Abstract

Xestospongia sp. is one of marine sponge belongs to demosponges class that mainly found in Southeast Sulawesi and the secondary metabolites contained in Xestospongia sp. suspected to have immunomodulatory activity. A previous study exhibited the immunomodulatory of Xestospongia sp. ethanolic extract (XEE) at dose of 300 and 400 mg/Kg BW by affecting the phagocytic activity of macrophages. Thus, this study aims to investigate the effect of XEE towards interferon-gamma (IFN-γ) and tumor necrosis factor-alpha (TNF-α) at dose of 300 and 400 mg/Kg BW. Wistar male rats are divided into 4 groups (n=6) randomly and treated for 7 days orally each as follow: group I (XEE dose of 300 mg/KgBW); group II (XEE dose of 400 mg/KgBW); group III (0.5% NaCMC); and group IV (commercial phylantii extract). On day 8, animals were infected with Staphylococcus aureus and left for 1 hour. Animals were sacrificed and the blood was drawn by cardiac puncture (3 mL), followed by analyzed under ELISA Kit for IFN-γ and TNF-α. Collected data were analyzed statistically using SPSS®. The IFN-γ levels obtained were 350.113; 392.970; 118.416; and 61.958 ρg/mL, respectively and the TNF-α were 2808; 1308; 778; and 845.5 ρg/mL, respectively. According to results obtained, both doses of XEE are affecting the IFN-γ and TNF-α levels (p<0.05) compared to group III as negative control, and group IV as positive control. As conclusion, XEE of both doses is increasing IFN-γ and TNF-α levels of animals that respond to phagocytic activity

Abstract

Drug interaction is a type of Drug-Related Problems (DRPs) that caneventually increase morbidity and mortality rates. CKD patients have asignificant risk of developing polypharmacy due to comorbid diseases andpharmacokinetics' alteration. The literature review was conducted byexploring all of the articles related to the drug interaction using druginteraction analysis program in CKD patients, which obtained from threedatabases, namely Google Scholar, PubMed, and Science Direct, usingseveral keywords combination. Based on the comprehensive reviewsconducted, it is known that the most common effects of antihypertensivedrug interactions in CKD patients are decreasing effects of antihypertensivedrugs, hypotension, and hyperkalemia. Handling management used for theemergence of potential drug interactions is based on the severity of the druginteractions and complete knowledge of the patients' clinical condition. Themanagement of drug interaction by monitoring blood pressure, diuresis, andpotassium levels; Monitor the related effect symptoms; Monitor the fluidand body weight; Monitor the kidney and heart function. On the conditionwhere the handling management of potential drug interactions is not carriedout, elevated morbidity and mortality rates are the risks of complicationsarising from the drug interactions.

Abstract

Breastfeeding is essential for the health of newborn infants and mothers. Basic health research in 2018 showed that exclusive breastfeeding in Bali has not reached the national target. High rates of cesarean section delay breastfeeding initiation, the important predictor of continued breastfeeding. Main concern for breastfeeding mothers about medications increased the use of herbal galactagogue to aid lactation. The use of traditional medicinal plant in Bali is 40% higher than national average and its existence is strengthened by the issuance of Bali Governor Regulation No. 55/2019 concerning Traditional Balinese Health Services. Despite increasing popularity of traditional medicine, there are currently limited data available on the plant list and pattern of use herbal galactagogue. This study aims to explore the galactagogue plants, pattern of use, perceived effectiveness of herbal galactagogues during breastfeeding among breastfeeding mother’s in Bali. This study was conducted using a questionnaires and in-depth interviews. The questionnaire and interview guideline were validated by lactation consultants and traditional health practitioners in Bali. The study was approved by Human Research Ethic Committee of Bina Usada Bali. The use of herbal galaktagogue is mostly practiced by highly educated mothers, mother’s occupation as health workers and primiparous. There are 26 types of plants that are often used by lactating mothers in Bali. Majority of respondent begin to use herbal galactagogue after giving birth (82%) for approximately less than 1 month. Majority of lactating mothers in Bali perceived effectiveness of herbal galactagogue usage based on breastfeeding adequacy indicators. About 95% respondents feel confidence and have self-empowerment after using herbal galactagogue, possessing psychological benefits. The use of herbal galactagogue is common amongst breastfeeding mothers in Bali, while information about efficacy and safety is lacking. Further research is needed to give evidence-based information to support exclusive breastfeeding
