Abstract

Background: Beta thalassemia major is a genetic disorder that leads to abnormal hemoglobin production, requiring regular blood transfusions. A common complication is iron overload, which potentially damaging vital organs including the lungs. Oxidative stress due to iron overload is suspected to impair pulmonary function via lipid peroxidation. Objective: This study aims to analyze the correlation between serum ferritin levels and the peroxidative index with pulmonary function in pediatric patients with beta thalassemia major undergoing routine transfusions at RSUD Ulin Banjarmasin. Methods: This study was an analytical observational study with a cross-sectional approach involving children aged 6–18 years. Measured parameters included serum ferritin, peroxidative index (H2O2 to catalase ratio), and pulmonary function spirometry (FEV1/FVC ratio). Result: The Pulmonary dysfunction was identified in 68.85% of subjects, predominantly restrictive (65.57%). Mean ferritin level was 5,423.23±3,139.96 µg/L and mean peroxidative index was 46.42±66.38. There was no statistically significant correlation between serum ferritin (p = 0.239) or peroxidative index (p = 0.147) with pulmonary function. Conclusion: Although elevated ferritin and peroxidative index were observed, no significant association was found with pulmonary dysfunction. Monitoring these parameters may aid in early detection and management of pulmonary complications in thalassemia.

Abstract

Background: Ovarian cysts are common in women of reproductive age and can vary from simple functional cysts to large benign or malignant neoplasms. Giant ovarian cysts are rare and may contribute to infertility due to mechanical and hormonal disruptions. Objective: To report the diagnosis and management of a giant ovarian cyst in a young woman with long-standing primary infertility, emphasising the importance of individualised surgical planning for fertility preservation. Case presentation: A 26-year-old nulligravida woman presented with progressive abdominal distension over three months. Imaging showed a 22 cm cystic adnexal mass with suspicious features and elevated CA-125 (84 U/mL). The patient underwent exploratory laparotomy and bilateral salpingo-oophorectomy. Histopathological examination confirmed a benign serous cystadenoma. The patient recovered well and was referred for fertility counselling. Conclusions: Early evaluation and careful management of large ovarian cysts are crucial in reproductive-age women with infertility. Although tumor markers and imaging guide diagnosis, histopathology is definitive. Surgical decisions must balance oncologic safety and fertility potential to optimise patient outcomes.

Abstract

Background: Giant congenital melanocytic nevus (GCMN) is a rare condition present at birth, with an estimated incidence of fewer than 1 in 20,000 live births. While smaller congenital melanocytic nevi are common, GCMN poses significant clinical concerns due to its potential malignant transformation into melanoma. Early recognition and long-term monitoring are crucial to mitigate risks. Objective: This case report aims to highlight the clinical presentation, diagnostic approach, and management challenges of GCMN to emphasize the importance of timely intervention. Case Presentation: A 24-year-old male presented with a progressively enlarging blackish facial lump that bled upon scratching. Laboratory tests indicated leukocytosis, and histopathology confirmed tumor growth in the epidermis with papillomatous architecture and hyperkeratosis, leading to a diagnosis of GCMN in the fronto-nasal region. Result: The histopathological findings confirmed GCMN, underscoring the need to its malignant potential. Despite its benign appearance, the lesion’s progressive growth and bleeding tendency warranted close monitoring. Conclusion: GCMN, though rare, requires early detection, appropriate treatment, and lifelong surveillance to prevent malignant transformation. Delayed intervention, often due to cosmetic concerns, may increase complications. Thus, a multidisciplinary approach involving regular follow-ups is essential for optimal patient outcomes.

Abstract

Background: Facial skin care is needed to maintain and preserve moisture and elasticity of the facial skin. The use of organic masks can be used to treat and provide nutrition to the facial skin to overcome facial skin problems. Objective: The purpose of this study was to measure effectiveness of the combination of moringa leaves mask (Moringa oleifera L.) and honey with butterfly pea flower mask (Clitorea ternatea L.) on facial skin moisture. Methods: The research is an analytical experimental study, 30 women aged 40-50 years old were given a combination of moringa leaves mask and honey on the right side of the face and butterfly pea flower mask and honey on the left side of the face, then before using the mask and after using the mask, the moisture value of the facial skin was measured with a skin moisture analyzer. The treatment in this study was given twice a week for three weeks, from April to May 2025 at the Wijaya Surabaya doctor's practice and clinic, using statistical analysis of the Pearson Correlation test with a p value of 0.000 (α <0.005). Result: The combination of moringa leaves and honey experienced an increase in moisture value of 70% moist skin and 30% normal skin, and the combination of butterfly pea flower mask and honey experienced an increase of 60% moist skin and 40% to normal skin. Conclusion: The combination of moringa leaves mask and honey with butterfly pea flower mask and honey only has a difference of 10%, meaning that the use of both combinations of these masks is equally useful in increasing moisture in facial skin.

Abstract

Background: The liver's presence of fat has a histological picture with lipids>5%, and no secondary cause is called NAFLD. Rhizophora apiculata can serve as a therapeutic or prophylactic agent in NAFLD due to its flavonoid content. Objective: to ascertain the impact of Rhizophora apiculata extract administration on SGOT levels in white rats subjected to dexamethasone induction. Methods: The experimental animals were categorized into four groups: K- group, no treatment; K+ group, which was administered dexamethasone at a dosage of 5 mg/kg BW from the 15th to the 21st day; KP1 group, which received Rhizophora apiculata ethanol extract at a dosage of 56 mg/kg BW from the 8th to the 21st day, along with dexamethasone at 5 mg/kg BW from the 15th to the 21st day; and KP2 group, which was given Rhizophora apiculata ethanol extract at 56 mg/kg BW and dexamethasone at 5 mg/kg BW from the 15th to the 21st day. Result: The data were not normally distributed (p < 0.05), so the Kruskal-Wallis test was carried out, which yielded p < 0.05. Consequently, the post hoc test was continued, namely the Mann-Whitney U test. The post hoc test of the K-group and K+ obtained p < 0.05. The K+ group and KP1 and KP2 showed P > 0.05, indicating no significant difference. Conclusion: The Rhizophora apiculata extract at a dose of 56 mg/kg BW for 7 days can reduce SGOT levels in the Kp2 group.

Abstract

Background: Basil seeds are a traditional spice with the name Ocimum basalicum L. Basil seed extract contains secondary metabolite compounds such as alkaloids, flavonoids and polyphenols which have the potential to act as antibacterials for Salmonella typhi. The aim of this research was to determine the antibacterial effectiveness of basil seed extract (Ocimum basalicum L) against the growth of Salmonella typhi bacteria. Objective: the aim of the research. Methods: The experimental research method was a post test only control group design using the well diffusion method. Result: The research results showed that the diameter of the inhibition zone formed at successive concentrations of 10%, 20%, 40% and 80% was 27.5 mm, 30.3 mm, 35.6 mm, 40.7 mm against Salmonella typhi. In the One-way Anova test, a p value of 0.000 <0.05 was obtained for the inhibition of the growth of Salmonella typhi bacteria. Conclusion: There is a potential anti-microbial effect of Salmonella typhi bacteria with an optimal concentration of 80%.

Abstract

Background: The routine use of postoperative therapeutic antibiotics after laparoscopic cholecystectomy remains controversial due to limited evidence of clinical benefit and concerns over antimicrobial resistance. Objective: To evaluate the effect of postoperative therapeutic antibiotics on surgical site infections (SSI) and laboratory outcomes after laparoscopic cholecystectomy. Methods: A quasi-experimental, post-testonly study with a non-equivalent control group was conducted at RSUP Prof. Dr. I.G.N.G. Ngoerah (September 2023–March 2024). Fifty-six patients were divided into two groups: postoperative antibiotics (cefoperazone followed by cefixime) versus no antibiotics. SSI was assessed on postoperative days 7 and 30 using the ASEPSIS scoring system. Laboratory parameters (leukocytes, neutrophils, lymphocytes, hemoglobin, and platelet counts) were compared between groups. No SSI occurred in either group. Laboratory outcomes showed nostatistically significant differences between the antibiotic and non-antibiotic groups (p > 0.05). Results: The mean age of participants was 45.2 ± 15.1 years, with female predominance. Conclusion: Postoperative therapeutic antibiotics did not reduce SSI rates or alter laboratory outcomes followinglaparoscopic cholecystectomy. Routine use is not supported in mild to moderate cases, and limiting unnecessary antibiotics may help reduce antimicrobial resistance.

Abstract

Background: Prolonged stress commonly induces specific coping strategies, including the adaptive problem-focused and the less adaptive emotion-focused coping. One example of emotion-focused coping is smoking. Objective: This study aimed to characterize the stress experienced by medical students and how they correlate with smoking behaviors. Methods: This descriptive research employed the Depression, Anxiety, Stress Scale – Stress Domain (DASS-S) and Silvan-Tomkin’s smoking behavior questionnaires as guides for semi-structured interviews. Five all-male medical students aged 19-22 years who were regular smokers were interviewed. Result: One respondent admitted the presence of significant stress. Despite the absence of significant stress, other respondents admitted occasional tension due to academic stressors. Two respondents associated smoking with obtaining positive emotions, while three associated smoking with coping with negative emotions. Conclusion: There is no clear correlation between stress and smoking behavior Future research might benefit from obtaining data from a larger sample number and considering the timing of the occurrence of stress when inquiring about smoking behavior to better establish the characteristics of stress and its correlations with smoking behavior.

Abstract

Abstract Background: Tuberculous spondylitis is the most common form of skeletal TB that can cause joint damage and disability if left untreated. Several studies have discussed risk factors associated with the severity of TB spondylitis. However, there are differences in findings among these studies, especially regarding age, sex, body mass index, immunocompromised conditions, and pulmonary TB history. Early detection through risk factor screening and adequate treatment can be useful to prevent disease severity. Objective: This research aims to find out the association between risk factors and the severity of TB spondylitis using GATA classification. Methods: Observational analytical methods with a retrospective hospital-based cross-sectional study design were used. Using consecutive sampling technique. The total research sample was 50 patients diagnosed with TB spondylitis between January 1, 2022-December 31, 2024 at Gatot Soebroto Army Hospital Jakarta. Result: The characteristics of TB spondylitis patients were mostly adults (68%), male (54%), had normal BMI (42%), had immunocompromised conditions (52%), and had a history of pulmonary TB (58%), with severe TB spondylitis (42%). Bivariate analysis showed a significant association between BMI (p=0.033) and immunocompromised conditions (p=0.047) and no significant association between age (p=0.482), sex (p=0.356), and history of pulmonary TB (p=0.469) with the severity of TB spondylitis. Conclusion: A significant association was found between BMI and immunocompromised conditions with the severity of TB spondylitis. However, there is no significant association between pulmonary TB history with the severity of TB spondylitis.Keywords: risk factors, tuberculous spondylitis, severity

Abstract

Background : Cancer is a leading global health challenge, driven by various proteins that promote malignant cell growth—one of which is HER2, a transmembrane receptor tyrosine kinase in the ErbB family. TAK-285 is a known HER2 inhibitor. Recently, natural compounds like quercetin have gained attention for their ability to suppress HER2 overexpression and offer therapeutic potential. Objective: This study is to compare the binding affinity between quercetin and the reference inhibitor TAK-285 with the HER2 protein, as well as to analyze the molecular interaction patterns of HER2 using In Silico study. Through this, the study seeks the determine the potential similarity in the inhibition mechanisms of HER2 quercetin and TAK-285. Methods: Experimental study using In Silico docking methods, was performed using AutoDock Vina via PyRx 0.8 to evaluate involving docking interactions between quercetin and HER2, as well as TAK-285 and HER2. Results : The results showed that quercetin binds to HER2 with a binding energy of -8.3 kcal/mol, while TAK-285 shows a stronger affinity with -9.0 kcal/mol. Despite having a slightly lower binding affinity, quercetin successfully interacts with key residues in the HER2 binding pocket and demonstrates potential as a natural HER2 inhibitor.Conclusion: These findings suggest that quercetin may serve as a promising lead compound for HER2-targeted therapy and support further investigation through in vitro and in vivo studies.
