Abstract

An epiphytic fern known as Davalia denticulata, which is a member of the Davalliaceae family, grows on oil palm. It has been reported that several different species within this genus have been used as anticancer agents, antimicrobial agents, and antioxidants. However, there has been no report of any biological activity associated with this species. As a result, the purpose of this study was to extract secondary metabolites and assess the cytotoxicity of the extracts. The maceration method was used to extract this plant with a variety of solvents (methanol, ethyl acetate, and n-hexane), and the MTS assay was utilized to determine the cytotoxicity of this plant in relation to lung cancer (A549 cell line). According to the findings, the extracts exhibited a wide variety of activity toward the A549 cell line. The IC50 for the activity of the ethyl acetate extract was found to be 317.59 ppm, whereas the IC50s for the water and n-hexane extracts were 575.41 and 806.06 ppm, respectively. These observations allow for a better understanding of the cytotoxicity of the species, which can then serve as a foundation for further research into the isolation and bioactivity of secondary metabolites.

Abstract

Diabetes is a complex chronic disease that requires ongoing medical care with multifactorial risk reduction strategies beyond glycemic control. This study aims to describe the direct medical costs from the perspective of Anwar Medika General Hospital in type 2 Diabetes Mellitus (DM) patients who use the National Health Insurance (JKN) facility and to see the difference between real costs (total direct medical costs) and INA CBG's costs. The study was retrospective for the past 1 year, namely January-December 2020. The subjects of this study were 54 patients. Mann- Whitney analysis is used to see the suitability of real costs with INA-CBG's tariffs. The results of this study show that the total real cost for JKN type 2 DM patients at Anwar Medika Hospital for the January-December 2020 period is Rp. 269,807,380 and there is a difference between real costs and INA-CBG's costs in class 3 of - 4,190,371 where INA-CBG's costs are -4,190,371. CBG's are less than the real hospital costs. The test results using the Mann-Whitney method show an insignificant difference between real costs and INA-CBG's rates. The result p=0.458 where p>0.05 is said to be insignificant. This could be due to a cross-subsidy between INA CBG’s rate claims of fully unused patients and patients who spend more on treatment costs exceeding INA CBG’s rates.

Abstract

Abstract: The trend of using peptides, short chains of amino acids, as therapeutic agents has been increasing in recent decades. Therapeutic peptides usually use the parenteral route as systemic delivery system, requiring an aqueous formulation. However, in aqueous formulation, peptides are often unstable. The most stable form of therapeutic peptide is a lyophilized powder. Unfortunately, for tropical and developing countries such as Indonesia, as well as from pharmaco-economic point of view, if stable, peptide formulations in aqueous solution are preferred. This systematic literature review provides the various degradation pathways potentially experienced by therapeutic peptides available in Indonesia in aqueous solution. Then present various known strategies to prevent peptide degradation based on recent research. Article were searched on the PubMed, ScienceDirect, Wiley Online Library, and Google Scholar databases. Search results with keywords: therapeutic peptide OR peptide drug AND stability OR stabilization AND degradation OR degradation pathway found 20 articles that met the inclusion and exclusion criteria. Results showed that aggregation is the most common degradation pathway, followed by deamidation, hydrolysis, β-elimination, and disulphide exchange. The main strategies to improve peptide stability in solution are buffer selection and/or pH adjustment, addition of antioxidants/preservatives, combination of buffer with divalent metal ions, avoiding light exposure, and storage in cold temperature.

Abstract

Abstract: Spare dadap plants contain flavonoids that can potentially lower glucose levels in the blood and reduce glucose absorption in the intestine. The purpose of the study was to determine the inhibitory activity of the α-glucosidase enzyme and the effect of giving Spare Dadap Leaf Ethanol Extract (EEDDS) on blood glucose levels of male white Wistar rats and its effective dose. The research method used the in vitro method with inhibition of α-glucosidase enzyme and the in vivo method Glucose Tolerance Test (TTGO) with amylum manihot induction using 25 rats divided into 5 groups with EEDDS doses of 100, 200, and 400 mg / kg BB, then measured blood glucose levels for 120 minutes. The results of the EEDDS α-glucosidase enzyme inhibition study showed the best % inhibition value of 41.81% at 100 ppm and the lowest % inhibition value at 10 ppm, which was 13.33% with an IC50 value of 146.30 ppm, which was included in the weak category in inhibiting α-glucosidase. The results of TTGO research induced amylum manihot, EEDDS 400 mg / KgBW have a significant effect on reducing blood glucose levels in the T60 to T120 range. The conclusion in this study is that spare dadap leaf ethanol extract has potential as an antihyperglycemic both in vitro and in vivo.

Abstract

Abstract: Tocilizumab is an anti-IL-6 which is recommended as cytokine release syndrome therapy in the treatment of Covid-19. One of the side effects of tocilizumab that can occur is hepatotoxicity, from mild elevated transaminases to severe drug-induced liver injury (DILI). Currently, research related to tocilizumab hepatotoxicity and the risk factors affecting Covid-19 patients is limited and still needs to conduct. This research is an observational study with cross-sectional design. Data collection was carried out retrospectively using secondary data obtained from medical records department, patients with Covid-19 at Universitas Indonesia Hospital in 2020-2021. Patients confirmed with positive Covid-19 receiving Tocilizumab therapy in medical record were included in this study. Patient demographics, clinical diagnoses, laboratory examinations, history of others medication were also reviewed. Patients with less than 18 years old, incomplete medical record data, and referred to other hospitals were excluded. The sampling technique used was total sampling, namely all patients who met the inclusion criteria were taken as subject in this research. Chi-square test was used to analyze the relationship between each risk factors and the incidence of elevated transaminases. The results of Chi-square test showed that several risk factors significantly increased the incidence of elevated transaminases in patients given tocilizumab therapy, including age, obesity, diabetes mellitus, and ceftriaxone as antibiotic therapy.

Abstract

Abstract: The results of the Walk Through Audit (WTA) BPJS stated that the waiting time for talking out the drugs at the Pekanbaru Medical Center Hospital was one of the factors for the lateness of services to patients. After conducting a preliminary study, the data was found that the waiting time for taking out the drugs was over than minimum value of Standard Service. The purpose of this study was to analyze the factors that affected the waiting time for outpatient prescription drug services at Pekanbaru Medical Center Hospital. This study used a combination method (Mixed-Methods Sequential Explanatory Design), namely by combining quantitative and qualitative descriptive methods. From 70 samples obtained quantitative research data for type of patient, type of prescription, number of items in prescription, number of prescriptions in shifts and number of prescriptions based on drug availability which were  not factors that affected the waiting time for outpatient drug prescription services, this was based on the test All Chi-Square values ​​> 0.05, this meant that Ho was rejected or in other words it was not proved related to the waiting time of service. On the other hand the qualitative research with in-depth interviews and observations resulted in insufficient number of human resources, lack of human resource competence, unclear service process flow, inadequate facilities and infrastructure, especially the Sim-PEC network system and pharmacy installation room layout which haven’t fulfilled the standard caused the lateness of services or the length duration of waiting time for outpatient drug service became longer.

Abstract

Primary Health Center still has problem in drug management, especially in drug availability, storage, and drug inventory control. This study aims to analyze whether drug management at the Margamulya Primary Health Center in 2021 met the standard indicators of drug management. This study used observational descriptive research methods. Data collection was carried out using secondary data tracing (retrospective), direct observation (concurrent), and interviews. The collected data were then analyzed with drug management indicators consisting of 26 indicators. The analysis results showed that drug management at the Margamulya Primary Health Center met 9 indicators, but did not meet 15 indicators, when compared to standards. In addition, 2 indicators could not be analyzed because of lack of data. Thus, it is necessary to review aspects of drug inventory control, to increase the number, soft skills and knowledge of pharmacist in pharmaceutical services, and to implement management information system in the Primary Health Center.

Abstract

Gastritis is a condition where the production of acid in the stomach can cause irritation. To treat gastritis, patients can do self-medication (self-medication). The factors that influence self-medication are sources of information such as advertisements on television. Self-medication behavior requires knowledge so that treatment is safe and rational. The purpose of this study was to determine the relationship between knowledge of gastritis medicine advertised on television and self-medication behavior in pharmacies in Bangkinang District, Riau Province. The type of research is observational, descriptive analytic method with cross sectional design and data collection simultaneously. Sampling was done by purposive sampling in order to obtain a total sample of 110 respondents who met the inclusion criteria. The results of the statistical test obtained a p value < 0.05, which means that there is a relationship between knowledge of gastritis medicine advertised on television and self-medication behavior in pharmacies throughout Bangkinang District, Riau Province.

Abstract

Tuberculosis (TB) is still a problem that must be addressed in the community, treatment programs and management guidelines have also been undertaken by the government to handle this case according to national standards. This is because there is still a high rate of non-adherence to treatment in TB patients. One of the causes of the high incidence of tuberculosis is due to knowledge, in this case the level of one's knowledge has a relationship with adherence to taking anti-tuberculosis medication. The number of people with pulmonary tuberculosis continues to increase from 2020 to 2022. This is because the high rate of non-adherence to treatment in tuberculosis (TB) patients is still high. Tuberculosis is caused by Mycobacterium tuberculosis which infects latently or progressively attacks the lungs. Purpose: This study was conducted to analyze the relationship between the level of knowledge of tuberculosis patients about tuberculosis and adherence to treatment of outpatient pulmonary tuberculosis patients at the Regional General Hospital of West Nusa Tenggara Province in 2022. Methods: This study was conducted using a Cross Sectional Study design. Population: in this study, the population used was patients with outpatient pulmonary tuberculosis at the Regional General Hospital of West Nusa Tenggara Province with a total sample of 57 respondents. Measuring tools used in the form of questionnaires and form TB 01. Data analysis using univariate and bivariate analysis with the Chi-Square test. Result: this study shows that there is a relationship between knowledge and medication adherence (p=0.025). It can be concluded that there is a significant relationship between knowledge and adherence to treatment in outpatient pulmonary tuberculosis patients at the Regional General Hospital of West Nusa Tenggara Province in 2022.

Abstract

The sweet orange (Citrus sinensis (L.) Osbeck) is a popular fruit. The part of the sweet orange that is consumed is only the fruit flesh, while the orange peel is only thrown away and not utilized. The sweet orange peel contains several secondary metabolites, namely alkaloids, flavonoids, tannins, phenols, saponins, and steroids, which have antimicrobial properties. This study aims to determine the effect of the concentration of the ethyl acetate fraction of sweet orange peel on antibacterial and antifungal activity in Staphylococcus aureus, Staphylococcus epidermidis, Escherichia coli, Salmonella typhi, and Candida albicans fungi using the disc diffusion method. The ethyl acetate fraction was prepared in several concentration series, namely 50%, 25%, 12.5%, 6.25%, 3.125%, and 1.5625% w/v. The results obtained showed that all concentrations of the ethyl acetate fraction series showed activity in inhibiting the growth of Salmonella typhi bacteria. Series concentrations of 3.125% to 50% have activity in inhibiting the growth of Staphylococcus aureus, Staphylococcus epidermidis, Escherichia coli, and Candida albicans fungi. The results of the data analysis using statistical one-way ANOVA showed that the antibacterial and antifungal activities between the concentration series were significantly different (p<0.05) from the negative control.
