Abstract

The price of drugs that win the e-Catalogue tender has long been considered too low by some pharmaceutical companies, and some even below their production cost. On the other hand, the fact that only pharmaceutical products with BPOM distribution licenses are eligible for the price tender, the quality of such e-Catalogue winners should not be of concern. This study aimed to evaluate the quality of several items of antibiotic products, an important drug category that can cause serious impacts if its quality is compromised. Samples used in the study were product items with the highest demands: Amoxicillin 500 mg, tetracycline 500 mg, cefadroxil 500 mg, and ciprofloxacin 500mg listed in the e-Catalogue 2017 as well as their generic and branded generic (OND) alternatives, and its respective originators. The quality parameters of the samples were evaluated according to the Indonesian Pharmacopoeia 5th Edition (FI-V). Antibiotic potency assays were conducted by using the Plate-Cylinder Method with S. aureus ATCC 25922 and E. coli ATCC 6538 as the testing bacteria. The results showed that all samples met the FI-V requirements on antibiotic content, weight uniformity, disintegration time, and dissolution. However, antibiotic potency of the e-Catalogue 2017 winners tend to be lower than their generic and OND alternatives as well as their respective originators, with a wide disparity: 10.3–18.2 percentage points, or a relative disparity of 12.0–26.4%, except for amoxicillin 500 mg caplet, in the assays using S. aureus. In conclusion, tetracycline 500 mg, cefadroxil 500mg, and ciprofloxacin 500 mg caplets/tablets/capsules listed in the e-Catalogue 2017 contain active pharmaceutical ingredients of lower qualities than their more expensive alternatives

Abstract

For the development of natural medicines from the ethanolic extract of Syzygium myrtifolium Walp leaves it is necessary to know the pre-clinical safety test so that its safety if used in the long term. The subchronic toxicity test of the ethanolic extract of Syzygium myrtifolium Walp leaves l was carried out by assessing the kidney and liver function of the test animals given after 60 days of test preparation, in the form of blood biochemical examination using the Mindray®BA-88A device and histological examination of the kidneys and liver. The results showed that the administration of ethanol extract of Syzygium myrtifolium Walp leaves at doses of 300, 600 and 900 mg/kgBW did not significantly affect the activity of SGOT, SGPT, serum creatinine levels (p>0.05). Microscopically on the percentage of damage to the glomerulus of the right and left kidneys. at doses of 600 and 900 mg/kgBW there was a significant difference to the control group (p<0.05). For qualitative microscopic examination, namely observation of sinusoids and hepatocyte cells at each level, there were abnormal changes. Meanwhile, quantitative microscopic examination at doses of 600 and 900 mg/kgBW caused an increase in the percentage of central vein damage (p<0.05). From the research, it was found that the ethanolic extract of Syzygium myrtifolium Walp leaves of white mice was safe for liver and kidney function at a dose of 300 mg/kgBW, but the doses group of 600 and 900 mg/kgBW gave a toxic effect on the kidneys and liver when compared to the control group.

Abstract

Curcumin has various activities such as antioxidant, anti-inflammatory, antitumor, and antibacterial. The bioavailability of curcumin is low and needs to be optimized. The manufacture of curcumin nanoparticles with several solvents is one way to increase the bioavailability of curcumin. This study aims to examine the safety of curcumin nanoparticles through acute and subchronic toxicity tests and the antioxidant activity of the formula. Acute toxicity test was performed by OECD 425 method, using AOT425StatPgm device for the LD50. Subchronic toxicity test was carried out by OECD 407 method using 4 groups: normal, a dose of 50;100; and 200 mg/kg BW for 28 days, then evaluate body weight, clinical biochemistry, and hematology. An antioxidant activity test was carried out using DPPH. The results showed were the LD50 value of curcumin nanoparticles was>5000 mg/kg BW and the results of the subchronic toxicity test (p>0.05) did not show any significant differences in AST, ALT, BUN, creatinine, albumin, and hematology compare to the normal group. The IC50 of curcumin and curcumin nanoparticles were 118.76 µg/mL and 90.87 µg/mL, respectively. It can be concluded that curcumin nanoparticles are safe and do not affect kidney and liver function and the antioxidant activity of the curcumin nanoparticle formulation is greater than that of curcumin.

Abstract

Sappan wood (Caesalpinia sappan L.) was reported to have natural antioxidant properties. Brazillin is the compound responsible for its antioxidant effect. Antioxidants are needed to prevent premature aging of the skin. Essence is a cosmetic that contains topical antioxidants. Essence has advantages over other skincare products, and essence is easier to absorb into the skin. The objective of the present study was to optimize and evaluate the Essence of sappan wood ethanolic extract for treatment as a cosmetic antioxidant. Simplex lattice design was adopted to evaluate the effect of butylene glycol and glycerin concentration on Essence characteristics. The method used in the extraction is maceration with ethanol solvent. The results showed that both butylene glycol and glycerin significantly enhanced the viscosity and pH value of the sappan essence formulation. The optimum Essence formula consisted of 10% butylene glycol with a predicted viscosity value of 2,944 dPas and a pH of 5,075. The characteristics of the optimum formula of sappan wood extract Essence have a thick texture, a characteristic odor of colored extract brownish yellow, homogeneous, and has a spreadabillity of 14 cm.

Abstract

Patient safety is a fundamental element in providing patient care and is a component of patient rights. Pharmaceutical services are one aspect of patient safety implementation. Patient safety is a variable used to monitor and evaluate the service quality of pharmacy staff in hospitals. This study aims to determine the relationship between the demographic characteristics of pharmaceutical personel with the implementation of patient safety at the Sultan Agung Islamic Hospital, Semarang City. This research is an analytic observational study using a cross sectional design. The research subjects who met the inclusion criteria were 39 pharmacists. Data collection was carried out in November 2020 using a questionnaire adapted from 6 patient safety aims by the 2018 National Hospital Accreditation Standard (SNARS) edition 1.1. Data analysis using Spearman's-Rho and Eta correlation test. The results of research conducted from 39 respondents showed that the implementation of patient safety has been carried out properly. This is due to there was a demographic correlation on patient safety including length of work for patient safety of 0.672 and a weak correlation between age and education status on the implementation of patient safety at the Sultan Agung Hospital, Semarang. The correlation coefficient values are 0.238 and 0.370 with a significance value of p<0.05. There is a significant relationship between length of work and the application of patient safety and there is no significant relationship between gender, profession, marital status, age, last education and place of work with a significance of p> 0.05.

Abstract

Propofol is one of the most commonly used intravenous drugs employed to induce and maintain general anesthesia. The effective induction dose of propofol is 1.0-2.5 mg/kg IV caused hemodynamic instability significant as hypotension. Stability hemodynamic in used propofol can optimized with midazolam-fentanyl. The aims of this study was to determine the comparison of side effects propofol-fentanyl and propofol-fentanyl-midazolam in patient undergoing elective surgery with parameter of blood pressure, pulse, respiratory rate (RR). This study was a observational with prospective method in 60 patients undergoing elective surgery with physical status ASA I-II conducted in the period of April to July 2019. The result showed the induction anesthesia used of propofol-fentanyl caused decreased on systolic blood pressure 17,2±3 mmHg, diastolic blood pressure 5,3±1,6 mmHg, pulse 3,9±1,5 beats/minute, and RR 1,4±0,01 breaths/minute. The induction anesthesia used of propofol-fentanyl-midazolam caused decreased on systolic blood pressure 11,8±0,4 mmHg, diastolic blood preasure 4,2±0,4 mmHg, pulse value 5,0±0,9 beats/minute, and RR 2,8±0,9 breaths/minute. Side effects in the form of blood pressure, pulse and respiratory rate from the administration of the combination of propofol-fentanyl and propofol-fentanyl-midazolam were not significantly different.

Abstract

This study aims to see the genetic diversity of the musk mango (Mangifera casturi) plant in South Kalimantan based on DNA barcode techniques using sequences from the rbcL gene and the matK gene. The plant samples used in this study were the leaves of the musk mango (Mangifera casturi) taken from eight regencies and cities in South Kalimantan Province, namely from Amuntai, Astambul, Barito Kuala, Peat, Kandanga, Babirik, Tabalong, and Tanah Laut. Isolation and DNA analysis of the musk mango plant was carried out in the research laboratory of the Faculty of Biotechnology, Atma Jaya Catholic University of Indonesia, Jakarta. The analysis of the genetic diversity of the musk mango began with the stage of amplifying genomic DNA (gDNA) using rbcL and matK primers. The results of the DNA isolation samples showed a high concentration level of 17-91 ng/µL with a purity of 1.72 -1.92 ng/µL. When PCR was carried out using rbcL and matK primers, from 8 areas in South Kalimantan, 5 samples (Babirik, Tabalong, Kandangan, Amuntai, Batola, Peat, and Tanah Laut) were amplified with rbcL primers and 6 samples (Tanah Laut, Babirik, Tabalong , Amuntai, Peat, and Kandangan) amplified with matK primer. The relationship between the musk mango in South Kalimantan can be seen from the phylogenetic tree.

Abstract

The most common causative factor in patients with type 2 diabetes is insulin resistance. Achyranthes Aspera Linn. is known as a medicinal plant which has been shown based on previous research to reduce total cholesterol, triglyceride, and blood glucose levels, but the effective dose is not yet known in insulin resistance rats. Further study is necessary to ensure the effect and also the dose. The purpose of the research was to determine the activity of the extract in reducing levels of total cholesterol, triglycerides, blood glucose, and OGTT in insulin resistance rats. 7 groups (each group consist of 4 animals) were divided, i.e. normal control (not induced), negative group (given with Na-CMC 0.5%), positive group 1 (fenofibrate), positive group 2 (metformin HCl), extract group dose 250, 500, and 750 mg / kg BW for 14 days of treatment. Induction of insulin resistance using streptozotocin. Total cholesterol, triglyceride, blood glucose, and OGTT levels were measured using a clinical spectrophotometer. Oneway ANOVA followed by the Tukey test were used to analyze the data. Data indicated that the third dose extract (750 mg/kg BW) could lower total cholesterol, triglycerides, blood glucose, and OGTT with the percentages of reduction of 48.83%, 48.95%, and 61.31%, respectively, and at OGTT the smallest AUC value was 23,991 mg/dL per minute.

Abstract

This research aims to prove the provision benefits of Rosella (Hibiscus sabdariffa Linn.) calyx extract to increase hemoglobin and hematocrit levels in female adolescents anemia. This research is a true experiment with a randomized pre-test and post-test control group design. Totals of samples were 40 female adolescents with anemia according to the inclusion and exclusion criteria divided by randomization sampling technique, consisting of two groups intervention groups and control groups. Data analysis used paired t-test statistical test to determine the difference between pretest and post-test each group and an independent t-test to determine the difference between the different groups. The average difference in hemoglobin and hematocrit levels in the intervention group was higher than in the control group. The average difference in hemoglobin levels in the intervention group was 1.8 g/dL in and the control group was 0.9 g/dL, (p < 0.05), The difference in the average hematocrit level in the intervention group was 5.6% and the control group was 2.7% (p < 0.05). The researchers also found significant statistics differences between hemoglobin and hematocrit levels of female adolescents from intervention and control groups s with a p-value < 0.05. The combination of Rosella (Hibiscus sabdariffa Linn.) calyx extract and Fe2+ tablet was effective to increasing hemoglobin and hematocrit levels in female adolescents with anemia.

Abstract

The boat lily (Tradescantia spathaceae) leaves have purple pigments produced from anthocyanins. This compound is thought to have sunscreen activity. The purpose of this study was to test the potential of sunscreen for both infusion and gel preparations of boat lily leaves in vitro using a spectrophotometric UV-Vis and determine its stability. The gel formula with a concentration of 12.5%; 25% and 50%. The parameters observed in this study were organoleptic, homogeneity, pH, dispersion test, viscosity, the value of Sun Protection Factor (SPF), and storage time test. The greater concentration of infusions, the lower the viscosity and inversely proportional to the dispersion value. The preparation has a good pH and follows the skin’s pH, which is in the range of 5.87-7.00. The boat lily infusion has SPF values: 3.64; 6.83, and 12.73. In comparison the boat lily gel has SPF in succession, namely: 7.48; 9.55, and 14.06. Based on the research, it can be concluded that the infusion of the boat lily can be made in a gel dosage form. Both infusion and infusion gel preparations of pineapple shellfish at a concentration of 12.5% to 50% have low protective power and boat lily infusion gel is unstable during storage.
