Abstract

Diare masih menjadi salah satu masalah kesehatan yang sering terjadi di masyarakat, sementara praktik swamedikasi yang tidak didukung pengetahuan yang memadai berpotensi menyebabkan penggunaan obat yang tidak rasional. Kegiatan pengabdian kepada masyarakat ini bertujuan meningkatkan pengetahuan Generasi Z di Pondok Pesantren Zamzami mengenai penyakit diare, penggunaan oralit, penanganan awal diare, serta penggunaan obat secara rasional dalam swamedikasi diare. Kegiatan dilaksanakan pada 25 September 2025 dengan melibatkan 54 santri tingkat Madrasah Tsanawiyah dan Madrasah Aliyah. Intervensi dilakukan melalui penyuluhan kesehatan menggunakan metode ceramah interaktif, diskusi, dan tanya jawab dengan media slide presentasi dan leaflet. Evaluasi menggunakan desain one-group pretest-posttest dengan pengukuran tingkat pengetahuan sebelum dan sesudah penyuluhan menggunakan kuesioner terstruktur. Data dianalisis secara deskriptif dan inferensial menggunakan paired t-test dengan tingkat kepercayaan 95%. Hasil menunjukkan peningkatan rerata skor pengetahuan dari 58,15 sebelum intervensi menjadi 82,37 setelah intervensi. Peningkatan tersebut bermakna secara statistik (p < 0,001). Proporsi peserta dengan kategori pengetahuan baik meningkat dari 18,5% menjadi 74,1%, sedangkan kategori pengetahuan kurang menurun dari 29,6% menjadi 3,7%. Temuan ini menunjukkan bahwa penyuluhan kesehatan berpotensi meningkatkan pengetahuan santri mengenai penyakit diare, penggunaan oralit, penanganan awal diare, dan penggunaan obat yang rasional dalam swamedikasi. Kegiatan edukasi serupa perlu dilakukan secara berkelanjutan untuk mendukung perilaku swamedikasi yang tepat di masyarakat.

Abstract

Introduction: Renal impairment affects the pharmacokinetics of various antibiotics eliminated through the renal route, potentially leading to drug accumulation, toxicity, or therapeutic failure if doses are not appropriately adjusted. The variability in antibiotic response in this population remains a clinical challenge due to the complexity of pharmacokinetic/pharmacodynamic (PK/PD) parameters, degree of renal dysfunction, comorbidities, and dosing strategies. Objective: This systematic review aims to explore the relationship between clinical pharmacokinetic parameters and antibiotic response variability in adult patients with renal impairment, as well as to identify implementable PK/PD-based individualized therapy strategies. Methods: A literature search was conducted through PubMed/MEDLINE, Scopus, Embase, and Cochrane Library (2016–2026) following PRISMA 2020 guidelines. Studies involving adult patients (≥18 years) with renal impairment that reported PK/PD parameters and clinical outcomes were included. A narrative synthesis was performed on 18 articles meeting the inclusion criteria. Results: Renal function is a major determinant of antibiotic exposure, particularly for renally eliminated drugs. PK/PD parameters such as AUC/MIC, trough concentration, and %fT>MIC play important roles in assessing therapeutic adequacy, although their association with clinical outcomes is not always consistent due to population heterogeneity, infection severity, and differences in dosing strategies. Higher vancomycin exposure is consistently associated with an increased risk of nephrotoxicity. PK/PD-based individualized approaches through therapeutic drug monitoring, Bayesian forecasting, and model-informed precision dosing have been shown to improve target concentration attainment and reduce toxicity risk. Conclusion: Clinical pharmacokinetic parameters are associated with antibiotic response variability in patients with renal impairment, with more consistent relationships observed for toxicity than for efficacy. Integration of PK/PD-based individualized dosing strategies into routine clinical practice is necessary to optimize antibiotic therapy effectiveness and safety, supported by the development of comprehensive predictive models in this population.

Abstract

Sweet soy sauce is a fermented soybean product widely used as a traditional Indonesian seasoning. Its production process involves specific microorganisms to develop unique taste, aroma, and texture characteristics. However, poor hygiene and sanitation can lead to microbial contamination, such as Coliform, which may degrade quality and pose health risks. This study aimed to evaluate the microbiological quality of sweet soy sauce using the Most Probable Number (MPN) method. Samples were collected from food vendors around the UPN "Veteran" Jakarta campus and tested using liquid media to detect the presence of Coliform. The results showed that sample 1 (street vendor) had an MPN value of 14 MPN/gram, exceeding the BPOM safety limit (3 MPN/gram), while sample 2 (packaged soy sauce from a minimarket) was within the safety limit at 3 MPN/gram. A confirmatory test detected the presence of Escherichia coli in sample 1 but not in sample 2. Proper product handling, implementation of Good Manufacturing Practices (GMP), and regular microbiological monitoring are necessary to ensure the safety of sweet soy sauce products.
