Abstract

Background: One of the impacts experienced by diabetes mellitus patients is a decrease in their quality of life. The Audit of Diabetes-Dependent Quality of Life (ADDQoL) is a widely used individualized diabetes-specific quality of life measure. However, there was no version available in the Indonesian language. Objective: This study is aimed to undertake linguistic validation, including a cultural adaptation of the ADDQoL questionnaire into the Indonesian language. Method: The original developer granted permission to use and modify the questionnaire. The international linguistic validation procedure developed by the Mapi Research Institute was used. There were six steps involved: forward translation, reconciliation, back translation, expert panel review by a psychologist and clinician, cognitive debriefing with diabetes patients, and proofreading. Result: Problems that arose during the linguistic validation process were resolved by finding conceptually equivalent alternatives and changing sentence structures to achieve equivalence in language, concept, and culture with the original version of the ADDQoL. The developer's team reviewed and discussed all actions taken. Cognitive debriefing interviews with five respondents showed that the ADDQoL questionnaire was simple to understand. Conclusion: The Indonesian version of the ADDQoL is linguistically and culturally validated. Further studies are needed to confirm the structure and reliability of the Indonesian ADDQoL.

Abstract

Background: Hypertension is currently a non-contagious disease that primarily affects the elderly population in Indonesia. Medication adherence is critical in managing hypertension and reducing the risk of morbidity and mortality. Previous research has found that loneliness and support received by older adults with hypertension influence medication adherence. Objective: This study aimed to examine the impact of feeling lonely and receiving social support on medication adherence in the elderly with hypertension at the Community Health Center in Surabaya. Methods: The study design of this research was a descriptive cross-sectional study from December 2021 to March 2022. A total of 235 eligible subjects fulfilled the inclusion criteria. The instruments used in the data collection were the patient's information form, UCLA-Loneliness Scale, MOS-Social Support Survey, and the ARMS (Adherence to Refill and Medication Scale). Results: The results revealed that the correlation between loneliness and social support was significantly associated with medication adherence in the elderly with hypertension (p < 0.05). In addition,  other factors, such as occupation status, living status, comorbidity, the number of drugs taken, and antihypertensive drug therapy, showed a significant correlation with medication adherence (p < 0,05). The most influential factor on medication adherence was loneliness (35.5%), followed by social support (24.4%), the number of drugs taken (7.1%), antihypertensive drug therapy (monotherapy or combination therapy (2.5%), occupation status (2.4%), comorbidity (1.6%), and living status (0.2%). Conclusion: This study confirms that feeling lonely and receiving social support affect medication adherence in the elderly with hypertension at the Community Health Center in Surabaya.

Abstract

Background: Type 2 diabetes mellitus (DMT2) is a metabolic disease due to abnormalities in insulin secretion. Insulin is one of DMT2 therapy. Objective: This study aimed to validate a modified the insulin adherence influence factor questionnaire based on the health belief model (HBM) among Indonesian patients with DMT2. Methods: The Indonesian insulin adherence influence factor questionnaire (IIAIFQ) was developed based on modified some literature reviews and internal expert discussions. The study included 30 participants aged ≥ 17 y.o years old who had been taking insulin in the previous two months. The questionnaire consists of seven dimensions to measure HBM : perceived susceptibility, perceived severity, perceived benefits, perceived barriers, self-efficacy, cues to action and insulin adherence. Results: The construct validity test showed that of the 34 question items in the questionnaire, 10 items were invalid, 24 others were demonstrated valid based on the Pearson Correlation (>r table 0.361; p<0.05; loading factor > 0.5). Furthermore, 24 valid items were tested for reliability at a significance level of 0.05, and the results showed that each size had a  Cronbach's Alpa > 0.6 with an overall score was 0.858, indicating that all domains in the questionnaire were reliable. Conclusion: IIAIFQ  based The HBM theory is a valid and reliable instrument for assessing insulin adherence in diabetes mellitus patients.
