Abstract

ABSTRACT Hirschsprung's disease is an abnormal condition of the colon with characteristics of the Meissneri submucosus, myenteric plexus and Aurbachi plexus without parasympathetic ganglion cells. Manifestations of Hirschsprung's disease are seen in the discharge of meconium that does not come out within 24 hours. Then there are signs of obstruction to the gastrointestinal tract, vomiting bile, constipation, and irregular bowel movements. In 2017, 2.5 million children died in the first month of life worldwide. 47% of deaths of children under the age of 5 years occur in newborns, which is about 7,000 deaths. Cases of Hirschsprung's disease itself occur in 1:5000-7000 births, with cases in boys 4 times more than girls. This study aims to determine the clinicopathological characteristics of Hirschsprung's disease at Sanglah Hospital Denpasar in 2019-2020 based on clinical symptoms, age, gender, and location of the aganglionic bowel segment. This study was carried out in 2021 at the Sanglah Hospital Denpasar Anatomical Pathology (PA) installation, using data from the years 2019-2020. It was conducted using a cross-sectional method, with secondary data in the form of patient data available at the Sanglah Hospital Denpasar PA Laboratory, and total sampling. SPSS version 26 was used to analyze the data. The results of this study showed that there were 78 patients with Hirschsprung's disease at Sanglah Hospital Denpasar in 2019-2020 and all of them met the criteria of inclusion. Most cases of Hirschsprung's disease in Sanglah Hospital Denpasar experienced the most flatulence, namely 38 cases with a percentage of 36.5%. Cases were dominated by patients within age of 0-12 months with total of 64 people with 82.1% percentage, and dominated by men as many as 50 people with 64.1% percentage. The aganglionic most frequently found in the rectum as many as 49 people with a percentage of 62.8%. It was concluded from the study result that Hirschsprung's disease patients at Sanglah Hospital in 2019-2020 were mostly male, mostly in the age range 0-12 months, with the most clinical symptoms being flatulence, and the most common aganglionic location found in the rectum. This result may then be used as a basis and also study material for further related research and learning Keywords : Hirschsprung's disease, congenital megacolon, aganglionic, clinicopathological characteristics

Abstract

ABSTRACT Dengue Hemorrhagic Fever (DHF) as one of the most common infectious diseases in the world is still a major concern even though the incidence and mortality rates have decreased. The study objective is to determine hematological examination results and the speed of recovery of DHF patients reviewed according to age, gender, BMI, degree of DHF, and duration of fever before being treated. This was cross-sectional study using secondary data from medical records of 193 DHF patients treated from year 2019 to 2020. In 193 patients, before being admitted to the hospital, the patient had an average fever for 4.12±1.16 days with an average 4.69±1.2 days of hospitalization. Hematological test on hospital admission showed the average platelet value was 83.06±28.74 x103 cells/µL with 187 (96.9%) patients had platelet values below the normal value. Average leukocyte count was 4.13±2.01 x 103 cells/µL with 126 (65.3%) patients having a leukocyte value below the normal value. Average hematocrit value was 40.31±4.96% with 113 (58.6%) patients had normal hematocrit values. The duration of hospitalization for DHF patients had a significant difference when viewed according to age (p=0.000), BMI (p=0.000), and duration of fever before being treated (p=0.000). The mean platelet values were significantly different when compared according to the duration of fever before being treated (p=0.018). The mean leukocyte values were significantly different when compared according to age (p=0.007) and gender (p=0.008). The mean hematocrit value was significantly different when compared according to age (p=0.000) and gender (p=0.008). Abstract

Introduction. One of the sexually transmitted diseases is syphilis which is caused by the bacterium Treponema pallidum. Patients with syphilis can experience severe systemic symptoms and can cause disability. However, in latent syphilis the initial symptoms are minimal or even asymptomatic, so that undetected syphilis sufferers can still escape and can carry out blood donation activities. To meet the demand and need for blood in Denpasar City, the Indonesian Red Cross Blood Donor Unit Denpasar City conducted blood donation activities. However, due to the ongoing Covid 19 world pandemic, including Indonesia, the activities of mobile blood donor units are limited. Providing safe blood is the duty of the blood donor unit. Screening test is a way to detect the presence of Transmitted Infection Through Blood Transfusion (IMLTD). The techniques used are Enzyme-linked Immunoserbent Assay (ELISA) and Chemiluminescence Immunoassay (CLIA). Methods. This research is a retrospective descriptive type by taking secondary data from the screening laboratory of the Denpasar City PMI blood donor unit for the period May-December 2021. Results. There were 21 blood samples with reactive syphilis results from May to August which were performed using the ELISA method with a total of 831 donor bags. A total of 28 blood samples from 1,338 bags that were donated showed reactive results from September to December using the CLIA method. Keywords: Syphilis, ELISA, CLIA, blood donation, prevalence

Abstract

Open tibial fracture is one of the most common lower extremity fractures in the world, especially in Denpasar City. Clinically, grade III open tibial fracture has higher risk of worse outcome, such as infection and bone healing complication than grade I and II, thus it should be evaluated further regarding risk factors affecting that occurrence. This research aims to know the relationship between several risk factors, those are age, surgery type, and time to surgery with postoperative outcomes in grade III open tibial fractures in terms of clinical, radiological, and functional based on Johner-Wruhs criteria. This research used cross-sectional method and used patients with grade III open tibial fracture who underwent surgery at Sanglah Denpasar General Hospital in 2018-2020. Based on the data collection, there were 31 patients fulfilled the inclusion and exclusion criteria. The data collected were characteristics and postoperative outcome assessed using Johner-Wruhs criteria which consisted of 4 categories. From Kendall Tau correlation test, there was no significant relationship between age and outcome (p=0,215) and a significant relationship between time to surgery and outcome (p=0,021). Moreover, the results of Fisher’s Exact Test did not find significant relationship between surgery type and outcome (p=0,720). Based on the results, it can be concluded that time to surgery (debridement) has significant effect on postoperative outcome compared to age and surgery type. Time to debridement also has unidirectional relationship with outcome, which means the longer time to debridement, the worse outcome experienced by the patient. Keywords: open tibial fracture, surgery type, outcome, age, time to surgery

Abstract

ABSTRACT Systemic lupus erythematosus (SLE) is one of the most common autoimmune diseases in the community. One of common clinical manifestations of SLE is Lupus Nephritis (LN). The World Health Organization (WHO) notes that there are five million people with lupus in the world and there are 100,000 more new sufferers every year. In Indonesia in 2016 it was reported that 2,166 patients were hospitalized with a diagnosis of SLE and 550 of them died. This study aims to determine the characteristics of LN patients at Sanglah Hospital Denpasar in 2016-2020 based on age, gender, histopathological class (categorized according to ISN/RPS), and proteinuria level and to determine the relationship between proteinuria level and histopathological class and the relationship between age and histopathological class. This research was conducted at the Anatomical Pathology (PA) laboratory of Sanglah Hospital Denpasar in 2021 with a cross-sectional method, using secondary data that available at the PA laboratory of Sanglah Hospital Denpasar, then processed using SPSS ver. 26. The results showed that the total number of LN patients at Sanglah Hospital Denpasar in 2016-2020 was 28 cases. Most cases were found at age range of 26-35 years (11 cases or 39.3%) and most were female (23 cases or 82.1%). The most common proteinuria level found was +3 (8 cases or 28.6%) and the most common histopathological class was class IV (13 cases or 46.4%). Chi square test showed no significant relationship between proteinuria level and LN histopathological class (p=0.069) nor between age and LN histopathological class (p=0.202). It was concluded that LN patients at Sanglah Hospital in 2016-2020 were mostly women, aged 26-35 years, with a proteinuria level of +3 and included class IV LN. Since there is no correlation between age or proteinuria level with histopathological class, it is recommended that histopathological biopsy be performed in all cases of suspected LN so that patients receive adequate therapy based on the level of kidney damage. Keywords : Lupus nephritis, age, gender, proteinuria level, histopathological class according to ISN/RPS
