Abstract

Chronic respiratory diseases such as asthma and chronic obstructive pulmonary disease (COPD) are major global health problems with significant impacts on quality of life. Inhaler therapy is the main treatment, but patient adherence remains a challenge affecting clinical outcomes. This study aimed to evaluate the relationship between inhaler adherence and clinical outcomes in pulmonary patients at RSI Sunan Kudus. This research used a prospective observational analytic design involving 108 respondents selected by purposive sampling. Data were collected using the MARS-10 questionnaire, observation, and medical records, and analyzed using the Mann-Whitney test. The results showed that most patients had moderate adherence with relatively stable clinical conditions indicated by normal respiratory rate and SpO₂ ≥90%. There was a significant relationship between inhaler adherence and clinical outcomes (p=0.001). In conclusion, higher adherence to inhaler use is associated with better clinical outcomes.

Abstract

This study aims to analyze the effect of Natural Resource Revenue Sharing (DBH-SDA), economic inequality measured by the Gini Index, and the Human Development Index (HDI) on economic growth in resource-rich provinces in Indonesia. The phenomenon of the natural resource paradox indicates that abundant natural resources do not necessarily lead to improved welfare. This study employs a quantitative approach using panel data from selected resource-rich provinces during the 2017–2023 period. The analytical method used is panel data regression with a Fixed Effect Model (FEM). The results show that DBH-SDA has no significant effect on economic growth, economic inequality has a negative effect, while HDI has a positive and significant effect on economic growth. These findings suggest that human development plays a more crucial role than natural resource dependence in driving sustainable economic growth.

Abstract

West Nusa Tenggara Province is a region with an economic structure influenced by the extractive sector, including mining. On the one hand, the 2025–2029 NTB Regional Medium-Term Development Plan (RPJMD) emphasizes economic transformation and investment; on the other hand, the Spatial Planning (RTRW) serves as an instrument for spatial control and environmental protection. This study analyzes the dynamics of mining policies in both documents and their implications for artisanal mining in Sekotong, West Lombok Regency. A qualitative approach was used, using document analysis and a political ecology-based case study. The results indicate a structural tension between the orientation of extraction-based economic growth and commitment to environmental sustainability. The RTRW has not effectively integrated artisanal mining into the formal spatial planning framework, triggering agrarian conflict, land degradation, and health risks from mercury use. This article argues that reform of artisanal mining governance based on environmental carrying capacity and spatial equity is a prerequisite for the transition to a green economy in NTB.

Abstract

Pneumonia remains a major infectious disease contributing to significant morbidity and mortality, especially among adults and elderly patients. Antibiotic therapy as the main treatment for pneumonia must be rational to ensure therapeutic effectiveness, reduce antimicrobial resistance, and improve clinical outcomes. Objective: This study aimed to analyze the rationality of antibiotic use in hospitalized pneumonia patients at Universitas Gadjah Mada Academic Hospital during January–June 2025 based on the National Guidelines for Adult Pneumonia Management 2023. This study used a descriptive non-experimental design with a retrospective approach. A total of 92 patients were selected using purposive sampling. Rationality evaluation was conducted based on appropriate patient, indication, drug, dose, and route of administration. Most patients were aged >60 years (56.5%), male (53.3%), and had comorbidities (97.8%). Antibiotic use evaluation showed 100% appropriate patient selection, 100% appropriate indication, 95.9% appropriate drug selection, 93.8% appropriate dosing, and 100% appropriate route of administration. Overall, rational antibiotic use was found in 86 patients (93.5%), while 6 patients (6.5%) were categorized as irrational. Antibiotic use in pneumonia patients at RSA UGM was generally rational, although inappropriate drug selection and dosing remained issues requiring attention to optimize antibiotic therapy.

Abstract

This study analyzes the dynamics of fiscal independence and fiscal dependence in West Nusa Tenggara (NTB) Province during the 2019–2023 period within a fiscal decentralization framework that requires local governments to strengthen their own revenue capacity while simultaneously managing the dominance of intergovernmental transfers. The study focuses on a more comprehensive understanding of changes in the regional revenue structure through three commonly used financial performance indicators: the Regional Independence Ratio (RKD), the Degree of Fiscal Decentralization (DDF), and the Transfer Dependence Ratio. Unlike descriptive ratio studies that tend to focus on a single indicator or ignore the financing component, this study emphasizes the RKD, which includes loans as a comparative measure (transfers + loans) to avoid biased interpretations of independence due to the use of short-term financing. The method used is a descriptive quantitative approach based on data processing from the NTB Provincial Government's Budget Realization Report/LKPD for 2019–2023. The analysis is conducted by calculating annual trends in the three ratios and interpreting changes in the central-regional relationship pattern using the RKD intervals (instructive, consultative, participatory, and delegative). This approach allows for a simultaneous understanding of strengthening Locally-Owned Revenue (PAD), changes in the portion of transfers, and the role of loans in shaping regional fiscal space. The results indicate an improvement in NTB's internal fiscal capacity during the observation period. The Regional Original Revenue (RKD) increased from 49.58% (2019) to 92.76% (2023), indicating that financing is increasingly supported by PAD. The Regional Debt-to-Debt (DDF) also increased from 33.15% to 48.12%, indicating a larger contribution of PAD to total revenue (PAD + transfers), although it has not yet exceeded the 50% threshold. Correspondingly, the dependency ratio decreased from 66.85% (2019) to 51.88% (2023), indicating a weakening of transfer dominance and a shift toward a more balanced revenue structure. However, annual dynamics show that changes are not always linear; in 2021, the RKD decreased slightly despite an increase in PAD due to loans increasing the denominator (transfers + loans), while the 2023 spike was relatively analytically "clean" because loans were zero. Interpretatively, NTB shifted from a consultative model (2019) to a participatory one (2020–2022) and then to a delegative one (2023). This finding underscores the urgency of strengthening the regional original revenue (PAD) base through continuous intensification and extensification, while managing transfer dependence as a fiscal risk to changes in regional allocation (TKDD) policies. The novelty of this research lies in the integration of three ratios within the provincial locus, emphasizing the role of loans in reading the Regional Budget (RKD), and linking ratio trends to central-regional relationship patterns to enrich policy implications.

Abstract

Primary dysmenorrhea is a complaint of menstrual pain that often occurs in women of productive age, especially among female students, which can disrupt daily activities. One of the non-pharmacological methods that can be used to reduce dysmenorrhea symptoms is regular exercise. Less active exercise habits are one of the risk factors, while regular exercise has been proven effective in reducing pain through increased endorphins and blood flow. This study aims to analyze the relationship between exercise habits and the severity of primary dysmenorrhea among the 2021 cohort of female students at the Faculty of Medicine, Baiturrahmah University. Method: This study used an observational analytic design with a cross-sectional approach on 68 respondents selected through consecutive sampling. Data were collected using the Numeric Rating Scale (NRS) pain intensity questionnaire and the Godin-Shepard Leisure-Time Physical Activity Questionnaire. The research findings indicate that the majority of respondents have less active exercise habits (69.1%), with 45.6% of respondents experiencing severe dysmenorrhea pain. Bivariate analysis shows a significant relationship between exercise habits and the intensity of primary dysmenorrhea (p-value = 0.000). Female students who regularly engage in exercise have a lower level of pain compared to those who are less active. This study concludes that exercise habits play an important role in reducing primary dysmenorrhea symptoms. Therefore, it is important for female students to maintain a regular exercise routine to improve their quality of life. This research also recommends the development of structured exercise programs to reduce the risk of primary dysmenorrhea.

Abstract

Inequality in children's access to learning facilities remains a major issue in achieving quality education in Indonesia, particularly in remote and underdeveloped regions. This study aims to analyze the factors contributing to this inequality and to understand the actual conditions in the field. The research employs a literature review approach by examining various scientific sources, including academic journals, books, and reports from international organizations. The findings indicate that inequality is influenced by economic factors, limited educational infrastructure, uneven distribution of teachers, and disparities in access to technology. In addition, the suboptimal implementation of education policies further exacerbates the problem. These findings highlight the need for comprehensive policies, improved supervision, and multi-stakeholder collaboration to create a more inclusive and equitable education system.

Abstract

The LRT Feeder is a supporting mode of transportation that connects areas not yet served by major transport systems to service hubs such as stations. This study aims to analyze the relationship between Vehicle Operating Costs (BOK), Ability to Pay (ATP), and Willingness to Pay (WTP) in determining the fare for the Musi Palembang LRT Feeder on Corridor II. A descriptive quantitative approach was used, with data collected through questionnaires distributed to 100 respondents. Calculations followed the guidelines of the Ministry of Transportation Regulation KM 251 Year 2022. The results show that the BOK is Rp616.64, ATP is Rp158.30, and WTP is Rp130.47. The fact that ATP is higher than WTP but still far below BOK indicates that users have the financial capacity to pay but are not yet able to fully cover operational costs, and their willingness to pay remains low. Determining an ideal fare should consider improving service quality or implementing subsidy interventions to bridge the cost gap and ensure service sustainability. Further research can examine an appropriate fare scheme for users.
