Abstract

Diabetes Mellitus (DM) is a metabolic disorder characterized by hyperglycemia due to impaired insulin secretion or insulin resistance. HbA1c levels can reflect long-term glycemic control, while the neutrophil-to-lymphocyte ratio (NLR) serves as an inflammatory biomarker associated with disease severity in type II DM, including diabetic foot. This study is a descriptive analytic observational study with a cross-sectional design aimed at analyzing the relationship between HbA1c levels and NLR with Wagner’s grading of diabetic foot ulcers in type II diabetes mellitus patients. Among 34 diabetic foot patients, the majority were over 50 years old (94.1%), with a higher proportion of females (55.9%), an average HbA1c level of 9.33, and an average NLR of 9.31, indicating variations in glycemic control and systemic inflammation in ibnu sina Hospitak in januari until september 2023. Wagner's ulcer grade ranged from 2 to 4, with an average of 2.82, reflecting moderate to severe severity. Correlation analysis showed that HbA1c levels (r = 0.523, p = 0.002) and NLR (r = 0.602, p = 0.000) had a significant and positive relationship with diabetic ulcer severity. The study results indicate a significant correlation between HbA1c levels and NLR with Wagner’s grading of diabetic foot ulcers, where higher ulcer severity is associated with increased HbA1c and NLR levels due to greater insulin resistance.

Abstract

Surgical Site Infection (SSI) is a major healthcare-associated infection that significantly increases morbidity, mortality, and healthcare costs. This study aims to analyze the risk factors for SSI in post-appendectomy patients at Massenrempulu Hospital, Enrekang Regency, using a cross-sectional method with secondary data from patient medical records. A total of 41 patients met the inclusion criteria. The results indicate that SSI was more prevalent in females (65.9%) compared to males (34.1%), with the highest incidence occurring in the 17-25 age group (34.1%). In terms of nutritional status, most SSI cases were observed in patients with abnormal BMI, including underweight (22%), overweight (29.3%), and obesity (7.3%). Additionally, 92.7% of patients had no comorbidities, while a small percentage had hypertension (4.9%) or fatty liver disease (2.4%). Surgical duration was also a critical factor, as most SSI cases (92.7%) were associated with moderate-duration surgeries (60-120 minutes), while only 7.3% occurred in short-duration surgeries (<60 minutes). These findings highlight the importance of monitoring SSI risk factors, particularly among younger patients, females, and those with abnormal nutritional status. The study emphasizes the need for improved infection control strategies, proper nutritional management, and enhanced postoperative care to minimize SSI occurrence. Future research should consider primary data collection and additional variables to further explore risk factors. Additionally, healthcare facilities should enhance infrastructure and transition from manual to electronic medical records to improve patient management and reduce infection rates.

Abstract

Background: Tuberculosis is one of the deadliest infectious diseases in the world, Indonesia ranks third in the world with 824 thousand cases per year. Symptoms of Tuberculosis require a thoracic photograph-like examination with a molecular rapid test (TCM) examination. Although thoracic photographs are effective, they cannot detect infections in other organs, while TCM can diagnose more quickly at a higher cost. Objective: To determine the correlation between thoracic photographs and the results of the Molecular Rapid Test (TCM) of tuberculosis patients at the Lasinrang Pinrang Regional General Hospital. Research Methods: This study uses an observational analytical method with a cross sectional approach. The data used is secondary data, namely the patient's medical records. Research Results: Of the 100 patients studied at Lasinrang Pinrang Hospital, it was found that 80% showed infiltrate lesions on thoracic photographs, followed by 12% with consolidated lesions and 8% with fibrosis lesions. Molecular rapid test (TCM) results showed that 86% of patients were detected positive for Mycobacterium tuberculosis and sensitive to rifampicin, while 14% of TCM results were negative. Conclusion: The study showed that there was a significant relationship between the results of thoracic photo examination in the form of infiltrate lesions, consolidation, fibrosis and calcification to positive and negative results of molecular rapid test (TCM).

Abstract

Background: Chronic rhinosinusitis is an inflammation of the nasal mucosa and paranasal sinuses that lasts for ? 12 weeks, involving sinusitis and rhinitis simultaneously. In Indonesia, the incidence data is uncertain, but a study in Palembang in 2019 showed the proportion of chronic rhinosinusitis in adults was 33.3%. Given the limited data, high prevalence, and impact of this disease, a study was conducted to analyze the characteristics of chronic rhinosinusitis patients at Ibnu Sina Hospital Makassar. Methods: This study used an analytic observational design with a cross-sectional approach, involving 83 subjects selected by consecutive sampling. Data taken from medical records included age, gender, clinical manifestations, antibiotic treatment, and sinus location. Univariate analysis was performed using Microsoft Excel. Results: The results of univariate analysis showed that most chronic rhinosinusitis patients were aged 17-25 years (41.27%), followed by patients aged 26-35 years (26.98%). Male patients slightly outnumbered females, with 50.79% male and 49.21% female. The most common clinical manifestations were facial pain (63.49%), nasal congestion (53.97%), and headache (63.49%), while less common symptoms included cough (11.11%) and bad breath (4.76%). In terms of antibiotic treatment, cefadroxil was the most commonly administered (33.3%), followed by ceftriaxone (31.7%) and cefixime (24.2%). Regarding the site of sinus involvement, multi-sinusitis was the most common (60.5%), followed by unilateral sinusitis (34.2%) and pansinusitis (5.26%). Conclusion: Chronic rhinosinusitis at Ibnu Sina Hospital Makassar was most common in males aged 17-25 years, with facial pain as the main symptom, multi-sinusitis as the most common location, and cefadroxyl and ceftriaxone as the main antibiotics used.

Abstract

Diabetic ulcers are a chronic complication of diabetes mellitus that often occurs in the elderly, characterized by open wounds on the skin due to impaired blood circulation and peripheral neuropathy. This condition can disrupt skin integrity, increase the risk of infection, and worsen the patient's quality of life. Nursing care plays a crucial role in meeting skin integrity needs, including assessment, nursing diagnosis, planning, implementation, and evaluation. This study or case report aims to describe the nursing care process for elderly patients with diabetic ulcers based on the nursing process approach. The method used is a case study with data collection techniques through interviews, observation, physical examination, and documentation studies. The results of nursing care indicate that the main problems encountered are impaired skin integrity related to diabetic ulcers, the risk of infection, and acute pain. Interventions provided include standard wound care, self-care education, blood sugar monitoring, and prevention of further complications. Evaluation shows an improvement in wound condition, a decrease in pain complaints, and an increase in patient and family knowledge in ulcer care. Comprehensive nursing care has been proven to help improve the fulfillment of skin integrity needs and the quality of life of elderly patients with diabetic ulcers

Abstract

Growth is the increase in the size and number of cells and intracellular tissues, meaning the increase in physical size and structure of the body in part or in whole, so that it can be measured in units of length and weight. This period is a period that occurs quickly and will not be repeated, therefore it is often called the golden age. One thing that is related to growth is the provision of vitamin A and the provision of exclusive breastfeeding. This study uses a quantitative method with a cross-sectional approach with a sample of mothers who have toddlers in Lawak Village, Ngimbang District, Lamongan Regency, totaling 30 mothers. The results of this study obtained the F count value (128.091) > F0.05 (3.35) meaning that there is a significant influence between the variables of vitamin A provision and exclusive breastfeeding with toddler growth in weight. The F count value (123.225) > F0.05 (3.35) meaning that there is a significant influence between the variables of vitamin A provision and exclusive breastfeeding with toddler growth in height. So it can be concluded that there is a relationship between vitamin A provision and exclusive breastfeeding with toddler growth in weight and height.

Abstract

Bullying is defined as any deliberate, persistent, and harmful kind of intimidation or aggressive behavior directed at others by a person or group of individuals who are stronger or more powerful.  A person's evaluation of themselves, both favorably and unfavorably overall, is based on their level of self-esteem.  Bullying behavior can have an impact on how self-esteem develops, leading to teenagers having low self-esteem.  In today's schools, it is common for teenagers to be shy, lack confidence, or remove themselves from their friends since they have been teased by them. There are 10 male students and 7 female students from class VII who feel inferior because they are often teased by their friends, some of them also admit that they often call their friends rude names (animal names). In order to examine the connection between bullying and self-esteem in SMPN 7 Palangka Raya adolescents in class VII, this study used a cross-sectional strategy and a correlational study design.  151 pupils in class VII of SMPN 7 Palangka Raya made up the study's population, while 33 respondents made up the sample. At the end, the p value is 0.000 <0.05 when the significant value is 0.05 using the Chi square test (0.000).  In order to support the H1 hypothesis, adolescents in class VII of SMPN 7 Palangka Raya exhibit a substantial correlation between bullying and self-esteem. Conclusion: The study's findings show a strong correlation between bullying and self-esteem among teenagers in SMPN 7 Palangka Raya's seventh grade.

Abstract

Cyberbullying can be defined as a form of bullying that utilizes digital technology to intentionally and repeatedly harm others. One effective effort to prevent cyberbullying is through health education, which aims to increase adolescents’ awareness and understanding of this issue. Early prevention plays a critical role in helping teenagers recognize, identify, and respond to cyberbullying behavior. Improved knowledge can enhance adolescents’ concern for their safety both in school and in their surrounding environments. This study aimed to examine the effect of health education on the knowledge of female students about cyberbullying at Palangka Raya Christian Middle School. The research employed a pre-experimental design with a one-group pre-test and post-test approach. A total of 31 female students were selected using total sampling. The results showed a significant increase in students’ knowledge after receiving health education. The Wilcoxon signed-rank test indicated a statistically significant difference, with a p-value of 0.000 (p < 0.05). In conclusion, the study demonstrates that health education has a significant effect on increasing the knowledge of teenage girls about cyberbullying. Preventive strategies such as providing relevant health education can be effective in reducing cyberbullying behavior and fostering a generation of digitally responsible and healthy adolescents.

Abstract

The National Health Insurance (JKN) mobile application provides convenience for BPJS participants. This convenience includes paying BPJS contributions, updating participant data, accessing family member data, and more. The purpose of this study was to specifically evaluate the perceived effectiveness and practical benefits of the JKN mobile application in improving service access among users in Sibolga City. This research employed a descriptive qualitative method, involving four purposively selected informants: two BPJS Health officers and two active users of the JKN application. Data were collected through in-depth interviews, direct observation, and document review. The study found that the JKN mobile application significantly enhances healthcare service accessibility and user autonomy. Informants reported that essential features-such as digital membership cards (E-ID), online queue registration (antrean online), and contribution tracking-were particularly useful in reducing administrative burden and time. Users no longer need to physically visit the BPJS Health office, as the application-readily available on the Play Store-enables them to access services anytime and anywhere.
