Abstract

Pengabdian ini bertujuan untuk menguatkan peran komunitas dalam kemandirian kesehatan melalui deteksi mandiri hipertensi dan kelainan irama jantung di Puskesmas Gajahan. Peningkatan prevalensi penyakit tidak menular, khususnya hipertensi dan fibrilasi atrium, memerlukan intervensi berbasis komunitas yang efektif untuk mengurangi beban penyakit ini. Metode pengabdian ini melibatkan pada kader kesehatan di wilayah kerja Puskesmas Gajahan dengan melakukan edukasi dan pelatihan kepada kader puskesmas Gajahan mengenai cara deteksi mandiri hipertensi dan kelainan irama jantung. Pelatihan ini mencakup cara pengukuran tekanan darah mandiri dan cara deteksi mandiri kelainan irama jantung dengan cara meraba nadi sendiri (MENARI), serta edukasi mengenai faktor risiko dan tanda-tanda kedua penyakit ini. Hasil dari implementasi pelatihan ini menunjukkan peningkatan pengetahuan dan keterampilan masyarakat dalam mendeteksi dini hipertensi dan kelainan irama jantung. Partisipan yang mengikuti pelatihan mampu melakukan pemeriksaan tekanan darah dan deteksi kelainan irama jantung secara mandiri dengan benar. Selain itu, terdapat peningkatan kesadaran masyarakat tentang pentingnya pemeriksaan kesehatan rutin dan perubahan gaya hidup untuk mengurangi risiko penyakit tidak menular. Kesimpulannya, penguatan peran komunitas melalui edukasi dan pelatihan deteksi mandiri terbukti efektif dalam meningkatkan kemandirian kesehatan masyarakat di wilayah kerja Puskesmas Gajahan.

Abstract

Coronavirus disease 2019 (COVID-19) causes severe acute respiratory disease in humans and has spread rapidly worldwide since its first identification in December 2019. The neutrophil-to-lymphocyte ratio (NLR) describes the balance between the severity of inflammation and the immune system to be used as an important systemic inflammatory marker. Rapid progression of clinical deterioration is characterized by severe respiratory symptoms related to high levels of pro-inflammatory cytokines, like interleukin-6 (IL-6), indicating that the occurrence of cytokine storms leads to increased mortality. This study aims to assess the correlation between IL-6 and NLR in predicting the severity of COVID-19. This prospective cohort study was conducted at the COVID-19 ward of Universitas Sebelas Maret Hospital in August–September 2021. This study involved 66 COVID-19 patients >18 years old with asymptomatic to critical degree and Charlson Comorbidity Index (CCI) value ≤3. Examination of laboratory parameters and serum IL-6 was carried out when the patient entered the Emergency Room. Statistical test with Pearson’s correlation test, significant if p<0.05. There is no significant correlation between IL-6 and NLR with p=0.56 and r=0.08, and a strong correlation between IL-6 and disease severity with p=0.000 and r=0.454. The conclusion is that IL-6 does not correlate with NLR and strongly correlates with disease severity in COVID-19 patients.

Abstract

Background: Dengue Shock Syndrome (DSS) is a severe complication of dengue fever that can lead to life-threatening conditions such as thrombosis and acute kidney injury (AKI). These rare complications highlight the importance of early recognition and multidisciplinary management to improve clinical outcomes. Purpose: This case study aims to report and discuss the clinical management and outcomes of a patient with DSS complicated by inferior vena cava thrombosis and AKI, focusing on nursing interventions and therapeutic strategies. Methods: This manuscript reports a case by utilizing the patient’s baseline data, clinical progression, and treatment outcomes. A middle-aged male patient presented to the emergency department with high fever, shock, and signs of plasma leakage. Laboratory and imaging findings confirmed DSS with AKI and inferior vena cava thrombosis. The patient received fluid resuscitation, inotropic support, anticoagulation therapy, and multidisciplinary care, including close nursing monitoring. Results: After seven days of hospitalization, the patient showed significant improvement, with resolution of shock, normalization of platelet count, and recovery of renal function. Comprehensive nursing care, including vital sign monitoring, medication management, and patient education, played a pivotal role in the recovery process. At a follow-up visit on day 12, the patient demonstrated full recovery with no complications. Conclusions: This case highlights the importance of early diagnosis and a multidisciplinary approach, including cautious anticoagulation therapy, to effectively manage DSS with thrombosis and AKI. Integrating nursing interventions and strict monitoring were crucial to achieving positive clinical outcomes. This report provides valuable insights into managing similar cases in endemic areas.

Abstract

Background:  Urban life, especially in Surakarta, is easy to access and full of high-carbohydrate food and drink. This has increased unhealthy behavior and diets. The prevalence of diabetes Mellitus (DM) in Surakarta is growing, reaching 24% in 2021. Physical exercise and poor diet are important risk factors for DM, coupled with genetics, causing obesity. HbA1c levels periodically assess DM patients' glycemic management.Objective: This study aims to determine the association between family history, body mass index, and regular exercise with HbA1c status in type 2 DM patients at UNS Surakarta Hospital.Methods: The research was an observational study; 24 patients diagnosed with type 2 DM were recruited from the outpatient clinic of Universitas Sebelas Maret Hospital, Surakarta, Indonesia, in October 2021. Measurements were conducted to determine their HbA1c level by enzymatic methods and body mass index by measuring body weight and height, and they were questioned regarding their physical exercise and family history of diabetes. The data collected were analyzed using multiple logistic regression to ascertain the association between family history, body mass index, and exercise regularity with HbA1c status.Results: The findings indicated no statistically significant association between genetic background, body mass index, and exercise frequency with HbA1c status (p>0.05).Conclusion: This research shows that genetic history, body mass index, and exercise regularity are not associated with HbA1c status in type 2 DM in included subjects.

Abstract

Kejadian gawat daruat bisa terjadi pada siapa saja, kapan saja dan dimana saja. Keadaan ini menuntut kesiapan para tenaga kesehatan untuk melakukan tindakan sesuai dengan kompetensinya. Salah satu profesi kesehatan yang bertanggung jawab dalam penatalaksaan pasien gawat darurat adalah perawat. Penelitian di medan tahun 2019 menyebutkan mayoritas perawat tidak memahami cara membaca rekam jantung yang baik. Setelah berkoordinasi dengan ketua PPNI Rumah Sakit Universitas Sebelas Maret, permasalahan utama yang dialami PPNI adalah Pengetahuan mengenai cara membaca EKG pada perawat masih terbatas. Perawat tidak mengetahui gambaran rekam jantung yang berbahaya. Pelatihan membaca rekam jantung jarang ditemui di era pandemic COVID-19, dan tidak ada buku panduan khusus untuk membaca rekam jantung bagi perawat. Tahap-tahap pengabdian dilakukan dengan mengadakan workshop pelatihan membaca EKG secara daring pada minggu ketiga bulan April 2021.Tahap pengabdian selanjutnya adalah buku petunjuk EKG dan brosur EKG. Terjadi peningkatan pengetahuan perawat terhadap EKG dengan peningkatan nilai pretes 55,5 menjadi 72,5. Pelatihan EKG bagi perawat meningkatkan pengetahuan perawat sehingga meningkatkan kualitas pelayanan Kesehatan.

Abstract

Psoriasis can develop into systemic psoriasis, which primarily affects the liver, bile ducts, and cardiovascular system. Assessing the severity of psoriasis was crucial for determining treatment and cardiovascular disease risk. The aim of this study was to determine the role of Gamma-Glutamyl Transferase (GGT), High Sensitivity C-Reactive Protein (HS-CRP), and apolipoprotein B (APO B) in assessing the severity of psoriasis. Methods: This study was a cross-sectional, observational analytic study. A total of 33 psoriasis patients were tested for GGT, HS-CRP, APO B levels, and the Psoriasis Area and Severity Index (PASI) score. Results: The average GGT level was 70.75±56.47 u/L, the average HS-CRP level was 0.91±1.03 mg/L, the average APO B level was 110.09±26.47 mg/dL, and the average PASI score was 11.70±4.54. 12.1% of patients had mild psoriasis, 51.5% had moderate psoriasis, and 36.4% had severe psoriasis. There was a positive and strong correlation between GGT and PASI score, HS-CRP and PASI score, and APO B and PASI score. The multivariate correlation between GGT, HS-CRP, and APO B with PASI score had a value of R = 0.874, F = 31.145, p = <0.001. Conclusion: The multivariate correlation between GGT, HS-CRP, and APO B with PASI scores shows a positive and strong correlation, thus it useful for assessing the severity of psoriasis and evaluating the likelihood of cardiovascular disease.

Abstract

Furniture industry workers in Sambirembe Village, Sragen, face significant health risks from exposure to wood dust, particularly an increased risk of Chronic Obstructive Pulmonary Disease (COPD). This research is prompted by the low awareness among workers regarding these risks and the minimal use of personal protective equipment (PPE). The objective of this program was to increase workers' and business owners' knowledge of lung health, promote early detection of COPD, and encourage the adoption of safer work practices. The method employed was an intervention that included health education and lung function screening via questionnaires and spirometry. The results revealed a critical knowledge gap, where over 85% of workers had never heard of COPD, even though 70% of them reported experiencing respiratory symptoms. Furthermore, over 90% of workers admitted to rarely using masks. The intervention, which combined education with personalized spirometry data feedback, proved effective in increasing worker awareness of tangible health risks. In conclusion, the education-based and health-screening intervention program successfully raised furniture workers' understanding of COPD risks. An approach that integrates information with personal medical evidence is crucial for promoting behavioural change in high-risk work environments.

Abstract

We report a case study of a 58-year-old lady diagnosed with symptomatic knee osteoarthritis (OA) who received umbilical cord-derived mesenchymal stem cell (UC-MSC) secretome via intra-articular injections once weekly for a total of five weeks. This treatment significantly reduced pain compared to the control, using a Visual Analog Scale, with pain scores reducing from 7/10 to 3/10. Moreover, the patient showed marked functional capacity improvement, as indicated by a Western Ontario and McMaster Universities Osteoarthritis Index score from 65 to 30. Her Kellgren-Lawrence grade also improved radiographically from 4 to 2 indicating either an initiation of cartilage regeneration or a reduction in degenerative change. These favorable results position the UC-MSC secretome as a novel and safe treatment for knee osteoarthritis, making it a potent non-invasive replacement therapy to classical treatments. The researchers advise that these findings must be confirmed in bigger, controlled trials before the therapeutic effects of UC-MSC secretome can be established in broader patient populations.

Abstract

Diabetes mellitus (DM) is a chronic metabolic disorder characterized by persistent hyperglycemia, which can trigger inflammatory responses contributing to various vascular complications. Inflammation plays a key role in the pathogenesis of DM-related complications through mechanisms involving oxidative stress and activation of NF-κB signaling, which influence platelet production and activity. Given inconsistent findings regarding the association between HbA1c and mean platelet volume (MPV) in previous studies, this study aims to investigate the relationship between HbA1c and MPV, a pro-thrombotic and inflammatory marker, in patients with type 2 diabetes mellitus. This study is an analytical observational study with a cross-sectional approach. Data were collected from the medical records of Universitas Sebelas Maret (UNS) Hospital, Indonesia, using purposive sampling. HbA1c and MPV data were obtained from patients’ medical records over the past year. Statistical analysis was performed using the Shapiro–Wilk normality test and Pearson correlation test. A total of 38 samples were included in the study. The mean HbA1c level was 10.37% (range: 6.50–16.10), while the mean MPV was 9.38 fL (range: 7.30–12.50). Pearson correlation test showed a significant relationship between HbA1c and MPV (r = 0.467; p = 0.003). There is a significant positive correlation between HbA1c and MPV in patients with type 2 diabetes mellitus.
