Abstract

The Indonesian government enforces a COVID-19 vaccination policy for all, but the achievement rate for the COVID-19 vaccination in Sangihe Islands Regency at the first dose is only 1.05%, while the achievement at the second dose to date is only 0.47%. With this in mind, the vaccination policy has not been in line with community participation. The purpose of this study was to find out what kind of health information is needed to increase the covid-19 vaccination rate. The method used in this research is a descriptive study with a purposive sampling technique which was held from 7 January to 8 February 2021 with a total sample of 85 respondents who are health workers in the Sangihe Islands Regency. The data analysis used is descriptive analysis with frequency distribution. The results in this study were 15.3% stated that they were not willing to vaccinate COVID-19 because 39% of COVID-19 survivors, 38% had comorbidities, 15% were unsure of safety and 8% feared side effects. The conclusion of this study is to increase health information regarding vaccines for COVID-19 survivors, information about comorbidities such as what is appropriate for COVID-19 vaccination, safety, and side effects resulting from the COVID-19 vaccine.

Abstract

Discoid lupus erythematosus is the most common forms of chronic cutaneous lupus erythematosus. It is characterized by clinical manifestations of erythematous macules, papules, or plaques with a coin-like shape and the face is the most common predilection site. Clinical features often resemble granuloma faciale. This case report aimed to distinguish discoid lesions on the face based on the histopathological examination. A 71-year-old male with a few reddish lumps appeared on his face since three months ago. Physical examination showed multiple discrete erythematous plaques with overlying squamous. Hematoxylin and eosin staining on the epidermis demonstrated basket weave type orthokeratosis, basal vacuolar cell degeneration, epidermal atrophy with flat rete ridges, and follicular plugging while in the dermis obtained inflammatory cell infiltrates, especially in periadnexal areas. Histopathological features of DLE are hyperkeratosis, pilosebasea gland atrophy, follicular plugging, basal membrane thickening, and cellular infiltrate in periadnexa or perivascular areas more visible than in other types of CLE. In DLE, no subepidermal gren zone and eosinophil infiltrate were found, like histological features of granuloma faciale. Histopathological examination can be used to establish a diagnosis for discoid lesions on the face, although serology examination remains as the gold standart. Keywords:  Discoid lupus erythematosus; Granuloma faciale; Histopathology

Abstract

Antioxidants are substances that can slow down the oxidation process of free radicals. Limpasu plant (Baccaurea lanceolata (Miq) Muell. Arg), an indigenous plant of Borneo, is a natural antioxidant source. The purpose of this study was to determine the antioxidant activity of the limpasu pericarpium extract. The extraction of the limpasu pericarpium was done by maceration method using solvents with increasing polarity ranging from n-hexane, ethyl acetate, and methanol. Antioxidant activities of the three extracts were measured by the DPPH and FRAP methods. The IC50 values of n-hexane, ethyl acetate, and methanol extracts, as well as quercetin using the DPPH method were 517,45 µg/mL, 530,64 µg/mL, 10,63 µg/mL and 6,83 µg/mL, respectively. Meanwhile, the IC50 values obtained from FRAP method were 198,96 µg/mL, 190,07 µg/mL, 661,36 µg/mL, and 7,09 µg/mL, respectively. The results revealed that the methanol extract is more potent than other extracts tested for antioxidant activity.

Abstract

Aedes aegypti is the main vector of DHF in Indonesia. The use of synthetic insecticide results in vector resistance. Mahkota Dewa fruits (Phaleria macrocarpa (Scheff) Boerl) contain flavonoids, saponins, and tannins. Thus, it has the potential as a natural larvicide against Ae. aegypti larvae. The objective of the study was to determine the effectiveness of methanol extract Mahkota Dewa fruit. This research was experimental, used methanol extract Mahkota Dewa fruit at concentration 0.1%, 0.15%, 0.20%, 0.25% and 0.30%, then added 25 larvae each concentration and repeated 3 times. After 24 and 48 hours of exposure, the mortality of larva was counted and then calculated LC50 and anterior midgut histopathology. LC50 values 0,554% (0,391-11,397) and anterior midgut histopathology shows epithelial damage. Mahkota Dewa fruits with methanol extract have activity as larvicide larvae instar III-IV Ae. aegypti.

Abstract

Hospitals are efficient if they can use all available resources to produce something without leaving unwanted things. The purpose of the study was to determine the application of lean management in reducing waste in the Hemodialysis Installation of Qolbu Insan Mulia Hospital (QIM) Batang. This study uses a qualitative method with an Action Research research design whose results are presented narratively. The findings of this study indicate that there are three highest wastes, namely: waiting for waste (26%), motion waste (18%), and Defect waste (16%). There is a problem with waste waiting caused by a lack of human resources, in this case, the number of nurses, motion waste is caused by the movement of nurses who are too frequent in order to carry out inspections of patients undergoing HD and the large number of movements caused by inspections still using the manual method, and a waste defect found that HD failure was caused by anemia. Furthermore, there are efforts to reduce waste waiting, waste motion, and waste defect in the Hemodialysis Installation of Qim Hospital, Qolbu Insan Mulia Hospital (QIM).

Abstract

Gas-containing kidney stones are rare cases. The available literature about gas in the kidneys, usually only discusses and distinguishes between two things, namely emphysematous pyelonephritis and emphysematous pyelitis, after getting rid of recent upper urinary tract instrumentation and the presence of fistula with the gastrointestinal tract. To date, there have been only 11 cases of gas-containing kidney stones. We report the 12th case of gas-containing kidney stones (the first from the field of radiology), which is a 69-year-old female with bilateral flank pain, no fever, and no history of other diseases. Physical examination revealed bilateral costovertebral angle tenderness. The plain abdominal radiograph showed a staghorn stone in the right kidney. The non-enhanced CT abdomen showed gas-containing staghorn stones in the right kidney accompanied by hydronephrosis. The patient underwent percutaneous nephrolithotomy. Kidney stone analysis showed oxalate, phosphate, ammonium, calcium, and uric acid components. The etiology and relationship of gas-containing kidney stones with other gas entities in the kidney is still unclear, making it difficult to determine their clinical significance. Radiological examination, especially CT scan, plays a very important role in determining the entity of gas in the kidney.

Abstract

Patellar malalignment is the imbalance relationship between patella and trochlea, in which clinical findings most of the time are obscured; hence diagnosis is often challenging. Magnetic resonance imaging (MRI) is the most sensitive tool to detect subtle patellar malalignment features, so diagnosis can be made early. However, there has been no clear consensus on the normal value of patella morphology until today. This study aims to determine patellofemoral morphology values in Indonesian using MRI. This was a retrospective study of 202 patients aged 18-40 years old with knee problems without patellar instability. Patellar morphology parameters including Insal Savati ratio (IS ratio), patellar tilt angle (PTA), sulcus angle (SA) and tibial tubercle-trochlear groove distance    (TT-TG) were evaluated and recorded for statistical analysis. There was no significant correlation between anthropometric values and patellar morphology values. There were significantly higher PTA, SA and TT-TG values in females compared to males. The mean value of the IS ratio in the Asian population using MRI was 0.99 ± 0.14, PTA was 9.09 ± 6.88, SA was 139.20 ± 6.38, and TT-TG distance was 8.00 ± 5.25. Further studies with larger samples and multi-center results are required.

Abstract

Withdrawal life-support is mainly categorized as part of euthanasia. When viewed from the aspect of criminal law in force in Indonesia, Indonesia does not permit active euthanasia by anyone (including doctors and medics). If the doctor ends the patient’s life by euthanasia is considered to violate criminal law. The study aimed to find out about life-support for patients with brainstem death in the hospital and to find out about the juridical implications of withdrawal life-support in cases of brain stem death. Based on the data discovered to be recorded, some conclusions that life support for brain stem death patients in the hospital is not needed because the element is futile. The juridical implications of withdrawal life-support in brain stem death patients in hospitals can be exempted from lawsuits if the doctor has implemented duties by medical ethics and acting in a medical professional manner, especially article 344 of The Criminal Code.

Abstract

Pregnancy impacts a women’s physical and mental state. These modifications will have an impact on pregnant women’s quality of life. Pregnant women’s poor quality of life will impact the fetus and mother’s health. As a result, nonpharmacological therapy, such as the use of progressive muscle relaxation techniques (PMRT), is required to improve the quality of life of pregnant women. PMRT research has so far been linked to anxiety and depression in pregnant women that it is still limited in its application to the quality of life of pregnant women in the third trimester. This study aims to see how beneficial PMRT is in improving the quality of life of pregnant women. A quasi-experimental approach with a pre-post test with Nonequivalent Control Groups is used in this investigation. With a total sample size of 46 participants, non-probability sampling was used to pick the sample. Questionnaires are used to collect data, and t-independent analysis is used to analyze it. There were p-value 0.05 variations in the quality of life of pregnant women in the control and treatment groups after receiving PMRT. The average improvement in quality of life in the control and treatment groups was significantly different (p-value 0.05). For each dimension, there was a significant difference in the quality of life between the control and intervention groups before and after the research (p-value). It is expected that health workers will provide PMRT in pregnancy health services to improve the quality of life of pregnant women.

Abstract

Used cooking oil is cooking oil that has been heated repeatedly, so that it can cause the formation of free radical compounds. Free radicals are toxic kidneys that generate cell injury via lipid peroxidation, DNA damage, and protein peroxidation. Black tea (Camellia sinensis) infusion has a strong polyphenol group, namely epigallocatechin gallate, thearubigin, theaflavin, the amino acid                L-theanine, flavonols, and catechins that can inhibit free radicals and protect organs from oxidative stress. This study was a post-only control group design experiment. The research subjects used were 24 Sprague Dawley male rats which were divided into six groups. The first group was the control group (K0). Treatment group 1 was given distilled water and used cooking oil after 12 times heating (K1), treatment group 2 was given black tea at a dose of 2.5g/ kg bw (body weight) and distilled water (K2), and treatment group 3 was given black tea at a dose of 3.75 g/kg bw and distilled water (K3). Treatment group 4 was given used cooking oil after 12 times heating and black tea with a dose of 0.50 gr/200 gr bw (K4), treatment group 5 was given used cooking oil after 12 times heating and black tea with a dose of 3.75 g/kg bw (K5). All groups were treated for 42 days. Then, the rats' kidney was collected, and data were analyzed by Kruskal Wallis and One Way ANOVA test. The results of the Kruskal Wallis and One Way ANOVA test showed a decrease in glomerular and tubular damage after the rats orally induced with black tea were given 12x heating used cooking oil (p<0.05). The mean histopathological score in the administration of black tea at a dose of 3.75 g/kg bw was lower than at a dose of 2.5 g/kg bw. This study shows that infusion of black tea (Camellia sinensis) can prevent glomerular and tubular damage induced by used cooking oil.
