Abstract

Disease cost analysis is an important element in the decision making process of chronic disease, because it can evaluate the cost of illness and illustrate diseases that require an increase in resources for the prevention or therapy. This study aims to determine the total cost of diabetes mellitus (DM), so it can be used as a health fund planning considerations related to the effectiveness and efficiency of medication costs for patients in Yogyakarta Regional Public Hospital. The method used is observational prevalence. The data were obtained retrospectively with patients primary diagnosed and secondary diagnosed of diabetes mellitus with or without complications in the period January to June 2014 in Yogyakarta Regional Public Hospital. The Financial perspective used is the hospitals perspective as health care providers to take into account the direct medical costs by using a bottom-up approach. Data analysis was performed using Kruskal-Wallis and Mann-Whitney Whitney to determine differences in the direct medical costs of outpatient diabetic patients in terms of age, the number and type of diabetes complications, as well as to determine the direct medical cost differences hospitalized diabetic patients in terms of age, the number of complications, type of diabetes, duration of hospitalization and inpatient classes.The results of the research showed that the patients who entered the inclusion criteria as much as 1.689 episodes of visits, which consisted of 1.531 episodes of outpatients visits and 158 episodes of inpatients visits. The total cost of DM based on hospital perspective Rp1.203.799.389,00 IDR within six months. There is a significant difference in direct medical costs in outpatients in terms of the number of factors of complications (p=0.004) and a significant differences direct medical costs inpatients in terms of the number of factors of complications, length of stay and inpatient class in hospitalization (p=0.000).

Abstract

Ketoprofen is included in the Class II of Biopharmaceutical Classification System (BCS) which has low solubility. Low solubility will affect the dissolution rate and the dissolved concentration, so the absorption and bioavailability are low as well. Several studies have been conducted to improve the solubility and the dissolution of drugs from these group, such as by solid dispersion system. This study aims to determine the effect of polyvinyl alcohol (PVA) on the dissolved concentration of ketoprofen in solid dispersion and in physical mixture, and to decidethe optimum formula.Solid dispersion and physical mixture of ketoprofen - PVA were formulated with the ratio of 1:1, 1:2 and 1:4, and then compared with the standard ketoprofen. Evaluation of solid dispersion was performed by the intervention test of PVA as a matrix on the maximum wavelength of standard ketoprofen using a UV-Vis spectrophotometer and the dissolution test in artificial gastric fluid media without pepsin using a basket stirrer at pH ± 1,2, the temperature of 37 ± 0.5ºC, and device speed of 50 rpm. The sample was collected at 10, 20, 30, 45, and 60 minutes. The amount of dissolved ketoprofen was determined by a UV-Vis spectrophotometer at a wavelength of 260 nm. The results showed that there was no shifting on maximum wavelength point in both solid dispersions and physical mixtures. The dissolved ketoprofen concentration that was represented in solid dispersion was greater than in physical mixture and standard ketoprofen. The highest dissolved ketoprofen concentration was indicated in solid dispersion formula with the ratio of 1:1.

Abstract

Pamelo citrus is a fruit plant having the potential to be developed in Indonesia. One of the centers for producing large oranges or Pamelo citrus in South Sulawesi is Pangkep Regency. One of the components of the Pamelo citrus is pectin. Pectin is a polysaccharide compound with a high molecular weight which is widely found in plants and it can absorb metals because they contain carboxylic groups. This study aimed to analyze pectin from albedo pamelo citrus fruit as an adsorbent for heavy metal plumbum (Pb), cadmium (Cd) and copper (Cu). The method used was atomic absorption spectrophotometer with the equation analysis of heavy metal adsorption by pectin of pamelo citrus determined by Langmuir and Freundlich adsorption isothermal method. The results showed that pectin of albedo pamelo citrus following the isothermal of Freundlich adsorption which showed the adsorbent capacity of Pb, Cd and Cu with the KF values 0.561 mg/g, 0.010 mg/g and 0.066 mg/g, respectively. Meanwhile, n values were 0.823 for Pb, 0.321 for Cd and 1.121 for Cu, and in commercial pectin following the isothermal of Freundlich adsorption with the KF values of Pb, Cd and Cu were 0.650 mg/g, Cd 0.015 and Cu of 0.77 mg/g, respectively. Then the n values were 0.641 for Pb, 0.811 for Cd and 1.183 for Cu.

Abstract

Dissolution and solubility rates are very important parameters in designing a pharmaceutical dosage form, especially for oral drug administration. Oral drugs that have low dissolution rates often require high doses loading to improve the absorption and effectiveness in order to achieve therapeutic concentration. The increasing of drug dose is a less safe alternative solution, therefore researchers have developed physics, chemistry, and other modification in order to increase dissolution rate. These methods such as salt formation, prodrug formation, particle size reduction (micro-crystallization), co-grinding, crystal modification, micellar solubilization and complex formation, solid dispersion and self emulsifying. This review paper will focus on different methods that will be compared with the present of surfactant in these methods. The present of surfactants was able to overcome the limitation of each methods with a mechanism for reducing surface tension, micellar formation, reducing contact angle and increasing of wetting behaviour. Therefore, the present of surfactant in modification of drug dosage form could be considered to increase dissolution rate in order to achieve therapeutic effect.

Abstract

Beta-beta wood (Lunasia amara Blanco) is one of the Rutaceae plants. It has been used traditionally in Indonesia, both in the form of a single extract or in a mixture of several herbs, for the treatment of swollen feet, skin diseases and inflammation or irritation of the eyes. This study aims to examine the antioxidant activity and the phytochemical profile of beta-beta wood extract. The research methods include plant extraction by maceration, testing of antioxidant activity by 2,2-diphenyl-1-picrylhydrazyl (DPPH) method and phytochemical profiling with specific chemical reagents. The results showed that beta-beta wood extract (Lunasia amara Blanco) has antioxidant activity with IC50 was 69.46 μg/ml (0.069 mg/ml) and it contained steroid, phenolic, saponin, alkaloid and coumarin compounds.

Abstract

Lycopene is a carotenoid group easily damaged due to the oxidation process (light, oxygen and temperature) and isomerization during storage. This damage can be minimized by microencapsulation processes. The objective of this study was to develop novel microencapsulation of lycopene extracts from tomato fruit by solvent evaporation method using ethyl cellulose as wall materials and to select the optimum formulation. Three microcapsule formulations were prepared containing the ratio of lycopene and ethyl cellulose (L:ES) of 1:1, 1:2 and 1:3. The morphology of the microcapsules was analysed by optical microscopy and scanning electronic microscopy. The encapsulation efficiency, particle size, recovery yield and moisture content were also examined. The result showed that all microcapsule formula were aggregated and irregular in shape with encapsulation efficiency of 6.13- 19.43%, moisture content of 1,63- 7,52%, recovery yield of 81-98,12% and particle size of 46,2-86µm. Microcapsule with a ratio 1:3 (L:ES) was the most optimum formula based on a maximum encapsulation efficiency than the others

Abstract

First-line drugs (Isoniazid and Rifampicin) for the treatment of tuberculosis are known to have experienced resistance to Mycobacterium tuberculosis. The aim of this study was to determine the minimum inhibitory concentration (MIC) of maltodextrin encapsulated rosella calyx extract and its ability to provide a synergistic effect with Isoniazid (INH) and Rifampicin (RIF) on M. tuberculosis R37rv. Rosella calyxs were macerated using 50% ethanol and encapsulated using maltodextrin. Antibacterial activity was carried out by determining MIC value using Microscopic Observation and Direct Susceptibility (MODS) method. The synergistic effect was carried out by calculating the Fractional Inhibition Concentration Index (FICI). The results showed that this extract was able to inhibit M. tuberculosis H37rv with MIC of 10 mg/ ml. The FICI value of the combination of extract with INH and rifampicin was obtained 1.25. This showed that rosella calyx extract is not synergistic with INH and rifampicin, might be due to rosella calyx extract has an antibacterial effect with a different mechanism with INH and Rifampicin.

Abstract

Antidotes drug is indicated in intoxication or poisoning patients, in addition to the antidote drugs, it can also be used as an appropriate therapeutic indication of his own medicine. The high increasing number of the antidote drug used every year in inpatient instalation Undata hospital, for exmple atropine, N-acetylcysteine and pyridoxine (vitamin B6). Therefore, this study aims to determine the percentage of drug use of antidote category indicated as an antidote therapy in Undata hospitals Central Sulawesi Province period 2016-2018. This is a descriptive study with retrospective approach and using purposive sampling techniques.The results showed that the sample size was 75 patients, with the percentage of drug use as antidotes category in Undata Hospital Central Sulawesi province period 2016-2018, namely pyridoxine (vitamin B6) of 49.33%, N-acetylcysteine drug of 42.66% and the atropine drug of 8%. The use of antidotes drug category are indicated as an antidote to the poisoning therapy drugs obtained 0%. It can be concluded that there is no antidote drug category that used as an antidote therapy on patients with drug poisoning in inpatient instalation Undata hospital Central Sulawesi Province Period 2016-2018.

Abstract

Meatballs are processed meat products that very popular in Indonesia. Making meatballs is not just using animal material only, but also can be used plant materials such as oyster mushrooms. The meatballs can be made in various forms, such as a round shape or a box shape. This study aims to determine the nutrient content (moisture, ash, fat, protein, and carbohydrates) in several formula meatballs box of oyster mushroom (Pleurotus ostreatus) and find the best formula on the meatballs box oyster mushrooms, the nutrients (fat, protein, and carbohydrates), as well as the ash and moisture content. The study was designed using completely randomized design (CRD), which consists of six treatments and two replications. Parameters measured were as hedonic organoleptic include color, aroma, flavor and texture, using 15 panelists and nutrient analysis includes the levels of fat, protein, carbohydrate, ash content and water content. The results showed that the formulation of meatballs box, panelists preferred the formula II from another formula with a value of 3.06 based on color, aroma, flavor, and texture. The results of nutritional analysis were fat content of 0.77, protein content of 16:28 value, carbohydrate content of 31.4, ash content of 1:46 and water content of 9.44.

Abstract

The rind of dragon fruit (Hylocereus lemairel (Hook.) Britton & Rose) is a waste material that has not been optimally utilized. The rind contains betacyanin pigment that has many benefits in pharmaceutical products. The study aimed to characterize the physicochemical properties, to evaluate the stability and to apply betacyanin in dragon fruit rind as a natural dye. The extraction process was done by using water as solvent which was sonicated at 50 kHz for 30 minutes at 25 °C. Extract was freeze dried for 48 hours. The dried extract was purified by using preparative TLC and physicochemically analyzed by using UV-Vis and FTIR spectrophotometer. The stability of betacyanin extract against pH and temperature was examined and applied as a dye in tablet imprinting. The result of this study indicated that betacyanin was found at Rf value of 0.6 same as the betacyanin standard. The maximum wavelength of betacyanin was obtained at 534 nm and the IR spectra showed similarity with betacyanin standard with the same functional groups between 4000-600 cm-1 although there was a slight shift in the wavenumber but it still in the range. The stability study were stable at temperature below 40 °C and at range pH 4-6. Betacyanin applications as natural dye of tablet have been successfully carried out with good color stability during 3 months of storage at room temperature.
