Abstract

The bending of the uterus restricts venous return and gravitational pull, which increases fluid retention and causes 80% of pregnant women in the third trimester to experience leg edema or swelling. Edema in pregnant women’s legs can be a sign of dangerous pregnancy conditions, such as pre-eclampsia. Edema is also very dangerous for pregnant women as it can lead to issues with the heart, kidneys, and other body parts, disrupting their functions. Foot soaking, or foot hydrotherapy, is an additional therapy that can be done independently. To improve blood circulation and trigger a systemic response (vasodilation) through the dilation of blood vessels, bathing with warm water will help. The authors aim to conduct research on "The Effect of Ginger Decoction on Edema in Pregnant Women's Legs at Kasih Ibu Pratama Clinic in 2024" based on the above information and data. They believe that foot soaks can also be combined with other herbal ingredients. The research method used a population of 100 with a sample of 17 individuals, employing a quantitative research design and a one-group pre-test post-test model, where observations were made before and after treatment with data analysis using the Wilcoxon test. The study found that ginger decoction has an effect on reducing edema in the legs of pregnant women. The test results showed a probability of 0.01. This means that the probability < level of significance (? = 0.05), so Ha is accepted, and H0 is rejected, indicating a significant difference between leg edema in pregnant women before (pre-test) and after (post-test) the administration of ginger decoction. Therefore, ginger decoction has an effect on reducing edema in pregnant women's legs.

Abstract

The mismatch in the healthcare workforce ratio leads to suboptimal healthcare services in community health centers (Puskesmas). The quality of healthcare services needs to be evaluated from the perspective of both the institution and the patients, as interpreted through the community satisfaction index. This study utilized cluster randomized sampling. Data collection techniques included observation and questionnaires. The population of this study consisted of the number of outpatient visits in 7 (seven) medium-sized community health centers in South Lampung Regency in 2020, totaling 287,279 visits. The sample size for this study was 347 samples in each group. The study was conducted from February to March 2023. The aim of this research is to analyze the influence of healthcare service quality on patient satisfaction index in Community Health Centers in South Lampung Regency, focusing on the dimensions of Reliability, Assurance, Tangibles, Empathy, and Responsiveness (SERVQUAL). The results of this study showed that the Community Satisfaction Index (CSI) for healthcare services had a mean quality scale of 3.09, indicating a "Good" index. Regarding healthcare service quality, in terms of the SERVQUAL measurement variables, there is still 1.2% of the population stating that the quality of services in community health centers is not good, specifically in the aspect of Empathy. The significance value (P=0.00) indicates that service quality significantly affects the CSI, with a coefficient of determination (R square) of 0.685. Recommendations from this study suggest that the government of South Lampung Regency should provide special training for healthcare workers in community health centers to improve their skills in terms of their behavior and empathy towards patients, as well as to improve the ratio between healthcare personnel and the number of patient visits.

Abstract

Indonesia’s aging population presents an increasing demand for accessible, community-based health services. However, the utilization of Rumah Kasih, a community health program for older adults, remains suboptimal. Objective to examine the reinforcing factors influencing its utilization, including support from healthcare workers, university students, community health workers (CHWs), village heads, family members, and peers. A quantitative, cross-sectional study was conducted involving 192 elderly respondents with total sampling. Data were collected using questionnaires and analyzed using chi-square tests and multivariate logistic regression. The bivariate analysis indicated that all support factors were significantly associated with service utilization (p < 0.05). Multivariate analysis identified healthcare worker support as the most influential predictor (OR = 48.346; p < 0.001), followed by student involvement (OR = 5.259; p = 0.005). Although family support was significant in the bivariate analysis, it was not retained in the multivariate model. CHW and peer support yielded high odds ratios but lacked statistical significance, possibly due to data variability or multicollinearity. These findings highlight the critical role of professional engagement and student participation in enhancing elderly involvement in community health programs. Strengthening these factors, along with targeted strategies to involve social actors, may improve the effectiveness and long-term sustainability of elderly care services.

Abstract

Tuberculosis (TB) is still a major health problem globally, due to serious health disorders in the community and the incidence of this disease increases every year. Currently, it is estimated that 1 in every 3 TB cases have not been detected by the Case Detection Rate (CDR) program, which is known from national data showing that CDR in Indonesia from 2012-2014 has decreased, namely case discovery in 2012 was 61%, and in 2014 the case discovery rate was 46%. The purpose of this study was to Analyze Factors Influencing the Implementation of the Pulmonary TB Patient Health Promotion Program at the Grandmed  Lubuk Pakam Hospital in 2024. This study is a qualitative descriptive study. The results of this study are that the implementation of the pulmonary TB patient health promotion program in hospitals is influenced by various interrelated factors, namely individual factors, the environment, health systems, policies, and health workers. Individual factors such as knowledge, socio-economic conditions, and treatment compliance determine the extent to which patients can accept and carry out health recommendations. Environmental factors, including social stigma and family or community support, also influence patient motivation and openness to health education. From the health system side, the existence of an effective information system and guaranteed drug availability are crucial technical supports. Meanwhile, the success of the program is highly dependent on management policies and the design of a structured and sustainable promotion program. The conclusion is that the role of health workers through quality communication and education greatly determines the effectiveness of information delivery and changes in patient behavior. Therefore, optimal implementation of health promotion requires a comprehensive and collaborative approach from all of these factors to support sustainable TB control efforts in hospitals.

Abstract

Occupational Safety and Health (K3) is an effort made so that workers always work safely and healthily in the workplace, and resources and production processes can be used safely and efficiently. The number of work accident cases in Indonesia in the period January-August 2024 was 278,564 cases, of which North Sumatra Province had 15,220 cases. Every company is required to implement an Occupational Safety and Health Management System (SMK3) that is integrated with the company's management system. The implementation of the occupational safety and health management system (SMK3) at Perumda Tirtanadi, North Sumatra Province has a good management system, this is proven by the certification audit process by the K3 management system audit institution by PT. Surveyor Indonesia and has received recognition from the Indonesian Ministry of Manpower. This study aims to determine five (5) principles, namely the determination of K3 policies, K3 planning, implementation of K3 plans, monitoring and evaluation of K3 performance, and review and improvement of K3 performance at Perumda Tirtandi. This study uses a descriptive qualitative approach through interviews. The interview results were processed using the NVIVO application. The results of the study are on the first principle of establishing K3 policies by establishing SOPs and annual work programs in accordance with national regulations, which must be informed to all employees. The second principle is K3 planning which emphasizes K3 culture through repeated socialization reflecting an ecological-social approach in health promotion. The third principle, namely the implementation of K3 in Perumda Tirtanadi shows a strong commitment to ensuring the use of PPE and compliance with K3 signs. The fourth principle, sustainability in SMK3 is reflected in the monitoring system of Perumda Tirtanadi, which involves visits from management and the K3 advisory committee. The fifth principle is a critical process that allows management to conduct objective assessments of the entire system. The study on the Implementation of the Occupational Safety and Health Management System (SMK3) requires socialization of SMK3 and considers K3 as something mandatory and a necessity when working.

Abstract

Objective: This study aims to analyze the factors associated with low back pain (LBP) complaints among computer-using office workers at Sri Pamela Hospital, Tebing Tinggi City, in 2025.Method: This research employed a quantitative approach with a cross-sectional design. The sample included all computer-using office workers at Sri Pamela Hospital. Data were collected using a questionnaire covering individual characteristics such as age, length of service, body mass index, smoking habits, physical activity, and working duration, as well as complaints of LBP. Data were analyzed using bivariate and multivariate methods. Results: The study found that age, length of service, body mass index, smoking habits, physical activity, and working duration were significantly associated with LBP complaints. Additionally, workplace ergonomics and sitting posture also contributed to the occurrence of LBP.Conclusion: LBP complaints among computer-using office workers are influenced by various individual and workplace factors. Preventive efforts should focus on improving ergonomics, increasing physical activity, and promoting healthy lifestyle management in the workplace.

Abstract

Stunting is a chronic nutritional problem characterised by a child’s height being below the standard growth for their age, which impacts cognitive development, immunity, and future productivity. This study aims to analyse the influence of family financial literacy and household food security on the prevention of stunting among children under five in Lubuk Pakam District, Deli Serdang Regency, and to determine the most dominant factor. The study employed a quantitative design with a cross-sectional method. The population consisted of 130,417 families with children aged 6–59 months, with a sample of 256 respondents selected using multistage cluster sampling. Data were obtained through anthropometric measurements of children under five and questionnaires on financial literacy and food security, both of which had been validated and reliability tested. Data analysis was conducted using univariate, bivariate (chi-square test), and multivariate (logistic regression) methods. The results revealed that low financial literacy increased the risk of stunting by 2.18 times (OR = 2.18; p = 0.004), while low food security increased the risk by 3.12 times (OR = 3.12; p < 0.001). Mother’s education (p = 0.082) and family income (p = 0.162) had no significant effect. Multivariate analysis identified food security as the most dominant factor in preventing stunting, followed by financial literacy. In conclusion, strengthening household food security and improving financial literacy through education and household financial planning should be prioritised as strategies to reduce stunting prevalence among children under five.

Abstract

Acalypha indica L., commonly known as Indian copperleaf, is a wild medicinal plant typically found along roadsides and in gardens. Its leaves contain various bioactive compounds, including saponins, flavonoids, alkaloids, and tannins. Traditionally, the plant has been used for its anti-inflammatory, antibacterial, antifungal, and antioxidant properties, which help combat oxidative stress caused by free radicals. This study aimed to evaluate the effectiveness of ethanol extract of Acalypha indica L. leaves in treating gastric ulcers in male Wistar rats (Rattus norvegicus). This research was a true experimental study involving the extraction of Acalypha indica L. leaves using ethanol, preparation of the extract suspension, and induction of gastric ulcers in rats using aspirin at a dose of 800 mg/kg body weight (BW) administered orally. A total of 30 male Rattus norvegicus rats were divided into six groups. Three control groups is Ulcer control, Negative control, Positive control. Three treatment groups receiving ethanol extract of Acalypha indica leaves (EEAIL) at doses of: EEDAA 50 mg/kgBB, EEDAA100 mg/kg BB, EEDAA 200 mg/kgBB. Ulcer induction was carried out daily until surgical examination was performed on days 4, 9, and 14. Macroscopic observations of the stomach were conducted to assess the number of ulcers and ulcer scores. Statistical analysis was performed using one-way ANOVA, followed by Tukey’s post hoc test to determine significant differences between groups. Macroscopic examination showed that the ethanol extract of Acalypha indica leaves (EEAIL) reduced the number of gastric ulcers. Complete healing of ulcers was observed in the positive control group, as well as in the groups receiving EEAIL at 200 mg/kg BW and 100 mg/kg BW, by day 14. In contrast, the 50 mg/kg BW group and the ulcer control group showed no signs of healing by the end of the observation period. The most effective dose was 200 mg/kg BW, demonstrating the highest capacity to reduce the number of gastric ulcers.

Abstract

Kekerasan seksual terhadap anak merupakan isu serius yang memerlukan intervensi layanan pendampingan yang efektif, memadai, dan responsif. Pusat Pelayanan Terpadu Pemberdayaan Perempuan dan Anak (P2TP2A) Kabupaten Padang Lawas menyediakan layanan khusus bagi anak korban, namun evaluasi menyeluruh terkait pengalaman, kepuasan, dan hambatan layanan masih terbatas. Penelitian ini menggunakan metode campuran (mixed methods) dengan pendekatan kuantitatif melalui survei menggunakan kuesioner skala likert kepada 20 anak korban kekerasan seksual, serta pendekatan kualitatif melalui wawancara mendalam dengan pihak P2TP2A dan keluarga korban. Analisis kuantitatif dilakukan untuk menghitung tingkat kepuasan, sedangkan analisis kualitatif dilakukan untuk menggali pengalaman layanan dan faktor penghambat. Hasil penelitian menunjukkan bahwa pengalaman penerimaan layanan dinilai cukup baik, dengan sikap petugas yang empatik dan proses pendampingan yang mendukung. Tingkat kepuasan secara keseluruhan berada pada kategori cukup puas, dengan aspek pemulihan psikologis mendapat nilai tertinggi (mean = 3,55), sedangkan aspek privasi dan kerahasiaan mendapat nilai terendah (mean = 2,65). Hambatan utama yang ditemukan meliputi keterbatasan tenaga psikolog, lamanya waktu tunggu, minimnya fasilitas seperti rumah aman, serta kendala geografis yang memengaruhi akses layanan. Layanan pendampingan di P2TP2A telah memberikan manfaat signifikan bagi anak korban kekerasan seksual, terutama dalam pemulihan psikologis. Namun, masih terdapat aspek yang perlu ditingkatkan, terutama terkait perlindungan privasi, percepatan respons, dan ketersediaan sumber daya pendukung. Rekomendasi perbaikan mencakup penguatan sumber daya manusia, peningkatan fasilitas, serta pengembangan strategi untuk menjangkau anak-anak di wilayah terpencil secara lebih merata.

Abstract

Post-claim audits conducted by BPJS Kesehatan serve as a supervisory mechanism to ensure accountability in the management of the National Health Insurance (JKN) funds. However, audit practices that result in claim corrections or deductions may cause financial losses to hospitals and raise concerns regarding procedural and distributive justice. This study aims to analyze the implications of post-claim audits on hospital financial losses from the perspective of accountability and the principles of justice at RSU Grandmed. This research employed a mixed-method approach with a cross-sectional design involving 97 respondents selected through purposive sampling. Quantitative data were analyzed using multiple linear regression, examining variables such as transparency of the claim process, auditor accountability, tariff adequacy, timeliness of payment, and allocation of claim funds. The findings indicate that, partially, transparency of the claim process, tariff adequacy, timeliness of payment, and allocation of claim funds significantly influence the hospital’s financial condition, while auditor accountability was not significant in the bivariate analysis. However, simultaneously all variables demonstrated a significant effect on hospital financial losses and financial sustainability (R² = 0.693). These findings demonstrate that post-claim audits are not merely administrative mechanisms but have direct implications for hospital financial stability. Therefore, audit mechanisms must be implemented in a transparent, consistent, objective, and justice-oriented manner to maintain a balance between public accountability and the sustainability of health service delivery.Keywords: Indonesia Health Social Security Agency; post-claim audit; accountability; principles of justice; hospital financial loss.
