Abstract

Latar Belakang: Pasien gagal ginjal kronik (GGK) yang menjalani hemodialisis (HD) sering menghadapi tantangan yang dapat menurunkan kualitas hidup mereka. Kepatuhan terhadap pengobatan, termasuk terapi HD, merupakan faktor penting dalam meningkatkan kualitas hidup pasien. Tujuan: Penelitian ini bertujuan untuk menganalisis hubungan antara kepatuhan pengobatan antihipertensi dengan kualitas hidup (QoL) dan Quality Adjusted Life Years (QALY) pada pasien Penyakit Ginjal Kronik (PGK) dengan hipertensi yang menjalani hemodialisis di RSUD R.T Notopuro Sidoarjo. Metode: Data dikumpulkan secara prospektif melalui kuesioner, rekam medis, Sistim Informasi Manajemen (SIM) Apotek, dan SIM Rumah Sakit pada September-November 2024. Sampel terdiri dari 73 pasien yang dipilih dengan teknik purposive sampling. Kriteria inklusi meliputi pasien GGK berusia ≥18 tahun yang menjalani HD, mengisi kuesioner lengkap, dan mampu berkomunikasi dengan baik. Instrumen yang digunakan adalah kuesioner HillBone untuk mengukur kepatuhan obat dan European Quality of Life Five Dimension Five Level Scale (EQ5D5L) untuk menilai kualitas hidup. Analisis data dilakukan dengan uji korelasi Spearman Rho, Mann-Whitney, Chi-Square, uji t independen, dan One Way Anova. Hasil: Mayoritas responden adalah laki-laki (61,65%) dengan usia 51-60 tahun (45,21%). Sebanyak 70 responden (95,89%) tidak patuh minum obat antihipertensi, dengan nilai rata-rata kualitas hidup 0,752, QALY 25,48, dan VAS 71,3. Hasil menunjukkan hubungan signifikan antara kepatuhan pengobatan dengan QoL dan utilitas dengan QALY (P=0,002), serta antara kepatuhan dan QoL (P=0,033). Kesimpulan: Sebagian besar pasien hemodialisis di RSUD RT. Notopuro Sidoarjo tidak patuh terhadap pengobatan antihipertensi, dengan korelasi positif antara nilai utilitas (EQ5D5L) dan QALY. Kepatuhan pengobatan mempengaruhi kualitas hidup pasien.

Abstract

Aegle marmelos is a traditional plant with many therapeutic benefits; for example, it is used as an immunomodulatory agent. Literature studies reveal many functional and bioactive compounds, such as alkaloids, phenolics, coumarins, flavonoids, terpenoids, and other antioxidants. Based on its chemical content, Aegle marmelos could be an immunomodulatory agent because it can stimulate the production of antibodies against foreign antigens, increasing IgM and IgG. This study aims to determine the effect of Aegle marmelos filtrate on IgM and IgG titers in Rattus norvergicus induced (sheep red blood cells) SRBC 2%. In this study, 30 rats were divided into five groups: negative control received standard diets, positive control received SRBC 2% injection, P1 received SRBC 2% + Aegle marmelos filtrate 25%, P2 received SRBC 2% + Aegle marmelos filtrate 50%, and P3 received SRBC 2% + Aegle marmelos filtrate 75%. The results showed that administration of Aegle marmelos filtrate could increase IgM titers at the highest dose of 75%, although it was not statistically significant. Meanwhile, Aegle marmelos filtrate does not affect IgG titers in SRBC 2% compared to treatment groups.

Abstract

Latar Belakang: Menyelam SCUBA adalah rekreasi populer namun berisiko, salah satunya adalah Penyakit Dekompresi (DCS) yang disebabkan pembentukan gelembung gas inert akibat penurunan tekanan lingkungan. DCS dapat bermanifestasi ringan hingga berat, termasuk gejala neurologis. Penanganan cepat sangat penting. Tujuan: Melaporkan kasus Penyakit Dekompresi Tipe II pada penyelam rekreasi dan penanganannya dengan Terapi Oksigen Hiperbarik (TOHB). Presentasi Kasus: Dilaporkan kasus laki-laki 70 tahun dengan gejala pusing berputar, vertigo, gangguan keseimbangan, mual, muntah, parestesia, dan nyeri sendi setelah dua kali penyelaman. Pemeriksaan fisik menunjukkan Romberg test positif. Manajemen awal dengan oksigen normobarik dan obat simptomatik hanya memberikan sedikit perbaikan. Hasil: Pasien menjalani dua sesi TOHB (menggunakan Tabel 6 dan Tabel 5 US Navy). Setelah terapi, gejala pasien membaik signifikan, termasuk hilangnya pusing, mual, muntah, parestesia, dan nyeri sendi. Romberg test kembali negatif. Kesimpulan: Terapi Oksigen Hiperbarik adalah terapi standar emas yang efektif dalam meredakan gejala Penyakit Dekompresi Tipe II pada penyelam rekreasi.

Abstract

A foreign body, especially an organic type, causes eye injuries due to the infectious reactions it may provoke. Therefore, it is crucial to promptly identify and locate the foreign body to facilitate its removal and prevent undesirable complications. This case report highlights the importance of establishing a diagnosis and providing appropriate management for a case of conjunctival foreign body in the palpebral area. This case involved a 50-year-old male patient presenting with complaints of discomfort and pain in the left eye, particularly when closing the eye. The patient reported a history of being struck in the left eye by a rice stalk two days ago. The visual acuity of the right and left eyes was recorded as 6/6, with intraocular pressure of 18 mmHg using a Schiotz tonometer. A fragment of rice stalk was found embedded in the inferior palpebral conjunctiva. In the eyelid eversion examination, there was no foreign body. The foreign body was only visible when the patient gazed to the left while we pulled down the lower eyelid. In conclusion, in this case, foreign body extraction was performed, followed by topical antibiotic therapy and analgesia. The patient was advised to return for a follow-up in one month.

Abstract

Indonesia's innovation index is low, presenting a challenge for healthcare services like RSUD Sidoarjo Barat, which faces issues with staff problem-solving skills and innovation motivation, leading to a decline in innovation output. This community service activity aimed to accelerate problem-solving and foster innovation through the establishment and implementation of the Prima Innovation Incubator at RSUD Sidoarjo Barat (INSPIRA). The activity involved structured steps including problem identification, analysis, solution development, and INSPIRA implementation with training and a digital platform in selected units. Results showed a significant increase in staff understanding (pre-post test scores) and a substantial rise in the number of innovations generated (from 4 to 16, a 300% increase). The INSPIRA program successfully transformed staff mindset and empowered them to generate new solutions. In conclusion, INSPIRA is an effective breakthrough addressing innovation challenges, significantly increasing innovation capacity and contributing to service improvement

Abstract

Criminal acts often leave traces analyzed by law enforcement, especially by the Indonesian National Police (POLRI). Sometimes, various items were found and analyzed to link the victim and the perpetrator. Blood, the most abundant body fluid, found in crime scenes, consists of cellular and liquid elements. The forensic focuses on red blood, which can be wet or dried at the crime scene. Sometimes, there was no blood or bloodstains found, but objects related to other body fluids, such as: condoms, toothbrushes, the mouth of glass cups, straws, etc, could be found. Blood group analysis could still be carried out based on secretor gene (Se) considering that 80% of the population was of the secretor group. There are two chromosomes related to blood regulation, namely: chromosomes no. 9 and 19. The former relates to the ABO blood group and the latter relates to the secretory phenotype. The basic principle of secretors was antigens found on the surface of red blood cells and all body fluids, except cerebrospinal fluid. The pattern of inheritance on chromosomes 9 and 19 is based on the probability law by Mendell. It stated that all of genotypes (homozygous dominant-recessive or heterozygous) would be passed on to their offspring.

Abstract

The identification process at the crime scene (TKP) is one of the essential tasks carried out by police investigators. One critical aspect is the collection and analysis of evidence found at the crime scene, referred to as biological evidence (BB). This evidence serves as a crucial source for determining whether an incident occurred and, if so, whether it was a suicide or another criminal act (such as murder, etc.). The biological evidence found can be analyzed using molecular serobiology approaches, which are rapidly advancing in the fields of forensic medicine and medicolegal practice. One such approach involves utilizing DNA traces, although a common challenge is that often the DNA recovered at crime scenes is degraded. Therefore, a DNA amplification method utilizing the Polymerase Chain Reaction (PCR) technique, along with the short tandem repeat (STR) – CODIS identification approach, is necessary. The analysis of DNA traces using the STR-CODIS approach enables investigators to determine whether the incident was purely criminal, a suicide, or another type of accident involving the victim. Furthermore, the DNA analysis using the STR-CODIS method can also aid in identifying the victims discovered at the crime scene.
