Abstract

Background: Consistent self-care is key for effective hypertension management, but is difficult for older adults in rural settings. Understanding how individual and contextual factors interact helps develop targeted nursing interventions. This study examined the determinants of self-care adherence among older adults with hypertension in rural Central Java by analyzing the direct and indirect links between hypertension knowledge, self-care profiles, and health care empowerment. Methods: A quantitative study with path analysis included 60 older adults (aged >60 years) with hypertension in Getasan District, Semarang Regency, Central Java. Data were collected from August to December 2025 via validated questionnaires and analyzed using SmartPLS and IBM SPSS version 24. Result: Health service empowerment had the strongest positive effect on self- care adherence (β = 0.402, p < 0.001). Hypertension knowledge did not directly influence adherence, but it did so through health care empowerment (β = 0.171, p = 0.004). The model explained 23% of the variance in self-care adherence (R2 = 0.230). Conclusion: Health care empowerment, rooted in patient motivation and perception, is the main factor in adherence and mediates health knowledge. Nursing interventions should shift from education to empowerment-based strategies to enhance independence in hypertension management among rural populations.

Abstract

Background: Premature infants often experience difficulties in oral feeding due to neurological immaturity and poor coordination of sucking, swallowing, and breathing. These challenges may delay the transition to full oral feeding and prolong hospitalization in neonatal intensive care units (NICUs). The Feeding Cues Follow-Up Form (FCF-UF) is designed to objectively assess feeding cues in premature infants; however, it has not been adapted for use in the Indonesian context. Methods: This cross-sectional study aimed to adapt and evaluate the psychometric properties of the Indonesian version of the FCF-UF. Data were collected from 266 NICU nurses working in several hospitals in Indonesia between June and August 2025. The cross-cultural adaptation process followed seven stages. Internal consistency was assessed using Cronbach’s alpha, and test-retest reliability was evaluated using the Intraclass Correlation Coefficient (ICC). Construct validity was examined through Confirmatory Factor Analysis (CFA), and known-group comparisons were conducted to explore differences in instrument scores across respondent characteristics. Result: The Indonesian version of the FCF-UF demonstrated excellent internal consistency (Cronbach’s alpha = 0.922) and high test-retest reliability (ICC = 0.99). CFA supported a one-factor structure with very good model fit (χ²/df = 0.87; RMSEA = 0.000; CFI = 1.000; TLI = 1.001), with strong factor loadings across all items. Known-group comparison showed that pediatric nurse specialists scored significantly higher than nurses with a bachelor’s degree (p= 0.001). Conclusion: The findings indicate that the Indonesian version of the FCF-UF is a reliable and valid instrument for assessing feeding cues in premature infants. This tool may assist NICU nurses in implementing cue-based care and supporting the transition to oral feeding.

Abstract

Background: Congenital heart disease (CHD) is a major cause of pediatric morbidity and mortality. Malnutrition often coexists with CHD and may be associated with poorer heart failure (HF) outcomes. The study aimed to assess the prevalence of malnutrition (underweight, wasting, and stunting) and its association with HF severity in children with CHD. Methods: This was a single-center cross-sectional study at a tertiary hospital in Indonesia. The study included 41 pediatric CHD patients, recruited using a total sampling technique. Anthropometric data were assessed based on World Health Organization standards. HF severity was evaluated using the Modified Ross Score. The Mann-Whitney, Kruskal- Wallis, Spearman correlation, and multiple linear regression were applied. Result: Among participants, 26.8% were underweight, 22.0% were wasted, and 36.6% were stunted. The mean HF severity score was 3.07 (SD = 1.40). Most were categorized as having mild HF (63.4%). Residence (Z = 1.967; p = 0.049), underweight (Z = -2.394; p = 0.017), stunting (Z = -2.497; p = 0.013), and wasting (Z = -2.236; p = 0.025) were significantly associated with higher HF severity scores. WAZ score significantly correlated with HF severity (r = -0.348; p = 0.026). Stunting (β = -0.334; 95% CI: -1.787 to -0.138; p = 0.023) and wasting (β = -0.316; 95% CI: -2.018 to -0.098; p = 0.032) were significant determinants of HF severity. Conclusion: Stunting and wasting were independently associated with greater HF severity. These findings suggest the potential clinical importance of early nutritional screening and multidisciplinary interventions to mitigate HF progression. Given the single-center design and small sample size, further multicenter studies are needed to confirm these findings and strengthen causal inference.

Abstract

Background: Natural products are increasingly favored in wound care due to concerns about antibiotic resistance and side effects of synthetic drugs. Carica papaya L., a tropical plant native to Central America and widely grown in Indonesia, contains bioactive compounds with properties essential for tissue repair. This study tests whether a topical Carica papaya formulation speeds up wound healing by measuring granulation and epithelialization. Methods: Using a quasi-experimental design. A total of 50 participants were divided into a control (standard wound care) and a treatment group (standard care + C. papaya extract). Analysis used the Paired T test. Result: Paired t-tests indicated no significant changes in the control group (p = 1.00). In contrast, the treatment group demonstrated statistically significant improvements in granulation (pre: 0.44 ± 0.51; post: 1.56 ± 0.51; t = -9.35, p < 0.001) and epithelialization (pre: 0.68 ± 0.47; post: 1.72 ± 0.46; t = -10.25, p < 0.001). Independent-samples t-tests confirmed significant differences between groups after the intervention (granulation: t = -9.35, p < 0.001; epithelialization: t = -10.25, p < 0.001). These results support the incorporation of C. papaya extract into evidence-based complementary wound management protocols. Conclusion: Carica papaya extract significantly enhances wound healing, promoting granulation and epithelialization. Thus, it may serve as an effective adjunctive therapy in clinical wound management.

Abstract

Background: Children with thalassemia require lifelong, repeated blood transfusions, which frequently induce anxiety due to invasive procedures and unfamiliar clinical settings. If left unaddressed, anxiety can compromise treatment adherence and adversely impact psychological well-being. The present study investigates the effectiveness of play therapy in reducing anxiety among children with thalassemia undergoing blood transfusion. Methods: A quasi-experimental one-group pretest–posttest design was used at Pringsewu Regional Hospital. A total of 26 children were recruited using a total sampling approach. The intervention consisted of coloring-based play therapy administered for 20–30 minutes prior to transfusion. Anxiety levels were measured using a standardized child anxiety questionnaire before and after the intervention. Data were analyzed using the Wilcoxon Signed Rank Test. Result: Anxiety levels dropped significantly after the intervention. The median (IQR) anxiety score went from 40.42 before play therapy to 15.65 after. The difference between pretest and posttest scores was statistically significant (p < 0.001). Play therapy reduced anxiety in the participants. Conclusion: Play therapy is an effective non-pharmacological intervention for reducing anxiety in children with thalassemia undergoing blood transfusion. Its implementation may support pediatric nursing care by improving emotional comfort and facilitating smoother treatment procedures.

Abstract

Background: Patients with type 2 diabetes mellitus (T2DM) who have low self-acceptance often develop negative perceptions of their abilities, which can hinder effective diabetes self-management. T2DM is a chronic metabolic disorder characterized by insulin resistance and progressive β-cell dysfunction and long-term complications that negatively affect patients' quality of life (QoL). This study aims to determine the relationship between family support and self-acceptance in patients with T2DM. Methods: A cross-sectional study design was used, involving 221 respondents from Community Health Centers under the jurisdiction of the Ketapang District Health Office. Data were collected using the Hensarling Diabetes Family Support Scale (HDFSS) and the Illness Acceptance Scale (AIS). Result: The results showed a significant value of p = 0.000 and a Spearman correlation coefficient of r = 0.546, indicating a moderate positive correlation. Conclusion: There is a significant relationship between family support in diabetes management and self-acceptance in patients with T2DM

Abstract

Depression and loneliness are prevalent mental health concerns among adolescents and young adults and may increase the risk of suicidal ideation. Adolescence and early adulthood represent transitional developmental stages characterized by various psychosocial demands, which may heighten vulnerability to emotional distress. This study aimed to examine the relationship between depression, loneliness, and suicidal ideation among adolescents and young adults. A quantitative approach with a cross-sectional design was employed. The study population consisted of 462 individuals, from which 215 respondents were selected using simple random sampling. Data were collected using the Depression Anxiety Stress Scales (DASS-42) to measure depression levels, the UCLA Loneliness Scale Version 3 to assess loneliness, and the MINI Plus 5.0.0 to identify suicidal ideation. Data were analysed using Spearman’s rank-order correlation test. The results revealed a statistically significant positive correlation between depression and suicidal ideation (r = 0.376; p = 0.01), indicating a weak to moderate association. In contrast, loneliness was not significantly correlated with suicidal ideation (r = 0.10; p = 0.10). These findings suggest that depression plays a more substantial role in the emergence of suicidal ideation than loneliness. Therefore, suicide prevention strategies targeting adolescents and young adults should prioritize the early identification and management of depression while also strengthening psychosocial protective factors.

Abstract

Hypertension remains a major global health problem with increasing prevalence and poor self-care practices among patients. In Indonesia, particularly in Banjarmasin, decreased self-care (DSC) behaviors such as poor medication adherence, irregular physical activity, and limited blood pressure monitoring contribute to uncontrolled hypertension. Innovative nursing interventions are urgently needed to address these challenges. This study aimed to analyze the effectiveness of the Ners Counseling–Short Message Service Intervention (NC-SMSI) on improving self-care among hypertensive patients in Banjarmasin, Indonesia. The methods is a quasi-experimental study with a one-group pretest–posttest design was conducted among 30 hypertensive patients recruited using accidental sampling. The intervention combined direct nursing counseling and structured short message service reminders based on the Self-Care Deficit Nursing Theory. Self-care was measured using the DSCAI-90 instrument. Data were analyzed using the Wilcoxon Signed-Rank Test after the Shapiro–Wilk test indicated non-normal distribution. Ethical approval was obtained from the local health research ethics committee. The results is before the intervention, most respondents (53.3%) demonstrated very poor self-care, while only 3.3% achieved high levels. After the NC-SMSI intervention, 63.3% of participants reached high self-care levels, and none remained in the very poor category. The Wilcoxon signed-rank test showed a significant improvement in self-care scores (Z = –4.783, p < 0.001, r = 0.84), indicating a large effect. The conclusion is NC-SMSI effectively improved self-care behaviors among hypertensive patients. Integrating nurse counseling with mobile-based reminders offers an efficient, adaptive, and scalable model for enhancing hypertension management at the community level. This intervention holds promise for broader implementation in primary healthcare settings.

Abstract

Background: Household food waste presents a significant barrier to sustainable food security, with notable environmental, economic, and nutritional implications. This study evaluated the effectiveness of visual meal-planning education in enhancing knowledge of menu planning, influencing food purchasing behavior, and reducing household food waste in Tanah Sareal District, Bogor City. Methods: A pre-experimental, one-group pretest–posttest design was employed with 50 housewives responsible for household food management, selected through total sampling. The intervention included an initial educational session and refresher sessions every four weeks over a 12-week period, utilizing visual meal-planning materials. Data collection involved structured questionnaires to assess knowledge and behavior, as well as direct measurement of food waste using a weighing method over eight consecutive days. Data were analyzed using descriptive statistics and the Wilcoxon Signed Rank Test. Results: Findings indicated a significant increase in knowledge scores at the third evaluation (p = 0.001), reflecting improved understanding of menu planning and food purchasing. While planning and purchasing behaviors improved, the changes were not statistically significant. However, householdfood waste decreased significantly by 41 g/capita/day (60%) following the intervention (p = 0.000). Conclusion: In summary, visual meal-planning education effectively enhances knowledge and substantially reduces household food waste, although its influence on behavior change is limited.

Abstract

Background: Insufficient knowledge of reproductive health among female adolescents contributes to inadequate prevention and early detection of cervical cancer. Cervical cancer frequently develops in early adulthood, despite being preventable through the timely administration of the Human Papillomavirus (HPV) vaccine. This study evaluates the effectiveness of health education media in improving female adolescents' knowledge and attitudes toward the HPV vaccine. Methods: A quasi-experimental quantitative design with a pretest- posttest control group was employed. The sample consisted of 66 participants selected via probability sampling with stratified sampling. Result: Statistical analysis indicated that both the engklek and crossword puzzle intervention groups demonstrated significant improvements in knowledge and attitudes following the intervention (p < 0.05), while the control group showed no significant changes (p < 0.05). One-way ANOVA revealed significant differences in knowledge and attitudes among the engklek, crossword, and control groups (p = 0.000). Post hoc and N-gain analyses indicated that crossword puzzles were the most effective in enhancing adolescents’ knowledge. Conclusion: Crossword puzzle media can serve as an effective tool for disseminating information about the HPV vaccine. Enhanced dissemination is anticipated to increase HPV vaccine coverage and reduce cervical cancer risk.
