Abstract

 Off-label drug use is still widely found, especially among vulnerable patients. Pediatrics is one of the vulnerable patient groups who still often get off-label drug therapy. This study was conducted with the aim of looking at the description of the use of off-label drugs given to pediatric patients. This research is a type of research with the literature review method, with data obtained from several articles, the search for articles was carried out through the Google Scholar database. The results of this study from 10 articles used as research data, found the use of off-label drugs in pediatric patients with the most results in the off-label dosage category and followed by off-label dosage category, off-label age category, and off-label route of administration category, while the off-label contraindication category was not found. Based on the results obtained, it is concluded that the use of off-label drugs in pediatric patients is quite high, so it is necessary to monitor the use of off-label drugs to reduce the incidence of unwanted side effects

Abstract

This study aims to evaluate the effectiveness of education on the use of pain medication for outpatients at KHZ Musthafa Regional Hospital, Tasikmalaya Regency. Using a qualitative descriptive approach, this study describes in depth the counseling process carried out by pharmacists through the delivery of materials and interactive question and answer sessions. The education provided includes types of pain medication, rules of use, side effects, drug interactions, and the importance of stomach medication as a companion therapy. Data were collected through direct observation and documentation of interactions during counseling activities. Analysis was carried out using thematic methods to identify patterns of patient understanding and response. The results showed that direct and communicative education can improve patient understanding of pain medication, encourage adherence to therapy, and contribute to improving patient quality of life. These findings emphasize the importance of the active role of pharmacists in providing accurate and easy-to-understand drug information as an integral part of holistic health services that are oriented towards patient safety.

Abstract

Triage is a crucial process in the management of emergency patients to determine the priority of treatment based on the severity of the patient's condition. One of the triage systems used is the Emergency Severity Index (ESI). This study aims to determine the effect of triage Hospital Emergency Severity Index (ESI) video media on the knowledge of anesthesiology nursing students at Universitas 'Aisyiyah Yogyakarta. This research employed a quasi-experimental design with a non-equivalent pretest-posttest control group approach. The sample consisted of 40 students divided into two groups: 20 in the intervention group and 20 in the control group. The research instruments included an educational video and a knowledge questionnaire. Data analysis was conducted using the Wilcoxon Signed Rank Test and Mann-Whitney test. The results showed a significant increase in knowledge in the intervention group after the video media was provided. The average knowledge score increased from 36.60 (pretest) to 70.05 (posttest), with the Wilcoxon test yielding a p-value of 0.001 (p < 0.05). In contrast, the control group showed no significant change with a p-value of 1.000, indicating a significant difference between the two groups. The triage Hospital Emergency Severity Index video media is effective in enhancing the knowledge of anesthesiology nursing students.

Abstract

General anesthesia is a procedure that eliminates consciousness and pain during surgery. Common devices used to secure the patient’s airway during surgery include the endotracheal tube (ETT) and the laryngeal mask airway (LMA). However, these devices can cause side effects, one of which is postoperative nausea and vomiting (PONV). PONV is a physiological reaction that frequently occurs within the first 24 hours after surgery. This study aims to compare the incidence of postoperative nausea and vomiting in patients undergoing general anesthesia using endotracheal tubes and laryngeal mask airways at RSUD Wates. This study employed quantitative approach with descriptive comparative design. The sample consisted of 40 postoperative patients at RSUD Wates who received either ETT or LMA. The statistical test used was the Mann-Whitney test. The Mann-Whitney test revealed a significant difference in the incidence of PONV between ETT and LMA use, with a p-value of 0.008 (p < 0.05). In the ETT group, 10 respondents (50%) experienced nausea, and 5 respondents (25%) experienced retching or vomiting. In the LMA group, 6 respondents (30%) experienced nausea and 1 respondent (5%) experienced retching or vomiting. There is a significant difference in the incidence of postoperative nausea and vomiting between the use of endotracheal tubes and laryngeal mask airways in patients undergoing general anesthesia.

Abstract

Spinal anesthesia is commonly used in surgical procedures but often leads to hypotension due to sympathetic blockade, which can seriously affect the patient's hemodynamic stability. To prevent this, the administration of lactated Ringer crystalloid fluid using preloading (before anesthesia) and coloading (during anesthesia) methods is widely implemented. This study aimed to compare the effects of preloading and coloading on blood pressure stability in patients undergoing spinal anesthesia at Roemani Muhammadiyah Semarang Hospital. A quantitative research method with a comparative observational design and cross-sectional approach was used, involving 40 patients divided equally into preloading and coloading groups. Blood pressure was measured before and after fluid administration, and data were analyzed using paired t-test and independent t-test. Results showed significant changes in blood pressure in both groups (p=0.000; p<0.05). Furthermore, the independent t-test indicated a significant difference between the two methods (p=0.019; p<0.05), with preloading proving more effective in maintaining blood pressure stability than coloading. In conclusion, there is a significant comparison between preloading and coloading of lactated Ringer crystalloid fluid on blood pressure in spinal anesthesia patients, with preloading showing a superior stabilizing effect on blood pressure.

Abstract

Indonesia is located at the meeting of the earth's plates, causing catastrophic events, such as earthquakes. Lhokseumawe is one of the cities in Indonesia located in Aceh Province, which has a high potential for the threat of earthquakes. Earthquakes not only cause death, injury, and damage to various health facilities but also have an impact on public health problems. The main factor that causes disasters to cause large casualties and losses is a lack of knowledge about disasters. In this case, health education or counseling can be an important factor that aims to increase public knowledge about disaster preparedness. The purpose of this study was to determine the effect of earthquake disaster preparedness counseling on community preparedness in Pusong Village, Banda Sakti District Lhokseumawe City. This is a quasi-experimental type of research with 150 respondents. Samples were taken using purposive sampling. The results showed that before the extension, the majority of the people were in the medium category (56.0%), while after the extension, the majority of the people were in the high category (67.3%). The Wilcoxon test is used to analyze data. The statistical analysis resulted in a p value of <0.05. The conclusion of this study is that there is a significant effect between earthquake preparedness counseling and community preparedness in Pusong Village, Banda Sakti District, Lhokseumawe City.

Abstract

Antibacterial substances such as thymoquinone, carvacrol, thymol, and p-cymene are found in black cumin (Nigella sativa), a type of herb. This study reviewed 12 recent scientific publications to assess the antibacterial efficacy of N. sativa against clinical pathogenic microorganisms. A comprehensive literature review of in vitro, in silico studies, and applications was conducted. The study found that black cumin extract and oil were more efficient against Gram-positive bacteria such as Staphylococcus aureus than Gram-negative bacteria such as Escherichia coli and Salmonella typhi. The antibacterial activity increased with the concentration of the extract, especially in the polar form (ethanol). Active compounds such as thymoquinone also showed high binding affinity to MRSA bacterial proteins molecularly. The application of N. sativa in the form of ointments, antibacterial textiles, and dental health products also showed good effectiveness. Based on these findings, Nigella sativa has great potential as a safe and environmentally friendly natural antibacterial alternative, although further clinical studies are needed to validate its efficacy and safety.

Abstract

Cerebral palsy (CP) is a non-progressive motor disorder caused by early brain injury. The spastic type is characterized by increased muscle tone and limited voluntary movement, often leading to delayed gross motor development in children. This case report aims to describe physiotherapy management in a spastic CP infant and evaluate the response after two weeks of intervention. The patient was a 10-month-old male infant (corrected age 8 months 19 days) with a history of prematurity, neonatal asphyxia, and diagnosed with spastic CP. Physiotherapy interventions included neurosensory stimulation, Bobath positioning, head and trunk control training, and a home program provided to the parents. Evaluation was conducted through clinical observation and the Hammersmith Infant Neurological Examination (HINE). After two weeks of therapy, the patient showed minor improvement in head control and reduced head lag, but no significant development in gross motor skills such as rolling or sitting. HINE score was 34, indicating a non-ambulatory prognosis. Short-term physiotherapy management in spastic CP can support early postural control, but long-term intervention and strong family involvement are essential to achieve optimal motor development.

Abstract

Myocardial infarction (MI), a subtype of coronary heart disease (CHD), is still a major cause of morbidity and mortality around the world. Non-ST-segment elevation MI (NSTEMI) is more common than ST-segment elevation MI (STEMI). Antithrombotic therapy, including antiplatelet and anticoagulant agents, is essential in MI management to prevent thrombus formation and reduce ischemic complications. Although international and national clinical guidelines (PERKI and ACC/AHA) provide recommendations tailored to MI type and patient factors, discrepancies in clinical practice, especially in developing countries, may impact treatment outcomes. This retrospective descriptive study aimed to evaluate the use and appropriateness of antithrombotic therapy in NSTEMI and STEMI patients at Hospital X, Kediri City, in 2018. Data were collected from medical records of hospitalized acute myocardial infarction (AMI) patients. Aspirin combined with clopidogrel was the most commonly used regimen for NSTEMI, whereas STEMI patients frequently received aspirin, clopidogrel, and enoxaparin; fibrinolytics were administered in nine STEMI cases. Dosage evaluation showed compliance with guidelines in NSTEMI patients, while 36.84% of STEMI patients, particularly those receiving enoxaparin, were given incorrect dosages. These findings highlight the need for improved adherence to clinical guidelines to optimize antithrombotic therapy outcomes in MI management

Abstract

Personal hygiene can affect food contamination can be for several factors including the cleanliness of eating utensils because the human body can also be a source of pollution including eating utensils if they do not maintain cleanliness. Contamination does not only occur in food and water sources, but food equipment that does not meet health requirements is also a cause. In Indonesia, regulations have been made in the form of Regulation of the Minister of Health of the Republic of Indonesia No. 1096 / Menkes / SK / VI / 2011 that for the requirements for hygiene and sanitation of catering services. Based on the description above, the problem in the study can be formulated, namely how is personal hygiene in fast food in Latta Village, Teluk Ambon Baguala District. To find out Personal hygiene in fast food in Latta Village, Teluk Ambon Baguala District. This type of research is descriptive research by interviewing and observing. The population in this study was 22 fast food traders. The sample used was 5 (five) fast food traders in Latta Village. The results of the study have been carried out that the examination of five traders I, traders II, traders III, traders IV, and traders V with the criteria did not meet the requirements according to Permenkes No. 1096/Menkes/Per/VI/2011 namely 0 CFU The results obtained were five food handlers for personal hygiene did not meet the requirements in Latta Village, Teluk Ambon Baguala District. The five samples met the requirements and five food handlers in Latta Village, Teluk Ambon Baguala District had Personal Hygiene that did not meet the requirements.
