Abstract

The global rise in obesity, including in Semarang, Indonesia, has been associated with an increased risk of insulin resistance, which can be assessed using the Homeostatic Model Assessment for Insulin Resistance (HOMA-IR). Diet plays a crucial role in modulating systemic inflammation. The Dietary Inflammatory Index (DII) is a validated tool used to quantify the inflammatory potential of dietary intake. However, few studies have explored the relationship between the ((DII) and HOMA-IR in obese individuals. This study aimed to analyze the association between DII score and HOMA-IR in obese adults in Semarang City. A cross-sectional study was conducted from September to November 2024, involving 66 obese adults aged 26–50 years (Body Mass Index [BMI] ≥25 kg/m²), selected through purposive sampling. Dietary intake was assessed using a semi-quantitative food frequency questionnaire (SQ-FFQ), and DII scores were calculated based on established algorithms. Fasting Blood Glucose (FBG) and fasting insulin levels were measured to calculate HOMA-IR. Data analysis was performed using Pearson’s correlation and multiple linear regression adjusted for age and BMI. A DII score > 0 indicated a pro-inflammatory diet and HOMA-IR values >2,0, indicating insulin resistance. The results showed a significant positive association between the DII score and HOMA-IR (p = 0,002). Multiple regression analysis showed that the DII remained a significant predictor of HOMA-IR after adjusting for age and BMI (Model 1: β = 0,362, p = 0,007; Model 2: β = 0,381, p = 0,002; Model 3: β = 0,368, p = 0,002). The final model explained 24,3% of the variance in HOMA-IR (R² = 0,243), indicating a moderate effect size. In conclusion, a higher Dietary Inflammatory Index score was significantly associated with increased insulin resistance among obese adults in Semarang, independent of age and BMI. These findings highlight the potential role of diet-induced inflammation in metabolic dysregulations.

Abstract

The adolescent age group had a tendency to have an unhealthy diet, such as consuming more sugar (53,1%) than the other age groups. The increasing consumption of sugar-sweetened beverages (SSBs) is a major contributor to the increasing prevalence of obesity in adolescents in Indonesia. Increased consumption of SSB in adolescents is thought to be one of the factors contributing to the increase in obesity prevalence. Jember Regency as one of the areas with modern lifestyle trends shows a trend of consumption of these drinks that needs to be analyzed in more depth. The purpose of this study was to analyze the consumption patterns of SSB in adolescents and the relationship between the characteristics of respondents and the type, amount, and frequency of SSB consumption. This was an analytical observational study with a cross-sectional design. The study was conducted from June to September 2024. The independent variables of the study consisted of respondent characteristics, including gender, age, and education, while the dependent variable was the consumption pattern of sugar-sweetened beverages in packaging (SSB), including the type, size, amount, and frequency of SSB consumption. Data collection on SSB consumption patterns in adolescents was carried out with an instrument in the form of a semi-quantitative food frequency (SQ-FFQ) questionnaire online using the Google form. The research data were analyzed univariately and bivariately using the chi-squared test. The results showed that almost all respondents (97,75%) consumed more than one type of SSB. Tea type SSB (37,39%) was the most consumed/favored, with a frequency of consumption of 2-3x/week by (48%). In addition, the absence of significant differences in the consumption patterns of SSB in terms of the type of SSB consumed and the characteristics of respondents indicates that SSB is favored by adolescents with a fairly high frequency, regardless of differences in individual characteristics.

Abstract

Stunting, a chronic form of undernutrition, is linked to impaired growth, delayed cognitive development, and adverse health and economic outcomes across the life course. Environmental, socioeconomic, and behavioral variations between urban and rural settings contribute differently to stunting risk, although the evidence remains fragmented. This study systematically reviewed the determinants of stunting in urban and rural Indonesia to provide a comprehensive and contextual understanding. The review followed the PRISMA guidelines, with searches conducted in PubMed, Scopus, and Web of Science restricted to the past ten years. The methodological quality of the eligible studies was appraised using the Joanna Briggs Institute (JBI) Critical Appraisal Tools. From 659 initial records, 11 studies met the inclusion criteria: 4 focused on urban settings, 4 on rural settings, and 3 covered both. The prevalence of stunting in rural areas of Indonesia reached 53,3%, compared to 34,9% in urban areas. Rural stunting is primarily associated with structural and service-related limitations, including inadequate health services, sanitation, and maternal education. In contrast, urban stunting was more strongly associated with behavioral factors, particularly unhealthy dietary practices such as excessive snack consumption. These findings highlight the need for context-specific interventions, prioritizing structural improvements in rural areas and promoting healthier dietary behaviors in urban areas.

Abstract

Non-communicable disease (NCD) is a chronic health condition caused by various factors, such as age, physical activity, obesity, and economic status, which can increase the risk of unhealthy living conditions. This study aimed to identify factors associated with the occurrence of central obesity in patients with NCDs using a quantitative approach with a cross-sectional study design calculated using the Slovin formula, involving 207 people who filled out the questionnaire instrument. Sample collection using stratified sampling with inclusion criteria for patients with NCD at five community health centers in Gorontalo City from June to October 2024. After normality testing, the independent sample t-test and Mann-Whitney U test were used to analyze the data. The research results showed the characteristics of the respondents with diabetes mellitus (52,7%) and hypertension (25,2%). The overall incidence of central obesity was 75,8%. Bivariate analysis indicated a relationship between physical activity (p=0,008), sex (p=0,000), smoking habits (p=0,000), long illness of NCDs (p=0,049), and the incidence of central obesity, while there was no association between family history of NCDs (p=0,935), income (p=0,941), and central obesity. The factors related to the incidence of central obesity in NCDs patients are physical activity, sex, smoking habits, and prolonged illness.

Abstract

Based on the Indonesian Nutritional Status Survey and Indonesian Health Survey data, the prevalence of stunting in Indonesia since 2021 has decreased to 2023 24,4%, 21,6%, and 21,5% by 2023. However, research on the influence of supplementary feeding (SF) on stunting is limited. Thus, this study described the determinants of stunting, including SF consumption in the 6-23 months participant group, as this age group is considered part of The First 1000 days. This case-control study was conducted in the Tlogosari Wetan Community Health Center area, Semarang City, between July and August 2024, with 120 study participants aged 6-23 months (60 stunting cases and 60 controls). The prevalence of stunting at the Tlogosari Wetan Community Health Center was 14,1%. SF had a significant effect on stunting (p<0,023), and early breastfeeding initiation (p=0,053) and breastfeeding (p = 0,073) were weakly associated with stunting. However, per capita income (p=0,116), birth length (0,120), fathers’ education (0,328), and fathers’ body height (p=0,141) had no significant effects on stunting. In conclusion, SF was significantly associated with stunting and accelerated the decline in stunting rate. It is necessary to pay attention to SF consumption to prevent stunting and to increase the knowledge of early initiation breastfeeding (EIB), as it affects the practice of breastfeeding.

Abstract

Modern lifestyles that lead to sedentary behavior and unbalanced diets are major health concerns for adolescents. Sedentary behavior and unbalanced diets have a negative impact on nutritional status. This study aimed to identify the relationship between sedentary behavior, diet, and nutritional status in adolescents in Makassar City. This observational, analytical, quantitative, cross-sectional study was conducted on adolescents aged 14-15 years from five schools in Makassar from October to December 2024. A total of 286 respondents were selected using a Proportionate Stratified Random Sampling technique. Data were collected through questionnaires, interviews, and anthropometric measurements and then analyzed using the chi-square test. The results showed a significant relationship between sedentary behavior (p = 0,001) and diet (p = 0,031) with nutritional status. The majority had moderate sedentary behavior, poor diet, and good nutritional status. The study concluded that adolescents have diverse nutritional needs based on both direct and indirect factors

Abstract

The rapid development of digital technology has driven an increase in the utilization of online food ordering services in Indonesia, particularly among young adults, who constitute 45% of the users. This trend presents nutritional challenges that necessitate intervention through digital media education. Podcasts are regarded as effective and accessible educational mediums; however, previous studies have primarily focused on their impact on knowledge and attitudes. This quasi-experimental study aimed to analyze the influence of podcast-based nutrition education on improving nutritional knowledge and altering online food ordering intentions, frequency of orders, and food choices. This study employed a pretest-posttest control group design involving 128 non-health students from Muhammadiyah University of Semarang (December 2024 to March 2025), divided equally into experimental and control groups. The experimental group received four podcast episodes, each under 10 minutes, over four weeks, whereas the control group received no intervention. Data were collected through pre- and post-test assessments and engagement evidence, with purposive sampling based on inclusion and exclusion criteria. Data analysis using the Mann-Whitney U test revealed significant improvements in nutritional knowledge (p < 0,001), changes in ordering intentions aligned with the Theory of Planned Behavior constructs (p < 0,001), decreased ordering frequency (p < 0,001), and healthier food choices (p < 0,001). In conclusion, the findings suggest that podcasts have the potential to be an innovative strategy in nutrition education to promote healthy eating habits in the digital era.

Abstract

Older adults are a vulnerable group prone to degenerative diseases and decline in quality of life, often characterized by geriatric syndromes such as frailty. Nutritional intervention plays an important role in addressing these challenges. However, evidence of the effectiveness of dietary programs combined with structured nutrition education in nursing homes remains limited. This study aimed to evaluate the “Bijak Garam” program, a combination of a low-salt diet and nutrition education, on the dietary intake, blood biomarkers, physical function, and health status of older adults. A quasi-experimental study was conducted using a pre-post, one-group design with purposive sampling involving 100 participants. Dietary intake was assessed using a Semi-Quantitative Food Frequency Questionnaire (SQ-FFQ), physical function was measured using handgrip strength and the FRAIL scale, and blood biomarkers were analyzed using standard laboratory methods. Data were analyzed using paired t-test or Wilcoxon signed-rank test. The Bijak Garam intervention effectively improved dietary intake and reduced HbA1c, total cholesterol, LDL, triglyceride, and diastolic blood pressure (p < 0,05), while HDL levels increased significantly (p < 0,05). Most of the participants preferred plant-based protein sources. These findings demonstrate that the Bijak Garam program can improve dietary patterns and several health indicators among older adults, with the potential to be implemented more widely in geriatric nutritional interventions and public health policies.

Abstract

Diabetes mellitus is one of the major health issues in Indonesia, and Diabetes mellitus is a significant health issue in Indonesia, and consuming low-glycemic index foods can help manage blood sugar levels. This study aimed to develop a cookie formula incorporating red ginger by-products and analyze its proximate composition, fiber content, antioxidant activity, and glycemic index. The research methods included an organoleptic test involving 30 panelists, chemical composition analysis, and a glycemic index test on 20 subjects. Organoleptic test results showed that cookies with red ginger by-products had a more preferred color and taste than control cookies. Chemical analysis revealed an increase in dietary fiber content and antioxidant activity in cookies containing red ginger byproducts. The glycemic index test indicated that these cookies contributed to a more stable reduction in the blood sugar levels. The study concludes that incorporating red ginger by-products into cookie formulations can provide additional nutritional benefits for patients with diabetes mellitus while supporting sustainability through a zero-waste approach. These findings highlight the potential of red ginger by-products as a functional food ingredient that benefits both health and environmental sustainability, encouraging further exploration of food by-product utilization in functional food development.

Abstract

Autism is often associated with macronutrient deficiencies due to food sensory preferences, as well as gluten and casein allergies, which can trigger digestive disorders. In this study, gluten- and casein-free (GFCF) biscuits were developed by substituting kidney bean flour and keluih seed flour, followed by an evaluation of their energy content, macronutrient composition, and sensory characteristics to determine the best formulation. Four formulas were tested: F0 (100:0:0), F1 (30:52.5:17.5), F2 (30:35:35), and F3 (30:17.5:52.5). Macronutrient content was analyzed using standard laboratory methods (Kjeldahl, Soxhlet, and difference), whereas energy was calculated based on macronutrients. The hedonic test involved 50 untrained panelists, and statistical analysis was conducted using One-Way ANOVA, Kruskal-Wallis test, and a multi-attribute decision compensatory model. The biscuits contained 432–455 kcal of energy, 3-8% protein, 30-32% fat, and 32-40% carbohydrate. Significant differences were observed in energy (P =0,028), protein (P <0,001), fat (P =0,005), and carbohydrate (P <0,001), as well as sensory preferences for taste (P <0,001), aroma (P =0,002), and overall preference (P =0,017). F1 was selected as the best formulation because of its favorable sensory score and balanced nutrient content. Kidney bean flour improved protein and sensory properties, while keluih seed flour increased energy and fat but lowered preference levels. This product offers practical potential as a GFCF snack to support the dietary management of children with autism.
