Abstract

One of the efforts to provide labor protection for Occupational Safety and Health (K3) in hospitals is by providing Personal Protective Equipment. The purpose of this study is to analyze the factors that affect the use of PPE in Nurses in Bina Kasih Hospital Medan. This type of research includes quantitative research with a cross sectional approach. The population in this study was 152 nurses with a sample of 60 people based on the results of sample calculation using the Slovin formula. Data collection in the study used questionnaires and processed into SPSS with univariate, bivariate and multivariate analysis. The results of the study with the chi square statistical test found that the variables of age, working period, knowledge, attitude, availability of PPE, supervision and social support were related to the p<sig value of α (0.05). Based on the results of the multivariate analysis, it is known that the most dominant variable influences, namely, the attitude variable (p=0.003, Exp(B) 14.931), namely attitude has an influence of 14.9 times on the use of PPE, the working period (p=0.004, Exp(B) 13.169), namely the working period has an influence of 13 times on the use of PPE, and social support (p=0.012 Exp(B) 8.592), namely social support has an influence of 8.5 times on the use of PPE. It can be concluded that the variables of working period, attitude and social support are the factors that most affect nurses in using PPE. It is recommended to Ananda Bekasi Hospital, especially to nurses, to always improve the performance of nursing care provided to patients by paying attention to the completeness of PPE in accordance with the SOPs that have been set by the work agency.

Abstract

The Social Security Administration Agency or BPJS Kesehatan recorded a total of 267.31 million participants as of December 31, 2023. If they need follow-up services by a specialist doctor, BPJS Kesehatan participants can be referred to second-level health facilities or secondary-level health facilities. The causes of the high number of referrals at the Teladan Health Center are due to: inadequate availability of doctors, delays in the availability of drugs and frequent vacancies in drug stocks, lack of availability of medical facilities and medical equipment and the implementation of the FKTP function as a gatekeeper does not follow the established rules. This research is a qualitative descriptive research, which is a research method that is carried out with the main purpose of making an overview or description of a situation objectively, with a semi-structured interview method, which is a type of interview that includes the category of in-depth interviews recorded using a tape recorder where in its implementation it is freer when compared to structured interviews. The results of the study obtained from this study are that there are still many referrals on their own request because patients do not trust the health center, resulting in a high number of referrals. The number of health workers at the Teladan Health Center has met the standards, but neurologists and cardiologists are needed because many of the patients referred have internal diseases. Medicines, medical devices, medical device facilities and infrastructure facilities at the Teladan Health Center are incomplete and there is a lack of public knowledge about the gatekeeper concept so that there are frequent APS referrals. It is hoped that the Exemplary Health Center to the Head of the Health Center will propose the addition of Specialist Doctors to the Medan City Health Office, re-evaluate drug planning, provide socialization to the public about JKN, especially the tiered referral system and about the gatekeeper concept. The Medan City Health Office is also expected to collaborate with health centers in completing medical equipment and medicine facilities.

Abstract

The high prevalence of cancer in Indonesia needs to be observed with preventive measures and early detection through Visual Examination of Acetic Acid/IVA test. The low interest of women of childbearing age to do IVA Screening is one of the health problems. Many media have been used by the government and researchers before to socialize cervical cancer prevention to the public. In the digital era, the internet is one of the dominant information media, the most accessed is WhatsApp (95.9%). Researchers use the Whatsapp autoreply feature to provide information about Ca Cervix which can be accessed 24/7. The use of this technology is expected to be useful so that users are motivated to conduct IVA screening. The study used a quasi experiment with a pretest-posttest control group design model. The data were analyzed using the non-parametric statistical test Wilcoxon Signed Rank Test to compare the differences between the pre-test and the post-test and the Mann Whitney U Test to compare the differences between the control group and the experimental group. In the control group, the results of the wilcoxon test pre-test post-test knowledge context showed a-Symp 0.227, at the input information a-Symp 0.916, in the process of providing information a-Symp 0.083 and at Interest a-Symp 0.317, the value was >0.05 Ho was rejected, there was no difference in the pre-test post-test of the control group. In the experimental group, the results of the wilcoxon test pre-test post-test in the context of knowledge, showed a-Symp 0.000, at the input of a-Symp information 0.000, in the process of providing information a-Symp 0.000 and in the interest of a-Symp 0.000, the value <0.05 Ho was accepted, there was a difference in the pre-test post-test of the experimental group. The results of the Mann Whitney U Test showed that A-Symp was 0.000< 0.05 There was a difference in the control group and there was an increase in the experimental group that was given the treatment. The interest of women of childbearing age to do IVA screening can increase after being given information about Ca Cerviks and IVA Test through whatsapp autoreply.

Abstract

Improving the performance of hospital honorary staff is one of the most important things to maintain the quality of health services provided to patients. Many hospitals face obstacles in improving employee performance, such as high attendance rates, low productivity, and unsatisfactory service quality. In general, the decline in work morale and seriousness of work is due to the dissatisfaction of the workers concerned, both materially and non-materially. The purpose of this study is to evaluate the implementation of salary payments for honorary personnel in Aceh Singkil Regency which has an impact on the performance of honorary personnel at BLUD Aceh Singkil Hospital. This study uses a descriptive qualitative approach, to describe the phenomena that occur in the object of research. Data collection techniques are carried out by observation, interviews, and documentation. Data analysis through data collection, data condensation, data presentation, conclusion drawn. This study also used 10 informants consisting of 1 key informant, 3 main informants, and 6 supporting informants. Based on the results of observations, interviews and documentation of the Performance of BLUD Honorary Personnel of Aceh Singkil Hospital is not in accordance with the Optimization of the Implementation of SOPs for Inpatient and Outpatient Services has not been carried out properly, it is still found that honorary personnel have not complied in carrying out hand hygiene and the use of personal protective equipment. As well as the service waiting time which is relatively slow. The quality of service is considered poor. The work motivation of honorary staff is still relatively low and lack of discipline in work In addition, the Patient Satisfaction Level is still low because the service has not been in accordance with their expectations. This also has an impact on the Implementation of Salary Payments for Honorary Personnel of the Aceh Singkil Hospital against the decline in performance which results in less than optimal services at the BLUD Aceh Singkil Hospital. Advice to the management to be able to pay attention to honorary personnel, especially in the implementation of salaries to provide motivation or work spirit to improve performance at BLUD Aceh Singkil Hospital.

Abstract

The cumulative number of ODHIV (People with HIV) in Indonesia based on SIHA (HIV AIDS Information System) data reported as of March 2022 is 329,581 people. At the Teladan Medan Health Center, the number of HIV cases has increased every year. Where from 2019 to 2021 HIV cases at the Teladan Health Center reached 600 people who had routinely received ARV therapy and in 2023 there was an increase of 290 people to 890 people who had routinely received ARV therapy. Sexually transmitted infections, family history of diseases, socioeconomic circumstances, blood transfusions, homosexuality, heterosexuality are risk factors that affect the incidence of HIV. The researcher's goal is to find out the risk factors that affect HIV cases at the 2023 Medan City Health Center. Methods: Using observational analytical methods using a Case Control research design. The sample amounted to 148 respondents, 74 case groups and 74 control groups were taken by consecutive sampling. Data analysis was carried out univariate, bivariate and multivariate with logistic regression tests. The results of the study on the analysis of the bivariate test with chi-square showed factors that affected the incidence of HIV, namely the history of sexually transmitted infections p = 0.034, family disease history p = 0.021, socioeconomic status p = 0.045, drug p = 0.042, blood transfusion p = 0.024, homosexual p = 0.014, and heterosexual p = 0.029 against the incidence of HIV at the 2023 Medan City Teladan Health Center. Meanwhile, the multivariate test found that the most influential factor with the incidence of HIV was more than one sex habit variable (p= 0.001; Exp(B) 3,920). Conclusion The dominant risk factor affecting the incidence of HIV cases at the Medan City Health Center is the habit of having more than one. suggestions to improve promotive preventive services about the dangers of HIV, the factors that cause HIV, and create a culture of loyalty to couples and also demonstrate the issue of the dangers of HIV both among the community and students.

Abstract

Hypertension is one of the non-communicable diseases that is a very serious health problem today. Uncontrolled hypertension can lead to a 7 times greater chance of having a stroke, 6 times greater chance of developing congestive heart disease, and 3 times greater developing a heart attack. At the Teladan Health Center, there are still 36% of hypertension patients who have not been controlled, where data on the number of hypertension patients in 2021 is 842 people and in 2022 as many as 1162 people. Some of them are factors that affect non-adherence to taking medication, namely understanding of instructions, level of education, treatment, beliefs, attitudes, and personality as well as family support, The purpose of this study is to determine the influence of health promotion on knowledge, attitudes and actions with adherence to taking hypertension medication. This study uses a combination of quantitative and qualitative methods (mixed method) with a sequential explanatory approach  is a combined research method that uses data collection and quantitative analysis in the first stage and followed by qualitative data collection and analysis in the second stage, in order to strengthen the results of quantitative research conducted in the first stage. The research was conducted by 51 people in the experimental group and 51 people in the control group, in the qualitative research of informants as many as 8 people where 3 people were the main informants, 3 people were supporting informants, and 2 people were key informants, in quantitative research it was carried out by univariate analysis and bivariate analysis of the test which was carried out in the quantitative research of independent samples test T-Test, while in qualitative research data processing was carried out by means of data reduction,  Data presentation, and drawing conclusions The test used in this study is a data credibility test, which is carried out using source triangulation techniques, discussions with peers and membercheck. Source triangulation is a technique for collecting information from various sources. The results of the study showed that there was an influence of health promotion on knowledge, attitudes, and actions with medication adherence. In groups Control also there is an average change for theHUAN of 0.45 with a p-value of 0.004. Mean value of attitude on results Post Test The experimental group experienced an increase with an average change of 2.78. In the control group, there was also an average difference of 0.33 and a sig. p-value 0.010 < 0.05 . Mean value of action on results Pre Test The experimental group also experienced an increase with an average change of 3.04.  For actions in the experimental group, the SIG value was shown. p-value 0.000 < 0.05. In the control group the mean value for the action at the time Pre Test also experienced an increase of 0.75 but with a p-value of 0.000 < 0.05 means that there is a significant difference after health promotion.and there is a reinforcing factor, namely family support, support of health workers which can increase compliance with taking hypertension medication. The conclusion of this study is that there is an increase in adherence to taking hypertension medication after health promotion and there are driving factors that affect adherence to taking hypertension medication, namely the support from family and support from health workers, in addition to the factor of affordability of access to health services. Using the flip sheet media as a promotional medium about medication adherence and improving health services, especially for hypertension patients who are generally in the elderly.

Abstract

Posyandu (Integrated Service Post) for the Elderly is a community resource health service (UKBM) forum to serve the elderly population, whose formation and implementation process is carried out by the community together with non-governmental organizations (NGOs), across government and non-government sectors, the private sector, social organizations and others, with an emphasis on health services on promotive and preventive efforts. The purpose of the study is to find out the factors that affect the utilization of elderly posyandu in the Working Area of the Glugur Darat Health Center, East Medan District, Medan City. The research design uses the Analytical Survey method with a Cross Sectional approach. The population in this study is all elderly as many as 11,050 people. The sample was 99 with stratified random sampling. Data analysis uses univariate, bivariate and multivariate analysis. The results of logistic regression showed that there was an influence of knowledge p = 0.008, perception p = 0.006, distance p = 0.003 and family support p = 0.002, while what did not have an influence was the attitude and support of health workers towards the utilization of posyandu for the elderly in the Working Area of Glugur Darat Health Center, East Medan District, Medan City, the dominant factor was family support p = 0.000 Exp (B) 41,303 with 95% CI = 5,389-316,539. The conclusion of this study is that there is an influence of knowledge, perception, distance and family support on the utilization of elderly posyandu in the Working Area of the Glugur Darat Health Center, East Medan District, Medan City. It is recommended to the Glugur Darat Health Center to further increase its participation in providing training to health workers and be able to promote the use of elderly posbindu and it is better for Door To Door health workers to provide counseling on the use of elderly posbindu.

Abstract

Komplikasi kehamilan dapat terjadi di tahapan manapun, mulai dari fertilisasi hingga kelahiran. Diagnosis awal faktor risiko untuk komplikasi atau awal serangan komplikasi akan mengarah pada awal pengobatan dan mencegah bahaya pada ibu ataupun janin. Desain penelitian yang digunakan dalam penelitian ini adalah kuantitatif dengan rancangan cross sectional. Populasi penelitian adalah 460 orang ibu hamil. Jumlah sampel yang akan diteliti adalah 82 orang. Analisa data dilakukan dengan analisis univariat, bivariat dan multivariat. Uji statistik yang digunakan adalah uji chi square dan regresi regresi linear berganda. Hasil penelitian uji chi square menunjukkan bahwa ada perbedaan pengaruh pengetahuan, ekonomi, budaya, informasi, dukungan keluarga, dukungan tenaga kesehatan dan sarana prasarana terhadap Perilaku Ibu Hamil pada kelompok intervensi dan kelompok kontrol Di Puskesmas Tarempa Kecamatan Siantan Kabupaten Kepulauan Anambas dengan p-value 0.025<0.05; 0.034<0.05; 0.027<0.05; 0.002<0.05; 0.014<0.05 dan 0.047<0.05. Hasil analisis multivariat diperoleh bahwa variabel faktor dukungan keluarga merupakan variabel yang paling besar / dominan pengaruhnya terhadap perilaku ibu hamil pada kelompok intervensi dan kelompok kontrol di Puskesmas Tarempa Kecamatan Siantan Kabupaten Kepulauan Anambas. Kesimpulan ada pengaruh promosi kesehatan menggunakan media booklet tentang tanda bahaya kehamilan terhadap perilaku ibu hamil di Puskesmas Tarempa Kecamatan Siantan Kabupaten Kepulauan Anambas, khususnya bagi ibu hamil agar meningkatkan pengetahuan dan kepatuhan ibu hamil untuk mencegah terjadinya resiko-resiko selama kehamilan dan mencegah terjadinya kehamilan ibu hamil.

Abstract

Nutrition is a contribution to a person's health and development. Good nutrition can build a strong immune system, prevent disease, and improve health. The purpose of the study was to determine the effect of health education on increasing the consumption of nutritious food. This type of research is research quasi-experimental using the pretest-posttest design method, namely by providing a pretest (initial observation) before the intervention is carried out, after the intervention is given, then a posttest (final observation) is carried out. Health education in the form of balanced nutrition animation videos, balanced nutrition PPTs and food models. The statistical test used was Paired test T-Test. The results of the analysis of the pair t-test pre-test and post- test knowledge obtained a p value = 0.000, so it can be concluded that there is an effect of health education on increasing knowledge of nutritious food consumption. The results of the analysis of the pair t-test pre-test and post- test attitude obtained a p value = 0.000, which means is an effect of health education on increasing attitudes of nutritious food consumption. The results of the analysis of the pair t-test pre-test and post- test Action obtained a p value = 0.159, which means so it can be concluded that there is no effect of health education on increasing actions to consume nutritious food. This study is that there is an influence of health education on increasing knowledge and attitudes towards consuming nutritious food at SMP Santo Ignasius Johor. Video media can be recommended as an effective communication media because it can contain a lot of information about educational materials

Abstract

A research investigation evaluates public health strategies along with their ability to stop infectious diseases from spreading in rural populations. This study measured disease incidence through a quantitative research design focused on evaluating how health education combined with vaccination coverage alongside sanitation access and vector control methods affect disease transmission. The researchers collected their data through surveys which they analyzed by describing it statistically along with applying inference through regression analysis. Health education showed clear effectiveness in cutting down the occurrence of respiratory infections together with diarrheal diseases and vector-borne diseases. The level of vaccination preparedness demonstrated a powerful inverse link to the occurrence rates of disease prevention cases along with malaria and additional vector spread illnesses. The study confirmed access to sanitation facilities as the main factor for cutting down waterborne disease occurrences but also proved how vector control strategies like insecticide-treated nets played an essential role in fighting malaria and dengue infections. Empirical evidence demonstrates that rural areas benefit from combined public health interventions because this research fills gaps in scientific knowledge about these strategies in rural regions. The research demonstrates why rural communities should use multiple integrated strategies to combat their heavy infectious disease load. Research conclusions demonstrate an acute need for policymakers with public health practitioners to implement these interventions which will lead to better health results in underserved populations.
