Abstract

Background: Ankle sprain is a musculoskeletal injury with the highest incidence rate in the world. Risk factors for ankle sprain are age, gender, sport, anatomical abnormalities, and body mass index (BMI). There is still contradiction as to whether gender is a risk factor for ankle sprain. Furthermore, there has been no research on ankle sprain regarding members of the Police Force as a risk factor. Objective: This research aims to determine the relationship between patient profile and acute ankle sprain at Pusdokkes Polri Bhayangkara Tertiary Hospital. Methods: This study took 124 research subjects, with consecutive sampling analysis, was an observational analytic study with a cross sectional research design involving patients diagnosed with ankle sprain in 2022 – 2023 at Pusdokkes Polri Bhayangkara Tertiary Hospital who met the inclusion and exclusion criteria. Result: As many as 79% of the subjects were aged 19 – 44 years (p=0.333). It was found that 52.4% of the subjects were male (p=0.944). As many as 46% of the subjects were members of the Police Force (p=0.181). Conclusion: Age, gender, and occupation had no significant association with the incidence of acute ankle sprain in this research. Male police officers aged 45 – 59 years have a higher risk of experiencing acute ankle sprain. The implication of this study is to prevent at-risk groups and/or ankle sprain patients.

Abstract

Background: The human gut microbiota plays a unique role in metabolism, immunity, and nutrient absorption.  Various studies on the gut microbiota have identified a link between changes in bacterial abundance in the gut known as dysbiosis (imbalance) and the pathogenesis of certain disorders. It is known that an imbalance in the ratio of the phyla Firmicutes and Bacteroidetes has been associated with various chronic diseases, including inflammatory bowel disease and metabolic disorders. The Firmicutes/Bacteroidetes (F/B) ratio is a measure used to assess gut microbiota composition, which is believed to play a significant role in intestinal health and metabolism. Studies suggest that a higher F/B ratio is often associated with obesity and other metabolic conditions. Objective: This study aims to carry out metagenomic analysis of third-trimester pregnant women's faecal samples to predict degenerative diseases using third-generation whole genome sequencing, namely PromethION nanopore technology.  Method: Nanopore Technology: DNA sequencing of faecal samples from pregnant women in the third trimester was performed using the Oxford Nanopore Technology (ONT) device based on the Ligation Sequencing gDNA - Native Barcoding Kit 24 V14 and following its guidelines. Result: Based on the quality and quantity of total DNA reads from 20 stool samples from pregnant women, only 4 samples were continued for analysis (1A, 2A, 3A and 4A). The results of the abundance of intestinal microbiota based on the most abundant phylum are Proteobacteria, Bacteriodetes, and Firmicutes. Conclusion: Meanwhile, based on species abundance, it shows that Prevotella copri (phylum Bacteriodetes) is abundant in samples 2A, 3A and 4A, while in sample 1A the most abundant species is Succinivirio dextrinosolvens (phylum Proteobacteria). Bacteroidetes are a group of Gram-negative bacteria that are often found to increase in individuals with type 2 diabetes and obesity

Abstract

Abstract Background: Knee osteoarthritis (OA) is a globally alarming condition that is closely related to mechanobiological factors. Based on several previous studies, it is known that knee OA is correlated with knee injury and that play an important role in the expansion of knee OA. Objective: This research aims to find out the relationship between knee injury history with knee OA severity and to estimate the risk of knee OA after knee injury. Methods: This research used observational analytical methods with a retrospective hospital-based cohort study design. Using consecutive sampling technique. The total research sample was 68 participants who Indonesian National Police Mobile Brigade Corps had knee OA period January 1 2021-December 31 2023 at the RS Bhayangkara Tk. I Pusdokkes Polri Jakarta. Result: There was a significant relation between knee injury history with knee OA severity (p = 0.011) and odds ratio 5.0 (95% CI: 1.6-15.9). Characteristics of members of the Indonesian National Police Mobile Brigade Corps with the highest risk of knee OA, namely in the pre-elderly with an age range of 45 - ≤ 59 years (66.2%), female (51.5%), work unit of the Headquarters Mobile Brigade Corps (45.6%), had a history of knee injury (70.6%), with mild degrees (1-3) (73.5%), and the majority experienced anterior cruciate ligament (ACL) injury (23.5%).  Conclusion: The study concluded that there was a statistically significant relation between knee injury history with knee OA severity and that the risk of developing knee OA increased fivefold after knee injury.

Abstract

Background: Schizophrenia is a chronic psychiatric disorder characterized by significant cognitive impairments, including deficits in attention, processing speed, and executive function. Brain-Derived Neurotrophic Factor (BDNF) is known to play a key role in neuroplasticity and cognitive processes; however, its relationship with cognitive function in schizophrenia remains unclear. Objective: this research aims to determine correlation between serum BDNF levels and cognitive function  with Trail Making Test. Methods: A cross-sectional study was conducted on 50 schizophrenia patients. The population is schizophrenia patients at at Reksodiwiryo Padang Hospital Padang. The sampling method is a consecutive sampling.  The statistical test used is Spearman’s correlation. Results:   A significant proportion of participants demonstrated cognitive impairments (50% in Trail Making Test-A (TMT-A) and 60% in Trail Making Test-B), no statistically significant correlation was found between serum BDNF levels and TMT scores (TMT-A: r = -0.073, p = 0.612; TMT-B: r = -0.263, p = 0.065). Conclusion: This research concluded there was no statistically significant correlation between BDNF levels and TMT A and B. The findings suggest that serum BDNF may not directly correlate with cognitive impairments in schizophrenia, spesificly executive function and attention, as assessed by the TMT.

Abstract

Background: Flooding often leads to malnutrition among children under five, driven by limited access to nutritious food and unsanitary conditions. Sidomulyo Village in Batu City, prone to flooding due to the Paron River overflow, has faced challenges affecting the nutritional status of children. Objective: this research aims to analyze the nutritional status and its determinants using the CIAF Index for children under 5 years old affected by flooding in Sidomulyo Village, Bumiaji District, Batu City, Indonesia. Methods: This analytical observational study involved 20 pairs of mothers and children under five residing in Sidomulyo Village. Anthropometric measurements were conducted by calculating their z-scores and classifying their nutritional status using the Composite Index of Anthropometric Failure (CIAF). Interviews were conducted to gather data on maternal, child, and sanitation characteristics. Data analysis was performed using SPSS version 23.0, including descriptive analysis and Chi-square tests to examine relationships (P < 0.05). Results: The findings revealed that 68.75% of toddlers in Sidomulyo had a normal nutritional status. Factors influencing nutritional status included maternal characteristics (maternal height, education level, and family income) and child-related factors (frequency of food consumption). Conclusion: The study concluded that most toddlers affected by flooding in Sidomulyo have a normal nutritional status. Also, the determining factors of children’s nutritional status are maternal height, education level, family income and frequency of food consumption. The results of the study can be used as a reference for further research with the aim of maintaining children’s nutritional status in this area.

Abstract

Background: Adolescent idiopathic scoliosis is a spinal deformity characterized by Cobb angle deviation of more than 10o. Objective: This study aims to determine the association between low body mass index (BMI) and curve magnitude in adolescent idiopathic scoliosis. Methods: This was a retrospective cohort study using medical record data of patients diagnosed with scoliosis at Bhayangkara Tk I Pusdokkes Polri Hospital. BMI classification was based on WHO BMI-for-ages z-scores in children and adolescents. Result: The study included 22 AIS subjects, with the majority aged 14–17 years and a higher proportion of females than males. The p-value for the association between low body mass index and adolescent idiopathic scoliosis is 0.169. Conclusion: Curve magnitude is not associated with low body mass index.

Abstract

Background: Primary Sjögren syndrome is chronic systemic autoimmune disorder characterized by lymphocytic infiltration of the exocrine glands, most commonly presenting with dryness of the mouth and eyes. This case is very rare, which the antibody in the Sjogren's syndrome case concerns platelets. Objective: This case report emphasizes the occurrence of epistaxis in Sjögren Syndrome. Case Presentation: This case describes a 30-year-old male presenting with recurrent epistaxis as the initial clinical manifestation. Further clinical evaluation revealed hypertensive crisis and dryness in the mouth and nasal mucosa. The clinical presentation raised suspicion of an underlying autoimmune condition, prompting further testing, which confirmed the presence of anti -Sjögren’s syndrome type B (SSB) antibodies on an antinuclear antibody (ANA) profile.  Result: The criteria for Sjogren's syndrome are based on the criteria if eye and mouth symptoms, eye and mouth clinical signs, and one of autoantibodies are found Anti-Ro (SSA), Anti-La (SSB), Antinuclear antibodies (ANA), Rheumatoid factor (RF). From this case has found symptoms of mouth and antibodies SSB. Conclusion: This case highlights that epistaxis can be caused by autoimmune cases, for epistaxis it is better to do screening for autoimmune examination test, emphasizing the need for further research to elucidate the diverse clinical manifestation, progression and prognosis factor.

Abstract

Background: Bladder cancer ranks as the second most prevalent urinary tract cancer globally, following prostate cancer, accompanied by a significant mortality rate, and cause several complications, for example weight loss, urinary tract infections, metastases to other organs, and can cause chronic kidney failure due to obstruction of urine flow Objective: This case presented the challenging progression of muscle invasive bladder cancer with chronic kidney disease complications and refuse therapies as a clinical lesson for case management. Case Presentation: This case reports the clinical course of a 78 year old man who presented to our hospital's urology clinic presenting with symptoms of hematuria, along with urinary dripping and discomfort. Result: The patient was diagnosed with muscle invasive bladder carcinoma, with a complication of chronic kidney disease, and refused therapies. Conclusion: Controlling bladder cancer that has invaded to the muscular layer and chronic kidney disease was difficult and give poor prognosis linked to their age, type of bladder cancer, and refusal to undergo therapies.

Abstract

Background: Internal mammary artery (IMA) is the preferred conduit for coronary artery bypass grafting (CABG) due to its superior long-term patency. However, its harvesting disrupts sternal vascularization, potentially leading to impaired sternal healing and complications such as sternal wound infection (SWI). This review aims to synthesize current evidence on how IMA harvesting affects sternal healing after CABG, with a focus on surgical techniques, patient risk factors, and postoperative complications. Objective: This review aims to provide a comprehensive synthesis of current evidence regarding the impact of IMA harvesting on sternal healing following CABG. Methods: A review was conducted using the PubMed and ScienceDirect databases, including studies published from 2015 to 2025. Studies published before 2015 were considered if they provided foundational knowledge and were relevant. Randomized controlled trials (RCTs), propensity studies, and observational studies analyzing IMA harvesting techniques and sternal healing were included. The primary outcomes assessed were SWI, non-union, and delayed healing. Results: A total of 13 studies met the inclusion criteria, comprising 6 observational studies, 3 propensity-matched studies, and 4 RCTs. Bilateral IMA grafting was associated with an increased risk of SWI, especially in diabetic and obese patients. However, skeletonized IMA harvesting was linked to improved sternal perfusion and reduced wound complications compared to pedicled techniques. Additionally, interventions such as continuous insulin infusion and low-level laser therapy showed promise in enhancing sternal healing outcomes. Conclusion: IMA harvesting impacts sternal healing, particularly in high-risk patients undergoing bilateral IMA grafting. Skeletonized harvesting techniques and optimized perioperative care can mitigate the risk of complications.

Abstract

Background: Acute myeloid leukemia (AML) is a heterogeneous hematologic malignancy characterized by the clonal proliferation of immature myeloid precursor cells in the bone marrow and peripheral circulation. Hematologic manifestations such as anemia, thrombocytopenia, and leukocytosis are welldocumented, its association with oral involvement, particularly tonsillitis, is a rare presenting feature. Objective: This case report emphasizes membranous tonsilitis as the primary presenting feature in acute monocytic leukemia (AML-M5). The aim of writing this case study is to provide an illustration that the causes of membranous tonsillitis, beside from the infection, can also be caused by infiltration of leukemia cells. Case Presentation: We report a case of a 27-year-old female presented with fever and was found to have a fluctuating mass on the right side of the neck along with gingival swelling and hyperemic, hypertrophic tonsils with a membranous exudate. The diagnosis from bone marrow aspiration and immunophenotyping revealed as acute monocytic leukemia (AML-M5). Bacterial and fungal infections were excluded through gram staining, potassium hydroxide (KOH) examination, and throat swab culture. The patient was initially stabilized with hydroxyurea, with subsequent clinical improvement, before being referred to a tertiary center for definitive management. Result: This diagnostic workup altogether from all the clinicians leads to the diagnosis of acute myeloid leukemia (AML-M5). Conclusion: This case highlights primary tonsilitis as the primary presenting feature of acute myeloid leukemia (AML-M5), emphasizing the need for continued monitoring and regular follow-up assessment to detect potential relapses and ensure optimal disease management
