Abstract

FMEA (Failure Mode and Effects Analysis) is a method to identify potential failure modes of a process or product, which has been used since 1950 inside an aviation control system. To implement more efficient improvement of FMEA, criticality analysis was then added into every failure mode, termed Failure Mode, Effects, and Criticality Analysis (FMECA). However, those methods are yet to integrate cost variable, which are essential in the rapid growth of manufacturing industries. Priority-Cost FMECA (PC-FMECA) emerged to reach such objective. PT. Ebako Nusantara is a high-end furniture manufacturing with several steps of production. Interview with QC Department and Defect Event Finding Data in October 2015 portrayed Smoothmill Facility as the area responsible for the largest defect event total in October 2015, which are 32,78%. This research is done to investigate the failures occurring during production so that defect event could be reduced, if not eliminated. This research uses PC-FMECA method, which takes profitability of action into account. In this method, the New RPN is calculated with the adoption of AHP technique, where profitability values are then formulated. Criticality matrices are drawn using priority-profitability diagram to formulate priority of failure. Upon this method, the recommendations are then customized to not exceed the budget of the company.

Abstract

PT. Sango Ceramics Indonesia sudah dikenal di seluruh dunia, karena telah mengekspor hasil produksinya berupa barang pecah belah. Kegagalan produksi yang dialami oleh perusahaan masih tinggi yaitu sebesar 3,1% dari total produksi sedangkan target cacat yang diinginkan perusahaan sebesar 2%. Dalam penelitian ini menggunakan metode Failure Mode and Effect Analysis (FMEA) dan metode Fault Tree Analysis (FTA) untuk mengidentifikasi kegagalan yang terjadi dan memberikan solusi pencegahan. Tujuan penelitian ini adalah Mengidentifikasi jenis-jenis cacat yang terjadi pada produk piring di lantai produksi piring PT Sango Ceramics Indonesia berdasarkan pada hasil penelusuran studi literatur dan deep interview dengan para pegawai dari PT. Sango Ceramics Indonesia, mendapatkan resiko kegagalan proses produksi terbesar dalam nilai RPN (Risk Priority Number ) dari metode FMEA yang kemudian dianalisis kembali menggunakan metode FTA dan menentukan usulan perbaikan yang diperlukan berdasarkan analisis defect yang terjadi pada produk piring. Berdasarkan pengolahan data dengan menggunakan metode FMEA dan FTA dapat mengidentifikasi moda kegagalan yang terjadi pada proses pembuatan piring di PT Sango Ceramics Indonesia.r ABSTRACT PT. Sango Ceramics Indonesia is already known throughout the world, because it has been exporting its products such as glassware. Production failures experienced by companies is still high at 3.1% of total production while the company wants the target of defect is 2%. In this study using Failure Mode and Effect Analysis (FMEA) method and Fault Tree Analysis (FTA) method to identify the failure and find the solution. The purpose of this study are identifying the types of defects that occur in the product plates on the production floor of PT Sango Ceramics Indonesia is based on a literature search results and deep interviews with the employees of PT. Sango Ceramics Indonesia, risk failure of the largest production processes in the value of the RPN (Risk Priority Number) from the FMEA method then reanalyzed using the method proposed FTA and determine the necessary improvements based on the analysis of product defects that occur. Based on the data processing by using FMEA and FTA can identify failure modes that occur in the process of making the plate in PT Sango Ceramics Indonesia.

Abstract

The information system is one of the strategic means in patient care-oriented management. So far, Regional Hospital RAA Soewondo Pati has never conducted an evaluation related to the management information system (HMIS), so it is not known that the existing system is effective and efficient to answer patient needs. The purpose of this study was to determine the effect of the hospital management information system with the structural equation modeling method approach. This research is a case study research with data collection method using a questionnaire. The data that has been collected is then analyzed using SEM analysis. The results of his research stated that the quality of the system had a significant positive effect on the use of information systems. There is a significant effect of system quality on user satisfaction. There is a significant positive effect between the quality of information on the intensity of the use of information. In this study, the intensity of the use of information systems does not play a role in increasing user satisfaction in using information systems. User satisfaction has a significant positive effect on net benefits.

Abstract

Suppliers play an important role in determining product quality, costs, product development, and access to finance for companies. If the company is less selective in choosing its suppliers, the costs incurred may become ineffective. This study aims to develop or design a supplier selection model for non-critical items in Fast Moving Consumer Good companies. In this study, a literature review approach is used by examining 10 journals that have discussed supplier selection procedures. Each journal used as a basis for this study also has different problem objects, models, variables, methods and results. The results of the cross-reference analysis are used to design a procedural model for selecting suppliers for non-critical items. The design of the non-critical item supplier selection model uses several criteria which will later be used as the basis for selecting non-critical item suppliers. From several journals that have been studied, several criteria are obtained that are in accordance with the company that the researcher will conduct research, and will later be re-elected according to the company's needs. This study aims to develop or design a supplier selection model for non-critical items in FMCG (Fast Moving Consumer Good) companies. By using the F-AHP (Fuzzy – Analytical Hierarchy Process) and TLS (Traffic Light System) model, companies that have never had a procedure for selecting suppliers for non-critical items can carry out this procedure so that in the future this procedure can become a standard in companies in terms of choosing supplier.

Abstract

The increasing competition in the global market, driven by globalization and technological advancements, has caused many companies to face challenges in maintaining their market position. As a result, effective quality management systems are essential to enhancing employee performance and delivering high-quality products and services to satisfy customers. This study aims to analyze the effect of Total Quality Management (TQM) implementation on employee performance, the influence of employee performance on customer satisfaction, and the mediating role of employee performance between TQM and customer satisfaction. The research was conducted on companies certified with ISO 9001:2015, using questionnaires filled out by 80 respondents. Data were analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM). The results indicate that TQM significantly improves employee performance, positively affecting customer satisfaction. Additionally, TQM directly enhanced customer satisfaction, with employee performance as an effective mediator in this relationship. These findings suggest that improving TQM practices and employee performance can substantially increase customer satisfaction. The study provides recommendations for companies in Indonesia to optimize TQM implementation, aiming to enhance both employee performance and customer satisfaction.

Abstract

Abstrak PT Unitex merupakan salah satu perusahaan manufaktur yang bergerak dibidang industri tekstil yang ada diIndonesia yang memproduksi bahan baku benang menjadi bahan jadi kapas. Perencanaan bahan baku yangtelah dilakukan oleh PT Unitex saat ini menggunakan perhitungan manual dengan metode yang tidak pastisehingga hasil perhitunganya yang didapatkan kurang akurat. Oleh karena itu penelitian ini bertujuan untukmerancang peramalan bahan baku dari salah satu jenis benang yaitu benang AC 40 dengan menggunakanmetode time series serta memberi usulan safety stok dan kuantitas setiap kali pemesanan dengan pendekatanMin-Max. Berdasarjan permasalah tersebut maka dilakukan perhitungan menggunakan beberapa metodeperamalan yaitu metode 3 Double Moving Average, Double Exponential Smoothing, dan AutoregressiveIntegrated Moving Average untuk mengetahui seberapa besar pemakaian bahan baku benang AC 40 selamaperiode 2023. Berdasarkan hasil pengolahan data didapatkan bahwa metode peramalan terbaik menggunakanmetode Double Exponential Smoothing dan didapatkan minimum inventory bahan baku benang AC 40 periodeJanuari-Desember 2023 untuk jenis kapas Australia, Greek, Brazil, Supima, Egytian, Polyester ECO, dan EsterTifico secara berturut-turu adalah sebesar 88 kg, 131 kg, 196 kg, 45 kg, 47 kg, 23 kg, 11 kg untuk service level99%, serta maksimum inventory berturut-turut adalah sebesar 138 kg, 240 kg, 349 kg, 63 kg, 56 kg, 29 kg, dan16 kg untuk service level 99%. Serta usulan safety stock persediaan bahan baku secara berturut-turut adalah 39kg, 23 kg, 43 kg, 27 kg, 38 kg, 18 kg, dan 6 kg untuk service level 99%.Kata kunci: Forecasting, Time Series, Safety Stock, Metode Min-Max Abstract PT Unitex is a manufacturing company engaged in the textile industry in Indonesia which produces yarn rawmaterials into finished cotton materials. The raw material planning that has been carried out by PT Unitexcurrently uses manual calculations with uncertain methods so that the calculation results obtained are lessaccurate. Therefore, this study aims to design raw material forecasting from one type of yarn, namely AC 40yarn using the time series method and provide a discussion of safety stock and quantity for each order using theMin-Max approach. Based on these problems, calculations are carried out using several forecasting methods,namely the 3 Double Moving Average, Double Exponential Smoothing, and Autoregressive Integrated MovingAverage methods to find out how much AC 40 raw material is used during the 2023 period. Based on the resultsof data processing, it is found that the best forecasting method is using Double Exponential Smoothing methodand obtained a minimum inventory of AC 40 yarn raw materials for the January-December 2023 period forAustralian, Greek, Brazilian, Supima, Egytian, Polyester ECO and Ester Tifico cotton respectively of 88 kg, 131kg, 196 kg, 45 kg, 47 kg, 23 kg, 11 kg for 99% service level, and the maximum inventory is 138 kg, 240 kg, 349kg, 63 kg, 56 kg, 29 kg and 16 kg respectively for 99% service level. Abstract

Abstrak PT. XYZ adalah perusahaan galangan kapal swasta yang terletak di bagian utara KotaSemarang yang berfokus pada pembuatan, perawatan, dan perbaikan kapal. Sebagaiperusahaan galangan yang telah berpengalaman dalam bidang reparasi dan pembangunan kapaltentunya PT. XYZ juga memiliki permasalahan dalam pencapaian target yang dikarenakanketerlambatan proyek dan beban mental yang terjadi pada pekerja. Berangkat daripermasalahan ini, Tujuan dari penelitian ini adalah untuk menilai beban kognitif yang dialamioleh personil yang diharuskan untuk melakukan banyak aktivitas yang berhubungan denganpekerjaan di galangan kapal PT XYZ, dengan menggunakan metode NASA-TLX (Task LoadIndex). Hasil dari perhitungan metode NASA-TLX, skala indikator beban kerja yang palingdominan adalah Effort/Usaha, dengan rata-rata 15,23, yang termasuk dalam kategori bebankerja mental yang tinggi.Kata kunci: NASA-TLX, galangan kapal, beban mental.AbstractPT. XYZ is a private shipyard company located in the northern part of Semarang city thatfocuses on the manufacture, maintenance, and repair of ships. As a shipyard company that hasexperience in the field of ship repair and construction of course PT. XYZ also has problems inachieving targets due to project delays and mental burden on workers. Departing from thisproblem, the purpose of this study was to assess the cognitive load experienced by personnelwho are required to perform many work-related activities in the shipyard of PT XYZ, using theNASA-TLX (Task Load Index) method. As a result of NASA-TLX method calculations, the mostdominant workload indicator scale is Effort,with an average of 15.23, which belongs to thecategory of high mental workload.Keywords: NASA-TLX, shipyard, mental workload.

Abstract

AbstrakPT Toba Pulp Lestari, Tbk merupakan perusahaan penghasil bubur kertas (pulp). Perusahaan ini menggunakan kayu berjenis Eukaliptus (Euchalyptus spp) sebagai bahan baku. Selain penggunaan kayu sebagai bahan baku, produksi pulp juga membutuhkan bahan pendukung berupa bahan-bahan kimia agar dihasilkan produk berkualitas baik. Salah satu bahan kimia pendukung tersebut adalah Hidrogen Peroxide (H2O2) yang digunakan untuk meningkatkan derajat kecerahan produk pulp. Pada proses pengadaan Hidrogen Peroxide (H2O2), perusahaan tidak menentukan jumlah yang akan dipesan, namun bergantung kepada jumlah yang dikirimkan oleh pemasok dengan waktu kirim yang cukup panjang. Hal tersebut menyebabkan kekurangan bahan kimia kerap terjadi sehingga tindakan pengurangan kadar terpaksa dilakukan dan berdampak pada rendahnya kualitas produk dan meningkatkan proses repulping yang menyebabkan naiknya biaya produksi pada perusahaan. Penelitian ini bertujuan untuk melakukan pengendalian persediaan terhadap bahan kimia Hidrogen Peroxide (H2O2). Penelitian ini dilakukan dengan menggunakan metode peramalan time series, menghitung nilai safety stock dan reorder point. Dari hasil tersebut, dapat memberikan gambaran penggunaan bahan kimia pada periode selanjutnya sehingga perusahaan mempunyai dasar dalam melakukan pemesanan, terdapat persediaan pengaman sebagai antisipasi apabila kekurangan bahan kimia terjadi dan perusahaan memiliki titik acuan pemesanan kembali agar dapat dipastikan bahwa Hidrogen Peroxide (H2O2) yang dipesan dari supplier sampai di perusahaan sebelum persediaan di gudang habis digunakan.Kata Kunci : Persediaan, Bahan Kimia, Hidrogen Peroxide (H2O2), Peramalan, Safety Stock, Reorder Point.Abstract PT Toba Pulp Lestari, Tbk is a pulp producer company. In addition to using wood as raw material, pulp production also requires supporting materials in the form of chemicals to produce the good quality products. One of the supporting chemicals is Hydrogen Peroxide (H2O2) which is used to increase the brightness of pulp products. In the process of procuring Hydrogen Peroxide (H2O2), the company does not determine the amount to be ordered but depends on the amount sent by the supplier with a fairly long delivery time. This causes shortages of chemicals that often occur so that content reduction measures are forced to be carried out and have an impact on lower product quality and increase the repulping process which causes an increase in production costs for the company. This study aims to control the supply of the chemical hydrogen peroxide (H2O2). This research was conducted using the time series forecasting methods, calculating the value of safety stock and reorder points. From these results, it can provide a description of the use of chemicals in the next period so that the company has a basis for making an order, there is a safety inventory in anticipation of a chemical shortage and the company has a re-ordering reference point so that Hydrogen Hydrogen (H2O2) is ordered from suppliers has arrive at the company before inventory in the warehouse is used up.Keywords : Inventory, Chemical Material, Hidrogen Peroxide (H2O2), Forecasting, Safety Stock, Reorder Point.

Abstract

AbstrakTBBM Boyolali merupakan bagian dari PT. Pertamina (Persero) Marketing Operation Region IV yang bertugas melaksanakan kegiatan penerimaan, penimbunan, dan penyaluran BBM (Pertamax, Premium, Pertalite, dan Biosolar) untuk masyarakat. Penerimaan BBM/BBK TBBM Boyolali dari Lomanis, Cilacap melewati dua TBBM lain, yaitu TBBB Maos dan TBBM Rewulu. Moda transportasi yang digunakan adalah moda transportasi Pipa Cilacap – Boyolali (CY 2). Penerimaan BBM/BBK TBBM Boyolali dijadwalkan oleh tim dari MOR IV. Divisi Receiving, Storage, and Distribution akan menerima Batching Program setiap bulan dari MOR IV. Masalah yang terjadi adalah adanya perubahan Batching Program dengan cara melakukan penjadwalan penerimaan produk pertamax dari supply ponit TBBM Lomanis, Cilacap ke TBBM Boyolali. Maka, dilakukan penelitian untuk meminimalisir masalah tersebut, menghitung peramalan terhadap produk pertamax, mengetahui penjadwalan lifting penerimaan produk pertamax dari supply point TBBM Lomanis, Cilacap ke TBBM Boyolali, serta mengetahui dan memberikan saran penjadwalan dengan metode Distribution Requirement Planning (DRP). Batasan penelitian yang dibuat adalah bahwa produk yang diteliti hanya pertamax untuk bulan Januari 2019, operasi penerimaan BBM/BBK dari supply ponit TBBM Lomanis, Cilacap langsung menuju TBBM Boyolali, dan dengan data yang digunakan adalah data hasil penjualan produk pertamax tanggal 1 Desember 2018 – 31 Desember 2018. Berdasarkan perhitungan dan penjadwalan DRP pemesanan dilakukan pada tanggal 6 Januari 2019. Sedangkan, pada realisasinya pemesanan dilakukan sebanyak 8 kali dengan kapasitas pengiriman yang berbeda-beda.Kata Kunci : Forecasting; Persediaan; Distribution Requirement Planning (DRP); Mean Absolute Percentage Error (MAPE); liftingAbstractTBBM Boyolali is part of PT. The mode of transportation used is the mode of transportation of Cilacap – Boyolali (CY 2). Receiving FUEL/BBK TBBM Boyolali is scheduled by the team from MOR IV. The Receiving, Storage, and Distribution divisions will receive the Batching Program every month from the MOR IV. The problem is a change in Batching Program by scheduling the acceptance of the first product from the supply of Ponit TBBM Lomanis, Cilacap to TBBM Boyolali. Therefore, research to minimize the problem, calculate the forecasting on the first product, know the scheduling lifting of first product receipt from the supply point TBBM Lomanis, Cilacap to TBBM Boyolali, and know and Provide scheduling suggestions with the Distribution Requirement Planning (DRP) method. The limitation of research made is that the product is researched only first for the month of January 2019, the operation of FUEL/BBK acceptance of the supply of Ponite TBBM Lomanis, Cilacap directly to TBBM Boyolali, and with the data used is the result data Sale of first product on 1 December 2018 – 31 December 2018. Based on the calculation and scheduling of DRP booking was conducted on 6 January 2019. Meanwhile, on the realization of the booking is done 8 times with different delivery capacity. Abstract

AbstrakPT Astra Sedaya Finance dikenal dengan brand Astra Credit Companies (ACC) yang merupakanperusahaan pembiayaan finansial terkemuka di Indonesia. Pada Divisi Human Capital terdiri dari 6departemen, salah satunya adalah Recruitment Management Departement. Faktor eksternal dan internalyang menyebabkan beban kerja mental berlebih berpotensi berpengaruh terhadap kondisi fisik dan psikiskaryawan. Tujuan dari penelitian ini adalah untuk mengidentifikasi besar beban kerja mental yang dialamikaryawan, melakukan analisis aspek dominan serta faktor penyebab yang berpengaruh. Bertujuan agardapat diberikan rekomendasi perbaikan yang sesuai. Subjek responden sejumlah 11 orang karyawandepartemen Recruitment Management Human Capital yang terdiri dari Kepala Departemen dan 5 bidangyang berbeda. Pada metode NASA-TLX terbagi menjadi dua tahap, yaitu pemberian bobot dan pemberianrating indikator yang bersifat subjektif. Pengolahan data menggunakan metode NASA-Task Load Index(TLX) berdasarkan 6 skala indikator yang berpengaruh pada beban kerja mental karyawan. Padapenelitian ini diperoleh hasil bahwa sejumlah 4 karyawan memiliki skor beban kerja mental pada kategoriBerat yaitu lebih dari 80, dengan nilai tertinggi yaitu sebesar 83,33, dan sejumlah 7 karyawan memilikiskor beban kerja mental pada kategori Sedang. Rekomendasi perbaikan diberikan pada aspek lingkunganfisik kerja dan sistem manajerial perusahaan, yang bertujuan meningkatkan optimalisasi kualitas kinerjakaryawan.Kata Kunci: Beban Kerja Mental; NASA-TLX AbstractPT Astra Sedaya Finance, known as Astra Credit Companies (ACC) as its brand, it is a leading financialfinance company in Indonesia. The Human Capital Division consists of 6 departments, which one of it isthe Recruitment Management Department. The external and internal factors that cause excessive mentalworkload have the potential to affect the physical, and psychological conditions of employees. This researchwas conducted to identify the amount of mental workload experienced by employees, and to analyze thedominant aspects as well as the causal factors that affect the mental workload. The recommendations forimprovement can be given according to the existing conditions. The subjects were 11 employees ofRecruitment Management Department, consisting of the Head Department and 5 different fields.
