Abstract

Background & Objective: This study is to determine the nursing care of Congestive Heart Failure (CHF) patients with nursing problems of ineffective breathing patterns with nursing interventions for head up 45o at Jenderal Ahmad Yani Metro Hospital. Method: Data collection using surgical nursing care assessment sheets and oxygen saturation observation sheets. The study used Congestive Heart Failure (CHF) patients. Result: The results of nursing care obtained the results of evaluations that researchers conducted on clients based on the criteria that researchers compiled against 3 diagnoses, the resolved diagnoses were ineffective breathing patterns, decreased cardiac output and activity intolerance. Diagnoses of ineffective breathing patterns experienced an increase in oxygen saturation which was initially 89% after being given a 45o head up position to 99%. Conclusion: Suggestions for patients and families can apply the 45o head up position when experiencing shortness of breath at home and remain compliant with taking medication.

Abstract

Background & Objective: The purpose of this study was to determine the effect of deep breath relaxation and lavender aromatherapy on reducing the pain scale of post-prostatectomy patients. Method: Descriptive type of research with a case study approach in multiple cases. Data collection methods using surgical nursing care assessment sheets and pain scale observation sheets using the Numeric Rating Scale. The study used 2 patients with the same criteria. Result: The results of this study are proven effectiveness in reducing pain intensity in both patients. There is a decrease between before and after being given deep breath relaxation techniques and lavender aromatherapy for 3 days, as evidenced by using the Number Rating Scale (NRS) score after being given deep breath relaxation techniques and lavender aromatherapy for 15 minutes for 3 days, the results showed a change in the pain scale in patient 1 and patient 2, with patient 1's pain scale from 6 to 3 and patient 2 from 5 to 2. Conclusion: Researchers advise nurses in overcoming pain with deep breath relaxation techniques and lavender aromatherapy as non-pharmacological therapy.

Abstract

Background & Objective: A head injury is a traumatic disruption of brain function with or without interstitial hemorrhage in the brain substance without a break in brain continuity. The purpose of this study was to determine Nursing Care for Traumatic Brain Injury Patients with Decreased Intracranial Adaptive Capacity Using the 30⁰ Head Up Position Intervention. Method: The method in this scientific work with a descriptive approach to nursing care involving 2 head injury patients, the instrument used is an assessment sheet, the application of the 30⁰ Head Up position is carried out for 4 days in patients 1 and 2, with the intensity of giving the 30⁰ Head Up position 3 times a day for 30 minutes. Result: The results of this study found that the main problem that arose in both patients was a decrease in intracranial adaptive capacity, the intervention that was arranged was intracranial management combined with giving the Head Up 30⁰ position, implementation was carried out for 4 days, nursing evaluation of intracranial decline improved to 2 in the first patient and day 3 in the second patient. Conclusion: The recommendation that the author can give is that the application of the Head Up 30⁰ position can restore intracranial decline quickly.

Abstract

Background & Objective: The purpose of this study was to determine the effectiveness of destructive phase II decubitus wound care using honey at Jendral Ahmad Yani Metro Hospital. Method: The method in this scientific work is a descriptive approach to nursing care involving 2 decubitus patients, the instruments used are assessment sheets, head to toe¸ and wound observation sheets, the application of wound care using honey is carried out for 4 days in patient 1 and 5 days in patient 2, with the intensity of giving honey once a day every day. Result: The results of this study found that the main problem that arose in both patients was damage to tissue integrity, the intervention was arranged by performing wound care using pure honey, the implementation was carried out for 4 days and 5 days in each patient, the wound nursing evaluation began to improve on day 3 of wound care using honey. Conclusion: The recommendation that the author can give is that honey can cure and accelerate wound healing, so it can be applied to other types of wounds not only decubitus wounds.

Abstract

Background & Objective: Diabetes is one of the problems caused by chronic metabolic disorders characterized by high sugar levels accompanied by metabolic disorders of carbohydrates, lipids, and proteins as a result of insufficient insulin function. The purpose of this study was to determine the effect of moist wound healing on post debridement diabetic ulcers. Descriptive research with a case study approach in multiple cases. Method: Data collection methods used surgical nursing care assessment sheets and Bates-Jensen Wound Assessment Tools (BWAT) wound assessment sheets. The study used 2 patients with the same criteria Result: The results of nursing care obtained in patient 1 are tissue integrity disorders, instability of blood glucose levels, and knowledge deficits. Patient 2 had problems with tissue integrity disorders, physical mobility disorders, and knowledge deficits. After nursing care was carried out, it was found that in patient 1 there was a decrease in wound score from 40 to 37 after moist wound healing treatment. Instability of blood sugar levels has not been resolved, patients get insulin therapy. The knowledge deficit was resolved on the second day. In patient 2, there was a decrease in wound score from 37 to 35 after moist wound healing treatment. Physical mobility disorders have not been resolved and the knowledge deficit was resolved on the second day. Conclusion: Suggestions for patients and families can use the booklet media made by researchers as a guide for wound care, wound observation, diabetic diets, and drug compliance.

Abstract

Background & Objective: Acute coronary syndrome (SCA) is a set of symptoms that occur due to the accumulation of atherosclerotic plaque in the walls of blood vessels, which is part of Coronary Heart Disease (CHD). The purpose of this study was to determine the description of nursing care with pressure dressing techniques in patients with nursing problems of Post PCI bleeding risk. Method: This type of research is descriptive research using the case study method. The action taken is the application of the pressure dressing technique using a mechanical pressing device on the puncture area where the catheter is inserted after the PCI procedure. The monitoring process was carried out for 2 x 24 hours after the application of the technique to assess the effectiveness of the intervention and monitor signs of bleeding. Result: After 2 x 24 hours of treatment, there were no signs of bleeding, the acral was warm and pain at the puncture area was reduced. Conclusion: Answering the research objectives. Should The risk of bleeding can be well managed through treatment with the pressure dressing technique.

Abstract

Background & Objectives: Management of acute pain in post-operative PCNL (Percutaneous Nephrolithotomy) patients is essential to support optimal recovery and improve patients' quality of life. One effective non-pharmacological method to manage pain is foot massage therapy, which provides a relaxing effect and helps reduce pain intensity. This scientific work aims to describe nursing care using foot massage interventions in post-PCNL patients with complaints of acute pain. Methods: This research design is a case study, with the research subject being a patient who was treated in the urology surgery room at General Ahmad Yani Metro Hospital. Foot massage intervention was carried out for two consecutive days, each for 20 minutes each session. The techniques used included effleurage and petrissage movements on the patient's foot area, designed to relieve pain and improve blood circulation. Results: The results showed a decrease in pain intensity in patients after foot massage therapy, from scale 7 to scale 4 within 30 minutes post intervention. Conclusion: This effect shows that foot massage is effective in reducing acute pain in post-PCNL patients, so it can be recommended as a complementary pain management method.

Abstract

Background and Objectives: Femur fractures are one of the leading causes of complications and death that require proper management to prevent morbidity, complications, and accelerate patient recovery. The management of femur fractures includes pharmacological and non-pharmacological approaches, one of which is ambulation support. The purpose of this scientific work is to describe nursing care in postoperative patients with femur fractures through innovative ambulation support interventions. Methods: This research design uses a case study on a postoperative patient with a femur fracture, with early ambulation interventions carried out every day for 3 days of care. Results: The results showed increased left limb mobility, decreased edema, and tingling, and decreased pain intensity from scale 5 to scale 3 after 3 days of treatment. Conclusion:  Early ambulation as part of postoperative management of femur fracture is effective in reducing pain, improving mobility, and accelerating patient recovery. This approach can be adopted in nursing practice to improve the postoperative rehabilitation process.

Abstract

Background & Objective: The purpose of this study was to determine surgical medical nursing care in hypertensive patients with the application of benson relaxation therapy to reduce the level of nyri in the Nerve Room of Rsud Jendral Ahmad Yani Metro City. Method: The method in this scientific work with a descriptive approach to nursing care involving 2 decubitus patients, the instruments used are assessment sheets, head to toe¸ and pain measurement sheets, the application of acute pain treatment using benson relaxation therapy is carried out for 3 days, with the intensity of administration twice a day for 15 minutes. Result: The results of this study found that the main problem that arose in the patient was acute pain, the intervention was arranged by performing benson relaxation therapy, the implementation was carried out for 3 days in the patient, the evaluation of acute pain nursing began to improve on day 3 where when assessing pain obtained a value of 4 and the last day became 2. Recommendations that the author can give are by applying benson relaxation can reduce pain in hypertensive patients and can be done anywhere and anytime without contra indications. Conclusion: Benson relaxation therapy can reduce the pain scale in patients suffering from hypertension.
