Abstract

Background: Periventricular leukomalacia (PVL) is a white matter lesion of the brain that can cause a variety of neurological disorders, including mental retardation and communication disorders. This condition often presents challenges in dental care due to the patient's limited ability to communicate and cooperate during dental procedures. Purpose: To address the management of multiple pulp necrosis in a child with periventricular leukomalacia. Method: A patient presented with a history of PVL, accompanied by mental retardation and communication disorders. Clinical and diagnostic examinations revealed severe pain with a high pain score, requiring comprehensive management. Given the patient's limited cooperation and the complexity of the procedure, treatment was performed under general anesthesia. The procedure included eliminating the source of infection, preserving the remaining teeth, and extracting teeth with poor prognosis. The entire procedure was completed in a single session without significant perioperative complications. Results: Post-procedure evaluation demonstrated a reduction in pain symptoms and improved eating ability, which positively impacted the patient's nutritional status. Patients with PVL who have intellectual disabilities and communication disorders require a specialized, individualized, and comprehensive approach to dental care. Conclusion: The use of general anesthesia is an effective and safe option for treating various dental infections in patients with limited cooperation, while improving quality of life through pain reduction, improved oral function, and improved nutritional status.

Abstract

Background: Waterborne diarrhea remains a major global public health problem closely associated with water quality, sanitation conditions, and hygiene practices. Various interventions have been implemented to reduce the incidence of diarrheal diseases; however, their effectiveness needs to be comprehensively evaluated to provide evidence for environmental health policy and program development. Purpose: To evaluating the effectiveness of sanitation and hygiene interventions on waterborne diarrheal diseases. Method: This study employed a literature review design using a descriptive-analytical approach. Relevant articles were retrieved from Google Scholar, PubMed, ScienceDirect, and nationally accredited SINTA-indexed journals. Articles were selected based on predefined inclusion and exclusion criteria. A total of 10 articles published between 2020 and 2025 were included and analyzed to identify the types of interventions, study findings, and the effectiveness of sanitation and hygiene programs. Results: The findings indicated that all reviewed studies reported a significant association between sanitation and hygiene interventions and a reduction in diarrheal incidence (p< 0.05). Effective interventions included the provision of safe drinking water, the use of improved sanitation facilities, household waste management, handwashing with soap, and integrated Water, Sanitation, and Hygiene (WASH) programs. Integrated WASH interventions demonstrated greater effectiveness than single-component interventions. Conclusion: Sanitation interventions and hygiene practices are effective in reducing the incidence of waterborne diarrheal diseases, particularly when implemented through an integrated WASH approach. Suggestion: Strengthening WASH programs and promoting sustainable behavior change interventions are recommended to support long-term diarrhea prevention efforts.

Abstract

Background: Maternal health services are one of the Minimum Service Standards (MSS) indicators in the health sector that must achieve a 100% target. However, the achievement of maternal health services in Muaro Jambi Regency in 2025 reached only 61.96%, while the achievement was 90.93% (496 of 546 targeted pregnant women). In addition, antenatal care coverage remained below the expected target, with K1 coverage at 67.40%, K4 at 61.36%, and K6 at 50.37%. These findings indicate that maternal health service implementation has not been optimal. Therefore, an evaluation using the Logic Model framework, consisting of input, activities, and output, was conducted. Purpose: To evaluate the achievement of minimum service standard health performance indicators in maternal health services. Method: This study employed a qualitative research design using the Logic Model evaluation framework. Informants consisted of the Head of the Community health center, the Maternal and Child Health (MCH) Program Coordinator, midwives, village midwives, and pregnant women selected through purposive sampling. Data were collected through in-depth interviews, observations, and document reviews. Data were analyzed using the Miles and Huberman interactive analysis model, including data reduction, data display, and conclusion drawing. Results: The input component generally supported the implementation of maternal health services. However, several challenges were identified, including suboptimal workload distribution among healthcare personnel , limited MCH clinic facilities, and delays in the distribution of Maternal and Child Health (MCH Handbooks). The activities component showed that all maternal health service activities had been implemented according to established standards. Nevertheless, delays in antenatal visits, low continuity of K1–K6  antenatal care visits, inadequate monitoring of iron tablet compliance, and delayed MCH Handbooks distribution affected service documentation and health education. The output component indicated that the achievement of maternal health service MSS indicators had not yet reached the national target of 100%. Conclusion: The achievement of maternal health service MSS indicators has not yet met the expected target. This was influenced by suboptimal workload distribution among healthcare personnel , delays in MCH Handbooks distribution, and inadequate continuity of  antenatal care visits through K6. Suggestion: Strengthening the community health center had not, optimizing resource management, and improving follow-up and monitoring of pregnant women are necessary to improve the achievement of maternal health service MSS indicators.

Abstract

Background: Acute Respiratory Tract Infection (ARI) is a major health problem in toddlers and remains a cause of high morbidity and mortality in Indonesia, including Jambi City. The incidence of ARI in toddlers is influenced by various risk factors, one of which is environmental factors, particularly indoor air quality. One such indoor air pollutant is PM10. In addition to PM10, other factors can influence the incidence of ARI, such as nutritional status, exclusive breastfeeding, the presence of smokers in the home, adequate ventilation, and the use of mosquito coils. Purpose: To analyze the association between indoor PM10 exposure and acute respiratory infection among children under five. Method: This study used an observational analytical design with a cross-sectional approach. The sample consisted of 90 respondents aged 6-59 months in the Putri Ayu Community Health Center working area in Jambi City, selected using a purposive sampling technique. Data were collected through PM10 levels measurements using a DAZ-400 Particle Counter and interviews using a questionnaire. Data analysis was performed using univariate, bivariate, and stratified methods using R software. Results: Results showed that children living in homes with PM10 levels that did not meet the criteria were at risk of developing ARI (PR = 1.63; 95% CI: 1.19-2.23; P = 0.027). After stratification, PM10 remained a risk factor in each stratum, but with ventilation as an effect modifier. Conclusion: Indoor PM10 exposure was significantly associated with ARI among children under five. Improving indoor air quality, reducing indoor pollutant sources, and strengthening household ventilation are recommended to reduce ARI-related risks.

Abstract

Background: Cancer is a malignant disease caused by uncontrolled abnormal cell growth and is one of the leading causes of death worldwide. Breast cancer is a malignancy of breast tissue originating from the ductal or lobular epithelium and is the cancer with the highest incidence in women globally. Purpose: To determine factors of age and educational level influencing delayed diagnosis in female breast cancer patients Method: This study used an observational analytic study with a cross-sectional approach. A total of 52 respondents were included in the study, which influenced delayed diagnosis in women with breast cancer at Abdul Moeloek Hospital in 2025. Results: The results showed that the majority of respondents experienced delayed breast cancer diagnosis (27 respondents (51.9%), while 25 respondents (48.1%) did not experience delays, thus making delayed diagnosis still considered high. Bivariate analysis using Fisher's Exact Test showed that age significantly influenced delayed diagnosis (p = 0.011; OR = 5), with elderly women having a five-fold greater risk of experiencing delays compared to late adulthood. Furthermore, education level also significantly influenced the risk (p = 0.004). Conclusion: There is a relationship between age and education level, which influence late diagnosis in women with breast cancer.

Abstract

Background: The Free Nutritious Meal Program is a government initiative aimed at improving the health and nutritional status of the population. However, the provision of nutritious meals through this program does not automatically lead to improvements in nutritional status. The success of the Free Nutritious Meal Program requires sustained and appropriate healthy eating behaviors among beneficiaries. Purpose: To analyze the relationship between nutritious food consumption behavior and the nutritional status of students Elementary School who are beneficiaries of the Free Nutritious Meal Program. Method: This study employed an analytical correlational design with a cross-sectional approach. The sample consisted of 241 students from grades 1 to 6. Data were collected using a nutritious food consumption behavior questionnaire and nutritional status measurements. Statistical analysis was performed using Spearman’s rank correlation test. Results: Most students demonstrated good nutritious food consumption behavior (64.7%). The majority had normal nutritional status (59.8%), with a mean BMI-for-age Z-score of 0.67 ± 1.59. Spearman’s rank correlation analysis showed a correlation coefficient of 0.103 with a significance value of p = 0.109 (p > 0.05). Conclusion: There was no statistically significant relationship between nutritious food consumption behavior and the nutritional status of students at Gedongkiwo Elementary School. However, based on school records prior to receiving the Nutritious Meal Program program, 11 students who were at risk of stunting, 20 students who were underweight, and 4 students who experienced wasting had achieved normal nutritional status at the time of the study. Suggestion: These findings should be interpreted with caution, as the cross-sectional design does not allow for the observation of changes in nutritional status before and after the implementation of the Free Nutritious Meal Program. Further longitudinal studies or studies that consider students' baseline nutritional status are recommended

Abstract

Background: Stunting is a condition of growth failure in children caused by chronic malnutrition, characterized by a height-for-age (HFA) score below -2 SD according to WHO standards. An inappropriate diet is one of the factors that can influence the incidence of stunting in preschool-aged children. Stunting is not just a short-term health problem but also disrupts brain development, intelligence, immunity, and increases the risk of chronic diseases in adulthood. Children who are stunted experience limitations in learning, motor skills, and socio-emotional development, which lowers the quality of human resources. Therefore, stunting is a serious public health problem that requires immediate intervention. Purpose: To examine parental feeding practices and stunting status among preschool children. Method: This study employed a quantitative method with a cross-sectional design. The study sample consisted of 36 respondents selected using total sampling. Dietary data were collected using the Child Feeding Questionnaire (CFQ), while the prevalence of stunting was measured by height using a microtoise and calculated based on the height-for-age (HAZ) indicator according to WHO standards. Data analysis was performed using univariate and bivariate methods, employing Fisher’s Exact test with a significance level of α = 0.05 Results: The results of the Fisher’s Exact test show a p-value > 0.05. Based on the results of research conducte, it can be concluded that the eating patterns of preschool children are entirely in the appropriate category (100%), the incidence of stunting mostly does not occur, with 96.7% of children not stunted and only 3.3% experiencing stunting, and there is no relationship between eating patterns and the incidence of stunting in preschool children at both kindergartens (p value = 0.942). Conclusion: These findings suggest that adopting a dietary pattern that considers the type, quantity, and frequency of food is a crucial aspect in supporting the growth and development of preschool-aged children. A balanced and regular diet plays a role in meeting children's nutritional needs, although stunting can still be influenced by other long-term, multifactorial factors. Suggestion: It is recommended for parents and the school to continue maintaining the child's good eating habits and to increase periodic monitoring of nutritional status so that the risk of stunting can be prevented as early as possible.
