Abstract

Pneumonia is one of the leading causes of morbidity and mortality due to lower respiratory tract infections, with treatment heavily reliant on antibiotic administration. However, irrational use of antibiotics significantly contributes to the rise of antimicrobial resistance. This study aimed to evaluate the rationality of antibiotic use in adult pneumonia patients at RSUD Bahteramas, Southeast Sulawesi Province, using the Gyssens method. A total of 100 hospitalized patients who met the inclusion criteria from January to December 2024 were included. Data were analyzed using the Gyssens method to assess the appropriateness of antibiotic therapy based on indication, dosage, duration, route, and timing of administration. The results showed that only 41% of antibiotic use was classified as rational (category 0), while 59% was deemed irrational (categories I–V). Levofloxacin was the most frequently used antibiotic (63%), although its use often did not align with clinical guidelines. Most patients required antibiotics for 3–4 days, consistent with recommendations for mild to moderate pneumonia. Nonetheless, inappropriate use was still observed, particularly in prolonged therapy durations and antibiotic combinations without clear indications. These findings underscore the importance of strengthening adherence to clinical guidelines and conducting regular evaluations to prevent resistance and improve treatment outcomes.

Abstract

Mefenamic acid belongs to a class of the Non-steroidal Anti-Inflammatory drugs that work as an analgesic. But mefenamic acid can cause gastrointestinal disorders, has unpleasant odors and tastes and sensitive to the influence of light and temperature. Microencapsulation technology is a technique where the active substance is coated by a thin layer so that the active substance is protected from environmental influences. The aim of this research was to formulate and characterize mefenamic acid in the form of microencapsulation using ionic gelation methods. Preparation was done by comparing 3 variations of concentrations of sodium alginate polymers. Success parameters include the entrapment efficiency, particle shape, particle size distribution, and dissolution test. The results showed that the entrapment efficiency respectively 98,69%, 96,38% and 93,98%, with spherical shape, and particle size that fulfilled the microencapsulation size range of 1,268 μm, 1,343 μm and 1.386 μm and the release of the active ingredients in an acidic medium of pH 1.2 was 8.811 mg/L, 6.751 mg/L and 5.965 mg/L, also on a base medium of pH 7.4 was 79.908 mg/L, 63.394 mg/L and 40,312 mg/L. Abstract

The continuous growth in the number of pharmacies in Kendari City has intensified competition in this business sector. In such a competitive landscape, brand image and service quality become crucial factors influencing consumer preferences when choosing a pharmacy. Apotek PS, which has been serving the Kendari community for over a decade, is recognized for its strong brand reputation and reliable service standards. This study aims to explore the impact of brand image and service quality on customer loyalty at Apotek PS. Data were collected through a Google Form-based questionnaire completed by 100 customers who had visited at least twice. The collected data were then analyzed using descriptive statistical methods and the Structural Equation Modeling (SEM) approach with the assistance of SmartPLS software. The findings of this study confirm that both brand image and service quality contribute positively and significantly to customer loyalty. Together, these two variables account for 78.4% (indicating a strong influence) of customer loyalty, with a Q-squared value of 0.772, signifying a high level of predictive accuracy. Furthermore, the PLS model evaluation demonstrates an acceptable level of model fit. Abstract

Pneumonia is one of the leading causes of morbidity and mortality worldwide. The irrational use of antibiotics in pneumonia management can reduce therapeutic effectiveness and increase the risk of antimicrobial resistance. This study aimed to evaluate the rationality of antibiotic use in pneumonia patients at Hospital X using the Gyssens method. This research employed a descriptive observational design with a retrospective approach, utilizing medical record data of adult inpatients diagnosed with pneumonia during the period of January–December 2024. A total of 86 patient records meeting the inclusion criteria were analyzed. Evaluation was conducted using the Gyssens method by comparing the appropriateness of therapy with the Guidelines for the Management of Adult Pneumonia issued by the Indonesian Ministry of Health (Regulation No. 28 of 2021) and the Infectious Diseases Society of America/American Thoracic Society (IDSA/ATS, 2023) guidelines. The results showed that the most frequently used antibiotics were Ceftriaxone (34.88%), Azithromycin (32.56%), and Levofloxacin (32.56%). The most common duration of therapy was five days (53.9%). Based on the Gyssens categories, rational antibiotic use (category 0) was found in 57 cases (66.28%), while the remaining cases were categorized as I (6.98%), IIa (11.63%), IIIa (9.30%), and IIIb (5.81%). These findings indicate that most antibiotic use at Hospital X was rational; however, continuous monitoring and periodic evaluation through the Antimicrobial Resistance Control Program (PPRA) are still necessary to optimize therapeutic effectiveness and prevent antibiotic resistance.

Abstract

Background: chronic kidney disease (CKD) is often undetected until reaching advanced stages, posing a serious public health concern. In Kendari City, Indonesia, lifestyle shifts and increased sedentary behaviour have raised concerns about rising CKD prevalence. Physical inactivity is suspected to be a modifiable risk factor, but local empirical data remain limited. Objective: This study aimed to investigate the association between physical activity levels and early kidney function decline, as indicated by urine protein concentration, among adults in Kendari City, Southeast Sulawesi. Methods: A cross-sectional quantitative study was conducted from June to August 2023, involving 119 adults aged 17–75 years selected through purposive sampling across three urban sub-districts. Physical activity was assessed using the International Physical Activity Questionnaire–Short Form (IPAQ-SF) and categorized into low (<600 METs/week), moderate (600–2999), and high (≥3000). Kidney function was measured through morning urine protein dipstick tests and classified into normal, low, moderate, or severe proteinuria. Data were analysed using chi-square, linear regression, and multinomial logistic regression tests. Results: The average physical activity level was 1560.37 ± 1516.78 METs/week (moderate), and the mean urine protein level was 0.64 ± 1.08 g/L (low risk). A statistically significant inverse relationship was found between physical activity and urine protein levels (p < 0.001; β = -0.394). Participants with high physical activity had 100% normal proteinuria, while those with low activity had the highest rates of moderate (32.6%) and severe (30.4%) proteinuria. Multinomial regression indicated that individuals with high activity were 27 million times more likely to have normal protein levels compared to those with low activity (p < 0.001), suggesting a strong protective effect. Conclusion: Lower physical activity is significantly associated with increased risk of impaired kidney function in urban adults. Public health strategies should prioritize structured physical activity promotion and early screening to prevent CKD progression in Kendari City. Further research is needed to examine other contributing factors such as diet, hydration, and lifestyle behaviours.

Abstract

Drug abuse involving Narcotics, Psychotropics, and Other Addictive Substances (NAPZA) poses a serious threat to the physical and mental health as well as the future of young generations. Adolescents, as a vulnerable group, require systematic and sustainable educational interventions to enhance their knowledge and awareness of the risks and consequences of drug abuse. This community service activity aimed to improve students’ understanding of the types, risk factors, and impacts of NAPZA abuse through participatory health education. The implementation method consisted of preparation, inter-institutional collaboration, human resource readiness, and educational activities including pretest, material delivery through lectures and discussions, brief consultations, and posttest evaluation. The activity involved 35 students of SMAN 4 Konawe Selatan and was conducted by students of the Pharmacist Professional Education Program, Universitas Halu Oleo, in collaboration with the South Konawe District Health Office. The evaluation results demonstrated a significant improvement in students’ knowledge levels, with the proportion of students in the good knowledge category increasing from 42.86% before the intervention to 71.43% after the intervention, while the poor knowledge category decreased to 8.57%. These findings indicate that health education on NAPZA is effective in enhancing students’ knowledge and awareness of the dangers of drug abuse. This program is expected to serve as a sustainable collaboration model between educational institutions and health authorities in promoting preventive efforts against drug abuse among adolescents.
