Abstract

Background: HIV/AIDS is a terminal disease that remains the leading cause of death worldwide, including in Indonesia. This disease has a wide impact, not only for sufferers but also for their social environment. Caregivers of people living with HIV/AIDS (PLWHA) often experience psychological distress, especially when they are not prepared to care for sick family members. Their ability to fulfill the five family duties in health is greatly influenced by their psychological well being. Objectives: This study aims to describe the psychological well-being of caregivers of people living with HIV/AIDS (PLWHA) in Sumedang regency.Methods: The method used in this research is descriptive quantitative. The sample in this study amounted to 196 respondents selected by simple random sampling technique. The instrument used was the Ryff Psychological Well-Being Scale, which has a validity range of 0.279 to 0.660 and a reliability score (Cronbach's Alpha) of 0.885.Results: The results of the study showed that most caregivers (52.1%) were in the moderate psychological well-being category, while 47.9% were in the high category. Among the six dimensions of psychological well-being that were studied in this study,  only purpose in life and personal growth were predominantly in the high category (50-60%). The demographic analysis revealed that most respondents were aged 20–55 years (M = 36.3, SD = 10.9), the majority were female (56.1%), and most worked as housewives (44.8%). The duration of caregiving was predominantly 1–5 years (61.7%; M = 2.7, SD = 2.5).Conclusions: It can be concluded that most caregivers of People Living with HIV/AIDS (PLWHA) exhibit moderate levels of psychological well-being. This indicates that caregivers can generally accept their strengths and weaknesses, build positive relationships, adapt to their environment, set clear life goals, demonstrate independence, and engage in personal growth. Therefore, it is necessary to enhance family counseling programs within supportive care and treatment services, as well as strengthen family psychoeducation initiatives in nursing and healthcare institutions

Abstract

Background: Traumatic brain injury (TBI) is one of the leading causes of morbidity and mortality worldwide, often resulting in long-term disability or death. The critical period for survival is within the first 6–12 hours after trauma, when secondary brain injury may occur due to hypoxia, hypotension, and systemic complications. To assist clinicians in predicting patient outcomes, several trauma scoring systems have been developed, including the Trauma and Injury Severity Score (TRISS), Glasgow Coma Scale–Age–Systolic Pressure (GAP) Score, and Kampala Trauma Score (KTS). Objectives: This study aims to evaluate the accuracy of these three scoring systems in predicting survival outcomes in TBI patientsMethods: This research employed a quantitative observational analytic design with a cohort approach involving 212 patients who presented to the Emergency Department (ED) with TBI. Patients were assessed using TRISS, GAP Score, and KTS, and survival status was observed for up to 6 hours. Data were collected by trained ED nurses using standardized procedures, including measurement of Glasgow Coma Scale (GCS), age, respiratory rate, neurological status, systolic blood pressure, and the number of severe injuries. Data analysis included diagnostic tests such as sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and overall accuracy. Results: The results indicate that GAP Score had the highest sensitivity (99,5%), while GAP score demonstrated the highest specificity (85,7%). The highest PPV was found in TRISS and GAP score (97,1%), whereas KTS had the highest NPV (71,4%). GAP Score achieved the highest accuracy (96,7%), making it the most effective method for predicting TBI patient survival.Conclusions: The GAP Score is recommended as the most accurate and practical tool for predicting survival in traumatic brain injury patients presenting to the emergency department, especially in clinical environments that require rapid decision-making.

Abstract

Background: Exclusive breastfeeding for infants aged 0–6 months provides major health benefits, yet many mothers struggle to maintain it due to low motivation. “Hypnolactation,” which combines hypnosis to build confidence and reduce anxiety, with lactation massage to stimulate milk flow, may serve as an effective holistic care intervention.Objectives: This study develops an innovative holistic care model incorporating hypnolactation and lactation massage to enhance maternal motivation for exclusive breastfeeding. Methods: This quasi-experimental study used a pretest–posttest control group design with 42 postpartum mothers at Kagok Health Center, Semarang (March–December 2024). Data were collected using a validated breastfeeding motivation questionnaire and observation sheets. Statistical analysis employed the Wilcoxon test and the independent t-test.Results: Both hypnolactation (Z = –2.33, p = 0.02) and lactation massage (Z = –2.00, p = 0.046) significantly improved maternal motivation, with hypnolactation showing greater effectivenessConclusions: The findings demonstrate that the holistic care model combining hypnolactation and lactation massage significantly improves maternal motivation for exclusive breastfeeding. This intervention effectively addresses psychological barriers such as anxiety and lack of confidence while also providing physical comfort that facilitates the breastfeeding process. Future studies should expand the sample size and evaluate the long-term impact on exclusive breastfeeding success up to six months.

Abstract

Background: Clinical leadership among nurse managers plays a vital role in ensuring the quality and effectiveness of hospital services. Nurse managers are responsible for integrating professional competence, managerial skills, and interpersonal collaboration to enhance patient outcomes and team performance. However, in Indonesia, there remains a lack of standardized and contextually relevant instruments to measure clinical leadership competencies.Objectives: This study aimed to develop and validate a clinical leadership competency assessment instrument specifically tailored to the Indonesian hospital context and to evaluate its feasibility for practical application in nursing management.Methods: The study employed a research and development design conducted in Tasikmalaya City. The sample consisted of 38 nurse managers selected through accidental sampling. The research process consisted of several stages: instrument design, expert validation, revision, and limited testing. Content validity was examined using the Content Validity Index to ensure conceptual and contextual relevance, while item validity and reliability were analyzed through appropriate statistical methods to ensure measurement accuracy and internal consistency. Results: The developed instrument included 49 items across five core dimensions: communication, collaboration, decision-making, team empowerment, and change management. Validation results showed a CVI of 0.86 (>0.80), and high reliability (Cronbach’s Alpha 0.950). Non-valid items were removed, resulting in a final instrument that is valid, reliable, and contextually appropriate for use by nurse managers.Conclusions: The instrument is valid, reliable, and feasible for assessing nurse managers’ clinical leadership competencies. Beyond its psychometric soundness, it provides a strategic framework to strengthen leadership accountability, improve team coordination, and enhance hospital service quality. The instrument can also serve as a valuable tool in hospital management and nursing education to guide professional development and promote evidence-based leadership practices

Abstract

Background: Perineal laceration is a common complication during vaginal delivery, and its severity may influence maternal recovery and postpartum quality of life. Pelvic floor muscle exercises, including Kegel exercises and Pelvic Floor Muscle Training (PFMT), are believed to enhance perineal elasticity and reduce the risk of laceration. Objective: To assess the effectiveness of Kegel exercises and PFMT in reducing the degree of perineal laceration among postpartum mothers. Methods: A quasi-experimental study with a posttest-only nonequivalent control group design was conducted among 60 pregnant women, who were assigned to either an intervention group (antenatal exercises with PFMT) or a control group. The degree of perineal laceration was assessed after delivery. Data were analyzed using an independent t-test. Results: The findings indicated a statistically significant difference between the intervention and control groups. The mean degree of perineal laceration was lower in the intervention group (0.77 ± 0.879) than in the control group (1.73 ± 0.583), with a p-value of 0.001 and a t-value of 5.915. Conclusions: This study demonstrated that Kegel exercises and Pelvic Floor Muscle Training (PFMT) are effective in reducing the degree of perineal laceration among postpartum mothers. The intervention group showed significantly lower laceration grades compared to the control group, indicating that these pelvic floor exercises can serve as a beneficial preventive strategy. These findings confirm that Kegel exercises and PFMT meet the study objective by contributing to a measurable reduction in perineal trauma during childbirth.

Abstract

Background: Pre-Menstrual Syndrome (PMS) is a common condition among adolescent females that can negatively affect daily activities, emotions, and overall quality of life. According to PKRR data from 2020, approximately 90% of women of reproductive age experience PMS, with 80% being adolescents reporting significant disruption in daily life. One contributing factor is sleep disturbance, often associated with hormonal fluctuations prior to menstruation. Additionally, excessive social media use has been shown to influence sleep patterns and mental health, potentially worsening PMS symptoms. This issue is particularly relevant among female university students, who are frequently exposed to social media and academic stress, both of which may impact sleep quality and emotional regulation. Objectives: This study aims to analyze the relationship between social media addiction and sleep disturbances with the incidence of PMS among female students at the Faculty of Health Sciences, Universitas Muhammadiyah Purwokerto.Methods: This study used an observational analytic design with a cross-sectional approach. A total of 92 female students were selected using proportionate stratified random sampling. Research instruments included the Bergen Social Media Addiction Scale (BSMAS), the Pittsburgh Sleep Quality Index (PSQI), and the Shortened Premenstrual Assessment Form (SPAF). Data were analyzed using the Spearman rank correlation test.Results: The findings revealed significant associations between social media addiction and PMS (p = 0.002), as well as between sleep disturbances and PMS (p < 0.001).Conclusions: Social media addiction and sleep disturbances are significantly related to the incidence of PMS among female university students.

Abstract

Background: Affective touch aims to convey affection to children. This touch is closely related to emotional needs and social bonds, which strengthen relationships between individuals. Children in orphanages often experience a lack of affective touch, which causes persistent stress.Objectives: To determine caregivers' perceptions of the social-emotional development of babies and children who receive baby massage as daily affective touch at the Metta Mama and Maggha Foundations.Methods: This research is a qualitative study with a phenomenological approach. The sample consisted of 25 caregivers, using the total population. Data was collected through focus group discussions (FGDs), with each group consisting of 8-9 caregivers.Results: Some participants have the perception that a socially healthy child is a child who is able to adapt and interact with others, some participants have the perception that children need social-emotional aspects such as the need for affection, attention, the need to be loved, a sense of security, and a desire to be understood, some participants have the perception that the role of caregivers is very large because it replaces the role of parents, most participants said that baby massage has been done on children aged 0-6 months, some participants have the perception that baby massage can affect the calmness, comfort of babies so that they can be more optimal in carrying out daily activities, some participants have the perception that children who are socially-emotionally healthy behave more understandingly, confidently and more quickly accept the information given and all participants have the perception that it is very important to apply parenting patterns with touch, because of the lack of touch in orphanages.Conclusions: According to caregivers at the Metta Mama & Maggha Foundation, it is important to implement parenting patterns with touch in the form of baby massage as Daily Affective Touch in children's social-emotional development

Abstract

Background: Postpartum depression is a major maternal mental health concern that remains insufficiently addressed in rural areas such as Malaka District, East Nusa Tenggara, where structural and social vulnerabilities may heighten psychological risks. Limited evidence from these settings underscores the need to understand the contextual determinants influencing maternal mental health. Objectives: The objective of this study was to examine the relationship between family support, sociodemographic characteristics, and the risk of postpartum depression among mothers in Malaka District.Methods: This cross-sectional analytic study involved 100 postpartum mothers selected through purposive sampling. Data were collected using a structured questionnaire that included demographic characteristics, a family support scale, and the Edinburgh Postnatal Depression Scale (EPDS). Statistical analyses comprised descriptive tests, chi-square tests, and multivariate logistic regression. Results: The results indicated that 40% of participants were at risk of postpartum depression (EPDS ≥13), and more than half (55%) experienced inadequate family support. Bivariate analysis revealed a significant association between family support and postpartum depression (p=0.001). Multivariate logistic regression demonstrated that inadequate family support was the strongest predictor of postpartum depression, with affected mothers having 3.8 times higher odds of developing depression (AOR 3.8; 95% CI: 1.7–8.3). Low maternal education was also a significant predictor (AOR 2.5; 95% CI: 1.1–5.8), whereas maternal age and employment status were not associated with depression risk. Conclusions: These findings emphasize the critical role of family involvement and maternal education in mitigating postpartum mental health problems. Strengthening the active participation of husbands and family members, improving maternal mental health literacy, and integrating family-centered approaches into antenatal and postnatal care are essential steps for reducing the burden of postpartum depression in Malaka District.

Abstract

Background: The nutritional status of pregnant women affects fetal growth. Inadequate nutrition intake can lead to poor maternal nutritional status, ultimately resulting in insufficient nutrients being transferred to the fetus. A good and correct dietary pattern and nutritional intake will meet the nutritional needs of pregnant women. Objectives: To determine the relationship between dietary pattern and nutritional intake with the nutritional status of pregnant women, including CED status, anemia status and weight gain in Pancoran mas, Depok City . Methods: This study was a prospective cohort conducted on 115 pregnant women. Respondents were observed for 7 days over 4 weeks to measure their dietary patterns and nutritional intake using Food Frequency Questionnare (FFQ) and the 24-hour Food Record and then processed using Nutrisurvey. Their nutritional status was then measured. CED and anemia status were measured using MUAC and hemoglobin tester after observation. Weight gain was measured before and after observation. The data were analyzed using the chi-square test. Results: The average energy intake of respondents was 1.884,2 ± 383,30 kcal/day; the average protein intake was 79 ±  14,75 gram/day; the average iron intake was 24,8 ± 12,3 mg/day; and the average folic acid intake was 359,6 ± 83,04 mcg/day. There was a relationship between dietary patterns and the risk of CED, anemia status, and weight gain (p<0.05). There was a relationship between energy and protein intake and weight gain in pregnant women (p<0.05).Conclusions: Dietary patterns are related to the nutritional status of pregnant women. The average intake of energy, protein, iron, and folic acid in pregnant women is still far from the nutritional adequacy level. Health workers are expected to strengthen nutrition education, and pregnant women are expected to pay attention to their dietary patterns and nutritional intake during pregnancy.

Abstract

Background: Exclusive breastfeeding (EBF) is the practice of giving only breast milk to infants aged 0–6 months without any additional food, except for vitamin supplements and medicines. The prevalence of exclusive breastfeeding among working mothers is significantly lower in urban areas due to inflexible work schedules, early return to work, short maternity leave, fatigue, and lack of support for expressing breast milk at the workplace. The web-based mHealth application “Mama Bekping” was proposed by researchers to provide educational features for working mothers regarding breast care, methods to increase milk supply, proper pumping techniques, and notifications to remind mothers to express breast milk.Objectives: To determine whether the web-based mHealth application “Mama Bekping” can improve knowledge and adherence of working mothers in breast milk pumping.Methods: This study used a quasi-experimental design with two group pretest–posttest. The sample consisted of 80 breastfeeding mother selected using purposive sampling, with a 1-month intervention. Parameters measured were maternal knowledge and adherence to breast milk pumping. Data normality was tested using the Shapiro–Wilk test, while the effect was analyzed using the dependent t-test.Results: The average pumping frequency from week 1 to week 4 was consistent at 14 times per week. However, within 4 weeks, an increase was observed from 12–13 times to 15 times per week. The mean knowledge score before and after intervention showed a 20-point difference, with a minimum difference of 35 points and a maximum difference of 40 points. The dependent t-test revealed a p-value = 0.000 (p < α), indicating a significant effect.Conclusions: The web-based mHealth application “Mama Bekping” effectively increases the knowledge and adherence of working mothers in breast milk pumping, thereby supporting the success of exclusive breastfeeding programs
