Abstract

This study aims to compare the effectiveness of three trauma scoring systems—Injury Severity Score (ISS), Revised Trauma Score (RTS), and Trauma and Injury Severity Score (TRISS)—in predicting mortality among trauma patients at Inche Abdoel Moeis Regional General Hospital Samarinda's emergency room. Trauma assessment tools are crucial for reducing mortality rates and enhancing patient outcomes. While ISS focuses on anatomical factors, RTS integrates physiological parameters, and TRISS combines both for a comprehensive evaluation. Previous research suggests their effectiveness in mortality prediction, albeit with variations in performance. The absence of a dedicated trauma assessment system at the hospital underscores the urgency of this research. By evaluating the performance of ISS, RTS, and TRISS, this study aims to inform clinical practice and enhance trauma patient management, ultimately reducing mortality rates and improving outcomes in the emergency room setting. The study's methodology involves retrospective analysis of trauma patient data, including demographics, injury characteristics, and outcomes. Statistical analysis will compare the performance of the three scoring systems in predicting mortality with implications for trauma care protocols and resource allocation. Results will contribute to evidence-based decision-making in emergency room settings, facilitating more accurate triage and treatment strategies for trauma patients.

Abstract

The World Health Organization (WHO) sets the average cesarean sections (CS) standard at 5-15% per 1000 births. However, cesarean section rates in private hospitals exceed 30%, compared to 11% in government hospitals. This study investigates the prevalence and complications of surgical wound infections post-cesarean section and the effectiveness of modern wound dressing techniques in reducing these infections. Data analysis reveals cesarean wound infections occur during hospitalization (27%), re-admission (1%), and post-discharge (71%). Surgical wound infections (SWI) significantly increase morbidity, treatment duration, and healthcare costs, with a direct death rate ranging from 3% to 75% globally. The study evaluated modern wound dressings adhering to "moist wound healing" principles, such as hydrocolloids, alginate, and foam, which maintain a moist environment to enhance epithelial cell proliferation, accelerate wound healing, and reduce scarring. Findings indicate that moist wound dressings significantly improve healing outcomes, promoting epithelialization and collagen synthesis, with techniques showing a 30-50% increase in epithelialization and a 2-5 times faster re-epithelialization rate. These results highlight the need for adopting advanced wound care techniques to reduce surgical wound infections and improve patient recovery post-cesarean section. The study implies that healthcare providers should integrate modern wound care practices to enhance maternal health outcomes globally.

Abstract

Triage at the hospital's Emergency Room (ER) is critical for prioritizing the care of emergency patients. Triage errors can be dangerous and endanger patient safety, so nursing students should have a solid understanding of triage. This study investigates the impact of ESI triage learning media containing PowerPoint and animated films on knowledge, triage decision-making (TDM), and participant satisfaction. The research used a quantitative approach with a Quasi-Experimental pretest-posttest method involving 36 5th semester Diploma Nursing students at the Makassar Ministry of Health Polytechnic. The study was divided into two groups, experimental (animated video learning) and control (PowerPoint learning), to evaluate the effect of treatment on knowledge, triage decision-making, and student satisfaction in ESI triage learning. The results of the data analysis show a significant increase in knowledge about ESI Triage and student satisfaction after the intervention, with significant values ​​of 0.001 and 0.018, respectively (p-value < 0.05). Although there was no significant increase in Triage decision-making abilities between the groups that used animated videos and Microsoft PowerPoint (p = 0.440), the use of animated videos overall increased student responses and satisfaction. MANOVA analysis confirmed that the implementation of animated videos significantly influenced knowledge and satisfaction in students with a p-value <0.05. The use of animated videos increases student knowledge and satisfaction significantly compared to PowerPoint, although it does not affect triage decision-making.

Abstract

The hospital provides health services such as inpatient, outpatient, and emergency departments. The quality of nursing care is crucial as patients rely on nurses to address their complaints. This study aims to determine the relationship between nurse burnout and patient safety culture with the quality of nursing care. An analytical observational design with a cross-sectional study approach was conducted in two Type B Hospitals in Central Sulawesi Province with 235 nurse respondents in the inpatient room, using purposive sampling. Data analysis included the Pearson product-moment test and multivariate modeling. Results indicated a significant relationship between nurse burnout and nursing care quality (p-value <0.05), as well as between patient safety culture and nursing care quality. The dominant variable related to the quality of nursing care was patient safety culture (β = 0.723). The study highlights that nurse burnout negatively impacts the quality of nursing care, whereas a strong patient safety culture improves it. Theoretically, this research expands the understanding of the interplay between nurse burnout, patient safety culture, and nursing care quality, emphasizing the need for organizational support and effective communication. Practically, it suggests that hospital administrators should implement strategies to reduce nurse burnout and foster a positive patient safety culture, such as stress management programs and a supportive work environment, to enhance nursing care quality and improve patient outcomes.

Abstract

Recent evidence indicates that elderly who have suffered rib fractures may develop delayed massive hemothorax. Delayed enormous hemothorax after thoracic trauma is uncommon, but it is nevertheless linked to high rates of mortality. This single case report provides an overview of the ordinary and important delayed hemothorax caused by thoracic trauma with multiple rib fractures in limited healthcare facilities. We observe symptoms and chest X-rays before and after the development of delayed hemothorax. A 56-year-old man came to the hospital with complaints of shortness of breath and pain in the chest and right shoulder. The patient said that four days ago, He fell from a height of around 6 meters. After visiting the hospital four days prior, the patient was diagnosed with multiple fractures of the inferior aspect of the right ribs (costa 3-7) as well as a fracture of the right clavicle. However, the patient declined surgery and was made to return home. At that time, the patient did not experience shortness of breath. Currently, the patient is experiencing dyspnea. Blood pressure 138/91 mmHg, heart rate 89 beats per minute, respiration rate 26 times per minute, Spo2 91% room air. The chest x-ray currently shows a right hemothorax. After stabilization, the patient asked to be referred to another hospital for further treatment. Patients with multiple fractured ribs, especially with rib displacement, are at high risk of evolving delayed hemothorax. Close monitoring and observation for several days are necessary.

Abstract

Leprosy ulcer is one of the worsening manifestations of leprosy that often causes disability if not appropriately handled. Healing of ulcers requires a long enough duration so that a good immunological response is needed to encourage the wound healing process, one of which is transforming growth factor-  (TGF-), which is necessary of the plants that plays a role in the expression of TGF-  to spur wound healing is Curcuma longa rhizome or turmeric rhizome. This study is expected to prove that administering Curcuma longa rhizome extract can increase serum TGF-β levels in leprosy ulcer patients. This study is an experimental study with a pre and post-randomised single-blind controlled trial design involving 22 subjects with leprosy ulcers. The control group obtained standard therapy for leprosy ulcers, and the treatment group received standard therapy and adjuvant Curcuma longa rhizome extract at a dose of 1 g per day for 30 days. TGF-β levels were measured by the ELISA method. Data analysis used the Wilcoxon test and the Mann-Whitney test. The results showed no significant difference in TGF-β levels after and before the intervention in the control group (p=0.79) and the treatment group (p=0.14). However, there was a significant difference in TGF-β levels after the intervention in the control group and treatment (p=0.01). Curcuma longa rhizome extract did not show an important role in changes in serum TGF-β levels in leprosy ulcer patients. Abstract

Type 1 leprosy reaction / Reversal reaction (RR) is an inflammatory episode in existing leprosy lesions and aggravates the pain rate. Long-term therapy with steroids causes a variety of side effects, so an adjuvant therapy alternative is needed to alleviate steroid use. Curcuma longa rhizome extract has become an anti-inflammatory therapy choice in various previous inflammatory diseases. This study aims to determine the anti-inflammatory effect of Curcuma longa rhizome extract as an adjuvant therapy for reversal reaction patients by analyzing serum levels of IL-6 as a marker of inflammation. The pre and post-test randomized single-blinded controlled trial was conducted on two groups, namely control (RR patients with steroid therapy & placebo) and treatment (steroid therapy & Curcuma longa rhizome extract 1 gram/day). IL-6 serum levels were analyzed from sampling before and after the intervention for one month. The serum levels of IL-6 post-test in the control group were significantly higher than those of the pre-test. The treatment group showed lower serum levels of IL-6 post-test compared to pre-test, although there was no significant difference. Curcuma longa rhizome extract 1 gram/day for one month as adjuvant therapy for patients with type 1 leprosy reaction did not significantly reduce serum IL-6 levels.

Abstract

The study investigates the relationship between obesity and cardiovascular disease (CVD) risk, comparing the use of body mass index (BMI) versus other biomarkers in predicting CVD risk among obese individuals. It conducts a systematic literature review following the PRISMA guidelines, searching databases for relevant articles published from 2017-2022. The review analyzes 12 eligible articles and finds that factors beyond just BMI, such as genetics, physical activity, metabolic disorders, previous heart disease history, nutrition, fat distribution, and changes in BMI, can significantly impact the prognosis of heart disease in obese individuals. Importantly, the study shows that measures of fat distribution, like waist-to-height ratio, waist circumference, log-transformed body shape index (LBSIZ), and the ratio of visceral adipose tissue (VAT) to subcutaneous adipose tissue (SAT), are superior to BMI in predicting CVD risk among those with obesity. The key takeaway is that while obesity is strongly linked to CVD risk, BMI alone often fails to predict that risk accurately. Fat distribution measures may be a more effective tool for identifying obesity status and predicting associated CVD risk compared to relying solely on BMI.

Abstract

A healthy lifestyle requires discipline. It's not easy, particularly adolescent who feel free and are looking for their existence. Its implementaiton needs to be applied to adolescent due to many cases of chronic diseases developing among them, following their lifestyle. Sedentary/lazy to move, smoking, alcohol, lack of rest and not being able to manage mental health. Generally, adolesecent at the educational level of Junior and Senior High School. The objective of this study is to analyze aspects of the impact and sustainability of chronic disease prevention programs for adolescents. This study is a systematic review using the PRISMA (Preferred Reporting Items for Systematic Reviews & Meta Analyses) method combined with open-source search engine. The database used for searching data and information is PubMed. The keywords used during the search process are "Prevention of Chronic Disease" AND "Chronic Disease" OR "Non-communicable Disease" OR "Infectious Disease" AND “Impact of Chronic Diseases” AND “Teenagers” OR “Adolescents” AND “School”. The most variable aspects of disease prevention are more information related to chronic diseases, knowledge, attitudes and stakeholder support. The CERDIK program provides knowledge, education and information among adolescents. These factors must be taken into consideration to improve lifestyle, maintain health so that you are not vulnerable to health problems during the teenage period

Abstract

Hypertension is one of the main risk factors for heart problems. Apart from causing heart failure, hypertension can result in kidney failure and cerebrovascular disease. Patient behavior with high sodium and fat intake, unhealthy lifestyle and low compliance with taking antihypertensive medication can result in the failure to achieve therapeutic goals resulting in a decrease in the quality of life of hypertensive patients. Therefore, a method is needed to improve these things, one of which is the counseling method. The counseling method used is medication-taking counseling. The results of this research use the journal review method by analyzing 6 national journals and inter national journals. The results of the study explain that non-adherence to antihypertensive treatment not only has a major impact on blood pressure control, but also on cardiovascular morbidity and mortality. Therefore, clinical pharmacy services provided by pharmacists in pharmacies in the form of counseling are very important to achieve success. Various studies show that counseling provided by pharmacists and PILS (Patient Information Leaflets) has an influence on the level of compliance in taking anti-hypertension medication.
