chemoteraphy,Sleep Quality, PSQI  ABSTRAK Kanker payudara dan kanker servik merupakan jenis kanker yang menjadi penyebab utama kematian dalam kasus kanker dengan jumlah penderita yang meningkat setiap tahunnya. Kemoterapi menjadi salah satu terapi bagi pasien kanker yang menimbulkan efek samping penurunan kualitas tidur. Kualitas tidur penting untuk diidenfifikasi karena memilik peran penting dalam kualitas hidup pasien. Mengetahui prevalensi gangguan tidur dan domain yang berperan dalam penurunan kualitas tidur. Penelitian menggunakan pendekatan deskriptif kuantitatif pada 80 pasien kanker payudara dan kanker servik yang menjalani kemoterapi. Kualitas tidur diukur dengan kuesioner Pitssburgh Sleep Quality Index (PSQI) dengan skor >5 menunjukan kualitas tidur yang buruk. Data dianalisis teknik analisa data univariate untuk variable yang diteliti. Sebanyak 70% responden melaporkan kualitas tidur yang buruk. Pada domain kualitas tidur responden melaporkan kualitas yang buruk pada gangguan tidur malam (51.2%) dan kesulitan memulai tidur atau latensi tidur (46.2%). Penurunan kualitas tidur merupakan permasalahan yang signifikan dialami oleh pasien kanker payudara dan kanker servik yang menjalani kemoterapi. Kesulitan tidur yang dialami sebaiknya dikaji secara rutin sehingga intervensi dan treatment yang sesuai dapat diberikan untuk meningkatkan kualitas tidurnya Kata Kunci: Kanker Payudara, Kanker Servik, Kemoterapi, Kualitas Tidur, PSQI

Abstract

Background: The level of medication adherence among the elderly with hypertension is decreasing globally, including in Indonesia. Medication adherence is a major factor in treatment effectiveness for hypertensive patients. The Health Belief Model is one of the theories illustrating factors influencing patients’ medication adherence. Purpose: This study aimed to analyze determinant factors of medication adherence among the elderly with hypertension based on the application of the health belief model. Methods: This is a descriptive-analytic study with a cross-sectional approach. Sampling used a proportional stratified random sampling technique involving 193 patients with hypertension recruited from one of the Regional General Hospitals in Denpasar City Center. Data were obtained using three questionnaires, namely demographic characteristics, the health belief model (HBM), and the Morisky Medication Adherence Scale (MMAS-8). Data were analyzed using binary logistic regression. Results: The research results showed the majority of respondents (62.7%) were compliant in taking medication. Multivariate test results showed that perceived susceptibility (p <0.001; AOR: 5.163), perceived benefits (p <0.001; AOR: 12.061), self-efficacy (p <0.001; AOR: 3.213), perceived barriers (p <0.001; AOR: 0.133) and cues to action (p <0.001; AOR: 3.480) is related to medication adherence in the elderly with hypertension. Conclusions: Perceived benefit is the strongest determining factor of medication adherence among the elderly with hypertension. Respondents with high belief in perceived benefits tend to adhere to medication 12.061 times compared to respondents with low belief in perceived benefits. Health workers are advised to increase the elderly's belief in the perceived benefits and perceived susceptibility to increase their medication adherence.
