Identification of patients at high risk for clinical deterioration is critical to ensure access to intensive care for severe conditions in a timely manner. According to (Hajjar et al., 2021), it is stated that in cases of severe respiratory failure, as often seen in ARDS associated with SARS-CoV-2, severe hypoxemia can cause a persistent increase in respiratory effort, with consequent induced lung injury. itself (P-SIL). In addition, other factors such as fluid overload or myocardial injury caused by SARS-CoV-2 can also play an important role in exacerbating the condition through pulmonary congestion. Critical care support optimized by facilitating Covid-19 patients to use ventilators is the best strategy to increase patient survival (Shang et al., 2020). Research Objectives: To determine the risk factors that influence the use of ventilators in COVID-19 patients in the ICU. Research Method: This research uses the literature review method using the Google Scholar, Pubmed, and ProQuest with a time limit for searching articles for 2019-2021, full free text in English and Indonesian and using predefined keywords. Results: The results of this literature review show that there are 3 risk factors that affect the use of ventilators in Covid-19 patients in the ICU, including sociodemographic factors (age, sex, BMI), comorbid factors (two or more co-morbidities), such as diabetes mellitus, hypertension, COPD, cardiovascular, cerebrovascular, and obesity, and lifestyle factors (smoking). Conclusion: The use of ventilators in Covid-19 patients is often used in developed countries as the best strategy to increase patient survival by taking into account the severity of the patient's condition, especially those with respiratory system disorders. In the use of the ventilator there are 3 main factors that must be considered, sociodemographic factors, comorbidity factors, examination results factors, and lifestyle factors.

Abstract

This study aims to identify what nurses experience in handling COVID-19. The research uses a literature study with PI(E)O (patients, intervention/exposure, and outcome), and searches were carried out on the Proquest and PubMed databases. Abstract

The COVID-19 pandemic had an impact on nursing students’ academic and clinical performance. Nursing institutions have provided remote learning to continue the learning process. However, students have met several obstacles that caused a decrease in the quality of learning. Remote learning for education requires the support of information and communication technology. However, studies related to how the COVID-19 pandemic affected nursing education in Indonesia are still rarely done. Accordingly, this study was conducted to evaluate the implementation of remote learning for nursing education during the COVID-19 pandemic. A cross-sectional study was conducted with 331 members of the Association of Indonesian Nurse Education Center (AINEC). The instrument used was the Guidelines of Filling Out Remote Learning. The results showed that 51% of the study programs implemented remote learning, while 69% of the regulations for remote learning were created by rectors. The most common teaching materials were visual, text, and audio-visual content. These materials were distributed through WhatsApp Group, Learning Management System (LMS), and video conference. To support the process of remote learning, nursing institutions need to improve information technology staff and cooperate agreement with other institutions. Overall, the remote learning process needs to be further improved to increase the students’ learning quality.

Abstract

Background: Patients need help from others to meet their needs during hospitalization in the Intensive Care Unit (ICU). The role of the family is significant in the care of critical patients in the ICU because it helps fulfill the satisfaction and providing supportive care of patients related to their intimacy.Objectives: This study aims to explore the role of families in the hospitalization of critical patients in the ICU.Method: This study used a qualitative method with a phenomenological approach. Data retrieval is done through in-depth interviews, focused group discussions, and observations. The participant recruitment technique uses pur-posive sampling. The research participants were 24 people and were following with the inclusion criteria. Source triangulation is done to find out the validity of research data and the roles performed by the family. This qualitative data was analyzed using N-Vivo 12 plus methods.Results: There are five themes accompanied by 12 sub-themes in them, including developing emotional relationships with patients, providing physical support to patients, becoming information partners with health personnel, fami-lies, and patients, supporting patient facilities and infra-structure, families meeting patients' spiritual needs.Conclusion: Family members are involved in patient care because families are given the opportunity and sufficient time to visit patients. Suggestions for further research can consider the family background in the involvement of roles and factors that influence family roles during hospitalization of patients in the ICU.

Abstract

Chronic Renal Failure (CRF) is a slow, progressive and irreversible decline in renal function can lead to the inability of the kidneys to dispose of residual waste products and unable to maintain fluid and electrolyte balance. Hemodialysis treatment measures performed on CRF patients in order to survive. Fluid monitoring in CRF patients using telemonitoring (remote monitoring equipment), the application contains fluid control and diet to facilitate patients at home. To find out the nutritional monitoring of Chronic Renal Failure patients using the electronic application of bartocar (nutritional fluid monitoring sheet). In this literature review were use 3 data bases namely proquest, pubmed and google scholar, he last 5 years from 2016-2020. Inclusion criteria: Quantitative, qualitative and mix method research design, have communication tools such as mobile phones/computers, Based via online (smartphones and websites), Chronic renal failure undergoing hemodialysis, Free, the last 5 years 2016-2020. Exclusion criteria: Paid journals, Patients undergoing hemodialysis. From the results of the journal search obtained 35,944 English journals discussed only 10 journals discussed different topics. Electronic bartocar (fluid monitoring sheet) in patients very helpful to facilitate diet, restriction of incoming and outgoing fluids and nutrients. This mHealth app is designed to help with early diagnosis of CRF and self-monitoring. This app can be accessed via online, android 4.0/ios mobile web application and website app. Bartocar serves to help monitoring fluids and nutrients in chronic renal failure patients at home.

Abstract

Latar Belakang: ICU adalah bagian khusus di rumah sakit yang menyediakan perawatan intensif untuk pasien yang menghadapi trauma yang mengancam jiwa, termasuk cidera kepala atau trauma capitis. Cidera kepala bisa berakibat gangguan neurologis atau bahkan kematian. Di Indonesia, tingkat cidera kepala tertinggi terdapat di Provinsi Gorontalo (17,9%), terutama akibat kecelakaan. Sulawesi Utara memiliki angka kejadian cidera kepala paling tinggi karena kecelakaan (3,5%), sebagian besar terjadi di jalan raya. Posisi kepala 30 derajat digunakan untuk mengurangi tekanan di kepala pasien dan meningkatkan oksigenasi otak.Tujuan: Untuk mengetahui efektifitas posisi kepala 300 pada pasien cidera kepala di ruang ICU RSUD Kabupaten Temanggung.Metode: Metode penelitian ini merupakan penelitian diskriptif yang menggunakan  studi kasus. Studi kasus dilakukan pada pasien yang berada di ruang ICU Rumah Sakit Umum Daerah Kabupaten Temanggung. Penelitian ini dilakukan dengan memberikan asuhan keperawatan pada pasien yang di rawat di ruang ICU dengan memberikan posisi head up 30 derajad.Hasil: Studi kasus menunjukan bahwa oksigen pada pasien baik dan menunjukan hasil spo2 di jam 09.00 100%, pukul 10.00 spo 98 %, kemudian tekanan darah pasien berangsur normal dari 142/ 95 kemudian 127/67 mmhg.Kesimpulan Dari studi kasus disimpulkan bahwa penerapan posisi kepala 300 dapat meningkatkan oksigen ke jaringan otak dan tanda tanda vital pasen berangsur membaik Effectiveness Of Giving Head Up 300 Position In Head Injury Patients' OxygenationBackground: The ICU is a specialised part of the hospital that provides intensive care for patients who have suffered life-threatening trauma, including head injury or trauma capitis. Head injuries can lead to neurological disorders or even death. In Indonesia, the province of Gorontalo has the highest head injury rate (17.9%), mainly due to accidents. North Sulawesi has the highest incidence of head injuries due to accidents (3.5%), mostly on highways. A 30-degree head position is used to reduce pressure on the patient's head and improve oxygenation of the brain.Objective: To determine the effectiveness of 300 head positions in patients with head injury in the intensive care unit of Temanggung Regency Hospital.Methods: This research method is descriptive research using case studies. Case studies were conducted on patients who were in the ICU room of Temanggung Regency Regional General Hospital. This research was conducted by providing nursing care to patients who were treated in the ICU room by providing a 30 degree head up position.Results: The case study showed that the patient's oxygenation was good and showed spo2 results at 09.00 100%, at 10.00 spo 98%, then the patient's blood pressure gradually normalised from 142/ 95 then 127/67 mmhg.Conclusion : From the case study, it was concluded that the application of 300 head position can increase the oxygen to the brain tissue and the patient's vital signs gradually improved.

Abstract

Latar Belakang:Stroke adalah kondisi dimana sebagaian sel otak mengalami kematian karena adanya gangguan aliran darah yang berupa sumbatan atau pecahnya pembuluh darah di otak sehingga menjadi pemicu terjadinya stroke non hemoragik sehingga fungsi syaraf menurun, aliran darah menuju otak tidak adekuat dan transportasi oksigen di otak tidak maksimal yang mengakibatkan gangguan hemodinamik termasuk saturasi oksigen. Pemantauan saturasi oksigen penting dipantau karena menentukan keadekuatan oksigenasi atau perfusi jaringan.Tujuan: memberikan analisis pelaksanaan asuhan keperawatan masalah gangguan pertukaran gas pada pasien stroke non hemoragik.Metode:Penelitian ini menggunakan metode studi kasus menggunakan instrument pengukuran saturasi oksigen sebelum dan sesudah intervensi. Salah satu intervensi keperawatan untuk mengatasi saturasi oksigen adalah dengan pemberian elevasi kepala 30°. Pemberian elevasi kepala 30° dilakukan selama 3 hari.Hasil: Berdasarkan hasil penelitian yang didapatkan menunjukkan bahwa klien mengalami peningkatan pada jumlah saturasi oksigen sebelumnya 89% menjadi 96%.Kesimpulan: darihasil penelitian dibuktikan buktikan bahwa intervensi pemberian elevasi kepala 30° selama 3 hari mampu meningkatkan saturasi oksigen pada pasien stroke non hemoragik.The Effects of 30° Head Elevation on Increasing Oxygen Saturation in Patients with Non-Hemorrhagic StrokeBackground: Stroke is a condition where part of the brain cells die due to blood flow disorders in the form of blockage or rupture of blood vessels in the brain so that it becomes a trigger for non-hemorrhagic stroke so that nerve function decreases, blood flow to the brain is inadequate and oxygen transportation in the brain is not optimal which results in hemodynamic disturbances including oxygen saturation. Monitoring oxygen saturation is important to monitor because it determines the strength of oxygenation or tissue perfusion.Objective: to provide an analysis of the implementation of nursing care for gas exchange disorders in non-hemorrhagic stroke patients.Methods: This study uses a case study method using oxygen saturation measurement instruments before and after the intervention. One of the nursing interventions to overcome oxygen saturation is to provide 30 ° head elevation. Giving 30 ° head elevation is done for 3 days.Results: Based on the results of the research obtained, it shows that the client experienced an increase in the amount of oxygen saturation from 89% to 96%.Conclusion: the results of the study proved that the intervention of giving 30 ° head elevation for 3 days was able to increase oxygen saturation in non-hemorrhagic stroke patients

Abstract

Discharge planning is a systematic and methodical process. Family readiness is crucial and entails preparation in caregiving, educational provision, delineation of roles within the home, and the anticipation of obligations prior to the patient's departure. Video serves as a medium via which families can make informed decisions, enhance abilities, adjust to new circumstances, and refine cognitive patterns. Object. To assess the impact of video-based discharge planning on fulfilling daily demands and joint range of motion exercises on familial preparedness to care for stroke patients. Methods: This study employed a quantitative methodology with a pretest-posttest control group design. This study utilized a sample of 60 family members who will subsequently provide home care for stroke patients, employing a complete sampling technique. The subjects of this study were family members of stroke patients treated at PKU Muhammadiyah Gamping Hospital. The utilized instrument was a knowledge and skills questionnaire modified from prior research employing bivariate analysis methodologies. Results: The Mann Whitney U Test yielded a p-value of 0.001, which is less than α (0.05), indicating significant differences in preparedness outcomes between the intervention and control groups. In conclusion, video-based discharge planning for daily activity needs and joint range of motion exercises is deemed effective in enhancing family preparedness to care for stroke patients. p value (0.001) < α (0.05) means that there are differences in readiness results in the intervention group and control group. Video-based discharge planning on fulfilling daily activity needs and joint range of motion exercises is considered capable of increasing family readiness to care for stroke patients.

Abstract

Background: The accumulation of toxins and fluid in the body due to decreased kidney function can lead to various potentially life-threatening systemic complications. Adherence to strict fluid restrictions is crucial to prevent intravascular fluid overload, which can increase the risk of edema, hypertension, and cardiovascular disease. Family support is an important social factor influencing health behaviors in patients with chronic kidney disease (CKD). Purpose: To describe in depth the clinical phenomena, patient responses, and the role of family support in improving adherence to hemodialysis therapy in patients with chronic kidney disease (CKD). Method: This case study was conducted in a hemodialysis unit at a healthcare facility on March 11, 2025. The subject was a 65-year-old female patient diagnosed with Chronic Kidney Disease (CKD) undergoing twice-weekly hemodialysis. Results: Two ethical dilemmas were identified in the case: autonomy and beneficence. Autonomy allows patients to make informed choices regarding their treatment options. Beneficence, in the form of healthcare professionals' obligation to provide the best possible care for the patient's well-being. Conclusion: Family support has been shown to be crucial in maintaining motivation, assisting decision-making, and improving adherence to therapy.   Keywords: Chronic Kidney Disease; Case Study; Family Support.   Pendahuluan: Akumulasi toksin dan cairan dalam tubuh akibat penurunan fungsi ginjal ini dapat menimbulkan berbagai komplikasi sistemik yang berpotensi mengancam jiwa. Kepatuhan terhadap pembatasan cairan yang ketat untuk mencegah kelebihan cairan intravaskular yang dapat meningkatkan risiko edema, hipertensi, dan penyakit kardiovaskular. Dukungan keluarga (family support) merupakan faktor sosial penting yang memengaruhi perilaku kesehatan pasien kronis.  akut Tujuan: Untuk menggambarkan secara mendalam fenomena klinis, response pasien, serta peran dukungan keluarga yang dapat diberikan dalam kepatuhan menjalani terapi hemodialisis pada pasien dengan Chronic Kidney Disease (CKD). Metode: Penelitian dengan desain studi kasus (case report) yang dilakukan di unit hemodialisis salah satu fasilitas pelayanan kesehatan pada tanggal 11 Maret 2025. Subjek penelitian adalah seorang pasien perempuan berusia 65 tahun dengan diagnosis Chronic Kidney Disease (CKD) yang menjalani terapi hemodialisis dua kali seminggu. Hasil: Terdapat dua Dilema Etik pada kasus yaitu Autonomy dan Beneficence. Autonomy pada pasien berhak menentukan pilihan terhadap pengobatan yang dijalaninya. Kemudian Beneficence berupa tenaga kesehatan memiliki kewajiban untuk memberikan perawatan terbaik demi kebaikan pasien. Simpulan: Dukungan keluarga terbukti sangat penting dalam menjaga motivasi, membantu pengambilan keputusan, serta meningkatkan kepatuhan terhadap terapi.   Kata Kunci: Chronic Kidney Disease; Dukungan Keluarga; Studi Kasus.

Abstract

Background: Diet is a key aspect of diabetes mellitus management, as it aims to achieve and maintain near-normal blood glucose and lipid levels, prevent acute and chronic complications, and improve the patient's quality of life. Purpose: To determine the strategies for implementing diabetes mellitus diet education as an application of the ethical principle of beneficence in nursing. Method: The research design used a literature review using Google Scholar as the e-resource data source, using the keywords "Diet Education: Amount, Type, and Schedule (3J)" and "DASH Diet Education." Results: The literature search found that 3J and DASH diet education can be used as strategies for implementing the ethical principle of beneficence. Diet is a key aspect of diabetes mellitus management. Simpulan: Nurses can implement the ethical principle of beneficence by providing 3J and DASH diet education to maintain and sustain blood glucose levels within the normal range, maintain stable blood pressure, and support a more stable health condition.   Keywords: Diabetes Mellitus; Diet Education; Nursing Ethical Principles.   Pendahuluan: Pola makan atau diet menjadi aspek utama dalam penatalaksanaan diabetes melitus, karena bertujuan untuk mencapai dan mempertahankan kadar glukosa serta lemak darah mendekati normal, mencegah timbulnya komplikasi baik akut maupun kronis, dan meningkatkan kualitas hidup penderita. Tujuan: Untuk mengetahui jenis strategi pelaksanaan edukasi diet diabetes melitus sebagai penerapan prinsip etik beneficence dalam keperawatan. Metode: Desain penelitian literature review dengan sumber data e-resource Google Scholar menggunakan kata kunci “Edukasi Diet Jumlah, jenis, dan Jadwal (3J)” dan “Edukasi Diet DASH”. Hasil: Pencarian literatur didapatkan bahwa edukasi diet 3J dan diet DASH dapat digunakan sebagai strategi pelaksanaan prinsip etik beneficence. Pola makan atau diet menjadi aspek utama dalam penatalaksanaan diabetes melitus. Simpulan: Perawat dapat menjalankan prinsip etik beneficence dengan memberikan edukasi diet 3J dan diet DASH dengan untuk menjaga dan mempertahankan kadar glukosa darah dalam rentang normal, menjaga kestabilan tekanan darah, dan mendukung kondisi kesehatan yang lebih stabil.   Kata Kunci: Edukasi Diet; Diabetes Melitus; Prinsip Etik Keperawatan.
