Abstract

The Transition of Primary Healthcare Centers to Regional Public Service Agency (BLUD) status allows them to independently manage their finances, including medicine procurement. This change is expected to impact the effectiveness of planning and procurement. This study evaluated the effectiveness of medicine planning and procurement in Primary Healthcare Centers that had transitioned to BLUD status. The study used retrospective observations based on data from 2022 to June 2024. Samples were selected using purposive sampling based on inclusion and exclusion criteria. The independent variable in this study was the implementation of BLUD financial management, while the dependent variables included compliance with FORNAS, medicine stockouts, and excess medicines. The study involved 30 Primary Healthcare Centers and the KruskalWallis test was used for statistical analysis. The results showed that the average scores for all indicators did not meet the expected standards. After BLUD implementation, there was a decrease in medicine compliance with FORNAS, although it remained above the minimum performance standard of 80%, and a significant reduction in stockouts (p < 0.05). The percentage of excess medicines increased, but not significantly (p > 0.05). In conclusion, medicine planning and procurement were ineffective. The BLUD financial management model impacted medicine planning and procurement by reducing compliance with FORNAS, and decreasing stockouts.

Abstract

The Head of the Hospital Pharmacy Department (HPD) plays a vital role in achieving the department's performance goals. Each HPD adopts a unique leadership style that has implications for their job satisfaction. Therefore, it is necessary to examine the influence of the HPD's leadership style on performance achievement and job satisfaction. A survey was conducted using questionnaires. The questionnaire consisted of three instruments: the MLQ,  The BSC, and a job satisfaction questionnaire . Spearman correlation analysis was used to evaluate the data. The transformational leadership had a significant positive impact on all dimensions of performance, with innovation being the most influenced dimension (r = 0.720, p < 0.01), as well as on motivation (r = 0.516, p < 0.01) and hygiene factors of job satisfaction (r = 0.613, p < 0.01). Transactional leadership also showed a positive relationship with performance but was less effective in enhancing job satisfaction. Conversely, laissez-faire leadership showed no significant relationship and even negatively affected innovation and motivation. Transformational leadership emerged as the most effective style for improving the performance and job satisfaction of HPDs in East Java, particularly in fostering innovation. Transactional leadership remains relevant for operational efficiency, while laissez-faire leadership should be minimized. The practical implications for the Head of the Hospital Pharmacy Installation are that the application of transactional and reward-based leadership styles is effective in improving staff performance in the short term, while transformational leadership is also important for building an innovative work culture, with a long-term empowerment and motivation approach

Abstract

Patients with type 2 diabetes mellitus require regular evaluation of the medication's effectiveness and safety through blood glucose and HbA1c testing. This continuous monitoring is one of the primary roles of pharmacists in collaboration with physicians and supported by health insurance to examine the fluctuation patterns of blood glucose levels in response to diabetes medications. This study aimed to identify the need and feasibility of frequent blood glucose monitoring from the perspective of physicians, pharmacists, national health insurance officials, and diabetes patients. A qualitative study using semi-structured interviews was conducted in six Public Health Centers and seven pharmacies in Banjarmasin, Indonesia, from November 2022 to September 2023. A total of twenty-one respondents were included in this study. Data were tape-recorded, transcribed verbatim, and categorized using manual thematic content analysis. This study reveals that inappropriate patient conditions, irregular visits to Public Health Centers, patients' negative emotions, limitations in self-monitoring blood glucose, and lack of patients' knowledge create barriers to conducting monitoring of diabetes. On the other hand, the frequent blood glucose and HbA1c testing was supported by positive perceptions of the importance of testing, ease of access, and the active roles of physicians, pharmacists, and insurance officials. The barriers and facilitators of continuous diabetes medication monitoring are key factors that pharmacists, physicians, and insurance officials should consider to ensure that medication effectiveness and safety are monitored effectively.

Abstract

Background: The management of narcotic drugs in healthcare facilities requires strict control because these medicines have significant therapeutic benefits while also posing a high potential for misuse. Inappropriate practices in narcotic drug use, storage, and administration may increase the risk of medication errors and regulatory violations. Objective: This study aimed to evaluate the use, storage, and administration of narcotic drugs in pharmacies, hospitals, and community health centers through a comprehensive literature review. Methods: This study employed a systematic literature review design following the PRISMA 2020 guideline recommended by the EQUATOR Network. Literature searches were conducted in the Google Scholar database using keywords related to the evaluation of narcotic drug use, storage, and administration in healthcare facilities. Articles published between 2020 and 2024 were screened based on predefined inclusion and exclusion criteria. From 236 identified records, 21 articles met the eligibility criteria and were analyzed using a thematic synthesis approach. Results:The review showed that narcotic drug use in pharmacies was predominantly associated with codeine-containing preparations, whereas hospitals maintained a broader range of narcotic drugs including fentanyl, morphine, and pethidine. Community health centers mainly utilized methadone as part of methadone maintenance therapy programs. Storage compliance was generally high in pharmacies, while hospitals showed moderate variation and community health centers demonstrated more limitations in storage infrastructure. Administrative compliance was generally adequate, although several studies reported incomplete documentation of physician and patient information. Conclusion: Narcotic drug management across healthcare facilities is generally implemented according to regulatory standards, although variations in compliance remain in terms of use, storage, and administrative procedures. Strengthening monitoring systems, improving pharmaceutical workforce capacity, and ensuring consistent implementation of standard operating procedures are necessary to enhance the safety and accountability of narcotic drug management.
