Abstract

Background: As the population ages, it is increasingly crucial to develop community-centered health interventions. This study aims to describe and assess sociodemographic information, health characteristics, health utilization, and out-of-pocket expenses among older people in Ngestiharjo Village, Bantul, Yogyakarta. It was part of an initiative to empower community health workers to understand and address the health needs of older people. Methods: A cross-sectional survey was conducted from April to July 2023 involving 330 participants, with a targeted sample size of 356 elderly individuals. Data on sociodemographics, health status, health utilization, and expenditure were collected through interviews with older people or their companions. Results: Preliminary data showed that most participants were female (58.77%), married (64.91%), and primarily received health information from health workers (47.37%). Hypertension and joint and bone diseases were prevalent, with 16.67% likely experiencing depression. 11.4% of the respondents were hospitalized in the past year, with an average of 2.15 hospitalizations. Private hospitals (69.23%) were the most common healthcare facilities used for hospitalization, and 76.92% consistently utilized health insurance. The average out-of-pocket expenditure for hospitalization was 437,500 IDR. Community health centers (36.36%) and private hospitals (45.45%) were predominantly used for outpatient care, and 77.27% always used health insurance. The average out-of-pocket expenditure for outpatient care was 126,200 IDR. Conclusions: The study underscores the importance of empowering community health workers to effectively disseminate health information, fortify integrated health posts, manage common health issues, and tackle barriers to healthcare utilization and expenditures among older people.

Abstract

Introduction: The global prevalence of obesity is on the rise, including in Indonesia. High energy intake from sugar is a growing concern worldwide due to its association with weight gain, tooth decay, and non-communicable diseases. A prior study revealed that 10.9% of Indonesian adults consumed energy from sweetened beverages in excess of the WHO recommendation. This study aimed to assess the consumption of sweetened beverages in male and female college students in Indonesia. Method: The study was conducted at Universitas Indonesia and involved 107 subjects aged 19-21 years who resided in student dormitories. Result: The result found the median BMI in female was higher than male. In female, 3 out of 10 students was overweight-obese, while 1 out of 10 in male. Interestingly, the median daily intake of sweetened beverages significantly higher in male than female; 387 (25–1580) vs 246 (0–762) ml/day (p=0.001). Added sugar content in beverages in males was higher than half of Indonesian recommendation (28.4 [1.9–134.4] vs 19.2 [0–61.1]) which contributed to energy as much as 170.9 (12.4–625.7) kcal/ day or about 8.9% of total energy intake than female (108.5 [0–394.4] kcal/day). Compared to Indonesian guideline, 12% of male subjects consumed added sugar higher than 50 g/day. Simultaneously, 7 out of 10 subjects consumed energy from sweetened beverages higher than 10% of their total energy intake (WHO recommendation). The BMI and prevalence of overweight-obese were higher in female, while the sweetened beverage was higher in male. It might be caused by the physical activity in male was higher than female. Although male had higher energy intake from SSB, their energy output through physical activity balanced with energy input. Conclusion: These finding alerted the university as educational institution should take an action to prevent this issue since the prevalence of overweight-obesity and sweetened beverage consumption has occurred in young age.

Abstract

Psychosocial health is one of the most important factors in achieving optimal growth and development in children. The increase in psychosocial problems among children has been accompanied by a decrease in the rate of exclusive breastfeeding over the past two years. Another important aspect is the history of natural disasters such as tsunamis, military operations, and the implementation of Islamic law in Aceh. This research aims to explore the relationship between exclusive breastfeeding and psychosocial problems in children in the city of Banda Aceh.The research method employed in this study is an analytical observational design with a cross-sectional approach. The population of this research comprises all children aged 4-6 years old. Samples were obtained using a cluster sampling technique, involving 296 children from 5 villages around one of the public health centers in Banda Aceh. Data analysis in this study is conducted using a logistic regression test.The results of the study indicate that the percentage of children aged 4-6 years in Banda Aceh who have received exclusive breastfeeding is 261 children (88.18%), while the number of children with psychosocial problems based on the Pediatric Symptom Checklist 17 is 27 children (9.12%). There is a significant relationship between exclusive breastfeeding and psychosocial problems in children aged 4-6 years in the working area of the Baiturrahman Public Health Center in Banda Aceh. Children who did not receive exclusive breastfeeding have a 5 times higher risk of experiencing psychosocial disorders compared to children who received exclusive breastfeeding during the first 6 months of life

Abstract

 Introduction: As one of Indonesia's largest regional government-owned hospitals, Hospital X faces high nursing staff resignations, with turnover rates surpassing the ideal rate of 4%. Job dissatisfaction and burnout indication contribute to nurses' intention to leave. Consequently, this study analyses job satisfaction and burnout on nurses' turnover intention at X Regional Hospital. Methods: A cross-sectional study was conducted between November 14 and December 14, 2022. Data was obtained through Google Forms and distributed to 293 nurses with a total sampling technique. Data was analyzed using Structural Equation Modelling (SEM) with the assistance of SmartPLS 4 software. Results: As many as 274 respondents were declared willing to participate in this study. The job satisfaction has an insignificant negative influence on nurses' turnover intention. However, job satisfaction has a significant negative relationship with job burnout, and job burnout has a significant positive effect on nurse turnover intention and a significant adverse effect on job satisfaction. Demographic factors also significantly influence nurses' job satisfaction and job burnout. Then, in the indirect effect test, job burnout mediates the relationship between job satisfaction and nurses' intention to leave work. At the same time, demographics affect nurses' intention to leave work through their level of job burnout. Conclusion: However, the results indicate no significant influence of job burnout mediated by job satisfaction and demographic factors mediated by job satisfaction on turnover intention. Hence, HR management should support and implement proactive measures, including revising the SOP for nurse schedule exchanges. Additionally, counseling programs should be considered to address low personal achievement issues among nurses.

Abstract

The subject matter of science, especially related to human anatomy, is one of the most challenging to understand. To help understand science materials, especially human body anatomy, we conducted community service by developing the Anato-Smart book as a learning media for students and teachers of 5th grade of Muhammadiyah 1 Elementary School Surakarta in 2021. The method of implementation in this service includes several stages, including determining the topic of the body system for the learning media that would be developed through the Focus Group Discussion for teachers at Muhammadiyah 1 Elementary School Surakarta, followed by the development of interactive and educational story media Anato-Smart with the title “The Adventures of Andi and Si Balung”, then media trials to 5th graders at Muhammadiyah 1 Elementary School Surakarta, as well as interviews and outspread questionnaires for the evaluation of Anato-Smart media. This service activity resulted in an Anato-Smart teaching media developed to hone the students' analytical skills on limb and skeletal topics. Teachers can use this book to augment interactive lessons and as one of the educational resources for students to learn about potential risks and disorders to the limbs and skeleton in daily life. From the evaluation, the story and education concepts in this Anato-Smart media book are intriguing and facilitate comprehension, however, it is better to lessen the words and increase the number of images to make it more interesting for children. We will use this feedback to improve the final results of this media so that it can be used by other schools, not only by Muhammadiyah 1 Elementary School Surakarta. Collaboration with other parties to develop teaching media in the future is needed to produce educational and interactive teaching media.

Abstract

Introduction: The COVID-19 pandemic has significantly impacted the mental health and safety of the elderly, particularly affecting elderly women. Research indicates a rise in anxiety and depression among older adults, with pre-existing elevated depressive symptoms exacerbating their resilience during this period. Increased isolation, financial strain, and restricted access to support services have led to a surge in elder abuse cases, disproportionately affecting women. Effective strategies, such as raising awareness and ensuring timely provision of resources, are crucial to safeguarding the mental well-being of the elderly. Indonesia's commitment to sustainable development and health equity, exemplified through initiatives like the Sustainable Development Goals, must address the specific challenges faced by elderly women in communities like Panggungan Village. The demographic shift towards an aging population, coupled with the pandemic's effects, further magnifies vulnerabilities. Empowering community-based resources can provide holistic support to elderly women, contributing to sustainable health development and social equity. Methods: The approach targets 15 Posyandu Lansia cadres and 20 elderly women in Panggungan Village through an offline community service program adhering to health protocols. Tools used in this study were the Mental Health Improvement Module, Geriatric Depression Scale questionnaire, and knowledge/skills assessment for Posyandu Lansia cadres. A combined quantitative and qualitative analysis was applied. Results: Results showed that 55% prevalence of verbal violence, 45% neglect, and 35% exploitation violence among elderly women. Post-training, these figures improved to 25% for verbal violence, 10% for exploitation violence, and 15% for neglect violence. Depression tendencies witnessed a 25% shift. Cadres’ knowledge increased from 58.2 to 82.1, and skills improved from 63.5 to 79.2. Conclusion: Elevated violence and depression tendencies that were quite high, negatively impact elderly women's mental health in Panggungan Village. Cadre’s training significantly enhances residents' well-being and mental health, offering a meaningful solution.

Abstract

Introduction: Stunting remains a significant public health concern in Indonesia, particularly in East Nusa Tenggara Province, where it is the most prevalent among all Indonesian provinces at about 42.6 per cent. Empowerment initiatives represent a crucial approach to addressing this issue. Women's empowerment holds profound significance as it governs and nurtures human resources within the household, especially children, who represent the nation's future generation. This community service aims to implement a women's empowerment program by establishing a group of Women Providing Health Information (Perempuan Pemberi Informasi Kesehatan) and enhancing women's access to national health services. Methods: We conducted an experimental quantitative study involving mothers with children aged 12 to 59 months. Inclusion criteria included mothers with children aged 12 to 59 months residing in Kesetnana Village. Data were analyzed using the Wilcoxon test. The mothers' classes aimed to enhance mothers' knowledge regarding the first 1000 days of life and the nutritional requirements during this period. The study included 35 participants, with the questionnaire on stunting knowledge serving as the primary instrument. Results: The findings revealed a notable increase in participant knowledge in the post-test compared to the pre-test, with a mean rank of 88.00 and a p-value of 0.000. Conclusions: The results underscore the continued need for a gender- and culture-based approach to health promotion to mitigate the incidence of stunting. Effective health education on stunting reduction requires collaborative efforts between health workers and local communities, with mutual support being essential.

Abstract

Introduction: High-risk pregnancies are a major cause of maternal morbidity and mortality, both on a global and national scale. Tuminting Public Health Center, which is a partner in the Community Partnership Empowerment activities, has a significant number of patients with high-risk pregnancies. This indicates an urgent need for the care of high-risk pregnant women. Pregnant women with high-risk pregnancies require priority and continuous care, including early screening/detection, monitoring, and support. However, a situation analysis at the partner site has identified several issues, including: 1) Posyandu cadres are not actively involved in supporting high-risk pregnant women, resulting in these women not receiving appropriate healthcare for their pregnancy conditions. 2) Health promotion media for high-risk pregnant women is limited, preventing these women from receiving accurate health information tailored to their condition.  The objectives of these activities are to enhance the role of cadres in supporting high-risk pregnant women and to improve the availability of health promotion media as a support tool for cadres in their role. Methods: To address these issues, the proposers offer the following solutions: 1) Training on early detection, monitoring, and support for posyandu cadres using a psychoeducational approach.2) Training on the use of digital media for health promotion for high-risk pregnant women for cadres. Results: This training was attended by 37 cadres at Tuminting Public Health Center. Participants' knowledge levels were evaluated before and after the activity.  The evaluation of these activities showed a p-value of 0.000, indicating that there was a difference in the knowledge of the training participants before and after the training. Conclusion: Training activities with a psychoeducational approach can improve the knowledge of cadres, enabling them to enhance their role in early detection and support for high-risk pregnant women.

Abstract

Introduction: The use of lipstick, blush-on, and various colored cosmetic products is highly popular among teenage girls. However, the adverse effects of hazardous colorants in cosmetics include respiratory irritations, skin irritations, eye injuries, gastrointestinal toxicity, and carcinogenic properties. Red Dragon Fruit contains a significant amount of red anthocyanin antioxidants, making it a viable natural colorant for food, cosmetics, and textiles. This community engagement project aimed to enhance the understanding of teenage girls, especially students of MAN 1 Model City of Bengkulu, regarding illegal cosmetics containing harmful and prohibited ingredients. It also aimed to improve the girls' skills in crafting natural Lip Tint and Blush-On preparations from Red Dragon Fruit (Hylocereus Polyrhizus), promoting a return to nature in cosmetic choices. This initiative was expected to inspire creative ideas among teenagers for designing alternative economic activities centered on producing natural and halal cosmetics made from herbal ingredients. Methods: The project involved educational and awareness-raising activities about the dangers of forbidden cosmetic ingredients, followed by workshops on making Lip Tint and Blush-On formulations. These workshops were led by faculty members from the School of Medicine and the School of Pharmacy, with assistance from students of the Faculty of Mathematics and Natural Sciences at the University of Bengkulu. Results: Evaluation results showed that workshop participants experienced a 63.2% increased in knowledge and gained the ability to independently create Lip Tint and Blush-On products from red dragon fruit. Conclusion: The workshop successfully produced natural Lip Tint and Blush On and the project also heightened the students' interest in the Pharmacy Program.

Abstract

Background: Tuberculosis (TB) is an infectious disease with high mortality and morbidity rates in Indonesia, caused by the bacterium Mycobacterium tuberculosis. The Case Detection Rate (CDR) in Banyuwangi is still relatively low at 54% compared to the set target. In Banyuwangi, one of the Non-Governmental Organizations (NGOs), Yayasan Bhanu Yasa Sejahtera Yabhysa, supports the TB control program in the region. Since 2021, this community has collaborated with the Banyuwangi Health Department to implement community-based TB interventions in 28 Community Health Centers (Puskesmas) spread across 16 districts, with a total of 66 active TB cadres. The aim of this community service is to train Tuberculosis (TB) cadres through socialization to improve the performance of TB suspects in Sempu Banyuwangi Community Health Center. Methods: The analysis of knowledge improvement data used a paired T-test with a 95% Confidence Interval. The test was conducted to evaluate the difference in knowledge improvement before and after education. Results: From the results of this community service, there was an increase in knowledge before and after socialization by 0.06 or 6%. The formation of TB cadres through contact investigation simulation carried out in community service activities resulted in the formation of four cadres. Conclusion: Based on the pre-test and post-test results, respondents showed a 6% increase in knowledge. The hope of this community service is to improve the achievement of TB suspect cases and reduce TB cases in Sempu Community Health Center.
