Abstract

The nose has a physiological function for example as a filter that is useful to clean the air of inspiration from dust, bacteria and viruses through a mechanism called the mucociliary transport system (TMS). This TMS mechanism functions to protect the upper and lower respiratory system by forming waves of sweeping on foreign objects such as dust and even microorganisms trapped in the mucous pallets. This study aims to determine whether there are differences in nasal mucociliary transport time in smokers and non-smokers. The material used in this study is sakharin which has a sweetness level 700 times sweeter than ordinary sugar. This study involved 50 research subjects who were divided equally into 2 groups, 25 smokers and 25 non-smokers. All subjects will be interviewed in the form of a questionnaire and then test the time of mucociliary transport of his nose using the sakharin test. The results of this study found that the average mucociliary transport time of the nose to non-smokers was 24.67 ± 5.36 minutes and the mean time of mucociliary transport of the nose to smokers was 9.42 ± 5.04 minutes. Obtained also the results of the average mucociliary transport time of noses in light smokers that is 24.2 ± 5.4 minutes and the average mucociliary transport time of noses in moderate smokers is 27.9 ± 3.94 minutes. It can be concluded that the difference in nasal mucociliary transport time was found in smokers and non-smokers where the mucociliary transport time in smokers was longer.

Abstract

Electronic games or often referred to as online games, have experienced very rapid development and progress. This can be seen in various small cities in all corners of Indonesia, where many game centers have emerged. Even lately, the type and theme of the game that develops leads to the theme of violence that triggers aggressive behavior in adolescents. In addition, as the number of game addicts increases, the number of criminal acts triggered by the game increases. The purpose of this study was to determine the relationship of addiction to playing games with personality disorders in game players. This research uses observational analytic method with cross sectional design. The research subjects were game players in the Makassar City Game Center which were taken in total sampling. Data is collected using Game Addiction Scale (GAS) questionnaire, Mini International Neurophysciatric Interview (MINI), and game player data sheets. Hypothesis testing the correlation between addiction to playing games with personality disorders was analyzed by the two free sample T Test. Of the 50 gamers in the Game center that were sampled, there were 48 people who experienced personality disorders. Bivariate analysis of addiction to play games associated with personality disorders obtained p value = 0,000, the type of game not related to personality disorders obtained p value = 0.745, and the duration of playing games related to personality disorders obtained p value = 0.001. The conclusion is that there is a relationship between addiction and the duration of playing games with personality disorders. It is recommended not to always use the game so that it does not experience personality disorders.

Abstract

EGFR-TKI is the first-line therapy for EGFR-mutant patients. Nevertheless, patients will have disease progression (median PFS 10 – 12 months) due to resistance. The treatment options are still limited in developing countries for such cases, thus double-platinum chemotherapy is the next option. Although IMPRESS study reported no difference in terms of PFS and OS between double-platinum alone and double-platinum plus EGFR-TKI, several local studies reported benefit of continuing EGFR-TKI in combination with double-platinum chemotherapy (treatment beyond progression). This study aimed to compare chemotherapy effects of double-platinum plus EGFR-TKI versus double-platinum alone on patients with NSCLC progression after EGFR-TKI treatment. This was an analytical descriptive study using prospective cohort design, involving 30 patients with disease progression following EGFR-TKI treatment that met inclusion criteria in Dr. Soetomo Hospital. Subjects were divided into two groups: arm A (double-platinum plus EGFR-TKI) and arm B (double-platinum alone). Subjects were observed until 4 cycles of double-platinum chemotherapy. Subjective response (body weight and EQ5D questionnaire) was analyzed, chest CT scans were evaluated using RECIST criteria, and adverse effects were monitored. This study was conducted in accordance with GCP principles and has received ethics certificate from Dr. Soetomo Hospital ethics committee (No. 08/Panke.KKE/I/2017). The results showed that subject characteristics between two arms were insignificantly different (p=0.05). The most common EGFR mutation was exon 21 (50% on arm A and 60% on arm B). Chi square was tested on subjective response parameter (EQ5D (p=0.483)). T2 free sample was tested on semi-subjective parameter (body weight (p=1.00)). Comparison test on both groups after cycle 2 and 4 showed p value=0.05. Statistical test on adverse effect between both groups showed p value=0.526. As a conclusion, there was no significant difference between double-platinum and double-platinum plus EGFR-TKI on patients who had disease progression following EGFR-TKI treatment.

Abstract

Introduction: Tuberculous lymphadenitis (TBLN) is a form of extra-pulmonary TB with clinical features ranging from lumps to abscesses. Human Immunodeficiency Virus (HIV) co-infection and diabetes mellitus alongside TBLN made the diagnosis and management exceptionally challenging. We reported 3 cases of TBLN, 2 among them had an existing HIV co-infection, and 1 had preexisting diabetes mellitus.Case: The first case, a 28-year-old man, previously diagnosed with HIV, complained of a lump in the neck; biopsy results suggested TBLN. The second case was a 36-year-old man with a neck abscess and HIV co-infection. Acid Fast Bacilli (AFB) pus was positive & Human Immunodeficiency Virus Enzyme-Linked Immunosorbent Assay (HIV ELISA) was reactive. The third case was a patient with a neck abscess with preexisting diabetes mellitus (DM) underwent wound debridement and was given anti-tuberculosis drugs. Conclusion: TBLN with HIV co-infection or diabetes had clinical features ranging from a painful lump to an abscess. The definitive diagnosis was taken by examining AFB from pus. If the abscess was more extensive than or equal to 3 cm, wound debridement was necessary. The primary treatment for TBLN was anti-tuberculosis drugs and required even greater attention if a patient had any preexisting comorbidities such as HIV and diabetes.
