Abstract

The aim of this research is to explore and delve deeper into the reasons families request passive euthanasia, the illnesses and conditions that lead individuals to consider such actions, and how the principle of patient autonomy is applied in euthanasia practices at KH. Daud Arif Regional General Hospital, Kuala Tungkal. The research employs a qualitative approach through literature review studies. Data were collected from journals, books, and previous studies, focusing on the withdrawal of life-support measures and the ethical principle of autonomy. The collected information was analyzed to support the research. Based on the findings, decisions regarding euthanasia, particularly passive euthanasia, should ideally be made by patients when they are mentally capable. However, in critical conditions where the patient is incapacitated, the responsibility is transferred to the patient's family, who can make decisions on their behalf, including opting for passive euthanasia. While active euthanasia remains illegal in Indonesia, passive euthanasia, such as the cessation of life-support interventions, is permitted under certain legal frameworks, such as Ministry of Health Regulation No. 37 of 2014. This regulation also emphasizes the importance of therapeutic contracts between patients and healthcare providers. The reasons families or patients choose passive euthanasia at KH. Daud Arif Regional General Hospital, Kuala Tungkal, include prolonged physical suffering and a significant decline in quality of life. The illnesses or conditions considered for passive euthanasia include chronic, incurable diseases that cause ongoing suffering, such as heart disease, kidney failure, or illnesses with poor prognoses.

Abstract

The number of people with diabetes increases from year to year, accompanied by an increase in complications of diabetic ulcers. Various kinds of interventions are carried out to accelerate the healing of diabetic foot wounds, one of which is the use of aloe vera extract. To determine the effect of aloe vera extract on wound healing in patients with post-hospitalization diabetes mellitus. This study used a Pre-Experiment research design with a group Pretest post-test design. The population was all patients with diabetes mellitus and diabetic ulcers, and the sample was taken using the accidental sampling technique, totalling 37 people. Data analysis using the Wilcoxon Sign Rank Test Most of the wound healing before being given aloe vera extract was quite good. Namely, 26 people, or 70.3%, most wound healing after being given aloe vera extract was good; 28 people or 75.7%, there was an effect of providing aloe vera extract to wound healing in Diabetes Mellitus patients with a value of p = 0.000. Nurses should apply wound care using aloe vera as a non-pharmacological independent nursing action.

Abstract

Health law reform in Indonesia is an important agenda in efforts to realize universal health that is fair and equitable for all levels of society. Along with social and economic developments, the Indonesian health system faces various challenges, such as unequal access to health services, unequal quality of services, and high financing burdens. Therefore, reforms in the health legal system are needed to increase the effectiveness and efficiency of health services, as well as ensuring the right of every citizen to receive adequate health services. This article discusses the main challenges in implementing health law reform in Indonesia, including unequal health infrastructure, limited human resources, and regulatory and bureaucratic obstacles. Apart from that, this article also explores existing opportunities, such as the use of digital technology, improving the National Health Insurance (JKN) system, and increasing community participation in decision making regarding health policy. Through analysis of various policies and experiences of other countries, it is hoped that recommendations can be obtained to improve Indonesia's health legal system in realizing universal health goals. With the right reforms, Indonesia can improve the quality of health services that are inclusive and accessible to all its citizens, without exception.

Abstract

Collage students are academic individual with high levels of intellectual, critical thinking skills, and thoughtful planning in their actions. The quality of students can be observed through the academic achievements they attainment. Various factors can influence the academic performance of medical students, such as self-motivation, family support, campus environment, active participation in organizations, and individual differences in the significance of each factor. One effort to boost the motivation to excel is through a reflection approach, or muhasabah. In the context of medical education, self-reflection helps students become more aware of their personal strengths and weaknesses, allowing them to assess the pros and cons of their learning methods, identify their shortcomings, and find ways to improve, thereby encouraging continuous development of better study skills. This study aims to examine the impact of self-reflection on the academic performance of medical students. The method used is a literature review with a narrative review design to identify and summarize previously published articles on the influence of self-reflection on academic achievement. From the 12 articles reviewed, it was concluded that self-reflection is positive impacts students ' academic performance. Through self-reflection, students can enhance self-discipline and reflective skills in evaluating their learning methods, identifying weaknesses, and seeking improvements, which foster continuous improvement in study skills, ultimately contributing positively to academic performance.

Abstract

Reflection is an important method in medical education that has long been used to improve the quality of communication and learning. Reflection come from the Latin word reflectere, meaning “ to bend ” or “ to turn,” and refers to the process of self evaluation through reflection and analysis of experiences. Students with self reflection skills can develop into reflective practitioners. This lifelong learning process must be applied in medical education so that medical graduates can continue to apply it continuously in the health field. In medical education, the abilities to reflect not only increases the individual's capacity to act more effectively in similar situations in the future, but also help in the formation of a more balanced, calm, and objective professional attitude. Reflection and feedback is key elements in medical education because they support effective learning, the development of professional competence, and the formation of good attitudes and communication skills.

Abstract

Diabetic Retinopathy is a microvascular disorder that occurs due to the long-term effects of diabetes mellitus. Diabetes mellitus (DM) is caused by metabolic disorders that occur in the pancreas organ which is characterized by increased blood sugar or often referred to as hyperglycemia which is caused by a decrease in the amount of insulin from the pancreas. This can cause damage to the retina that threatens vision and can cause blindness. Effective management requires a multidisciplinary approach that includes glucose control, medical intervention, and patient education. This study aims to determine the pathogenesis and management of Diabetic Retinopathy. This research is a literature study with a narrative review approach. The literature reviewed is literature obtained from databases that have been accredited or indexed by Sinta. The results of this literature study show that vascular dysfunction including loss of pericyte cells, formation of acellular capillaries (endothelial cell death), thickening of the basement membrane and increased capillary permeability to the closure of blood vessels leading to retinal ischemia-infarction is the pathogenesis of diabetic retinopathy. Management of diabetic retinopathy depends on the severity of the symptoms. Treatment does not only consist of observing blood sugar control, systemic hypertension and hypercholesterolemia, but also therapy such as laser photocoagulation, administration of Anti-Vascular Endothelial Growth Factor (Anti-VEGF), intravitreous steroids or IVTA (Intra Vitreous Triamcinolone Acetonide), and other procedures. pars plana virectomy.

Abstract

Endometriosis in the abdominal wall following a caesarean section is an uncommon occurrence, accounting for only 0.03%- 1.5%. Diagnosis is often late, so symptoms become more severe or the mass size increases. Delayed diagnosis increases morbidity, complications, and the complexity of treatment to reduce symptoms and remove the endometriosis mass. We report a 33-year-old female patient with a mass in the anterior abdominal wall that appeared over the past two months. The patient experienced mass enlargement and cyclic pain which began three days before menstruation. The patient had a history of one cesarean section and did not use contraception. Ultrasonography examination showed a mixechoic image of the anterior abdominal wall of the cesarean section scar measuring 2.48 x 2.45 x 1.38 cm. We performed a wide excision up to 1 cm outside the mass. Pathological anatomy showed endometrial glands and stroma between skeletal muscle tissue that were consistent with external endometriosis with regenerative skeletal muscle cells. Understanding the histology and immunohistochemistry of muscle cell regeneration, as well as basic clinical examination, is very helpful in establishing a diagnosis so that appropriate management can be carried out.

Abstract

Medication management at the Gunung Tinggi Community Health Center is an important aspect in providing health services. The aim of this study was to evaluate the drug management process from planning to disposal. The research method used was descriptive and qualitative with in-depth interviews with community health center managers, pharmacy staff and pharmacist leaders. The results of the study show that although there are several obstacles in the procurement and distribution of medicines, the management of medicines at the Gunung Tinggi Community Health Center has been carried out in accordance with applicable standard operating procedures (SOP). Management systems need to be improved to ensure optimal access to medicines.

Abstract

Chronic tonsillitis is a condition of enlargement tonsil and accompanied by repeated infections. Chronic tonsillitis is most commonly caused by Gram-positive bacteria such as Staphylococcus aureus. Provision of broad-spectrum antibiotics such as Ciprofloxacin and Amoxicillin is able to inhibit the growth of aerobic  bacterias that cause chronic tonsillitis.The main objective of this study is to determaine the comparison of Ciprofloxacin and Amoxicillin sensitivity against bacterias causes of chronic tonsillitis Departement of West Nusa Tenggara General Hospital. The design of this study is observation study with cross sectional approach. Forty-nine people with chronic tonsillitis had a throat swab. The sensitivity test was examined by dilution methods for Ciprofloxacin and Amoxicillin antibiotics. The result of the study was analyzed by Chi Square (X2) test with a significance level of p value <0.05. The 49 chronic tonsilitis patients who had throat swab, the most common cause of chronic tonsillitis was Staphylococcus aureus 36 isolates (73,5%). Sensitivity to ciprofloxacin was 41 isolates (83,7%) with an inhibitory zone 28 mm and sensitivity to amoxicillin was 8 isolates (16,3%) %) with an inhibitory zone 20 mm, Ciprofloxacin is  more sensitive than Amoxicillin (p<0,05). Ciprofloxacin was more sensitive to bacterias that cause chronic tonsillitis.

Abstract

Craniomaxillofacial (CMF) surgery is a specialized area of medicine that addresses a range of complex conditions affecting the skull, face, and jaw. These conditions can arise from congenital deformities, trauma, tumors, or diseases, and their treatment often involves highly intricate and technically demanding procedures. Traditional approaches to pre-surgical planning and treatment typically rely on 2D imaging and manual models, which can lead to increased time consumption, higher costs, and a greater likelihood of human error. Recently, the adoption of 3D printing technologies into CMF surgery has shown significant potential to improve both pre-operative planning and surgical outcomes. This article explores the transformative impact of 3D printing in CMF surgery, focusing on its application in creating patient-specific models that help optimize surgical precision. By utilizing detailed 3D scans (such as CT or MRI), These technologies allow the creation of highly precise, individualized physical models that replicate the anatomical structures of the patient. This level of precision helps minimize risks associated with surgery, such as the misalignment of bone structures or the inadvertent damage to critical tissues. Emulating the surgical process on a 3D-printed model allows surgeons to enhance their techniques and predict potential challenges, ultimately improving surgical outcomes. Nonetheless, the integration of 3D printing in CMF surgery presents certain challenges. High production costs, the need for specialized equipment, and a lack of standardized protocols pose barriers to widespread adoption. Moreover, regulatory and legal issues need to be addressed to ensure patient safety and adherence to medical standards. In conclusion, 3D printing represents a groundbreaking advancement in CMF surgery, offering substantial improvements in surgical accuracy, patient care, and recovery outcomes. As the territory of 3D printing develops, it stands to greatly improve craniomaxillofacial surgery through safer, more efficient, and personalized solutions.
