Abstract

An effort to improve the solubility of albendazole (ABZ), an anthelmintic drug has been successfully carried out through the formation of multicomponent crystal with dl-malic acid (MAL). Construction of phase solubility curve of ABZ in MAL solution and crystal morphological observations after recrystallization in the acetone-ethanol (9:1) mixture were performed for initial prediction of multicomponent crystal formation. ABZ-MAL multicomponent crystal was prepared by wet grinding or also known as solvent-drop grinding (SDG) with acetone-ethanol (9:1) mixture as a solvent followed by characterization of the multicomponent crystal formation by powder X-ray diffraction and Fourier transform infrared (FTIR) methods. The solubility of ABZ-MAL multicomponent crystal was tested in water at ambient temperature and in pH 1.2, 4.5 and 6.8 of buffered solutions at 37°C. The phase solubility curve of the ABZ in the MAL solution showed type Bs. The ABZ-MAL mixture has a different crystalline morphology than pure ABZ and MAL after recrystallization in the acetone-ethanol mixture (9:1). The powder X-ray diffraction pattern and the FTIR spectrum of ABZ-MAL from SDG different from intact ABZ and MAL powder X-ray diffraction patterns and these results can indicate the ABZ-MAL multicomponent crystal formation. The ABZ-MAL multicomponent crystal has better solubility than pure ABZ in all media used. These results can be concluded that ABZ-MAL multicomponent crystal can be prepared by solvent-drop grinding method with acetone-ethanol (9:1) mixture as a solvent and can increase the solubility of albendazole.

Abstract

Prevalensi diabetes melitus berdasarkan hasil Riset Kesehatan Dasar yang tertinggi di Indonesia terdapat di provinsi DKI Jakarta. Ketidakpatuhan berobat pasien diabetes melitus tipe 2 merupakan salah satu faktor yang mengakibatkan rendahnya kontrol glukosa darah dan meningkatnya resiko komplikasi. Penelitian ini dilakukan untuk mengetahui profil tingkat kepatuhan penggunaan obat di Puskesmas Kecamatan Makasar Jakarta Timur dan faktor-faktor yang mempengaruhi kepatuhan penggunaan obat pada pasien diabetes melitus tipe 2. Penelitian ini berdesain cross sectional yang dilaksanakan pada April 2019 sampai Juni 2019 dengan total 175 responden diabetes melitus tipe 2. Pengumpulan data dilakukan dengan teknik wawancara terstruktur menggunakan kuesioner kepatuhan MMAS-8. Data diolah dengan menggunakan software statistik dan dianalisa menggunakan analisa distribusi frekuensi dan uji chi square. Hasil penelitian menunjukkan bahwa 71 responden (40,6%) memiliki tingkat kepatuhan sedang dalam menggunakan obat antidiabetes. Alasan utama ketidakpatuhan responden adalah bosan (43,6%). Ada hubungan bermakna antara tingkat kepatuhan dan hasil tes gula darah. Untuk mengetahui lebih dalam hubungan antara tingkat kepatuhan dengan hasil tes gula darah maka perlu adanya penelitian lanjutan tentang hubungan studi prospektif hasil tes gula darah dengan tingkat kepatuhan.

Abstract

Mefenamic acid belongs to a class of the Non-steroidal Anti-Inflammatory drugs that work as an analgesic. But mefenamic acid can cause gastrointestinal disorders, has unpleasant odors and tastes and sensitive to the influence of light and temperature. Microencapsulation technology is a technique where the active substance is coated by a thin layer so that the active substance is protected from environmental influences. The aim of this research was to formulate and characterize mefenamic acid in the form of microencapsulation using ionic gelation methods. Preparation was done by comparing 3 variations of concentrations of sodium alginate polymers. Success parameters include the entrapment efficiency, particle shape, particle size distribution, and dissolution test. The results showed that the entrapment efficiency respectively 98,69%, 96,38% and 93,98%, with spherical shape, and particle size that fulfilled the microencapsulation size range of 1,268 μm, 1,343 μm and 1.386 μm and the release of the active ingredients in an acidic medium of pH 1.2 was 8.811 mg/L, 6.751 mg/L and 5.965 mg/L, also on a base medium of pH 7.4 was 79.908 mg/L, 63.394 mg/L and 40,312 mg/L. So that microencapsulation of mefenamic acid can be prepared with polymer chitosan and sodium alginate using the ionic gelation method.

Abstract

Songi (Dillenia serrata) is a tree endemic to Southeast Sulawesi and its stem bark been used in folk medicine. Nonetheless, only a handful scientific knowledge regarding chemistry and biological activities has been investigated on the plant. The present study aimed to investigate the antioxidant and xanthine oxidase (XO) inhibitory activity of the stem bark of the plant. Methanol extract and organic fractions (petroleum ether, ethyl acetate, and methanol) of the dried powdered stem bark of songi were evaluated for phytochemical screening, total phenolic (TPC), total flavonoid contents (TFC), DPPH, FRAP, and XO in vitro assays. Flavonoids, tannins, terpenoids, steroids, and saponins present in the extract. TPC and TFC in extract were 59.2 mg GAE/g and 23.4 mg QE/g. The contents in organic fractions were in solvent-dependent manner (methanol>ethyl acetate>petroleum ether). Extract and fractions scavenged DPPH radicals (48.2–59.7%) at 100 μg/mL compared to ascorbic acid, trolox, and gallic acid (90.3–93.8%). FRAP values varied from 0.8–3.4 μg/μg equivalent trolox amount (quercetin and gallic acid were 25.7 dan 32.4 μg/μg, respectively). They also inhibited xanthine oxidase (15.3–50.3%) at 100 μg/mL (allopurinol, 98.2%). The study concluded the potential of methanol extract and organic fraction of the stem bark of songi, thus highlighted the prospect of songi to be used in herbal and drugs development from nature.

Abstract

A growing problem in the medical field is the development of antibiotic resistant pathogens. One reason this development is so important is that in recent years there is a shortage of new antibiotics in development to combat resistant pathogens. It worths to mention that while 19 new antibiotics were released in the period 1980 to 1984, this had dropped to just three in the period 2005 to 2009. Ironically, the shortage of new antibiotics occur in the era where growing number of pathogens develop resistance to multiple antibiotics that previously effectively used to treat the infections. As a consequent, it is essential that the efficacy of last resort antibiotics, including the new antibiotics, be maintained as long as possible. Ceftaroline is a new antibiotic in Indonesia market which has methicillin-resistant Staphylococcus aureus activity and it belongs to the cephalosporins. Further understanding related to basic profile of ceftaroline, efficacy and safety, cost, and place in therapy is needed to optimize the responsible used of ceftaroline in daily medical practice

Abstract

Ficus elastica leaf is one of the Ficus species assumed to be rich in polyphenols so that it can act as an antioxidant. The research aimed to determine the antioxidant activity and IC50 value of methanol and water extracts of rubber leaf. In this research, the extraction was conducted by maceration using methanol and obtained the yield value of 7,03%, while the infundation using water obtained the yield value of 2,82%. The antioxidant activity test was conducted by DPPH free radical scavenging (1,1-diphenyl-2-phycrilhydrazyl) using a quercetin comparator. The qualitative tests were conducted by TLC method using dicloromethane:methanol eluent with a ratio of 99:1 for methanol extracts, and a ratio of 95:5 for water extracts, where all extracts of both methanol and water positively container antioxidants characterized by the formation of yellow spots with a purple background on TLC plates. The results of the quantitative test shower that the methanol extract had medium antioxidant activity with an IC50 value of 78,39 µg/mL, and water extract had an inactive antioxidant activity with an IC50 value of 319,11 µg/mL. While quercetin was classified as a very strong antioxidant with an IC50 value of 7,62 µg/mL.

Abstract

In this research, there was docking process of leonurine compounds and their derivatives to Cyclooxygenase-2 (COX-2) enzyme as an anti-inflammatory. The receptor code used is 6COX which is downloaded from the protein data bank site (PDB). The aims of this study was to investigate the interaction of leonurine compounds and their derivatives against COX-2 receptors. All compounds are docked using AutoDock 4.2 software. The docking validation is performed by tethering the ligand-receptor with the parameter of the Root Mean Square Deviation (RMSD) value <2 Å. From the docking validation results, have been obtained RMSD value = 0.31 Å. Analysis of docking results indicates that leonurine and its derivatives are predicted to have good interaction with COX-2 receptors. From the results of this research, it can be concluded that leonurine compounds and their derivatives have inhibitory activity against COX-2 receptors. The docking results showed the lowest binding energy value ligand-receptor (ΔG) best matched in the 11th derived compound which is -7.95 kcal/mol.

Abstract

Infection is one of the health problems. It is mostly caused by bacteria. The increased incidence of bacterial infection results in higher antibiotic use. It can lead to antibiotic resistance risk. Antibiotic resistance may occur in pediatric patients. Morbidity, mortality, and high cost of medication are impact of this condition. Evaluation of antibiotic use needs to be done to ensure responsible use of antibiotics. ATCD/DDD (Anatomical Therapeutic Chemical/ Defined Daily Dose) method can be used to evaluate antibiotic use quantitatively. ATC classification is a system of grouping active substances according to their location of action and DDD is a measurement system that is connected to the ATC code.The purpose of this study was determining the profile of antibiotic use and measuring quantitative evaluation with ATC/DDD method in pediatric patients. The data was collected by observing the hospital medical record. The results of this study showed that cephalosporin (46,22%) was the most used group and cefotaxime (31,15%) was the most used type of antibiotic. While, the quantitative evaluation with ATC/DDD method indicated that the highest of antibiotic use was ceftriaxone (11,30 DDD/100 patient days) and the lowest was amikacin (0,03 DDD/100 patient days).

Abstract

A pharmacy is a place where pharmacists do their pharmaceutical work, serving directly and responsibly for patients who need their professional services. One form of direct service is the provision of drug information and counseling. The purpose of this study was to describe the drug information services provided by pharmaceutical personnel in the South Tangerang area pharmacy for patients who redeemed prescriptions using antibiotics. Patient simulation method was used to collect data to 100 pharmacies that were randomly selected based on data in the city health office. Recommendations were obtained from the South Tangerang branch of the Indonesian Pharmacists Association while ethical clearance was obtained from the ethical committee of the Faculty of Medicine, University of Indonesia. The results of this study indicate that 85% percent of pharmaceutical personnel who provide drug information for simulated patients are non-pharmacists. The most widely conveyed drug information items were the frequency of drug use delivered by 82% of pharmacists, received by the intended use (61%) and time of use (44%). Not related to pharmacy officers who submit the following information: drug interactions and ways to prevent them, side effects of drugs and how to prevent them; food and drinks that must be avoided as well as how to store drugs. As many as 49% of pharmacists provide additional antibiotics requested by patients without a prescription. Drug information points provided by pharmacy staff specifically by pharmacist had not been maximally delivered.

Abstract

Hepatitis A is a viral disease with a high potential for morbidity and mortality. It is also the second highest viral disease in Indonesia after hepatitis B. Community’s knowledge regarding Hepatitis A disease dan vaccine is required to support such program for eradication of Hepatitis A. The aims of this study was to measure the knowledge of Indonesian community about hepatitis A disease and its vaccine indicated from knowledge score of correct answer towards questionnaire, as well as to analyze the factors that can influence it indicated from difference of mean of knowledge scores among groups of respondents characteristics. This study used an observational survey research design applying a cross-sectional multi-center approach. The number of sample was 500 respondents located in 5 provinces in Indonesia, namely DIY, Lampung, West Java, Central Kalimantan, and Central Sulawesi. Data collection was carried out using a questionnaire that was modified from the questionnaire in a similar previous study. The results showed that public knowledge related to diseases and hepatitis A vaccine in Indonesia was considered sufficient with the mean score obtained was 75.2% ± 12.2%. Statistical analysis using Mann-Whitney test showed that there were significant differences in score of knowledge between groups on the characteristics of experience of hearing a disease (p=0,000), experience of hearing a vaccine (p=0,000) and the experience of the child being vaccinated (p=0,000). There are 2 items of knowledge that werelack understood by respondents, namely treatment of hepatitis A that cannot be treated with antibiotics and immunizations and hepatitis A is not included yet in the national immunization program in Indonesia. It is concluded that community knowledge regarding hepatitis A disease and vaccine was fair (mean knowledge score between 65 – 80%). However, the government still needs to conduct educational programs for the Indonesian community about disease prevention through the vaccination process and provide counseling about the types of vaccines that are not included in the national health insurance program in Indonesia.
