Abstract

Breast milk production should be prepared from pregnancy, especially in the third trimester. Many pregnant women in their third trimester have not yet produced colostrum. This impacts breast milk production in the first week postpartum, thus triggering the use of formula milk. One effort to stimulate colostrum production is by consuming carrot juice and honey. This study aims to determine the effect of giving carrot juice and honey to pregnant women in their third trimester on the timing of colostrum production as an effort to achieve successful breastfeeding at Susiani Independent Midwife Practice Banyuwangi in 2025. This research method uses a quasi-experimental two-design. The study used a post-test only with a control design. The population of this study was 60 respondents, 30 of whom were in the intervention group and 30 in the control group, with consecutive sampling as the sampling technique. The intervention was administered and consumed for 7 days, twice a day. The control group was given basic IEC (Information, Education, and Communication)  care for the Third Trimester. Normality and homogeneity tests were carried out, and the Levene test obtained normal values , so data analysis used the Independent Samples T-Test. The results showed that the average time for colostrum release in the intervention group was 4.87 days, with the fastest time on day 2 and the longest on day 7. In contrast, in the control group, it was 8.10 days, with the fastest time for colostrum release on day 4 and the longest on day 12. The results of the Independent Samples T-Test with a p value = 0.000 (<0.05), which means that there is an effect of giving carrot juice and honey to pregnant women in the THIRD TRIMESTER on the time for colostrum release as an effort to achieve successful breastfeeding. The results of this study will enable midwives to provide IEC on carrot juice and honey and to implement it as support for successful breastfeeding.

Abstract

Introduction: Uremic pruritus or Chronic Kidney Disease-associated Pruritus (CKD-aP) is a common complication in hemodialysis patients, with a prevalence of 30-70%. Pharmacological therapy is often ineffective or has side effects, so non-pharmacological therapy is an important alternative. Objective: This systematic review aims to identify and evaluate non-pharmacological therapeutic interventions in reducing the intensity of uremic pruritus in hemodialysis patients. Methods: The systematic review uses the PRISMA 2020 guidelines. The databases used in the article search consist of EBSCO, PUBMED, ProQuest, CINAHL, and Wiley using relevant keywords based on the topic and title of the study. Article selection uses the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) diagram method, with a total of 4,127 articles obtained. 5 articles were obtained according to the inclusion criteria and analyzed descriptively narratively and have met the quality review using the JBI (Joanna Briggs Critical Appraising Methodology) guidelines. Results: Five research articles with non-pharmacological therapeutic interventions consisting of topical Anethum Graveolens oil, topical Sweet Almond oil, topical violet oil, acupressure and auricular pressure. Conclusion: The findings of this systematic review indicate that non-pharmacological interventions in the form of topical Anethum Graveolens oil, topical Sweet Almond oil, topical violet oil, acupressure and auricular pressure can reduce the severity of uremic pruritus in hemodialysis patients. Non-pharmacological therapy has the potential to be an effective, safe, and easy-to-implement intervention to reduce uremic pruritus in hemodialysis patients. However, high-quality studies are needed to ensure long-term effectiveness.

Abstract

AbstractBackground: Simulation-based training is an effective learning method to enhance cognitive and psychomotor competence in first aid education. However, vocational health students often experience difficulties in translating theoretical knowledge into practical skills. Objective: This study aimed to analyze the effectiveness of simulation-based first aid training in improving the knowledge and skills of students at SMK Kesehatan Bakti Nusantara Gorontalo. Method: A pre-experimental one-group pre-test–post-test design was used with a total of 60 respondents. Knowledge and skills were measured before and after the intervention. Data were analyzed using paired statistical tests. Results: The findings showed a significant improvement in both domains. Knowledge scores increased from 63.25 ± 6.42 to 85.47 ± 4.83 (Δ = 22.22; p = 0.000). Skills scores increased from 68.10 ± 5.98 to 88.92 ± 5.26 (Δ = 20.82; p = 0.001). Both results indicate that simulation-based training effectively enhanced students’ cognitive and psychomotor abilities. Conclusion: Simulation-based first aid training significantly improves knowledge and skills among vocational health students. Integrating structured simulation activities into the curriculum is recommended to strengthen competency achievement and support students' preparedness in real emergency situations.Keywords: simulation-based training, first aid, knowledge, skills, vocational health students

Abstract

AbstractPreeclampsia is one of the leading causes of maternal morbidity and mortality in Indonesia and is frequently underdiagnosed, particularly in primary healthcare facilities such as community health centers (Puskesmas). Delayed early identification increases the risk of severe complications for both mothers and infants. Currently, Antenatal Care (ANC) services at Puskesmas rely on routine vital sign monitoring without an integrated scoring system that could provide early warning of preeclampsia risk. This study aimed to develop and validate an Early Warning Score (EWS) model based on vital signs and clinical symptoms for early detection of preeclampsia in primary healthcare settings. A prospective cohort design was employed, involving 65 third-trimester pregnant women who were followed until delivery. Data collected included blood pressure, heart rate, respiratory rate, and symptoms such as headache, blurred vision, epigastric pain, and edema. Bivariate analysis using the Chi-Square test was conducted to identify variables associated with preeclampsia, followed by multivariate logistic regression to determine significant predictors for model construction. Model validity was assessed using the ROC curve for discrimination, the Hosmer–Lemeshow test for calibration, and Decision Curve Analysis (DCA) for clinical utility. The incidence of preeclampsia among respondents was 12.3% (8 out of 65 participants). Three significant predictors were identified: systolic blood pressure ≥140 mmHg (OR 5.21; p=0.003), headache (OR 6.45; p=0.002), and blurred vision (OR 4.78; p=0.015). The developed EWS model demonstrated excellent discrimination (AUROC 0.86) and satisfactory calibration (Hosmer–Lemeshow p=0.521). DCA results indicated meaningful net clinical benefit at probability thresholds of 10–30%. In conclusion, the proposed EWS model is accurate, well-calibrated, and clinically beneficial, and is recommended as a screening tool for early detection of preeclampsia in Puskesmas to improve ANC services and reduce maternal complications

Abstract

This study aims to implement nursing care in a non-hemorrhagic stroke case in an inpatient setting, using Dorothea Orem's nursing model theory. The focus of this case study is to provide comprehensive nursing care to Mr. S based on Orem's conceptual framework. The research method used is a case study with a nursing care approach based on Orem's theory. Data were collected through interviews, observations, physical examinations, and using an assessment format adapted to Orem's nursing theory.The initial assessment of Mr. S revealed a blood pressure of 180/100 mmHg, a pulse rate of 80 beats/min, SpO2 of 95% with 3 lpm of oxygen, and a respiratory rate of 26 breaths/min. From this data, two primary nursing problems were identified: the risk of decreased cerebral tissue perfusion and an ineffective breathing pattern. Nursing interventions aligned with the Indonesian Nursing Outcome Standards (SLKI). After three days of nursing care for Mr. S, it was concluded that both nursing interventions had partially resolved the problem. This study recommends that nurses consistently implement and improve the quality of nursing care using Dorothea Orem's self-care theory.

Abstract

Stunting is a chronic nutritional problem that has a long-term impact on the quality of human resources and needs to be prevented from the beginning of life. The delivery process is a crucial phase in determining the baby's initial condition through the quality of intranatal care, the prevention of complications, and the success of early breastfeeding initiation (IMD). However, the practice of intranatal care in health care facilities still faces obstacles in the form of suboptimal collaboration between professions and the use of digital technology. This study aims to analyze the influence of the TECH4HEALTH Digital Technology-Based Collaborative Intranatal Care Model on the optimization of healthy births in stunting prevention. This study uses a quasi-experimental design with a pretest–posttest with control group. The sample consisted of 80 maternity mothers who were divided into an intervention group (n=40) and a control group (n=40). The intervention group received collaborative intranatal care based on digital TECH4HEALTH technology involving midwives, doctors, nurses, and nutritionists, while the control group received conventional intranatal care. The research variables included the quality of intranatal care, the success of IMD, the incidence of asphyxia, the baby's birth weight, and the level of maternal understanding of stunting prevention. The results showed a significant improvement in the quality of intranatal care in the intervention group with an average score increasing from 72.4 to 88.6 (p<0.001), while the control group only increased from 73.1 to 76.5. The success of IMD in the intervention group reached 87.5% compared to 62.5% in the control group. The incidence of asphyxia decreased to 5% in the intervention group compared to 17.5% in the control group, and the proportion of infants with normal birth weight was higher (92.5% vs. 77.5%). The level of maternal understanding of stunting prevention increased significantly (p<0.001) after the application of the TECH4HEALTH model.

Abstract

Introduction: Stunting is a chronic malnutrition problem caused by inadequate nutritional intake over a long period. Mothers play an important role in the family to overcome nutritional problems, especially those related to parenting and family nutrition. Objective: The purpose of this study was to determine the relationship between education on complementary foods for breast milk made from local foods and the knowledge and motivation of mothers of toddlers as a strategy in preventing stunting in the 3T area. Method: Quantitative correlational research design with a cross-sectional approach. The number of research samples was 50 mothers with a simple purposive sampling technique. Results: The results showed a p-value = 0.001 which was smaller than α (<0.05). The correlation coefficient value (r = 0.465) shows that there is a relationship between education on complementary foods for breast milk made from local foods and the knowledge of mothers of toddlers in efforts to prevent stunting also the p-value = 0.000 which is smaller than α (<0.05) The correlation coefficient value (r = 0.554) shows that there is a relationship between the duration of education on complementary foods for breast milk made from local foods and the motivation of mothers of toddlers in efforts to prevent stunting. Conclusion: there is a relationship between education on complementary foods for breast milk made from local foods and the knowledge and motivation of mothers of toddlers as a strategy in preventing stunting in the 3T area

Abstract

High-risk pregnancy is a maternal health condition with a greater likelihood of complications that may adversely affect both mothers and newborns. Limited maternal knowledge regarding pregnancy risks often contributes to inadequate childbirth readiness, leading to delays in recognizing danger signs and seeking appropriate medical care. This study aims to analyze the relationship between maternal knowledge and childbirth readiness among high-risk pregnant women. A descriptive correlational design with a cross-sectional approach was employed. The study involved 100 high-risk pregnant women selected through purposive sampling in Kalibaru Wetan, Banyuwangi. Data were obtained through a validated and reliable questionnaire, and analyzed using the Chi-Square test. The results indicated that 67.0% of respondents had good knowledge, while 63.0% demonstrated high childbirth readiness. Cross-tabulation showed that mothers with good knowledge tended to have good readiness (89.6%), whereas poor knowledge was associated with low birth preparedness. Statistical analysis revealed a significant relationship between knowledge and childbirth readiness (p < 0.05), indicating that better maternal knowledge contributes to increased physical, psychological, and logistical readiness for childbirth. These findings highlight the importance of strengthening educational interventions targeting high-risk pregnant women, particularly through structured antenatal counseling, maternal health classes, and family involvement. Improved knowledge is essential not only for recognizing risk symptoms but also for enhancing confidence, decision-making, and preparedness to seek timely obstetric care. Health workers are recommended to implement participatory and continuous educational programs to optimize maternal readiness and ultimately reduce preventable maternal and neonatal complications.

Abstract

Analysis of the interpersonal relationship theory by Hildegard E Peplau explain the basic concepts of the theory that focuses on human, health, nursing and environmental aspects. This theory is also one of the most influential nursing models in psychiatric nursing practice, especially in treating patient at risk of violent behavior.  In its application, this theory forms a dynamic and continuous interpersonal relationship between nurses and patients. This interpersonal relationship is divided into four phases of therapeutic relationship between nurses and patients, namely the orientation phase, the identification phase, the exploitation phase and the resolution phase. Here we discuss the concept of Peplau’s theory and its application to psychiatric nursing care in the kutilang ward of the Bengkulu Provincial Mental Hospital for patient with medical diagnosis of schizophrenia who experiences auditory hallucination and is at risk of violent behavior. The application of the theory focuses on therapeutic communication, assessing, triggers of aggression, establishing trusting relationships and developing adaptive coping through. Structured interventions based on the SDKI-SLKI-SIKI. The result of the application show a decrease in improvement in patient’s self-esteem. The theoretical analysis shows that the Peplau model has advantages in building effective interpersonal relationships and increasing patient involvement in the recovery process. However, this theory has limitations in patients with severe psychosis who find it difficult to establish optimal therapeutic contact, and it underemphasizes the biological aspects often needed in the acute phase. Overall, Peplau’s theory has proven relevant and effective as humanistic, relationship-oriented approach to psychiatric nursing.

Abstract

The 2023 Indonesia Health Survey reported that 60.7% of deliveries in Jakarta occurred in hospitals, while only 14.2% took place in primary health care (puskesmas), indicating a gap between antenatal care (ANC) utilization and delivery services at puskesmas despite high ANC attendance. This qualitative exploratory study was conducted at Tanah Abang and Kemayoran Primary Health Care from July to October 2025, involving 114 respondents in focus group discussions, with data analyzed using thematic analysis, triangulation, and inter-rater agreement. The findings identified six major themes related to ANC and delivery place selection, influencing factors, perceptions of puskesmas services, and maternal health education. Distance and accessibility were the main reasons for choosing puskesmas for ANC, while hospitals were preferred for delivery due to perceived higher safety, availability of specialists, and more complete facilities, particularly for high-risk pregnancies. Utilization of maternal health services was strongly supported by the BPJS/JKN program, as well as family support, prior delivery experiences, and socioeconomic factors. Although puskesmas services were generally perceived positively, gaps were identified in lactation counseling and antenatal class implementation, indicating the need to strengthen health education and provider communication to enhance trust and utilization of puskesmas delivery services.
